    A Dual-Layered Structure of Reality

    This manuscript develops a dual-layered metaphysical framework, grounded in information theory and thermodynamics, to reconcile determinism with informational agency. It posits a structural stratification of reality into a deterministic lattice of potential (1) and a finite layer of realization (2), where agency emerges via structural compression reframing the epistemic veil as a constitutive necessity. The framework is justified by the External Reference Paradox (ERP), which employs continuous-time models and deductive arguments to demonstrate that single-layer matter-first ontologies constitute finite, informationally closed systems lacking a stable external reference for long-term error correction and are therefore structurally collapse-bound. This logical limitation is compounded by thermodynamic and dimensional deficits: closed single-layer models lack an entropy sink capable of sustaining biological and informational complexity against the Second Law, and fail to distinguish the static lattice of potential from the sequential traversal of realization. These failures are formalized through boundary conditions that distinguish thermodynamic Totality from Subsystem behavior and establish Asymmetric Persistence as the requisite test for structural viability. The dual-layered framework is therefore proposed as the structural minimum necessary to satisfy both the logical requirements for error correction and the physical requirements of an open, evolving system. Continuity is defined by conservation of information across layers rather than conservation of matter. Reality is derived as = (1) ∙ (2), unfolding through realignment, a process by which the informational layer instantiates finite life-paths from the deterministic lattice. Constrained by informational finitude, this structure yields a Finite-Worlds framework in which agents occupy limited branches per epoch, in contrast to unrestricted multiverse interpretations. Quantum probability is reinterpreted as the geometric density of pre-encoded trajectories within the lattice. Ultimately, agency is framed not as a violation of determinism, but as the evolutionary capacity of information to navigate an exhaustive space of possibilities. Quantum theory is treated as an interpretive and modeling lens that complements this ontology, which describes reality independently of any specific microphysical formulation

    Este projeto consiste em uma revisão de escopo que tem como finalidade mapear a produção científica publicada entre 2020 e 2025 acerca da prescrição de medicamentos realizada por enfermeiros, identificando suas características metodológicas, os modelos de prescrição adotados e os principais aspectos relacionados à prática prescritiva descritos na literatura. A prescrição constitui ato clínico estruturante da prática em saúde, traduzindo o julgamento profissional em condutas terapêuticas organizadas. No âmbito da enfermagem, a dimensão prescritiva integra formalmente o Processo de Enfermagem, reconhecido como método científico que orienta a tomada de decisão clínica e a individualização das intervenções assistenciais. No cenário brasileiro, a Resolução COFEN nº 736/2024 estabelece a prescrição como etapa obrigatória do Processo de Enfermagem no contexto hospitalar, enquanto a Resolução COFEN nº 801/2026 dispõe sobre a prescrição de medicamentos pelo enfermeiro na Atenção Primária à Saúde e em programas de saúde pública. Diante da ampliação normativa e do fortalecimento do escopo profissional da enfermagem, torna-se necessário compreender como a literatura científica recente tem discutido a prescrição de medicamentos por enfermeiros, especialmente no que se refere ao tipo de prescrição realizada, ao grau de autonomia profissional, aos modelos assistenciais adotados e aos desfechos clínicos, organizacionais e assistenciais associados. A pergunta de pesquisa que orienta esta revisão é: “Qual a produção científica de enfermeiros sobre prescrição de medicamentos?”, estruturada a partir do acrônimo PCC, considerando como população os enfermeiros, como conceito a prescrição e como contexto os medicamentos. A revisão será conduzida conforme as diretrizes metodológicas do Joanna Briggs Institute, seguindo as seis etapas do método: definição da pergunta, estabelecimento de critérios de elegibilidade, elaboração das estratégias de busca, seleção dos estudos, extração e mapeamento dos dados e análise/apresentação dos resultados. As buscas foram realizadas nas bases PubMed, Scopus, Web of Science, CINAHL, LILACS e Google Acadêmico, utilizando descritores DeCS e MeSH, com aplicação de filtro temporal para o período de 2020 a 2025. Os dados serão organizados em dois quadros analíticos complementares: um destinado à caracterização metodológica dos estudos e outro voltado à análise da prática prescritiva descrita, contemplando tipo de prescrição, categoria do enfermeiro prescritor, grau de autonomia profissional, desfechos reportados e conclusões dos estudos. Espera-se que o mapeamento das evidências permita identificar padrões, tendências e lacunas na literatura recente, contribuindo para o fortalecimento da reflexão acadêmica, da prática profissional e da consolidação científica da prescrição de medicamentos pelo enfermeiro

    Peyton method versus Halsted method for assessing the acquisition and retention of surgical skills in medical students: randomized educational trial

    Introduction: The teaching of surgical skills has evolved, with criticism of Halsted's traditional model ("see one, do one, teach one") and the emergence of structured methods, such as Peyton's, which uses four stages to promote skill acquisition. Previous studies have shown that Peyton's method is similar to Halsted's in terms of immediate knowledge acquisition and superior in terms of execution efficiency, but there is limited evidence on its effectiveness in long-term retention. Objective: To evaluate the long-term retention rate of Peyton's method compared to traditional Halstedian teaching, using the surgical procedure of water seal chest drainage as a reference. Method: A longitudinal, prospective, randomized, parallel-arm, single-masked study conducted in a simulated hospital. Fourth- to sixth-year medical students will perform the water seal thoracostomy procedure, with assessment of skill retention through practical tests at defined intervals. Expected results: It is hypothesized that the Peyton method will result in greater long-term retention of procedural skills compared to the Halsted model

This living overview will identify and include systematic reviews assessing the effects of psychotherapy, social transition, puberty blockers, cross-sex hormones, surgeries. The overview will also evaluate their risk of bias, synthesize the available evidence, and assess the overall quality of that evidence. This overview will provide a regularly updated, comprehensive synthesis that reveals the best available evidence, clarifies the strengths and limitations of the evidence to better inform clinical practice, research priorities, and policy decision-making
