Tormenta de citocinas secundaria a la COVID-19
Introduction:COVID-19 is the disease caused by a new coronavirus SARS-CoV-2 that was identified and characterized in January 2020 in China. This condition is accompanied by an aggressive inflammatory response with a huge liberation of pro-inflammatory cytokines. This event is called cytokines torment.Objective: to describe cytokines torments associated to COVID-19.Methods: a review of 29 bibliographic references was made, consulting articles from Elsevier, SciELO, ClinicalKey,Pubmed and Google Schoolar were reviewed, through different databases. Markers cytokines; coronavirus; infection; coronavirus infections; severe acute respiratory syndrome and epidemiology were used as descriptors.Development: all pathological process begins with defects in the T-cells activity and increasing activity of the macrophages and activation of all the immune system because cytokines torments. This potentially deadly affection is presented in patients with COVID-19.Conclusions:infection for COVID-19 is accompanied by an inflammatory aggressive response with the liberation of a huge amount of proinflammatory cytokines. This is called cytokines torments. This can show up with a variety of symptoms that go from a mild flu to a lifethreading disease.Introducción: la COVID-19 es la enfermedad causada por un nuevo coronavirus SARS-CoV-2, que fue identificado y caracterizado en enero de 2020 en China. Esta enfermedad se acompaña de una respuesta inflamatoria agresiva con la liberación de una gran cantidad de citoquinas proinflamatorias, en un evento conocido como tormenta de citoquinas.Objetivo: describir la tormenta de citocinas secundaria a la COVID-19.Métodos: se realizó una revisión de 29 referencias bibliográficas, para ello se consultaron bases de datos como Elsevier, SciELO, ClinicalKey, Pubmed y Google Académico. Como descriptores se utilizaron: citocinas; coronavirus; infección; infecciones por coronavirus; síndrome respiratorio agudo grave y epidemiología.Desarrollo: todo el proceso patológico que comienza con defectos en la actividad citolítica de linfocitos, pasando por el aumento de la acción de los macrófagos y la de todo el sistema inmunitario, resulta en una tormenta de citoquinas. Esta afección potencialmente mortal es una de las principales causas de muerte en pacientes con COVID-19.Conclusiones: la infección por COVID-19 se acompaña de una respuesta inflamatoria agresiva con la liberación de una gran cantidad de citoquinas proinflamatorias, en un evento conocido como "tormenta de citoquinas". Esta puede presentarse con una variedad de síntomas que varían desde leves, parecidos a la gripe, hasta graves que ponen en peligro la vida
Un estudio observacional sobre infecciones bacterianas en pacientes con coronavirus hospitalizados en un centro de tercer nivel de atención
Introduction: The latest global pandemic is caused by SARS-CoV-2 coronavirus. 6% of patients are admitted to the hospital, with 20% of those admitted to the intensive care unit (ICU) due to acute respiratory distress syndrome (ARDS).Aim: to characterise the bacterial infections in patients with coronavirus at an ICU of North Indian Hospital.Method: After receiving ethical approval from the institutional review board, a retrospective study was done on coronavirus subjects admitted to GMC Srinagar's ICUs between January and June 2021. Data on demographics, comorbidities, and microbiology were obtained retrospectively.Results: Overall, “394 COVID-19 patients were admitted to ICU. Median age was 58 years (IQR 51–69) and sex ratio (M/F) was 3. At admission the median SAPS II was 33 (IQR 24–49). Among patients, 232 had at least one co-morbidity and 162 were overweight (body mass index (BMI) = 27.8 kg/m2). In all, 358 patients received antibiotics (244/358 introduced before ICU and 114/358 during ICU stay)”.Conclusion: The current study is the first of its kind in our hospital setting to describe the bacterial persistence in the lungs despite adequate therapy, as well as frequent bloodstream infections possibly associated with bacterial translocations originating from the digestive or oropharyngeal microbiota, in COVID-19 ICU patients.Introducción: La última pandemia mundial está provocada por el coronavirus SARS-CoV-2. El 6 % de los pacientes ingresan en el hospital, con un 20% de los ingresados en la unidad de cuidados intensivos (UCI) debido al síndrome de dificultad respiratoria aguda (SDRA).Objetivo: caracterizar las infecciones bacterianas en pacientes con coronavirus en una UCI del North Indian Hospital.Método: Después de recibir la aprobación ética de la junta de revisión institucional, se realizó un estudio retrospectivo en sujetos con coronavirus admitidos en las UCI de GMC Srinagar entre enero y junio de 2021. Los datos sobre demografía, comorbilidades y microbiología se obtuvieron retrospectivamente.Resultados: en general, “394 pacientes con COVID-19 