Comparative Analysis Of Effectiveness And Safety Between Metformin Monotherapy And The Combined Use Of Metformin With Vildagliptin In Individuals Diagnosed With Type 2 Diabetes Mellitus.
Background: Diabetes mellitus is a heterogeneous chronic metabolic disorder principally characterized by persistent hyperglycemia resulting from defects in insulin action and/or insulin secretion. In course of time, prolonged hyperglycemia and associated metabolic aberrations result in tissue toxicity manifested as accelerated atherosclerosis and neuropathy leading to a variety of vascular, neurological, and focal complicationsAim and Objective: The aim of this study is to evaluate the efficacy and safety of metformin with combination of metformin and vildagliptin in patients with Type 2 diabetes mellitus.Materials and Methods: This is a longitudinal interventional study. A total of 100 patients were enrolled in the study. Those patients who were already on metformin 500 mg and 850 mg bid with poor glycemic control were included in the study. These 100 patients were divided into two groups each consisting of 50 patients. Group A patients received metformin 500 mg bid and metformin 850 mg bid. Group B patients received combination of metformin with vildagliptin 50/500 mg bid and metformin 50/850 mg bid. The total period of the study was 3 months.Results: After 3 months of treatment, both the groups caused a significant decline in blood glucose levels both fasting blood sugar (FBS) as well as postprandial blood sugar (PPBS) levels. There was a significant difference between the two groups in reducing the FBS levels and PPBS. Hemoglobin A1C (HbA1c) was also reduced significantly in both groups at 12 weeks. After 3 months of therapy, there was a reduction in HbA1C in B group. The reduction of HbA1C was not statistically significant between the two groups. Adverse effects were more with metformin group at the end of 3 months therapy. There was a significant difference in the incidence of adverse effects between both the groups.
Conclusions: Vildagliptin and metformin combination provided better efficacy comparable to that of metformin and resulted in better adverse effect profile with lower risks of hypoglycemia and weight gai
A Complete review on Detection, Prevention and Treatment of Pyorrhea a major problem in old age people
Pyorrhea an infectious diseases that can be damage the gums and destroys the jaw bones. Pyorrhea is mainly seen in people over 35 years of age that causes tooth loss over time. The current study is clear that gingival bleeding and visible inflammation are the first signs of gingivitis. Continuous bleeding gums could be caused by plaque formation on the teeth, which is connected to a number of systemic disorders in older persons with periodontal diseases. The most common causes of bleeding gum include roughly brushing or wearing fake teeth that do not fit properly and formation of plaque at the gum line, which leads to inflammation. A clinical trial was performed in order to evaluate the effectiveness of mouth wash with Promegranate and Chamomile plant extract, against Chlorhexidine 0.12% in the gingiva bleeding condition. Other treatment is give the prescription of vitamin supplement for bleeding gum. It can be an indication loe ,parfit and Massler depend on a decision about oral color, texture, and contour to determine the level of inflammation in the gums. The aim of this review is to examine the gingival response to dental plaque in gingivitis patients without gum damage, with either a low or high bleeding or plaque ratio. Bio-remedial Impact of Elaeocarpus sphaericus Seed Extract (ESSE) against Sodium Arsenite (As)-Induced Nephrotoxicity in Charles foster rats
The presence of arsenic (As) metalloid in groundwater poses serious threat to human and animal’s health. Approx. 300 million people of about 105 countries in the world are affected due to arsenite poisoning. Except mitigation there is no such mode by which the population can be prevented from being exposed to arsenic. Elaeocarpus sphaericus (E. sphaericus) is widely used in the folk medicine system for the treatment of various diseases. Hence, present study aimed to investigate the Bio-remedial Impact of ESSE against As-Induced Nephrotoxicity in Charles foster rats. Male twenty-four rats (weighing 160 ± 20 g) were randomly assigned into two groups, where Group-I (n=6) rats were used as control. Group-II (n=18) rats were treated with sodium arsenite at 8 mg/Kg body weight for 90 days daily and then further divided into three sub-groups. Sub-Group I (n=6); rats were sacrificed and data were collected, Sub-Group II (n=6); rats were left for 60 days for auto recovery (as As-pre-treated group), and Sub-Group II (n=6); rats were administrated with E. sphaericus at 20mg/kg body weight for 60 days. After the completion of entire experimental dose all the control and treatment group were sacrificed to evaluate the various parameters. As-Induced rats had Significant (p<0.0001) alteration in haematological parameters. As-Induced serum levels of urea, uric acid, creatinine and albumin had significant (p<0.0001) alteration. Level of MDA and BUN were significantly (p<0001) increased. However, ESSE administration significantly reduced the adverse effect related to test of nephrological functions, MDA level significantly (p<0.0001) reduced. Dose dependent ESSE administration combat As-Induced toxicity and significantly (P<0.0001) normalise the level of haematological parameters. Hence, the study concluded that E. sphaericus seed might be used as a nutritional supplement to combat the arsenic led toxicity among the exposed population
