    Effects of virtual reality application on pain, anxiety, and vital signs due to port catheter needle insertion: Randomized controlled study

    Purpose: This study aims to examine the effects of virtual reality applications on pain, anxiety, and vital signs during port catheter needle insertion in adult cancer patients. Methods: The research followed a randomized controlled design as intervention and control groups. In the intervention group, patients received virtual reality sessions with five images in addition to routine procedures during the port catheter needle insertion. We measured the pain, anxiety, and vital parameters resulting from the port catheter needle insertion. Assessments were measured twice: at baseline (before port catheter needle insertion, T0) and at the end of the proceduce (after port catheter needle insertion, T1). Results: The virtual reality application significantly reduced the levels of pain (p=0.45) and anxiety (p<0.001) at T1 in the intervention group compared to the control group. The virtual reality application showed no difference between T0 and T1 in the vital parameters of the intervention group. Conclusions: Virtual reality application during the port catheter needle insertion procedure reduced pain levels and anxiety levels in cancer patients. The virtual reality applications to reduce pain and anxiety levels during the port catheter needle insertion procedure can be recommended

    The Turkish validity and reliability of addenbrooke's cognitive examination III

    Background: Addenbrooke's Cognitive Examination III (ACE-III) was developed as a screening tool for cognitive disorders. Many countries have proven the cultural adaptation, reliability and validity of ACE-III. Aims: To make cultural adaptations of ACE-III for the Turkish population and to examine its validity and reliability. Methods & Procedures: First, ACE-III was translated and adapted into Turkish (ACE-III-TR), then its validity and reliability were examined. The study included 234 people: 93 with dementia (78 Alzheimer's disease (AD) and 15 frontotemporal dementia (FTD)), 46 with mild cognitive impairment (MCI) and 95 healthy. Two blinded speech and language therapists rated the ACE-III-TR simultaneously for interrater validity. The same practitioner retested the same participants 2 weeks later for test-retest reliability. The construct validity of the culturally adapted test was assessed by analysing subsection correlations with the ACE-III-TR total score. The association between the Mini-Mental State Examination (MMSE) total score, relevant subsections and ACE-III-TR total score was examined for criterion validity. Intergroup differences for healthy, MCI and dementia were studied for ACE-III-TR subsections and total score, and cut-off scores were calculated for total score with sensitivity and specificity in differential diagnosis. Results & Outcomes: Attention, memory and ACE-III-TR total scores showed a statistically significant difference between the three groups of dementia, MCI and healthy (p < 0.001). Statistically significant positive correlations ranging from 0.571 to 0.929 were found between ACE-III-TR subsections and total scores (p < 0.05). A highly significant positive correlation was found between MMSE total score and ACE-III-TR total score (r = 0.870). Between the second and first measurements, positive, moderately significant correlations were found for all subsections and ACE-III-TR total (ICC = 0.508-0.784, r = 0.477-0.646). A high level of agreement was found between two raters for all ACE-III-TR subsections and the ACE-III-TR total score (alpha = 0.9296-0.99995). The total ACE-III-TR cut-off score was 79.5 for healthy and MCI and 69.5 for MCI and mild stage dementia. Conclusions & Implications: This study found that ACE-III-TR is a sensitive and specific screening test for the diagnosis of MCI and dementia that has high validity and reliability. ACE-III-TR was found to be a valid and reliable tool in dementia, including AD and FTD, and in mild, moderate and advanced dementia. By providing a more comprehensive assessment of a person's cognitive profile, it can help the clinician make a differential diagnosis of MCI and dementia. ACE-III-TR may be useful in monitoring the progression of cognitive deficits in clinical practice, research studies and therapy follow-up processes

    Tip surgery in dorsal preservation rhinoplasty: the effect of modified low septal strip septoplasty on tip plasty