fueron admitidos en la UCI. La mediana de edad fue de 58 años (RIC 51-69) y la proporción de sexos (M/F) de 3. Al ingreso, la mediana de SAPS II fue de 33 (RIC 24-49). Entre los pacientes, 232 tenían al menos una comorbilidad y 162 tenían sobrepeso (índice de masa corporal (IMC) = 27,8 kg/m2). En total, 358 pacientes recibieron antibióticos (244/358 introducidos antes de la UCI y 114/358 durante la estancia en la UCI)”.Conclusión: El presente estudio es el primero de su tipo en nuestro medio hospitalario en describir la persistencia bacteriana en los pulmones a pesar de una terapia adecuada, así como infecciones frecuentes del torrente sanguíneo posiblemente asociadas con translocaciones bacterianas originadas en la microbiota digestiva u orofaríngea, en COVID-19 pacientes de la UCI
Disfunción temporomandibular en pacientes angolanos con maloclusión
Introduction: dental occlusion was accepted as one of the main etiological factors of dysfunctional disorders. Understanding occlusion in its anatomical and functional context allows for more accurate diagnoses and directs therapeutic procedures for alterations caused by occlusal disharmony. Comorbidity between malocclusion and temporomandibular dysfunction in Angolan patients is not recorded in the reviewed literature.Objective: to identify the degree of temporomandibular dysfunction in Angolan patients with malocclusion.Methods: a cross-sectional descriptive observational study was carried out in 26 patients with malocclusion, treated at the orthodontic consultation of the Meditex-Alvalade Dental Clinic, in Luanda, Angola, between August and December 2021. The following variables were used: degree of temporomandibular dysfunction, age, sex and molar ratio. Maglione index was applied.Results: 73,07 % of the patients showed temporomandibular dysfunction; 50,00 % of the patients aged 19-35 years presented mild temporomandibular dysfunction as well as 41,17 % of the female patients; most of the patients presented class II molar relationship, of which 64,28 % showed mild dysfunction.Conclusions: Angolan patients with malocclusion were characterized by mild temporomandibular dysfunction, mainly in the ages between 19-35 years and female; the class II molar relationship was the most representative.Introducción: la oclusión dentaria se aceptó como uno de los principales factores etiológicos de los trastornos disfuncionales. La comprensión de oclusión en su contexto anatómico y funcional permite realizar diagnósticos más certeros y encaminar los procedimientos terapéuticos de alteraciones causadas por la falta de armonía oclusal. No se registra en la literatura consultada la comorbilidad entre maloclusión y disfunción temporomandibular en pacientes angolanos.Objetivo: identificar el grado de disfunción temporomandibular en pacientes angolanos con maloclusión.Métodos: se realizó un estudio observacional, descriptivo y transversal en 26 pacientes con maloclusión; atendidos en la consulta de ortodoncia de la Clínica Dentaria Meditex- Alvalade, en Luanda, Angola, entre agosto y diciembre de 2021. Se emplearon las siguientes variables: grado de disfunción temporomandibular, edad, sexo y relación molar. Se aplicó el índice de Maglione.Resultados: el 73,07 % de los pacientes mostraron disfunción temporomandibular; el 50,00 % de los pacientes con edades entre 19-35 años mostró disfunción temporomandibular leve al igual que el 41,17 % de las pacientes femeninas; la mayoría de los pacientes presentaron relación molar clase II, de los cuales el 64,28 % mostró disfunción leve.Conclusiones: los pacientes angolanos con maloclusión se caracterizaron por presentar disfunción temporomandibular leve, principalmente en las edades entre 19-35 años y del sexo femenino; la relación molar clase II fue la más representativa
Coledocolitiasis con diagnóstico inicial de colangiocarcinoma. A propósito de un caso
Introduction: choledocholithiasis or choledocholithiasis is defined as the presence of stones in the extrahepatic biliary tract. Its diagnosis sometimes becomes complex due to the similarity of its clinical picture with other biliary pathologies, especially cholangiocarcinoma, which may lead to diagnostic errors. Objective: to describe the characteristics of a patient with choledocholithiasis who presented an initial diagnosis of cholangiocarcinoma.Case presentation: 75-year-old female patient, with apparent health history who began with diffuse abdominal pain predominantly in the epigastrium, radiating to the right hypochondrium and back, accompanied by fever, weight loss, nausea, decay and slight jaundice in skin and mucous membranes, for which she was admitted to the surgery department where a computed axial tomography was performed and cholangiocarcinoma and recurrent acute cholangitis were diagnosed. He was referred to the Institute of Gastroenterology for endoscopic retrograde cholangiopancreatography which resulted in a diagnosis