    Update in the Understanding, Diagnosis, and Management of Sturge Weber Syndrome: Case Report

    Sturge-Weber Syndrome (SWS) is a congenital, vascular, neurocutaneous, uncommon disease associated with facial angiomas port wine birthmark (PWB) or “nevus flammeus”, cerebral vascularity alterations (leptomeningeal vascular malformation), and ocular disorders. It is the third most common neurocutaneous syndrome after neurofibromatosis and tuberous sclerosis. GNAQ R183Q is the most frequent related mutation, caused by a postzygotic, somatic, gain-of-function. 75% of patients present seizures during the first year of life, mainly focal motor seizures, with or without consciousness impairment.We present the case of a 33-year-old female with a diagnosis of SWS, with refractory seizures that started at 4 months of age. In this admission, she presented upper and lower respiratory tract infections that culminated in a convulsive status epilepticus (CSE), the reason for which she required sedation and advanced airway management with adjustment of the anti-seizure medication (ASM). An electroencephalogram (EEG) was performed that reported epileptic activity, as well as an imaging study with data suggestive of calcification in the frontal and right parietal region, compatible with vascular malformation

    Deep Venous Thrombosis as the Clinical Expression of POEMS Syndrome

    This case suggests that POEMS syndrome does present with a constellation of signs and symptoms that may lead a clinician to a multitude of other possible diagnoses. Diagnosis is often difficult and delayed. A good history and physical examination as well as a careful review of all workups are paramount in establishing this particular diagnosis. The major criteria of diagnostic for the syndrome are polyradiculoneuropathy, clonal plasma disorder, sclerotic bone lesions, the presence of Castleman disease, and elevated vascular endothelial growth factor. Minor features include organomegaly, extravascular volume overload, endocrinopathy, characteristic skin changes, papilledema, and thrombocytosis. The diagnosis is made with three of the major criteria, two of which must include polyradiculoneuropathy and clonal plasma cell disorder, and at least one of the minor criteria.     The prevalence of intra-uterine dysfunction of ductus arteriosus is unknown and the clinical consequences are poorly understood. We report a case of prenatal diagnosis of premature closure of the ductus arteriosus due to maternal intake of metamizole during pregnancy. Fetal echocardiography at 37 weeks of gestation revealed a right ventricular hypertrophy and suspected stenosis of the tricuspid valve. A cesarean section led to an excellent neonatal outcome.     Impact of Traumatic Sports Injury on an Athlete’s Psychological Wellbeing, Adherence to Sport and Athletic Identity

    Introduction: Sports injuries can affect athletes across all ages, sexes, and levels of competition. The mental aspect of acute sports injury is often overlooked by coaches, trainers, and medical professionals. This study investigated if and how an acute traumatic sports injury affects an athlete’s psychological well-being, adherence to sport, and athletic identity. Methods: The study consisted of surveys sent to former or current athletes over 18 with one or more athletic injuries. The Qualtrics surveys were anonymous, and participants consented to the study within the survey. Results: There were 101 total participants (20.2% response rate) with an average age of 36. All reported one or more acute athletic injuries throughout their athletic careers. Specific survey sets were compared against each other using a variable correlation analysis (p - value < 0.05) and via Pearson’s Correlation.Conclusion: The results indicated that injury impacts the lives of athletes most significantly on the field and can harm their performance based on their perception of the severity of the injury. However, this decline in performance and decrease in confidence does not correlate to an athlete’s desire to leave their sport or how they identify as being an athlete.&nbsp

    Correlation between the Values of Immature Platelet Fraction and Mean Platelet Volume with the Extent of Coronary Artery Disease in Patients with Non-ST-Segment Elevation Myocardial Infarction

    Introduction: The identification of new markers of thrombotic risk and early diagnosis of Non-ST-segment Elevation Myocardial Infarction (NSTEMI) could allow the optimization of the therapy and predict short and long-term prognosis. Aims and objective: We aimed to assess the impact of Immature Platelet Fraction (IPF) and Mean Platelet Volume (MPV) levels on the extent of Coronary Artery Disease (CAD) in patients with NSTEMI undergoing coronary angiography. Methods: This is a prospective observational study in which 100 subjects of Non-ST Elevation Myocardial Infarction were recruited. For the measurement of platelet count, IPF and MPV samples were analyzed by an automated hematology analyzer (Sysmex XN 1000). Patients were subjected to coronary angiography as per institutional protocol and the extent of coronary artery lesion was noted. Result: A cutoff of MPV (fL) ≥ 10.6 can predict the involvement of the left anterior descending artery (LAD) with a sensitivity of 84%, and a specificity of 50%. With the cutoff of IPF (%) ≥ 2.4, it can even predict the type of disease i.e., Double Vessel Disease (DVD), or Triple Vessel Disease (TVD) with a sensitivity of 97%, and a specificity of 19%. Mean IPF values and MPV levels were significantly higher in patients with LAD involvement i.e., 4.40 ± 1.72% (p = 0.003) and 12.45 ± 1.88 (p = 0.030) respectively than in patients without involvement of LAD i.e., 2.78 ± 1.50% and 11.08 ± 2.19 respectively. Conclusion: Immature platelet fraction and mean platelet volume were significantly associated with the involvement of the left anterior descending artery which was the most commonly involved vessel in patients with NSTEMI. Mean platelet volume was also associated with TVD which was statistically significant. MPV and IPF can be useful early independent hematologic markers to identify patients with a higher risk for significant CAD as they are readily available and inexpensive

