    The BALM framework of clinical excellence: an adoptable model for sub-Saharan Africa.

    Clinical excellence is the ability to provide rigorous, evidence-based, and rewarding care to the right patients. The BALM (brilliance, attitude, leadership, and materials) framework is a novel model for adjudicating clinical excellence. There are a few models and frameworks for evaluating clinical excellence, but each has its flaws and does not consider the peculiarities of economically-challenged regions such as sub-Saharan Africa. Coincidentally, such low-resource areas have an urgent need for a framework to assess clinical excellence because of their low health-related indices. The five-step mode of professional excellence is a well-recognised tool and was the model upon which the BALM framework was built. So, the BALM framework determines to what extent, using the five-step stages of novice, advanced beginner, competent, proficient, and expert, a clinician has achieved in each of the four critical domains, namely brilliance, attitude, leadership and materials. Therefore, a clinician, who strives for excellence, aims to be an “expert” in each of the domains of the BALM framework. The framework is concise, practical, easy to use, and multidimensional, although it still needs to be applied widely to assess its reproducibility

    Cultivating Compassion: A Cross-Sectional Evaluation of Empathy Among Medical Undergraduate Students

    ABSTRACT: BACKGROUND: Empathy plays a crucial role in medical practice, profoundly affecting patient outcomes and treatment. Understanding the factors that influence empathy among medical students can help in developing strategies to enhance empathetic skills during medical training. This study aims to evaluate the correlates of empathy among medical undergraduate students using an epidemiological cross-sectional design. MATERIALS AND METHODS: A cross-sectional study was conducted among 270 medical undergraduate students from various academic years at JSS Medical College for four months (September 2023-December 2023). Convenient sampling was used to select study participants. Participants completed a standardized empathy questionnaire, the Jefferson Scale of Empathy Student Version (JSE-S) and a demographic survey. Statistical tests like Chi-square tests and Pearson’s correlation test were used. RESULTS: Among 270 students who participated in the study, 145(54%) were females and 125(46%) were males. The mean age of students was 22 + 1.8 years. The mean empathy score among males (79.72 + 9.83) and females (79.49 + 10.36). A correlation was made between the study participants\u27 Age and Total Empathy Score which showed a statistically significant positive correlation (r= 0.265). CONCLUSION: This study highlights key correlates of empathy among medical undergraduate students. The similar empathy scores across genders highlight that both male and female students possess comparable levels of empathy. The increase in age among medical students influences empathy development which encourages the cultivation of empathetic healthcare professionals. Community-based medical education is of vital relevance in the modern change in healthcare from curative to promotive and preventive medicine. By embracing a holistic and community centered approach, community medicine contributes significantly to the overall health of populations and the advancement of public health systems. Community medicine personnel are also involved in various training programs. Their expertise in epidemiology and disease surveillance aids in identifying health trends, responding to disease outbreaks, and developing evidence-based interventions.     Long Term Impact of Covid 19 On Psychiatric Health and Quality of Life in Recovered Patients

    Background: Covid-19 has a devastating impact on different dimensions of life, including health. Recent studies have highlighted the effects of prolonged Covid-19 symptoms on mental health. Aim: This study aims to 1) assess psychiatric illness among individuals previously diagnosed with COVID-19 infection and treated on an inpatient basis, and 2) evaluate the quality of life of individuals with and without psychiatric illness. Method: This hospital-based cross-sectional study was conducted at a tertiary care general hospital in South India and involved 200 patients with COVID-19 infection who received treatment through an inpatient facility. The study utilized a sociodemographic data sheet, a general health questionnaire, a Generalized Anxiety Disorder scale, WHO\u27s Quality of Life scale, and psychiatric diagnosis according to ICD-10 criteria. Results: The findings show that 39.5% of participants had a psychiatric illness. Mild to moderate depression (15.00%), adjustment disorder (9.50%), major depressive disorder (9.00%), and anxiety and panic disorders (4.50%) were frequently reported psychiatric conditions. Anxiety (p Conclusion: More than one third of COVID-19 patients developed psychiatric disorders, like anxiety, depression, and adjustment disorders, three months after leaving the hospital.     Prevalence of Alcohol Dependence Among Rural Residents of Mysuru District: A Community-based Cross-sectional study

    Psychoactive substance use, including alcohol and illicit drugs, poses substantial health risks. A community-based study in rural areas of Mysuru district, employed the AUDIT-C score to assess alcohol dependence. The prevalence of alcohol dependence among rural residents was 77.5%, with males (94.8%) exhibiting higher rates than females (5.1%), although not statistically significant (P \u3e 0.05). Sociodemographic factors showed no significant association (P \u3e 0.05) with alcohol consumption. The study underscores the considerable alcohol consumption in rural area of Mysuru, underscoring the need for targeted awareness programs aimed at controlling and preventing alcohol dependence to enhance public health outcomes

We report a case of a 70-year-old female patient who presented with history of loose stools, vomiting, and abdominal pain. Imaging revealed a small hyper-enhancing nodule in the small bowel, multiple liver lesions, and a mesenteric mass, all suggestive of metastatic carcinoid disease. Biopsy confirmed a neuroendocrine tumor. As these tumors can remain indolent for years, early detection and treatment are critical for effective management and better prognosis
