Incidentally Diagnosed Bladder Xanthoma in a 38-Year-Old Man
Xanthoma is a benign lesion characterized by the localized collection of lipid-containing histiocytes. It is mostly observed in patients with hyperlipidemia but can also be found in patients with normal lipid profiles. It is very rarely detected in the bladder. Xanthoma may be associated with bladder tumor or it can be seen isolated. Most of the cases in the literature are elderly patients. Isolated bladder xanthoma is mostly detected incidentally in cystoscopy performed for complaints like hematuria and lower urinary tract symptoms. A large lesion can be detected in radiological imaging and mistaken for a bladder tumor. We present a case of urinary bladder xanthoma incidentally detected in a 38- year-old man

Therapeutic Drug Monitoring in Pediatric Patients Treated with Anti-Tuberculosis Medications by High Performance Liquid Chromatography
© 2022 Marmara University Press.The aim of this study is to perform therapeutic drug monitoring for isoniazid (INH), rifampicin (RIF), and pyrazinamide (PZA) in pediatric tuberculosis patients. The study was carried out in 3 different training-research hospitals in Istanbul, Türkiye between 2011 and 2012. The pediatric patients (aged ≤14 years) who initiated the standard primary anti-tuberculosis therapy were included in this study. The serum samples were collected 3 hours after the first medication doses were given on the 5th day of treatment. Chromatographic experiments were performed on an Agilent 1100 High-Performance Liquid Chromatography (HPLC) system, and the separation was carried out on a Nova-Pak C18 (3.9x150 mm, 5 μm, Merck) analytical column. In this HPLC method, the gradient elusion delivered 3% to 40% (v/v) acetonitrile in phosphate buffer was used, and diode array detector. Twenty-three children (60.9% male) patients were included with a mean age of 111.70 ± 59.94 months. Plasma levels were measured sub-therapeutically for INH in 14, RIF in 10, and PZA in 5 patients, according to the normal range of adult patients. Maximum plasma concentrations after three hours were found between 0.53-14.02 mg/L for INH, 11.17-60.39 mg/L for PZA, 2.15-16.75 mg/L for RIF. In conclusion, this method has been successfully applied to simultaneously determine RIF, INH, and PZA plasma levels in pediatric tuberculosis patients. RIF and INH plasma levels were found to be lower in pediatric patients with tuberculosis compared to target range of adult patients
Çocuklarda Bitki ve Bitkisel Ürünlerin Kullanımı ve Ebeveyn Bilgilerinin Değerlendirilmesi: Kesitsel Araştırma
Amaç:Bitkisel ürünler çocuklarda hastalıklardan korunmak, hastalıkları tedavi etmek veya tedaviyi desteklemek amacıyla aileler tarafından kullanılmaktadır. Bu araştırmada, çocuklarda bitkisel ürünlerin kullanım gerekçeleri, kullanım sıklığı ve ailelerin bu ürünler hakkındaki bilinç düzeyini tespit etmek amaçlanmıştır.Gereç ve Yöntemler:Bu araştırma kesitsel bir çalışmadır. Araştırma verilerinin toplanmasında konu ile ilgili literatür taranarak araştırmacılar tarafından oluşturulan ailelerin demografik özellikleri ve araştırma konusu ile ilgili sorulardan oluşan anket formu kullanılmıştır. Türkiye\"nin farklı illerindeki ebeveynlere anketin linki gönderilerek ebeveynlerin araştırmaya katılımı sağlanmıştır. Araştırmaya toplam 620 ebeveyn katılmıştır. Araştırmadan elde edilen verilerin analizi için frekans, yüzde alma teknikleri ve ki-kare testi uygulanmıştır.Bulgular:Katılımcıların %80,3\"ü çocuğu için bitkisel ürün kullanmıştır. Katılımcıların 335\"i hastalıklardan korumak, 156\"sı tedavi, 202\"si tıbbi tedaviye destek amacıyla kullandığını belirtmiştir. Katılımcıların %48,9\"u bitkisel ürünleri eczaneden temin ettiğini belirtmiş, %38,7\"si bitkisel ürünü doktor tavsiyesiyle kullandığını belirtmiştir. Ebeveynlerin çocukları için en çok grip ve soğuk algınlığı, öksürük durumlarında ve bağışıklığı güçlendirmek amacıyla bitkisel ürün kullandığı tespit edilmiştir. Ebeveynlerin çocukları için en çok ıhlamur, nane-limon, zencefil, cadı fındığı, zerdeçal kullandığı saptanmıştır.Sonuç:Ebeveynler çocuklarını hastalıklardan korumak veya hastalıkları tedavi etmek gibi çeşitli amaçlarla ve farklı sıklıklarla bitki ve bitkisel ürünleri kullanmaktadır. Sonuçlarımıza göre ebeveynlerin yarısından fazlası doktor, eczacı gibi sağlık profesyonellerinin tavsiyesi ile bitkisel ürünleri kullanmakta, yaklaşık yarısı da bitkisel ürünleri eczanelerden temin etmektedir. Bitkisel ürün kullanan ebeveynlerin %79,37\"si fayda görmüş, yalnızca %0,2\"si zarar gördüğünü bildirmiştir. Çocuk sağlığının korunması ve geliştirilmesi için bitki ve bitkisel ürünlerin akılcı kullanımına yönelik yeterli verinin sağlanması, ailelerin ve sağlık personelinin bu konudaki duyarlılığının artırılması önemlidir
Is coracoclavicular ossification a complication or a good prognostic factor after surgical treatment of acromioclavicular joint injury?