    BackgroundThe aim in tip surgery is to provide rotation, derotation, projection and deprojection. In this study, we aimed to show the effects of modified low septal strip septoplasty, septal extension graft, TIG technique and additional maneuvers on tip shape in dorsal preservation rhinoplasty (DPR) and to discuss our clinical results.Patients and MethodsOne hundred eighty-nine patients who underwent DPR with modified low septal strip septoplasty between November 2021 and August 2023 were included in the study. Demographic data, complications, revision surgeries and follow-up periods of the patients were analyzed retrospectively.ResultsThe mean age of the patients is 29.58 +/- 9.04 (17-65). The mean follow-up period was 14, 50 +/- 2,98 months. Complications were observed in 1.1% of the patients (n=2/189). Revision surgery was performed in all these patients. Residual hump in 2 were observed and dorsum rasping was performed under local anesthesia. No tip revision was performed on any patient.ConclusionsA strong tip fixation is achieved with the modified low septal septoplasty technique described in this publication, and when combined with septal extension graft, tongue in groove technique and other suture techniques, an effective and permanent tip plasty can be performed in DPR.Level of Evidence IIThis journal requires that authors assign a level of evidence to each article.     Business analytics in customer lifetime value: an overview analysis

    In customer-oriented systems, customer lifetime value (CLV) has been of significant importance for academia and marketing practitioners, especially within the scope of analytical modeling. CLV is a critical approach to managing and organizing a company's profitability. With the vast availability of consumer data, business analytics (BA) tools and approaches, alongside CLV models, have been applied to gain deeper insights into customer behaviors and decision-making processes. Despite the recognized importance of CLV, there is a noticeable gap in comprehensive analyses and reviews of BA techniques applied to CLV. This study aims to fill this gap by conducting a thorough survey of the state-of-the-art investigations on CLV models integrated with BA approaches, thereby contributing to a research agenda in this field. The review methodology consists of three main steps: identification of relevant studies, creating a coding plan, and ensuring coding reliability. First, relevant studies were identified using predefined keywords. Next, a coding plan-one of the study's significant contributions-was developed to evaluate these studies comprehensively. Finally, the coding plan's reliability was tested by three experts before being applied to the selected studies. Additionally, specific evaluation criteria in the coding plan were implemented to introduce new insights.     Spectroscopic evaluation of DNA binding activities of copper (II) phthalocyanine complex consisting of tetrakis-(4-Tritylphenoxy) ligand

    The structure and basic properties of Cu(II) phthalocyanine compound possessing tetrakis-(4-tritylphenoxy) group were elucidated in a past study with the help of absorption and infrared spectroscopic equipments. The electronic spectra, emission spectroscopy, gel agarose electrophoresis and thermal melting were employed to reveal the DNA interaction functions of this complex at changing concentrations of CT-DNA. In this experiment, the binding constant for the Cu(II) phthalocyanine compound which contains the tetrakis(4-tritylphenoxy) group was computed to be 1.53 x 106 M-1. The data obtained from absorption and fluorescence spectroscopic studies revealed that the CuPc compound reacted with CT-DNA through an intercalating mechanism. Well as the above methods, melting temperature and electrophoresis were also employed to analyse the interaction feature of CuPc with DNA. The interaction of the CuPc compound with DNA was also confirmed by data from melting temperature and electrophoresis experiments.     Do nurses follow the “Right Response”? Mixed methods research