Beyond Policies: Exploring Organizational Culture\u27s Impact On Employee Well-Being
The well-being of employees in every firm is significantly shaped by the organizational culture. Identifying important cultural elements and evaluating how well organizational policies and practices support employee welfare are the main goals of this study, which looks at how organizational culture affects employee well-being in the Indian environment. Surveys and qualitative interviews were used in conjunction with a mixed-methods approach to gather data from 103 participants. The study set out to determine which aspects of Indian organizations\u27 cultures positively or negatively affect workers\u27 well-being; second, it sought to identify cultural elements that support or undermine workers\u27 well-being; and third, it sought to assess how well-functioning current organizational policies and practices are in fostering workers\u27 well-being. Descriptive statistics, regression analysis, correlation analysis, and qualitative interviews were all included in the study process. Positive opinions of the work environment, communication styles, and leadership style were found among the workforce using descriptive statistics. The significance of cultivating a good culture was highlighted by correlation analysis, which showed strong relationships between organizational culture dimensions and employee well-being indicators. Key cultural elements influencing worker well-being were found through qualitative interviews. These characteristics included incentives and recognition, communication styles, and leadership philosophies. The outcomes emphasized how important organizational policies and procedures are in fostering worker well-being. Key factors that have been identified as influencing employee wellbeing include flexible work schedules, employee support programs, performance evaluation systems, and health and wellness activities. Further highlighting the predictive efficacy of corporate culture factors on different aspects of employee well-being was regression analysis. In summary, this study offers insightful information about the intricate relationships that exist between employee well-being, policy, and organizational culture in Indian companies. Organizations may establish conditions that support employee well-being by emphasizing a healthy culture and putting in place sensible regulations. This will eventually lead to increased employee satisfaction and corporate succes
Phytochemical Standardization Of Aegle Marmelos Leaves Extract By Using High Performance Thin Layer Chromatography Fingerprinting (Hptlc)
Objective: Aegle marmelos is well known for its therapeutic properties in the Indian traditional medicinal system. The presence of important bioactive compounds in plants makes them pharmacologically valuable. Therefore, in the present study, the high-performance thin layer chromatography (HPTLC) analysis of hydroalcoholic extract of Aegle marmelos was performed for its phytochemical profiling.
Methods: Preliminary phytochemical screening was done. HPTLC studies were carried out. CAMAG make HPTLC system equipped with Linomat 5 applicator, TLC scanner 3, Reprostar 3 and WIN CATS were used. The HPTLC densitometric analysis of the hydroalcoholic extract of Aegle marmelos was carried out using CAMAG HPTLC system, and the results were obtained in the form of chromatograms (scanned at the wavelength of 254 nm and 366 nm) representing several peaks.
Results: Preliminary phytochemical screening of the extract showed the presence of alkaloids, flavonoids, glycosides, saponins, tannins, terpenoids, phenolic compounds, proteins, fats and oils. TLC resulted in identification of three spots found in the hydroalcoholic extract. The phytochemical profile of the plant was determined and presented in the tables showing the total number of peaks, peak heights, peak area, percent area, and Rf values. HPTLC finger printing of hydroalcoholic extract of Aegle marmelos under white light reveal presence of five components 0.04, 0.03, 0.29, 0.42, 0.49. Component number 2,3 and 4 at Rf 0.03, 0.29 and 0.42 showed maximum concentration.
Conclusion: The study concluded that hydroalcoholic extract of Aegle marmelos contains a rich variety of phytochemicals which might be accountable for its therapeutic value and thus justifies its traditional use in India.

