Deciphering PPAR-inducing pathway clarifying the link between Alzheimer’s Disease and diabetes

The complex sexuality of “Italian” Hikikomori and the need for better nosographic framing of psychopathological evidence
Objective: The definition of “hikikomori” evokes dysfunctional personality pictures already known in the literature and medical practice. The aim is to refute the hypothesis of the need to identify this clinical condition in a new descriptive framework. Materials and methods: Pubmed checklist, clinical interview, and psychometric tests. Results: In the entirety of the selected population, it emerges that the primary disorder is schizoid personality disorder; this is followed by depressive disorder, narcissistic covert disorder, bipolar disorder with depressive prevalence, obsessive disorder, avoidant disorder, and somatic disorder as secondary dysfunctional personality traits. Childhood and/or family trauma, capable of impacting the sexual and affective sphere, is present in almost the entire population. Conclusion: The syndrome should be framed as a specific phenomenon and not as a new psychopathological disorder, as the symptomatological descriptions are similar to the already known schizoid personality disorder; the symptomatological differences among patients should be framed according to a logic of correctives determined by the presence of one or more secondary psychopathological traits that draw a more complex personality picture than the simple nosographic diagnosis of the DSM-V. </p
Gum, sap and canker-colloid carcinoma -pancreas
Colloid carcinoma pancreas is an infiltrative ductal epithelial neoplasm of the pancreas characteristically denominating a preponderant (> 80%) component of enlarged pools of extracellular stromal mucin pervaded with suspended neoplastic cells. Colloid carcinoma pancreas is a microsatellite stable tumefaction and exhibits KRAS genetic mutation confined to codon 12. Tumefaction is posited to arise from the inverse polarization of cells with stromal mucin glycoproteins facing the intrinsic cellular surface. Cogent clinical symptoms such as abdominal or epigastric pain, pancreatitis, diarrhoea, hyperbilirubinemia or loss of weight are discerned. Tumefaction emerges as an enlarged, well-demarcated lesion with a mean diameter of 5 centimetres and a solid, firm, gelatinous cut surface. Neoplasm is predominantly comprised of enlarged, extracellular accumulates of stromal mucin with minimal carcinoma cells suspended within extra-cellular mucin pools. Cuboidal or columnar epithelial cells configure cribriform or stellate cellular clusters or miniature tubules and strips of columnar cells along with signet ring cells.</p

A remark on a perturbed Benjamin-Bona-Mahony type equation and its complete integrability
In the Letter, we study a perturbed Benjamin-Bona-Mahony nonlinear equation, which was derived for describing shallow water waves and possessing a rich Lie symmetry structure. Based on the gradient-holonomic integrability checking scheme applied to this equation, we have analytically constructed its infinite hierarchy of conservation laws, derived two compatible Poisson structure and stated its complete integrability.</p

The imaginary number has been idle for a long time since it was discovered. Investigation of the relationship of the use of the cellular and/or smartphone appliance with osteomioarticular lesions of the cervical spine and hand
Introduction: Considering that the cell phone and/or smartphones had become part of the human being and thus brought problems to people’s health. Objective: To investigate the existing relationship between the use of mobile telephone devices and the health problems of the user, focusing on musculoskeletal injuries of the cervical spine and hand and as hypotheses had the existence or not of this relationship. Methodology: A field study, cross-sectional, with a quantitative approach, of the descriptive type, was carried out in March and April 2019 with academics from the Physiotherapy course at Centro Universitário de João Pessoa- Unipê. Data collection was carried out through a questionnaire that explored questions related to socio-demographic data, the reasons why the devices are used, the quantification of hours of use per day and the postures adopted, among others. Discussions: The data were analyzed and described through tabulation in Microsoft Excel version 2016 through simple descriptive statistics and in that same program, graphs and tables were constructed for a better display of the results that were confronted with the scientific literature. The sample was mostly female, single, brown, with income between one and five minimum wages and right-handed. Conclusion: There are reasons for this relationship to exist and that the problems arising from this interaction continue to grow since users, despite feeling pain, heaviness, numbness and tingling, among others, do not abandon their devices and because it is a theme that involves the world, it needs to be special attention from the scientific community.</p
Robotic assisted laparoscopic radical prostatectomy with the new surgical robot HugoTM RAS in a man with proctocolectomy and restorative ileal pouch-anal anastomosis. The first European urological step-by-step procedure
Robotic-Assisted Laparoscopic Radical Prostatectomy (RARP) is the gold standard surgical treatment for localized clinically significant prostate cancer. In patients undergone previous abdominal or pelvic surgery, it can be difficult to perform and it is also contraindicated execution of radiotherapy.We have described step-by-step the first case of RARP performed with the robotic system HugoTM RAS in a patient previously subjected to proctocolectomy and ileal pouch-anal anastomosis packaging for ulcerative colitis. The procedure was found to be safe and feasible, with intra- and post-operative data overlapping those of the same procedure in the general population. Execution by an experienced surgeon is in any case recommended.</p
Clinical presentation, ICU management and outcome in severe COVID-19 disease – A prospective observational study
Aim: We conducted a prospective, observational study to describe the clinical characteristics, management, and outcomes of patients with moderate to severe coronavirus disease admitted to the Intensive Care Unit (ICU).Methods: The study was conducted from 1st July 2020 to 31st December 2020. The criteria for ICU admission included a positive Reverse Transcriptase-Polymerase Chain Reaction (RT-PCR) for the novel Coronavirus (SARS-CoV-2) and oxygen saturation of less than 90% with or without oxygen. Results: A total of 621 patients were included (median age, 59 years [interquartile range {IQR}, 48-68]; 75.8% were males. The common comorbidities were hypertension (323 [52%]), and diabetes mellitus. 96 (15.5%) patients underwent mechanical ventilation, 18 (2.9%) received renal replacement therapy, and 223 (36%) died. The median age in non-survivors was significantly higher [63 years (IQR 55-71)] vs. 57 years (IQR 46-66)] (p < 0.001). Hypoxia (oxygen saturation (SpO2 < 90%) and shortness of breath suggestive of impending respiratory failure were the most common symptoms necessitating ICU admission. A low absolute lymphocyte count (ALC), and high levels of inflammatory biomarkers which persisted for seven days after diagnosis were significantly associated with non-survival. Multivariate logistic regression analysis showed shortness of breath (SOB), hypoxia (SpO2 < 90% on oxygen), use of neuromuscular blockers, and chronic kidney disease as independent risk factors associated with mortality as were the severity scores (APACHE and SOFA). Conclusion: This single-center case series provides clinical characteristics and outcomes of patients with confirmed COVID-19 disease admitted to the COVID ICU. These findings are important for guiding research and preparing for future pandemics.</p
