    Insulin signaling in the long-lived reproductive caste of ants.

    In most organisms, reproduction is correlated with shorter life span. However, the reproductive queen in eusocial insects exhibits a much longer life span than that of workers. In Harpegnathos ants, when the queen dies, workers can undergo an adult caste switch to reproductive pseudo-queens (gamergates), exhibiting a five-times prolonged life span. To explore the relation between reproduction and longevity, we compared gene expression during caste switching. Insulin expression is increased in the gamergate brain that correlates with increased lipid synthesis and production of vitellogenin in the fat body, both transported to the egg. This results from activation of the mitogen-activated protein kinase (MAPK) branch of the insulin signaling pathway. By contrast, the production in the gamergate developing ovary of anti-insulin Imp-L2 leads to decreased signaling of the AKT/forkhead box O (FOXO) branch in the fat body, which is consistent with their extended longevity

    Coherent vibrational dynamics of Au-144(SR)(60) nanoclusters

    The coherent vibrational dynamics of gold nanoclusters (NCs) provides important information on the coupling between vibrations and electrons as well as their mechanical properties, which is critical for understanding the evolution from a metallic state to a molecular state with diminishing size. Coherent vibrations have been widely explored in small-sized atomically precise gold NCs, while it remains a challenge to observe them in large-sized gold NCs. In this work, we report the coherent vibrational dynamics of atomically precise Au-144(SR)(60) NCs via temperature-dependent femtosecond transient absorption (TA) spectroscopy. The population dynamics of Au-144(SR)(60) consists of three relaxation processes: internal conversion, core-shell charge transfer and relaxation to the ground state. After removing the population dynamics from the TA kinetics, fast Fourier transform analysis on the residual oscillation reveals distinct vibrational modes at 1.5 THz (50 cm(-1)) and 2 THz (67 cm(-1)), which arise from the wavepacket motions along the ground-state and excited-state potential energy surfaces (PES), respectively. These results are helpful for understanding the physical properties of gold nanostructures with a threshold size that lies in between those of molecular-like NCs and metallic-state nanoparticles

    Ninety-day complication, revision, and readmission rates for current-generation robot-assisted thoracolumbar spinal fusion surgery: results of a multicenter case series

    OBJECTIVE Robotics is a major area for research and development in spine surgery. The high accuracy of robot -assisted placement of thoracolumbar pedicle screws is documented in the literature. The authors present the largest case series to date evaluating 90-day complication, revision, and readmission rates for robot-assisted spine surgery using the current generation of robotic guidance systems. METHODS An analysis of a retrospective, multicenter database of open and minimally invasive thoracolumbar instrumented fusion surgeries using the Mazor X or Mazor X Stealth Edition robotic guidance systems was performed. Patients 18 years of age or older and undergoing primary or revision surgery for degenerative spinal conditions were included. Descriptive statistics were used to calculate rates of malpositioned screws requiring revision, as well as overall complication, revision, and readmission rates within 90 days. RESULTS In total, 799 surgical cases (Mazor X: 48.81%; Mazor X Stealth Edition: 51.19%) were evaluated, involving robot-assisted placement of 4838 pedicle screws. The overall intraoperative complication rate was 3.13%. No intraoperative implant-related complications were encountered. Postoperatively, 129 patients suffered a total of 146 complications by 90 days, representing an incidence of 16.1%. The rate of an unrecognized malpositioned screw resulting in a new postoperative radiculopathy requiring revision surgery was 0.63% (5 cases). Medical and pain-related complications unrelated to hardware placement accounted for the bulk of postoperative complications within 90 days. The overall surgical revision rate at 90 days was 6.63% with 7 implant-related revisions, representing an implant-related revision rate of 0.88%. The 90-day readmission rate was 7.13% with 2 implant-related readmissions, representing an implant-related readmission rate of 0.25% of cases. CONCLUSIONS The results of this multicenter case series and literature review suggest current-generation robotic guidance systems are associated with low rates of intraoperative and postoperative implant-related complications, revisions, and readmissions at 90 days. Future outcomes-based studies are necessary to evaluate complication, revision, and readmission rates compared to conventional surgery

    Patterns of association and distribution of estuarine-resident common bottlenose dolphins (Tursiops truncatus) in North Carolina, USA

