In the conventional reconstruction of microtia, skin grafting is an absolute necessity because of the scantiness of the skin tissue at the affected site. As the result, color- and texture- matching as well as sensory preservation remain to be improved. To solve these problems, we developed a surgical procedure for reconstruction of microtia by using a tissue expander, which allowed us to reconstruct the erect auricle without the need for skin grafting.In this paper, we introduce and discuss our method using the tissue expander with a review of the literature.We operated on a total of 38 patients with microtia. Most of the patients were around 10 years of age, but 3 were adults. Autogenous cartilage was used, and the helix, antihelix, concha, and tragus were reproduced. Initially, 70 ml custommade expanders were used; after that, 75 ml or 95 ml ready-made expanders were used.The erect auricle was reconstructed by the primary procedure in 33 patients. The reconstructed auricle was satisfactory in color- and texture- matching and had nearly normal sensation. No major absorption of the cartilage has been noted for 15 years, although a slight retraction of the expanded skin was observed in each patient.In conclusion, microtia repair by using a tissue expander is superior to that by conventional procedures in color- and texture- matching and sensory preservation of the reconstructed auricle.departmental bulletin pape
Heat shock proteins (HSPs) are highly conserved molecules and distributed widely in nature. They are also distributed in the skin, however, only limited information is available on the role of HSPs in the skin diseases. lmmunohistochemical study of HSPs in the skin revealed that HSPs are differently expressed in the epidermal cells of patients with systemic lupus erythematosus (SLE), atopic dermatitis, graft versus host disease (GVHD) and so on. In normal healthy skin HSPs are constantly expressed in the epidermal cells. HSPs are expressed in the skin according to the influence of both external and internal milieu of the diseased conditions. Cytokines released in the skin strongly affect to express HSPs in epidermal cells. HSPs expressed in the skin can be targets for infiltrated T cells to modulate immune response of skin diseases. Roles of HSPs in the pathogenesis of SLE, GVHD, atopic dermatitis and psoriasis are discussed in this review. HSPs play an important role in the pathogenesis of many inflammatory skin diseases. They can be the molecules to evaluate both diseased conditions and inflammatory process of the skin diseases.departmental bulletin pape
We report in this article a new technique of surgical treatment of enterocutaneous or enterovaginal fistula after total cystectomy for three cases of bladder cancer.The surgical outcome was successful in these patients, showing good performance status after this operation. It would seem that this technique is a good indication for enteric fistulas in the bottom of the pelvic cavity, which were difficult to be managed even under the control of total parenteral nutrition.departmental bulletin pape
Obturator prostheses have been extensively used in the functional rehabilitation of maxillectomy patients. The purpose of this study was to evaluate in vitro the vibration movements of three types of obturator prostheses. Three types of bulbs were used: the solid type, the buccal flange type, and the hollow type. Modal analysis wasperformed to identify the better form. The three types of obturator prostheses were excited by a shaker, and the frequency response functions were recorded on an FFT analyzer to identify their modal shapes. In addition, transient response simulations were carried out, and decay rates of the resultant transient response waves were compared to evaluate the energy absorption after the obturator prosthesis had experienced an impact. The results showed that the modal shapes were closely similar, while the amplitudes at the rests and clasps of the hollow type bulb were the smallest. In the transient response simulation, the decay rates of the hollow type were significantly the highest (p < 0.01). In this study, modal analysis demonstrated that the hollow type obturator prosthesis showed the steepest vibration decay slope, indicating that it might be the best among the three types of bulbs for use in clinical treatments.departmental bulletin pape
The reactions of periodontal tissues to the mechanical stimulation of teeth seem to be different in patients of different ages. Although the effects of aging on other structures of the periodontal tissues, i.e., collagen fibre, bone, and cementum, have been reported, there is very little information available concerning the effects of aging on periodontal oxytalan fibre, which is reportedly the only elastic element in the human periodontal ligament. The purpose of this research was to clarify the aging related changes in the distribution of oxytalan fibres in the periodontal ligament of mice. Histological sections of the periodontal ligament of the lower first molar of 10-week-old (control group) and 1-year-old (aged group) ICR mice were specifically stained for oxytalan fibres with aldehyde fuchsin after preoxidization, and observed by light microscopy and confocal laser scanning microscopy (CLSM). Inthe aged group, oxytalan fibres seemed to be more prevalent around blood vessels in comparison with the control group. Furthermore, CLSM clearly showed that oxytalan fibres in the aged group were relatively more tortuous and complex than those in the control group. This tortuosity and complexity might imply that the oxytalan fibres in aged mice have lost a considerable amount of their original elasticity. While further study of these changes is required, these results may help orthodontists to establish better treatment strategies and methods for adult orthodontic patients.departmental bulletin pape
