    Colitis Cystca Profunda causing colo-colic intussusceptions in an adult

    A 62 years old male with colo-colic intussusception caused by colitis cystica profunda is reported. Patient was admitted with features of complete intestinal obstruction. Patient had the history of altered bowel habits and per rectal discharge of altered blood and mucous for six months. Contrast enhanced CT abdomen showed a transverse colonic mass with colocolic intussusception. With provisional diagnosis of carcinoma colon as a causative lesion, left radical hemicolectomy was performed. Intussuscepted segment had colitis cystic profunda histopathologically.     QT Dispersion & Left Ventricular Hypertrophy in Essential Hypertensive Patients

    Background: Left ventricular hypertrophy (LVH) is an important indicator of target organ damage in chronic arterial hypertension. An increased risk for life-threatening arrhythmias and sudden death have been observed in hypertensive patients, associated with either left ventricular hypertrophy (LVH) or prolonged QT interval. Present study was conducted to compare QT dispersion in hypertensive patients with LVH & without LVH. Methods: Forty male subjects affected by essential hypertension with & without LVH, underwent 12 lead ECG recording for QT dispersion and 2D echocardiography performed to measure interventricular septal thickness, posterior wall thickness and left ventricular internal diameter & compared with normal age & sex matched control. Results: There was statistically significant increase in QT dispersion (p<0.05) in hypertensive subjects with LVH as compared to non-LVH group. Multivariate analysis demonstrated significant relationship between QT dispersion and systolic, diastolic & mean blood pressure, Left ventricular mass & Left ventricular mass index. Conclusion: Sympatho-vagal imbalance in hypertensive patients with LVH could contribute to abnormal ventricular repolarization process leading to prolongation of QT dispersion. Manual measurement of QT dispersion might be a simple, non-invasive screening procedure to identify the hypertensive at greatest risk of sudden cardiac death

    Clinical Profile of Ocular Involvement In Head Injury

    Aim: This study was done to evaluate the simultaneous involvement of ocular structures in cases of head injury. Method: 96 cases of head injury were studied to note the associated ocular affections. It was a prospective nonrandomized analytical observational study. Patients of head Injury were evaluated for any ocular affections .Detailed history, Ophthalmological and neurological evaluation was done. All the relevant data was compiled and analyzed. Ocular manifestations were evaluated in each patient. Appropriate investigations were done to diagnose the cases of traumatic brain injuries. Results-It was observed that eyes are commonly affected in cases of head injury. Maximum number of patients were in the age group of 21-30 yrs (56%). Males were commonly affected .Oedema & echymosis of eyelids was the commonest affection (79%), followed by conjunctival chemosis & subconjunctival haemorrhage (75%) and orbital wall fractures (54%). Among the posterior segment affections, papilloedema and post neuritic optic atrophy were the common manifestations. Conclusion- It is important to evaluate all patients of head injury for any ocular involvement as well; so that timely intervention can be done. A detailed ocular examination in such cases can save the life as well as the sight of the patients

    A retained Pessary in a 65 year old woman: Case Report

    Introduction: Vaginal pessaries are being used as a treatment of uterovaginal prolapse since a long time but forgotten pessaries may cause many complications. They are being used in cases of uterovaginal prolapse if patients are not fit for surgery or refuses for surgery. Case report: Our patient was a 65 year old woman who had pessary insertion 30 years back but luckily she didn’t have any serious complication. Instead her presentation was very unusual. Her pessary was covered on lateral sides by the vaginal epithelium like a loop, which was cut by cautery and the pessary was removed and patient finally underwent vaginal hysterectomy for third degree prolapse. Discussion: Vaginal pessary is a very simple and easy mode of treatment of pelvic organ prolapse and patients are satisfied of their symptoms by it but it can lead to severe complications if not changed in time or forgotten for long time. Before insertion of pessary, patient should be educated about its cleanliness and frequency of changing and should always be called for routine check- up

    Syringoma of the Palate-A Rare Presentation

    Syringoma is a benign adenexal neoplasm formed by well differentiated ductal elements. They are tumors of eccrine origin commonly found over upper part of cheek, lower eyelid, axilla, chest, abdomen, penis and vulva. A 40 year male reported in the OPD with an asymptomatic swelling over the hard palate for the past 15 year. The tumor was excised and sent for histopathological examination. Histological examination showed clusters of eccrine ducts lined by doubled layer epithelial cells. Lumen showed keratinous accellular eosinophillic material suggestive of Syringoma. The case of Syringoma is being reported for its unusual presentation over the palate suggesting it should be considered in the differential diagnosis of various swelling of the palat