© 2022Background: The purpose of this study was to investigate whether heterotopic ossification (HO) in the coracoclavicular (CC) space after surgical treatment of acromioclavicular joint (ACJ) injury is a complication or a sign of good prognosis. Methods: Fifty-nine consecutive patients who underwent CC reconstruction with or without augmentation of the ACJ for acute ACJ injuries were analyzed. Postoperative American Shoulder and Elbow Surgeons (ASES) score, Constant score (CS), subjective shoulder value (SSV), and visual analog scale (VAS) results were evaluated. For radiological evaluation, HO was evaluated, and CC distances were measured. Results: Fifty-one patients (11 women and 40 men; mean age, 36 years [range, 17-68 years]) were evaluated after a mean follow-up of 3 years (range, 2-8 years). The mean ASES score at the follow-up was 82.73 (range, 51.6-100), mean CS was 85 (range, 50-100), mean SSV was 80 (range, 40-100), and mean VAS was 1.9 (range, 0-5). It was observed that the clinical outcomes (ASES, CS, SSV, VAS) of patients who developed ossification in the CC space were better than those who did not although it was not statistically significant. No statistically significant differences were found in the clinical outcomes (ASES, CS, SSV, VAS) between patients who underwent CC reconstruction without augmentation of the ACJ and those who were combined (P > .05). Conclusion: HO in the CC space is a common finding following AC joint fixation injury. We suggest that HO is not a complication and might possibly have positive effects on clinical outcomes
A case report of hyperinsulinemic hypoglycemia caused by a heterozygous novel mutation in the insulin receptor gene
Introduction: Mutations in the insulin receptor (INSR) gene usually progress with insulin resistance and hyperglycemia. However, rare heterozygous INSR gene mutations that cause hyperinsulinemic hypoglycemia (HH) have also been described. A case with hyperinsulinemic hypoglycemia due to a new variant mutation in the INSR gene, is presented. Case: A 13-month-old male patient was referred to us with complaints of contractions in his entire body and low blood sugar in his history. He was born 3900 grams with spontaneous vaginal delivery as term birth. He had a similar seizure history 1 month ago. In family history; There was no consanguinity between father and mother. His father had a growth hormone treatment history and he had iris coloboma. Anthropometric measurements; Height: 78 cm (SDS: 0.24), weight: 12.1 kg (SDS: 1.08), head circumference: 45 cm (SDS: -0.37) was normal. In the physical examination of the patient, anterior fontanel was 0.5*0,5 cm open, elongated penis length was 3,5 cm, testicles were bilaterally palpable, other system examinations were normal. Laboratory values measured in hypo glycemic state (glucose: 35 mg/dL) are given in the table. In addi tion, the patient’s metabolic tests (tandem, blood amino acid, urine organic acid, ammonia) were normal.Hydrocortisone and diazoxide treatment were started simulta neously because the cortisol levels measured at the time of hypo glycemic attack were 3.9 ug/dL (low) and insulin was 7.6 uU/mL. Abdominal ultrasound was requested for the examination of pan creatic anomaly and it was found to be normal. Genetic result showed in INSR NM_000208.2 Exon 20 c.3634G>A p mutation. Anew mutation was detected in Val1212lle. During the follow-up the basal cortisol value measured was 16.2 ug/dL. After this result and genetical diagnosis, hydrocortisone treatment was discontin ued and diazoxide treatment was continued. The patient’s blood sugar is followed closely, and the follow-up continues in terms of conversion to diabetes. Conclusion: In conclusion, heterozygous INSR gene mutation causing hyper insulinemic hypoglycemia was detected in our case. This case is presented because INSR mutations are rare in the eti ology of hyperinsulinism in hypoglycemic patients and also the detected mutation is new