In mandibulectomy patients who have not undergone surgical reconstruction, the remaining mandibular segment is unstable and often deviated. Its movements have low reproducibility during mastication, deglutition and speech. The purpose of this study was to clarify three-dimensionally the differences in mandibular movements for each of these oral functions in mandibulectomy patients with and without mandibular continuity. Four mandibulectomy subjects (Group I) without mandibular continuity and three subjects (Group II) with mandibular continuity were selected. Their mandibular movements were recorded using a jaw movement tracking device with six degrees of freedom. Each movement was assessed graphically at the virtual incisor point and the rotational angles of the mandible in the frontal, sagittal and horizontal plane were analyzed. The findings were as follows ;1. In Group I, the border movements at the virtual incisor point exhibited an irregular and asymmetric envelope deviated to the resected side in the frontal plane, whereas Group II exhibited a smooth and symmetric envelope. 2. In Group I, the rotational angles in the frontal plane during border movements and mastication, in all planes during speech, and in the frontal and horizontal plane during deglutition were significantly larger than in Group II. A comparison among border and all functional movements in mandibulectomy patients revealed characteristic movements in the rotation of the mandible in the frontal plane. It is suggested that the rotational angle of the mandible is a useful parameter for assessment of mandibular movements in mandibulectomy patients.departmental bulletin pape
To elucidate the possible regulation of endometrial growth, we investigated the expression of tenascin, an extracellular matrix protein associated with cell proliferation and morphogenetic events, in relation to the mitotic marker ki-67 antigen in human endometrium throughout the menstrual cycle. The endometrial tissue was obtained from women with regular menstruation undergoing hysterectomy due to benign uterine disease. Cryostat sections were subjected to double immunohistochemistry to detect both tenascin and ki-67. During the proliferative phase, an intense deposition of tenascin fibrils occurred in the extracellular matrix of the stroma immediately subjacent to the uterine epithelium that showed the highest proportion of ki-67 positive cells. In the secretory phase, the amount of tenascin expression and the proliferative index of the epithelium were markedly reduced. The topological immunoreactivity of tenascin, however, was clearly related to ki-67 localization close to the tortuous endometrial gland and spiral artery. Expression of tenascin, showing changes depending on the menstrual phase, appears to be influenced by ovarian steroid hormones, and it might play a role in regulating human endometrial development by interaction with mitotic cells.departmental bulletin pape
Recent studies and literature reviews of temporomandibular disorders (TMD) do not strongly support the role of occlusal etiologic factors. For this paper we collected and classified studies dealing with occlusion as a contributing factor to TMD through reviews, and discussions of these difficulties and possible solutions. Related articles from 275 papers covering nearly the past twenty years were selected from a prosthetic point of view and reviewed.The validity and reliability of occlusal examinations, however, are questionable since the epidemiological studies dealt with numerous varied subjects. Although occlusal relationships, such as overbite, non-working side interferences, and discrepancy between the intercuspal position and the retruded contact position, have often been considered as contributing factors of TMD, there is no consistency among even those studies that support such an occlusal factor. A project should be initiated to create a revised, precise and practical method to determine the occlusal contact and to categorize an occlusal contact scheme. To detect whether the patients' occlusal schemeworks traumatically or not should be an important part of the occlusal examination. That is, the frequency and duration of tooth contact on the occlusal scheme should be studied.departmental bulletin pape
Midkine (MK) is a new member of the family of heparin-binding neurotrophic factors. MK has several important biological effects and plays an important role in the development and survival of neurons. The mechanism by which MK exerts its neurotrophic actions, however has not been sufficiently clarified. To understand the intracellular pathway activated by MK, we established an apoptosis-induction system with the neuronal cell line PC12 and studied the involvement of the mitogen-activated protein kinase (MAPK) cascade in neuroprotective actions of MK. We demonstrate here that MK rescued PC12 cells from apoptosis induced by serum deprivation in a dose-dependent manner. MK also activated extracellular signalregulated kinases 1 and 2 (ERK1 and ERK2), which are well known as signal transducer acting downstream several receptors. PD98059, an inhibitor of MAPK kinase (MAPKK), inhibited ERK activation and also prevented the trophic effect of MK. These results indicate that MK exerts its neuroprotective actions mainly via ERK activation.departmental bulletin pape
In this article, I review applications of the concentric circle model in clinical nursing. The concentric circle model is based on the cross-sectional shape of the body extremities at several points, and can be used in the areas ofboth kinesiology and nutritional science. This model makes it possible to calculate the cross-sectional area of muscles from measurement of the circumference of the extremities and the thickness of adipose (fatty) tissue. Then, changes in muscle strength or nutritional status can be inferred or assessed from these data. This model requires only simple and non-invasive measurements, and this is a significant and essential characteristic for its use by nurses, both in clinical and research applications.departmental bulletin pape
