    Capturing drug use patterns at a glance: An n-ary word sufficient statistic for repeated univariate categorical values

    Introduction The efficacy of treatments for substance use disorders (SUD) is tested in clinical trials in which participants typically provide urine samples to detect whether the person has used certain substances via urine drug screenings (UDS). UDS data form the foundation of treatment outcome assessment in the vast majority of SUD clinical trials. However, existing methods to calculate treatment outcomes are not standardized, impeding comparability between studies and prohibiting reproducibility of results. Methods We extended the concept of a binary UDS variable to multiple categories: “+” [positive for substance(s) of interest], “–” [negative for substance(s)], “o” [patient failed to provide sample], “*” [inconclusive or mixed results], and “_” [no specimens required per study design]. This construct can be used to create a standardized and sufficient representation of UDS datastreams and sufficiently collapses longitudinal records into a single, compact “word”, which preserves all information contained in the original data. Results We developed the R software package CTNote (available on CRAN) as a tool to enable computers to parse these “words”. The software package contains five groups of routines: detect a substance use pattern, account for a specific trial protocol, handle missing UDS data, measure the longest period of consecutive behavior, and count substance use events. Executing permutations of these routines result in algorithms which can define SUD clinical trial endpoints. As examples, we provide three algorithms to define primary endpoints from seminal SUD clinical trials. Discussion Representing substance use patterns as a “word” allows researchers and clinicians an “at a glance” assessment of participants’ responses to treatment over time. Further, machine readable use pattern summaries are a standardized method to calculate treatment outcomes and are therefore useful to all future SUD clinical trials. We discuss some caveats when applying this data summarization technique in practice and areas of future study

    Variation in the Drought Tolerance of Tropical Understory Plant Communities across an Extreme Elevation and Precipitation Gradient

    Little is known about how differences in water availability within the “super humid” tropics can influence the physiology of understory plant species and the composition of understory plant communities. We investigated the variation in the physiological drought tolerances of hundreds of understory plants in dozens of plant communities across an extreme elevation and precipitation gradient. Specifically, we established 58 understory plots along a gradient of 400–3600 m asl elevation and 1000–6000 mm yr −1 rainfall in and around Manu National Park in southeastern Peru. Within the plots, we sampled all understory woody plants and measured three metrics of physiological leaf drought tolerance—turgor loss point (TLP), cuticular conductance (G min ), and solute leakage (SL)—and assessed how the community-level means of these three traits related to the mean annual precipitation (MAP) and elevation (along the study gradient, the temperature decreases linearly, and the vapor pressure deficit increases monotonically with elevation). We did not find any correlations between the three metrics of leaf drought tolerance, suggesting that they represent independent strategies for coping with a low water availability. Despite being widely used metrics of leaf drought tolerance, neither the TLP nor G min showed any significant relationships with elevation or the MAP. In contrast, SL, which has only recently been developed for use in ecological field studies, increased significantly at higher precipitations and at lower elevations (i.e., plants in colder and drier habitats have a lower average SL, indicating greater drought tolerances). Our results illustrate that differences in water availability may affect the physiology of tropical montane plants and thus play a strong role in structuring plant communities even in the super humid tropics. Our results also highlight the potential for SL assays to be efficient and effective tools for measuring drought tolerances in the field

    Updates in classification, risk stratification and response assessment in myelodysplastic syndromes/neoplasms (MDS): A state-of-the-art report on behalf of the International Consortium of MDS (icMDS)

    The guidelines for classification, prognostication, and response assessment of myelodysplastic syndromes/neo-plasms (MDS) have all recently been updated. In this report on behalf of the International Consortium for MDS (icMDS) we summarize these developments. We first critically examine the updated World Health Organization (WHO) classification and the International Consensus Classification (ICC) of MDS. We then compare traditional and molecularly based risk MDS risk assessment tools. Lastly, we discuss limitations of criteria in measuring therapeutic benefit and highlight how the International Working Group (IWG) 2018 and 2023 response criteria addressed these deficiencies and are endorsed by the icMDS. We also address the importance of patient centered care by discussing the value of quality-of-life assessment. We hope that the reader of this review will have a better understanding of how to classify MDS, predict clinical outcomes and evaluate therapeutic outcomes.</p

    The myth of bureaucratic neutrality an examination of merit and representation

    "In a system discredited by political corruption, the notion of bureaucratic neutrality was presented during the Progressive era as strategy to restore legitimacy in government. However, bureaucratic neutrality also served as a barrier to equity in government. This book argues that neutrality is a myth that has been used as a means to oppress marginalized communities, largely disconnected from its origins within the field of public administration. A historical perspective of how the field has understood race and gender demonstrates how it has centered whiteness, masculinity, and heteronormativity in research and administrative practices, mistaking them for neutrality in public service. Using a historically grounded positionality approach, the authors trace the myth of bureaucratic neutrality back to its origins and highlight how it has institutionalized inequity, both legally and culturally. Ultimately, the authors demonstrate that the only way to move towards equity is to understand how inequity has become institutionalized, and to constantly work to improve our systems and decision making.     Improvements in operant memory of Aplysia are correlated with age and specific gene expression