    Environmental PM 2.5, Physical Activity Levels, and Aerobic Capacity, of Smokers and Non-Smokers

    Aerobic capacity of young men (19 years - 24 years) is high, but can be influenced by many factors like physical activity, smoking, and air pollution with environmental PM 2.5. Objectives: (a) - to estimate the aerobic capacity in young men (smokers and non-smokers) living in areas with higher PM 2.5 using Queen’s College Step Test (QCT). (b) - to find whether aerobic capacity is associated with the International Physical Activity Questionnaire (IPAQ)’ three classes, for smokers and non-smokers. Methods: In a cross-sectional study using criteria-based sampling a total of N = 60 smokers & non-smokers were included from the Delhi NCR region. IPAQ, Peak Expiratory Flow Rate (PEFR), Heart Rate, Systolic Blood Pressure, Diastolic Blood Pressure, and PM 2.5 and PM 10 levels were recorded. A comparison of smokers and non-smokers was performed using z test. Smokers and non-smokers were divided into three classes using physical activity levels and compared for aerobic capacity. The correlation of aerobic capacity with variables was seen using Pearson’s correlation coefficient. Multiple R was checked to study the model of cause and effect for aerobic capacity. Results: Significant difference is seen between smokers and non-smokers in the aerobic capacity (Mean ± SD smokers - 65.22 ± 8.73 ml/kg/min; Mean ± SD non-smokers 60.04 ± 7.7 ml/kg/min p value = 0.00). For non-smokers, a low level of physical activity shows a strong correlation with aerobic capacity (r = 0.78; p = < 0.05). No correlation of aerobic capacity is seen with physical activity levels among smokers. Aerobic capacity shows a significant negative and moderate correlation with PM 2.5 (r = -3.1; p = 0.016). The multiple R coefficient value for the model of cause and effect is 6.99 with a p - value of 0.0449 for this. Conclusion: Smoking affects aerobic capacity significantly for young men. High and moderate levels of outdoor physical activity do not increase aerobic capacity in areas with high PM 2.5, whereas low levels show a positive correlation among non-smokers only.&nbsp

    A Study of Complete and Incomplete Reactions of 12C + 169Tm System at Energy Range ≈ 4.16 –7.5 MeV/Nucleon

    An attempt was made in this study to measure the excitation functions of 169Tm(12C, 4n)177Re, 169Tm(12C, 5n)176Re, 169Tm(12C, αn)176Ta, 169Tm(12C, α2n)175Ta, 169Tm(12C, α3n) 174Ta, 169Tm(12C, α4n)173Ta and 169Tm(12C, 2α2n)171Lu reaction channels populated in the interaction of 12C projectile with 169Tm target were considered in order to investigate the mechanisms of complete and incomplete fusion reactions. The theoretically predicted excitation functions using the PACE4 code were compared with the previously measured excitation functions. For non α emitting channels cross-section values predicted by PACE4 in general were found to be in good agreement with the experimentally measured values. However, for α-emitting channels, the measured cross-section values were found to be higher than the values predicted by PACE4. The observed disagreement may be credited to projectile break-up in the vicinity of n-n interaction.&nbsp

    Cost Variation Analysis of Various Brands of Anti-Depressants Agents Currently Available in Indian Markets

    Depression is a mental, psychiatric medical condition or disorder in which individuals manifest some clinical syndrome characterized by sadness, mood swings, societal withdrawal, lack of interest, family issues, and education problems which affect the daily student life in which the individual does not participate in daily activities. Sometimes individual commits suicide due to exam stress and that swings the mood upon the condition of the individual. The cost of brand-name medications prescribed in such circumstances exacerbates the disease burden and may even result in noncompliance with therapy. IDR (Indian Depository Receipt) was used to calculate the cost of various antidepressant drug brands. Using the percentage cost ratio, one can ascertain the price of each brand’s 10 tablets in INR (Indian Rupees), the cost ratio, and the percentage cost variance. The difference between the greatest and lowest prices of the same drug produced by Indian pharmaceutical industries was calculated. There is a greater price disparity between agents on the market. The greatest expense variance was found to be amitriptyline 25 mg (195%), fluoxetine 50 mg (95%), sertraline 50 mg (83%) and the lowest % cost variation was of fluvoxamine 20 mg (13.8 mg), duloxetine 20 mg (16%) and escitalopram 10 mg (38%). On the Indian market, the average price disparity between antidepressant medications of various brands is quite high. If a pricey brand is prescribed, patients will incur additional costs

    An Urticaria Closet Simulating Leprosy in A 36-Year-Old Woman

    Urticaria corresponds to a papular edematous pruriginous fleeting rash on the skin whose cause is most often allergic (food, medicines, insect bites, etc.).Its management is often difficult because of its significant impact on quality of life. Its prevalence is estimated at 0.6% - 1.3% of the general population. Leprosy is a chronic infectious disease caused by Mycobacterium leprae (M. leprae) or Hansen’s bacillus (BH). Considered as the great simulator leprosy can pose a diagnostic problem with many other dermatological pathologies. This is a 36-year-old patient with a history of chronic urticaria treated with Loratadine 10 mg who consulted the Bamako Dermatology Hospital for a large erythematous cupboard, oedematous fixed on the buttocks evolving for more than 3 days. The interrogation found the notion of pruritus, hives, and physical examination, showed large erythematous plaques, edematous very limited, dark red color, and slightly copper resembling the closet infiltrated by leprosy. The pruriginous nature of the lesions and the absence of other cardinal signs of leprosy allowed us to make the diagnosis of chronic urticaria