    Amaç: İlaç uygulama ve yönetimi, klinik hemşirelik uygulamaları arasında önemli bir prosedür olup, hasta güvenliğini ve bakım kalitesini etkileyen önemli göstergelerden biridir. Hemşirelerin ilaç uygulaması sonrasında ilaç etkilerini takip etme durumlarının belirlenmesi amaçlanmıştır. Yöntemler: Araştırmada keşfedici ve karma araştırma yöntemlerinden sıralı açıklayıcı tasarım kullanılmıştır. Araştırmanın nicel verileri veri toplama formu, nitel verileri ise yarı yapılandırılmış görüşme formu kullanılarak Temmuz-Eylül 2023 tarihleri arasında derinlemesine ve bireysel görüşme yöntemi ile toplanmıştır. Yapılan görüşmeler ses kayıt cihazı ile kayıt edilmiştir. Bulgular: Hemşirelerin yaş, çalışma yılı, haftalık çalışma saati ve günlük bakılan hasta sayıları ortalamalarının sırasıyla 30.46±6.96, 7.1±6.31, 44.92±5.71, 15.42±11.89 yıl olduğu belirlendi. Hemşirelerin %54.2’si ilaç uygulamalarını yapmadan önce ilaç prospektüsünü kontrol ettiği, %74.5’inin ilaç uygulama öncesinde ilaç ve besin alerjisi öyküsünü her zaman hayati önemi olan bir durum olduğunu düşündüğü, %97.9’unun beklenmeyen yan/toksik etkileri bildirdiği saptandı. Araştırmaya katılan hemşireler ile yapılan derinlemesine görüşmeler sonucunda ise doğru ilaç uygulama ilkeleri yaklaşımı, doğru yanıt takip etme yaklaşımı ve hemşirelerin ilaç uygulama sonrası takip etme durumları olmak üzere üç tema belirlenmiştir. Sonuç: Çalışmada, hemşirelerin doğru ilaç uygulama ilkeleri konusunda bilgi eksikliği olduğu, iş yükünün fazla olması, hemşire sayısının az olması ve zaman yetersizliği nedenleriyle doğru yanıt ilkesini uygulayamadıkları belirlenmiştir. Hemşirelerin, doğru yanıt takibinin sorumluluğunu hasta veya hasta yakınına verdiği görülmüştür. Doğru yanıt ilkesinin uygulanmama nedenleri değerlendirilerek, hemşire sayısının artırılması, iş yükünün düzenlenmesi, hemşirelerin doğru ilaç uygulama ilkeleri konusunda ve özellikle doğru yanıtın önemi konusunda eğitimler alması önerilmektedir. Ayrıca hemşirelerin görev, yetki ve sorumlulukları konusunda da eğitim ve danışmanlık alması da önerilerimiz arasında yerini almaktadır.Aim: Drug administration and management is an important procedure among clinical nursing practices and is one of the significant indicators affecting patient safety and quality of care. It was aimed to determine the nurses’ monitoring of drug effects after drug treatment. Methods: In the study, sequential explanatory design from exploratory and mixed research methods was used. Quantitative data were collected using a data collection form and qualitative data were collected through in-depth and individual interviews using a semi-structured interview form. The interviews were recorded with a voice recorder. Results: It was determined that the average age, working years, weekly working hours, and daily number of patients cared for by the nurses were respectively 30.46±6.96, 7.1±6.31, 44.92±5.71, 15.42±11.89 years. It was found that 54.2% of the nurses checked the drug package insert before administering the medication, 74.5% of the nurses always considered the history of drug and food allergy to be of vital importance before drug administration, and 97.9% of the nurses reported unexpected side/toxic effects. As a result of in-depth interviews with the nurses participating in the study, three themes were identified: the approach of right drug administration principles, the approach of following the right response, and the follow-up status of nurses after drug administration. Conclusion: In the study, it was determined that nurses lacked knowledge about the right medication, and could not implement the right response due to high workload, low number of nurses, and lack of time. It was observed that nurses gave the responsibility of right response follow-up to the patient or the patient’s relatives. By evaluating the reasons for not implementing the right response, it is recommended that the number of nurses should be increased, the workload should be regulated, and nurses should receive training on the right drug administration and especially on the importance of the right response.     Glenoid-sided lateralization decreases scapular notching with a 135° humeral component Arthrex reverse shoulder arthroplasty

