Preparing for Surgical Residency: A Scoping Review of Surgical Anatomy Courses for Clinical Medical Students
The transition to surgical residency marks a critical point in the transition of medical education, yet over half of entering surgical residents feel unprepared for the transition. Third and fourth-year surgical preparatory courses have emerged as an effective intervention for addressing this gap. Despite growing implementation of fourth-year surgical preparatory curricula, no comprehensive synthesis has mapped the structural characteristics, educational strategies, and longitudinal outcomes associated with these programs. The objective of this scoping review is to synthesize information on surgical preparatory courses in the clinical years of U.S. and Canadian allopathic medical education. This review aims to (1) characterize the structural and curricular features of surgical preparatory courses, (2) synthesize reported subjective and objective outcomes, and (3) examine associations with early residency performance
The Role of Mindset: Who Benefits More from Gratitude Interventions
The present project forms part of a broader study aimed at examining the factors that determine whether certain mindsets (exchange and communal) benefit more from gratitude, and specifically whether differences in mindset underlie the cultural variations observed in the benefits of gratitude. This mixed-methods project complements another component of the study and seeks to generate both qualitative and quantitative experimental data to provide a more comprehensive understanding of whether this difference is also extended into gratitude interventions.
Gratitude is a stable predictor of various positive outcomes; grateful individuals tend to report higher levels of well-being and better overall mental health (e.g. SWB; Deichert et al., 2021). However, despite these consistent correlational findings, the application of gratitude within psychological interventions has produced mixed results, with positive outcomes not always replicating across different populations (Davis et al., 2016). This suggests an incomplete understanding of the underlying mechanisms by which gratitude operates. Recent research (Su et al., manuscript under review) has proposed that gratitude may promote well-being through different pathways, including self-enhancement, indicating that its effects may not be universally experienced or uniformly beneficial. Additionally, evidence suggests that interventions are most effective when they align with individual differences, a concept known as the person X activity fit (Proyer et al., 2015). In the case of gratitude, this means that individuals might benefit most when the intervention reflects their own lived experience or mindset around gratitude (Sin & Lyubomirsky, 2009). As such, variability in intervention outcomes may stem from mismatches between the design of gratitude interventions and the psychological orientations of participants, highlighting the importance of tailoring approaches to better fit individual needs and cognitive styles.
A market mindset, also known as an exchange mindset, is a trait characterized by the habitual tendency to approach social interactions through the lens of cost-benefit analysis, emphasizing fairness, reciprocity, and the expectation of return. Individuals with this mindset are more likely to track the balance of give-and-take in relationships, viewing help or generosity as part of an implicit transaction (Fiske, 1994; Chen & Unal, 2023). Such mindset is also reflected in gratitude, where it functions as a ‘social insurance,’ and grateful individuals are sensitive to the cost and benefit during interpersonal interactions (McCullough et al., 2008; Trivers, 1971). On the other hand, a communal mindset reflects a trait where people provide support based on need rather than expectation of repayment. This orientation prioritizes care, emotional closeness, and shared responsibility, often downplaying or disregarding strict calculations of exchange (Clark & Mills, 2012). Although communal orientations are often framed as the opposite of exchange or market mindsets, this paper does not adopt the assumption that only exchange mindsets foster gratitude. Prior research demonstrates that a sense of communal relationship orientation can, in fact, heighten feelings of gratitude (Simão & Seibt, 2014). However, when communality becomes a chronic or dispositional mindset, help from others may be perceived as normative rather than unexpected (Wood et al., 2008). As a result, expressions of gratitude may diminish over time, as individuals become habituated to both providing and receiving support (Shin et al., 2020; Yu & Chaudhry, 2024). Thus, this paper examines whether individuals with a high trait exchange mindset, characterized by a belief in give and return and similarly aligned with the mindset underlying gratitude, respond more effectively to gratitude intervention.
The scientific significance of this research lies in its potential to further clarify the underlying mindset associated with gratitude, shedding light on the psychological processes that drive its effectiveness. Additionally, it aims to identify the types of individuals who are most likely to benefit from gratitude-based interventions, thus enhancing the precision of targeted support. These insights can contribute to future intervention studies by informing the development of more tailored and effective designs, ultimately improving the overall impact of gratitude-based interventions
Cognitive State Machines - Markov Decision Processes, Monte Carlo Tree Search, and LLMs
Xybernetex introduces an event-sourced cognitive architecture that applies Monte Carlo Tree Search (MCTS) to reasoning paths generated by large language models operating as stochastic transition functions within a Markov Decision Process (MDP). Rather than accepting a single forward-pass output, the system samples multiple structured reasoning trajectories, scores them against a formal proxy reward function combining constraint satisfaction, internal consistency, log-likelihood alignment, and defensibility, and selects the highest-scoring path. Each decision is fully auditable and replayable, enabling deterministic reconstruction and counterfactual exploration. The architecture demonstrates that structured search over model probability space can produce more traceable and defensible outputs than single-pass inference, shifting performance emphasis from raw model scale to system design.

Ayurveda Interventions for ‘Khalitya’ (Hair Fall): A Scoping Review
This project aims to conduct a scoping review of Ayurvedic interventions for ‘Khalitya’ (Hair Fall). The review will map the breadth, characteristics, and outcomes of published clinical evidence on Ayurvedic therapies including oral formulations, external applications, Panchakarma procedures, and Rasayana therapies.
The review will follow PRISMA-ScR guidelines and use a structured PICO framework. Databases including PubMed, CENTRAL, Cochrane Library, Scopus, AYUSH Research Portal, DHARA, Shodhganga, and clinical trial registries will be searched.
The objective is to identify types of interventions studied, outcome measures used, safety reporting, and research gaps in the field.

Blood culture contamination rates in India: a systematic review
Blood culture contamination remains a major quality indicator in clinical microbiology laboratories and directly influences patient management, antimicrobial stewardship, and healthcare costs. Contaminated blood cultures may lead to unnecessary antibiotic use, prolonged hospitalization, additional investigations, and increased healthcare expenditure. International guidelines recommend maintaining contamination rates below 2–3%, yet published data suggest significant variability across institutions and regions.
India has diverse healthcare settings ranging from tertiary academic hospitals to peripheral centers, with heterogeneity in staffing, training, blood collection practices, and laboratory infrastructure. Despite the clinical importance of blood culture contamination, there is no consolidated synthesis of contamination rates reported from Indian healthcare institutions. Understanding the pooled contamination rate and identifying factors influencing variability is essential for benchmarking, quality improvement initiatives, and policy formulation.
This systematic review aims to synthesize available evidence on blood culture contamination rates in India and evaluate determinants associated with higher contamination, thereby providing national-level evidence to inform laboratory practice and infection control strategies
Population, Economy & Becoming Parents: Reproduction Needs Survival First
This project explores the philosophical relationship between population growth, economic structures, and the ethical responsibility of becoming parents. It argues that reproduction is not merely a matter of personal desire or social expectation, but part of a deeper natural order in which survival precedes continuation.
The central thesis proposes that reproduction requires survival first — biologically, economically, and psychologically. When individuals reproduce without securing stable survival conditions, instability may be transmitted across generations, intensifying social and economic vulnerability. Conversely, when survival is stabilized, reproduction becomes a conscious and responsible continuation of life.
The project integrates perspectives from population ethics, philosophy of economics, and social philosophy to examine how personal reproductive decisions intersect with broader structural realities. It frames parenthood not only as a private choice but as an ethical act that shapes future social consciousness.
This work contributes to contemporary debates on population, economic justice, and moral responsibility by proposing a hierarchical order: existence first, stability second, reproduction third