    The social structure of estuarine-resident bottlenose dolphins is complex and varied. Residing in habitats often utilized for resource exploitation, dolphins are at risk due to anthropogenic pressures while still federally protected. Effective conservation is predicated upon accurate abundance estimates. In North Carolina, two estuarine-resident stocks (demographically independent groups) of common bottlenose dolphin have been designated using spatiotemporal criteria. Both stocks are subjected to bycatch in fishing gear. The southern North Carolina estuarine stock was estimated at <200 individuals from surveys in 2006, which is outdated per US guidelines. Thus, we conducted a new capture-mark-recapture survey in 2018, identifying 547 distinct individuals, about three times higher than the prior abundance estimate. We compared those individuals to our long-term photo-identification catalog (1995-2018, n = 2,423 individuals), matching 228 individuals. Of those 228, 65 were also included in the 2013 abundance estimate for the northern North Carolina estuarine stock. Using sighting histories for all individuals in the long-term catalog, we conducted a social network analysis, which is independent of a priori stock assignments. The three primary clusters identified were inconsistent with current stock designations and not defined by spatiotemporal distribution. All three clusters had sighting histories in the estuary and on the coast, however, that with the highest within-cluster associations appeared to use estuarine waters more often. The within-cluster association strength was low for one cluster, possibly due to only part of that cluster inhabiting the southern North Carolina estuarine system. Between-cluster differences occurred in infestation rates by the pseudostalked barnacle, Xenobalanus globicipitis, but that did not predict clusters. We suggest the need to re-evaluate the stock structure of estuarine-resident common bottlenose dolphins in North Carolina and currently have insufficient information to assign an abundance estimate to a currently designated stock

    Outcomes of Gastrostomy Tubes in Newborns With Congenital Heart Disease and Comparison of Techniques

    Introduction: Gastrostomy tube (GT) placement for enteral access is one of the most com-mon procedures for infants with numerous conditions such as congenital heart disease (CHD). Discrepancies in the literature exist regarding outcomes of newborns with CHD undergoing GT placement. This study sought to characterize postoperative outcomes and readmission complications in this patient population.Methods: The Nationwide Readmission Database was queried from 2010 to 2014 for all newborns who underwent GT placement during their index hospitalization. Newborns with or without CHD other than an isolated atrial or ventricular septal defect were compared using standard statistical tests. A propensity score-matched analysis was performed among newborns with or without CHD using > 100 covariates.Results: Seven thousand seven hundred thirty six patients underwent GT placement. Newborns with CHD (27%) more frequently underwent open GT (59% versus 55%) and less frequently underwent laparoscopic (17% versus 19%) or percutaneous (24% versus 26%) GT placement compared to those without CHD, P = 0.043. GT-related complica-tions on index admission were similar between groups (7% versus 7%, P = 0.770). Newborns with CHD had higher overall readmission rates (39% versus 31%), more GT-related readmission complications (7% versus 3%), and higher readmission costs (35,787versus35,787 versus 20,073) compared to newborns without CHD, all P < 0.001. Laparo-scopic GT was associated with the lowest rate of GT-related complications (0%) and overall readmission rates (27%) compared to open or percutaneous endoscopic gastrostomy (all P < 0.001). Conclusions: Compared to newborns without CHD, newborns with CHD had higher rates of overall readmissions, readmission costs, and GT-related complications on readmission.     Durvalumab consolidation in patients with unresectable stage III non-small cell lung cancer with driver genomic alterations

    Durvalumab is the standard-of-care as consolidation therapy after chemo-radiotherapy in stage III unresectable non-small cell lung cancer (NSCLC); however, its activity across patients with NSCLC harbouring driver genomic alterations (dGA) is poorly characterised. Multicentre retrospective study including patients with stage III unresectable NSCLC treated with durvalumab after chemo-radiotherapy between April 2015 and October 2020 at 26 centres in Europe and America. Clinical and biological data were collected; dGA included: EGFR/BRAF/KRAS mutations (m) and ALK/ROS1 rearrangements (r). We evaluated progression-free survival (PFS) and overall survival (OS) based on dGA. Out of 323 patients included, 43 patients had one dGA: KRASm (n = 26; 8 G12C), EGFRm (n = 8; 6 del19/ex21), BRAFm (n = 5; 4 V600E) and ALKr (n = 4). The median age was 66 years [39–84], gender ratio 1:1, with 98% performance status (PS) 0–1 and 19% non-smokers; 88% had adenocarcinoma. PD-L1 was positive in 85% (n = 4 missing). In the whole cohort, the median PFS was 17.5 months (mo.) (95% CI, 13.2–24.9) and median OS 47 mo (95%CI, 47-not reached [NR]). No statistically significant differences in terms of the median PFS were observed between patients with dGA vs. non-dGA: 14.9 mo (95% CI, 8.1-NR) vs. 18 mo. (95% CI, 13.4–28.3) (P = 1.0); however, when analysed separately: the median PFS was NR (11.3-NR) in the KRASm G12C vs. 8.1 mo (5.8-NR) in the EGFRm del19/ex21 vs. 7.8 mo (7.7-NR) in the BRAFm V600E/ALKr (P = 0.02). We observed limited activity of durvalumab consolidation in patients with stage III unresectable NSCLC with EGFR/BRAFm and ALKr but not for those harbouring KRASm. Larger prospective studies are needed to confirm these findings. •Similar treatment outcomes to PACIFIC phase III trial.•Different benefits of durvalumab consolidation based on oncogenic alterations.•Significant increase in PFS with durvalumab in patients with KRAS mutations.•ALK-rearranged NSCLC seem to benefit the least to durvalumab consolidation.•Largest retrospective study evaluating durvalumab in oncogenic addicted stage III NSCLC