    Background: The incidence of scapular notching has dramatically reduced with the shift from 155° to 135° humeral inclination in reverse shoulder arthroplasty (rTSA). However, humeral inclination is not the only factor as notching persists with medialized glenospheres. The purpose of this study was to assess the effect of glenoid-sided lateralization on scapular notching with a 135° rTSA. Methods: A retrospective review was performed from a multicenter prospectively collected database to identify patients who underwent primary rTSA and had a minimum 2-year follow-up. All rTSAs were performed with the Arthrex system using a 135° inlay humeral component. Varying amounts of glenoid-sided lateralization were used from 0 to 8 mm in 2-mm increments based on surgeon preference. Postoperative radiographs were evaluated for scapular notching, distalization shoulder angle, and lateralization shoulder angle. The lateralization shoulder angle was further subdivided into humeral and glenoid contributions. Regression analysis was performed on component and clinical variables to assess for factors predictive of notching and the effect on clinical outcomes. Results: Overall, scapular notching was observed in 56 of 517 shoulders (10.8%). Metallic glenoid lateralization and glenosphere overhang were protective of notching (P = .030 and P = < .001, respectively). For women, 6 mm of lateralization reduced notching compared to 4 mm or less (5.3% vs. 15.6%; P = .016). For men, 8 mm of lateralization reduced notching compared to 6 mm or less but the difference did not meet statistical significance (7.6% vs. 13.6%; P = .161). Notching also was associated with decreased forward flexion by 13° (? ?12.79, 95% confidence interval: ?19.63 to 5.96 P = < .001). Discussion: With a 135° inlay humerus and lateralized glenoid rTSA, scapular notching decreases with metallic lateralization of 6 mm or more. Additionally, inferior glenosphere positioning is protective of notching.     Comparative evaluation of external chest wall fixator treatment effectiveness in patients with rib fractures

    Objective: External chest wall fixators may provide a new approach as part of multimodal treatment. This study aimed to investigate the effect of external chest wall fixator on patients' pain level, complication development and hospital stay in patients with rib fractures. Material and method: Patients who were admitted due to trauma and had serial rib fractures between December 2020 and December 2021 were evaluated. There were 14 patients in case group and 20 in control group. External chest wall fixator was applied to the case group in addition to standard treatment. Pain levels, development of complications and duration of hospitalization were recorded. Results: Pain levels in first and third months were lower in case group than control group. Mean pain levels in the first month were 1.79 (SD 0.80) in case group and 2.85 (SD 1.53) in control group, in the third-month were 0.43 (SD 0.64) in case group and 1.34 (SD 1.59) in control group, and the difference was significant (p = 0.022 and 0.032, respectively). Complications were more common in patients with more rib fractures (p = 0.002). While complications developed in 2 patients in the case group and 8 patients in the control group, the difference was not statistically significant (p = 0.216). Duration of hospital stay was shorter in the case group and the difference was significant (2.7 (SD 0.9) days versus 2.0 (SD 0.7) days, p = 0.049). Conclusion: It has been shown in our study that external fixator can be an effective method in reducing patients' pain and hospital stay.     Yoga for pregnancy

    Yoga is a mind-body practice involving posture, deep breathing, and a meditation system. Yoga training for pregnant women should be characterized by gentle, modified poses, an emphasis on breathing and mindfulness, a structured yet flexible session format, and the involvement of trained instructors. The integration of yoga into prenatal care has been supported by various studies that highlight its positive effects on pregnancy outcomes, maternal comfort, and overall wellbeing. From reducing stress, anxiety, and low back pain to improving sleep and flexibility, prenatal yoga is a safe, cost-effective, and feasible way to stay active and prepare for childbirth; it also influences the postpartum period and mother-infant attachment positively. Yoga in pregnancy has physical and mental health benefits for maternal and infant health, which affects the physiology of pregnancy, childbirth, and the postpartum period. The yoga poses mentioned in the section for pregnant women can help alleviate discomfort and promote throughout pregnancy. As more healthcare providers recognize the benefits of yoga, it is likely to become an integral component of holistic prenatal care.