    The transcription factor Aplysia CCAAT/enhancer binding protein ( ApC/EBP ) is expressed as an immediate early gene in the cAMP responsive element binding protein (CREB) mediated gene cascade, and it has essential functions in the synaptic consolidation of memory following a learning event. Synaptic consolidation primarily involves morphological changes at neuronal synapses, which are facilitated through the reorganization of the actin and microtubular cytoarchitecture of the cell. During early nervous system development, the transmembrane synaptic protein teneurin acts directly upon neuronal presynaptic microtubules and postsynaptic spectrin-based cytoskeletons to facilitate the creation of new synapses. It is reasonable to hypothesize that teneurin may also be linked to learning-induced synaptic changes and is a potential candidate to be a later gene expressed in the CREB-mediated gene cascade downstream of ApC/EBP . To assess the role of ApC/EBP and teneurin in learning and memory in the marine snail Aplysia californica , young (age 7–8 months) and aged (age 13–15 months; aging stage AII) siblings of Aplysia were trained in an operant conditioning paradigm—learning food is inedible (LFI)—over 2 days, during which they learned to modify the feeding reflex. Aged Aplysia had enhanced performance of the LFI task on the second day than younger siblings although far more aged animals were excluded from the analysis because of the initial failure in learning to recognize the inedible probe. After 2 days of training, ApC/EBP isoform X1 mRNA and teneurin mRNA were quantified in selected neurons of the buccal ganglia, the locus of neural circuits in LFI. Teneurin expression was elevated in aged Aplysia compared to young siblings regardless of training. ApC/EBP isoform X1 expression was significantly higher in untrained aged animals than in untrained young siblings but decreased in trained aged animals compared to untrained aged animals. Elevated levels of ApC/EBP isoform X1 and teneurin mRNA before training may have contributed to the enhancement of LFI performance in the aged animals that successfully learned

    MyTPill: study protocol for a cross-over randomised controlled trial comparing novel strategies to monitor antiretroviral adherence among HIV plus prescription opioid users

    IntroductionAdherence to HIV antiretroviral therapy (ART) remains the cornerstone of HIV treatment. For individuals with suboptimal adherence, electronic adherence monitoring (EAM) technologies have become an important component of multimodal adherence support strategies. Most EAM technologies detect pillbox opening, and therefore, assume but cannot verify actual ingestion of oral medication. In contrast, a digital pill system (ID-capsule manufactured by etectRX, here named My/Treatment/Pill) measures directly ingestion of medications. Identifying the superior method to measure ART adherence would improve virological suppression by enabling the delivery of real-time interventions to support ART adherence, particularly in high-risk populations.Methods and analysisCross-over, randomised trial with 1:1 variable block size randomisation comparing two EAM systems in prescription opioid-using HIV+patient on once daily oral bictegravir, emtricitabine and tenofovir alafenamide regimens and detectable viral load >200 copies/mL within 30 days of screening (n=80). The primary outcome is once daily ART adherence measurement efficacy as assessed by comparing the accuracy of each EAM system as measured by concordance of the respective EAM systems to dried blood spot ART concentrations. Secondary outcomes are the identification of multilevel factors that are prevalent in the target population most closely linked to ART non-adherence and EAM non-adherence.Ethics and disseminationThis protocol was approved by the institutional review boards of participating sites (The Ohio State University, The Fenway Institute and the University of Miami).     Does Problematic Substance Use Moderate Efficacy of Cognitive Behavioral Therapy for Adherence and Depression in HIV?

    Problematic substance use may attenuate the effect of treating depression in people living with HIV (PLWH). We examined the potential moderating effect of problematic substance use on depression and adherence outcomes in PLWH (N = 143) who participated in a randomized controlled trial of cognitive behavioral therapy for adherence and depression (CBT-AD) and were randomized to either CBT-AD or enhanced treatment as usual (ETAU). Problematic substance use was operationalized as either having substance use-related diagnosis or current substance use in the past 30 days with a lifetime history of problematic use. Acute (baseline to 4-month) and follow up (4, 8, and 12-month) general linear modeling with time, condition, problematic baseline substance use, and corresponding interactions demonstrated that substance use did not significantly moderate the effects of CBT-AD on adherence or depression improvements. Therefore, CBT-AD was beneficial for PLWH with depression, regardless of problematic substance use when starting depression treatment. Based on these results, clinicians should not withhold CBT treatment for depression in patients with HIV and problematic substance use to attain reductions in depression and gains in adherence