of choledochal lithiasis.Conclusions: the case in question served to demonstrate the need to deepen the knowledge of choledocholithiasis, as well as to highlight the clinical manifestations and the results of the complementary tests performed in this case as a learning tool for health personnel; these constitute tools to differentiate choledocholithiasis from other pathologies of the biliary tract that can help in decision making in other patients.Introducción: la coledocolitiasis o litiasis coledociana se define como la presencia de cálculos en las vías biliares extrahepáticas. Su diagnóstico en ocasiones se torna complejo debido a la similitud de su cuadro clínico con otras patologías biliares, en especial el colangiocarcinoma, lo cual puede llevar a errores diagnósticos.Presentación de caso: paciente femenina de 75 años, con antecedentes de salud aparente que comenzó con dolor abdominal difuso a predominio de epigastrio que se irradiaba hacia hipocondrio derecho y espalda, acompañado de fiebre, pérdida de peso, náuseas, decaimiento y ligera ictericia en piel y mucosas por lo cual se ingresó en servicio de cirugía donde se le realizó tomografía axial computarizada y se diagnosticó colangiocarcinoma y colangitis aguda recurrente. Se remitió al Instituto de Gastroenterología para realizar colangiopancreatografía retrógrada endoscópica la que arrojó un diagnóstico de litiasis coledociana.Conclusiones: el caso en cuestión sirvió para demostrar la necesidad de profundizar en los conocimientos sobre la colodocolitiasis, así como destacar las manifestaciones clínicas y los resultados de los exámenes complementarios realizados en este caso como un medio de aprendizaje para el personal de la salud; estas constituyen herramientas para diferenciar la coledocolitiasis de otras patologías de las vías biliares que pueden ayudar a la toma de decisiones en otros pacientes

Inmunopatología de la infección por virus SARS-CoV-2
Introduction: the daily increase of cases and deaths, the economic losses in the millions suffered by the affected nations and the consequent exhaustion of human resources involved in the reversal of this situation have made the COVID-19 pandemic an unprecedented international challenge. Objective: to describe the orchestrated immune response following SARS-CoV-2 virus infection.Methods: an updated bibliometric study was carried out on the type of articles stated in the objective, for which a total of 30 bibliographies were used. Documentary review and analysis-synthesis methods were used to prepare the final report. Resources available in the Infomed network were used to select the information, specifically: PubMed and SciELo, through the databases: Medline, Search Premier and Scopus. Development: the core elements in the immunopathology of COVID-19 involve innate immunity, with the sustained increase of proinflammatory interleukins associated with failures in the interferon system, which can trigger a potentially fatal cytokine storm. As for the elements linked to adaptive immunity, a marked lymphopenia is evident, which depending on the degree may indicate the severity of the disease.Conclusions: understanding the orchestrated immune response after SARS-CoV-2 infection as well as its temporal sequence allows choosing timely and effective therapies, specifically when selecting anti-inflammatory drugs and the moment of their application; since it is difficult to determine when they will be clearly beneficial, that they do not harm the response and that it is not too late, given the irreversibility of the process. Introdución: el incremento diario de casos y muertes, las pérdidas económicas millonarias sufridas por las naciones afectadas y la consecuente extenuación de los recursos humanos involucrados en la reversión de esta situación han hecho de la pandemia por COVID-19 un desafío intenacional sin precedentes.Objetivo: describir la respuesta inmune orquestada tras la infección por el virus SARS-CoV-2Métodos: se realizó un estudio bibliométrico actualizado sobre el tipo de artículos declarado en el objetivo, para lo cual se empleó un total de 30 bibliografías. Se utilizaron métodos de revisión documental y de análisis-síntesis para la confección del informe final. Se emplearon recursos disponibles en la red Infomed para la selección de la información, específicamente: PubMed y SciELo, a través de las bases de datos: Medline, Search Premier y Scopus.Desarrollo: los elementos medulares en la inmunopatología de la COVID-19 comprenden a la inmunidad innata, con el incremento sostenido de interleucinas proinflamatorias asociado a fallos en el sistema de los interferones, lo que puede desencadenar una tormenta de citocinas potencialmente fatal. En cuanto a los elementos vinculados a la inmunidad adaptativa se evidencia una marcada linfopenia que según el grado puede indicar la severidad de la enfermedad.Conclusiones: comprender la respuesta inmune orquestada tras la infección por SARS-CoV-2 así como su secuencia temporal permite elegir terapias oportunas y eficaces, específicamente al seleccionar los antiinflamatorios y el momento de su aplicación; ya que resulta difícil determinar cuándo serán netamente beneficiosos, que no perjudiquen la respuesta y que no sea demasiado tarde, dada la irreversibilidad del proceso