The Relationship between Early Maladaptive Schemas and Quality of Life with Couples\u27 Conflict Resolution Methods
The current research investigated the relationship between early maladaptive schemas (EMS) and quality of life with conflict resolution methods of couples. This research was applied in terms of purpose and descriptive survey of correlation type in terms of method. The statistical population included all the couples referred to the Ferdows Educational Counseling Center located in Tehran, with the scope of couple therapy counseling between August and December 2022. Among them, 85 people were selected using the available method who completed Yong\u27s schema questionnaire (2005), the World Health Organization\u27s quality of life questionnaire (1994), and Rahim\u27s (1997) standard questionnaire of workplace patterns. The data were analyzed using Pearson correlation, regression, and Darian analysis. The findings showed a significant negative relationship between the quality of life and schemas of emotional deprivation, mistrust, social isolation, defectiveness/shame, dependence, obedience, emotional inhibition, stubbornness, entitlement, pessimism, and punishment at the 0.05 level, and there is a positive and significant relationship with his workplace style at the 0.05 level. In addition, there is a negative and significant relationship between the quality of life and the schemas of abandonment, failure, self-sacrifice, and restraint at the 0.01 level. There is a positive and significant relationship between the styles of the workplace and emotional isolation schemas, abandonment, failure, vulnerability, obedience and self-sacrifice at the level of 0.05, and a significant negative relationship with schemas of social isolation of the untransformed self, stubborn self-restraint criteria, kindness and punishment at the level of 0.05, and a positive and significant relationship with the schemas of mistrust, defectness/shame, emotional inhibition, entitlement and job acceptance at the level of 0.01
Role Of TRPV I Receptor In Bilateral Common Carotid Artery Occlusion (BCCAO) Induced Vascular Dementia In Rat.
Dementia is a devastating disorder that commonly affects people over the age of 65. Alzheimer’s disease and vascular dementia are the most common forms of dementias, A number of studies have implicated cardiovascular risks as important factors in the development of dementia. Loss of cognitive function and vascular risk factor produce pathological changes into the brain. Changes are Infarcts, Ischemia hypoperfusion, Hemoragic Brain and Atrophy (Degeneration of cells), Neurofibrillary tangles impairment. Bilateral Common Carotid Artery occlusion (BCCAO) has been considered as a critical cause for the development of cognitive decline and dementia of vascular origin.
In our study hypoperfusion reduced oxidative stress, nitric oxide level, Acetylcholine, SOD (Superoxide Dismutase). TRPV1 Receptors have been reported to be beneficial in improving memory deterioration.
Aim of this study is to explore the role of capsaicin in Bilateral Common Carotid Artery occlusion (BCCAO) induced vascular dementia .In clinical research capsaicin role is of antioxidant and other one shows anti-inflammatory actions. It showed significant cognitive deficits, cholinergic dysfunction (increased acetyl cholinesterase-AChE) activity along with increased brain oxidative stress (brain thiobarbituric acid reactive species), glutathione, as well as superoxide dismutase with an increase in malondialdehyde levels) and TTC (2,3,5 Triphenyltetrazolium chloride staining).
Furthermore, treatment of capsaicin reduced BCCAO induced learning and memory deficits.(Morris water maze-MWM), locomotion (Actophotometer), and limited cholinergic dysfunction, oxidative stress, and tissue damage, Estimation of Brain Total Protein, infarct volume suggesting that capsaicin TRPV1 Receptors may provide benefits in BCCAO induced VaD
Assessment Of College Student’s Awareness About Tuberculosis In U.P. (India)
Tuberculosis is an infectious communicable disease caused by mycobacterium tuberculosis and easily spread through droplets. According to WHO 2022 report approx. 10.6 million people across the world were diagnose with tuberculosis, 4.5% increased from 2020, while 1.6 million people died due to this disease. India accounting 2/3rd (68.3%) of total TB count. Tuberculosis is preventable and curable disease. TB stands as major community health problem due to ignorance, unawareness, lack of knowledge of people.1
So it is a primary responsibility of health workers to develop awareness about prevention, care and treatment of TB among community. This survey is based on assessing the awareness of TB among undergraduate students and postgraduate students of nonmedical stream in selected colleges in U.P. In present study total number of students participated in study were 530 students out of that 290 (54.71%) male and 240 (45.28%) were female. Study represents that male student’s had higher awareness than female students. They had moderate knowledge about general awareness of TB, its symptoms, mode of transmission but very poor knowledge about treatment and diagnosis part of TB. Health workers should realize their role in health education programs in the community to improve the public awareness about the health burden diseases like tuberculosis

Phyto-Pharmacology Profile Of Holarrhena Antidysenterica: A Review
Medicinal plants have been recognized for centuries and are highly regarded worldwide as a valuable source of therapeutic agents for disease prevention and treatment. Holarrhena antidysenterica, commonly known as kauri, kurchi, or kutaj, has been employed since ancient times. Holarrhena antidysenterica (also known as H. pubescens), a member of the Apocynaceae family, is renowned in Ayurveda for its medicinal properties found in its stem bark, leaves, and seeds.