Cambios oclusales y temporomandibulares con la técnica ortodóncica de arco recto. Clínica Meditex, Angola
Introduction: The Straight Arch technique is characterized by a straight ttarch placed that allows the teeth to be inclined in all three dimensions of space.Objective: To evaluate the occlusal and temporomandibular changes with the Straight Arch orthodontic technique in patients with malocclusion.Methods: A quasi-experimental study, modality before and after, without a control group, was carried out in 26 patients with malocclusion, treated at the Meditex Clinic, in Luanda, Angola, between August 2021 and August 2022. The treatment was performed using the Arch orthodontic technique Right. The variables studied were: occlusion relationship, temporomandibular anatomical relationship and degree of dysfunction of the temporomandibular joint.Results: The occlusion relationship showed significant changes with the use of the Straight Wire technique in the molar relationship (2,110 ± 0,6637 mm; p= 0,000) and overjet (2,117 ± 1, 1,4644 mm; p= 0,000); the patients were characterized by presenting temporomandibular anatomical alteration before treatment, mainly in joint centrality (35,951), joint mechanical angle (26,412) and in the condyle-Frankfort relationship (-2,957), which improved with treatment (p= 0,000); patients presented temporomandibular dysfunction grade I and grade II in 46,15 % and 19,23 % respectively, after treatment it turned out that 69,23 % of patients remained with out joint symptoms.Conclusions: The Straight Arch technique constituted an orthodontic therapy that, in addition to improving the occlusion relationship, favors the joint anatomical relationship and constitutes a the rapeuticoption in temporomandibular dysfunction in patients with malocclusions.Introducción: la técnica de arco recto se caracteriza por un arco recto colocado que permite a los dientes inclinarse en las tres dimensiones del espacio.Objetivo: evaluar los cambios oclusales y temporomandibulares con la técnica ortodóncica de arco recto en pacientes con maloclusión.Métodos: se realizó un estudio cuasi-experimental, modalidad antes y después, sin grupo control en 26 pacientes con maloclusión, atendidos en la Clínica Meditex, en Luanda, Angola, entre agosto de 2021 y agosto de 2022. El tratamiento fue realizado mediante la técnica ortodóncica de arco recto. Las variables estudiadas fueron: relación de oclusión, relación anatómica temporomandibular y grado de disfunción de la articulación temporomandibular.Resultados: la relación de oclusión mostró cambios significativos con el uso de la técnica de arco recto en la relación molar (2,110 ± 0,6637 mm; p= 0,000) y el resalte (2,117 ± 1, 1,4644 mm; p= 0,000); los pacientes se caracterizaron por presentar alteración anatómica temporomandibular antes del tratamiento, principalmente en la centralidad articular (35,951), ángulo mecánico articular (26,412) y en la relación cóndilo-Frankfort (-2,957), que mejoraron con el tratamiento (p= 0,000); los pacientes presentaron disfunción temporomandibular grado I y grado II en el 46,15 % y 19,23 % respectivamente, luego del tratamiento resultó que el 69,23 % de los pacientes quedaron sin síntomas articulares.Conclusiones: la técnica de arco recto constituyó una terapéutica ortodóncica que además de mejorar la relación de oclusión, favorece la relación anatómica articular y constituye una opción terapéutica en la disfunción temporomandibular en pacientes con maloclusiones
Células madre hematopoyéticas. Implicaciones y retos de su trasplante en la medicina actual
Introduction: hematopoietic stem cells have an incredible capacity to multiply and give rise to all blood cells. This has made them the focus of worldwide research attention, especially transplantation research, with promising results. Objective: to describe the main mechanisms, implications and challenges of hematopoietic stem cell transplantation in medicine today. Methods: an exhaustive search was performed in the SciELO, SCOPUS and PubMed databases between June and August 2021. Seventy-eight articles were consulted, of which 29 were selected. Development: the most used and at the same time ancient method is the direct extraction from the donor to be implanted later, there are other alternatives that include genetic modification. The immunosuppression involved in classic transplantation represents the main barrier to its performance. It has been used to deal with hematologic and non-hematologic diseases such as COVID-19 based on specific cellular targets. Conclusions: hematopoietic stem cell transplantation is revolutionizing the way in which current medicine deals with different diseases, however, it has become a challenge to find safe and effective procedures for its performance; this does not prevent classical transplantation methods from being used with satisfactory results.Introducción: las células madre hematopoyéticas tienen una increíble capacidad de multiplicación y de dar origen a todas las células de la sangre. Esto las ha hecho centro de atención en investigaciones por todo el mundo, sobre todo estudios de su trasplante, lo que arroja resultados prometedores. Objetivo: describir los principales mecanismos, implicaciones y retos del trasplante de células madres hematopoyéticas en la medicina actual. Métodos: se realizó una búsqueda exhaustiva en las bases de datos SciELO, SCOPUS y PubMed entre los meses de junio y agosto del 2021. Se consultaron 78 artículos, de los cuales fueron escogidos 29. Desarrollo: el método más usado y a la vez antiguo, es la extracción directa del donador para ser implantadas seguidamente, hay otras alternativas que incluyen modificación genética. La inmunosupresión que conlleva realizar el trasplante clásico representa la barrera principal en la realización de este. Se ha usado para enfrentar enfermedades hematológicas y no hematológicas como la COVID-19 basándose en objetivos celulares específicos. Conclusiones: el trasplante de células madre hematopoyéticas está revolucionando la manera en la que la medicina actual se enfrenta a diferentes enfermedades, sin embargo, se ha vuelto un reto encontrar procedimientos seguros y eficaces para su realización; esto no impide que los métodos clásicos de trasplante sean utilizados con resultados satisfactorios
Tratamiento con bloques gemelos a pacientes con síndrome clase III de Moyers
Introduction: Moyers class III syndrome is characterized by discrepancy in arch length, skeletal problems, muscle dysfunction, dental problems, and the patient's facial profile. In the national and international literature there is no evidence of the efficacy of treatment with twin blocks of Moyers class III syndrome.Objective: to evaluate treatment with twin blocks in patients with Moyers class III syndrome.Method: a quasi-experimental intervention study was carried out before and after without a control group, at the Manuel Cedeño University Clinic of Stomatological Specialties, in the period from January 2018 to March 2021. The sample was 93 patients. The variables studied were the occlusion relationship, the skull-mandibular skeletal relationship and the temporomandibular joint dysfunction. Class III twin blocks were placed in all patients.Results: with the use of twin blocks, significant changes were obtained in the molar ratio (-7,030 ± 0,7287 mm) and the overhang (2,213 ± 1,6074 mm); the skull-mandibular skeletal relationship was improved (2,659° ± 1,8696°); the function of the temporomandibular joint was found to be without symptoms and mild in 64,86 % and 57,14 % respectively, after treatment.Conclusions: with the use of twin blocks, the relationship between dental occlusion and cranio-mandibular skeletal class III is corrected, in addition to being an orthopedic-functional option that improves temporomandibular joint dysfunction in patients with Moyers class III syndrome.Introducción: el síndrome clase III de Moyers se caracteriza por la discrepancia en la longitud del arco, problemas esqueléticos, disfunción muscular, problemas dentales y del perfil facial del paciente. En la literatura nacional no existe evidencia de la eficacia del tratamiento con bloques gemelos del síndrome clase III de Moyers.Objetivo: evaluar el tratamiento con bloques gemelos a pacientes con síndrome clase III de Moyers.Método: se realizó un estudio de intervención de tipo cuasi-experimental, en su modalidad antes y después sin grupo control, en la Clínica Universitaria de Especialidades Estomatológicas Manuel Cedeño, en el periodo comprendido entre enero de 2018 y marzo de 2021. La muestra fue de 93 pacientes. Las variables estudiadas fueron la relación de oclusión, la relación esquelética cráneo-mandibular y la disfunción de la articulación temporomandibular. A todos los pacientes se les colocó bloques gemelos clase III.Resultados: con el uso de los bloques gemelos se obtuvieron cambios significativos en la relación molar (-7,030 ± 0,7287 mm) y el resalte (2,213 ± 1,6074 mm); se mejoró la relación esquelética cráneo-mandibular (2,659° ± 1,8696°); la función de la articulación temporomandibular resultó ser sin síntomas y leve en el 64,86 % y 57,14 % respectivamente, luego del tratamiento.Conclusiones: con el uso de bloques gemelos se corrige la relación de oclusión dentaria y la clase III esquelética cráneo-mandibular, además de ser una opción ortopédico-funcional que mejora la disfunción de la articulación temporomandibular de los pacientes con síndrome clase III de Moyers
