    The liver-kidney axis: Is serum leptin a potential link in non-alcoholic fatty liver disease-associated chronic kidney disease?

    © 2023 Pan-Arab Association of GastroenterologyBackground and study aims: Non-alcoholic fatty liver disease (NAFLD) is an independent risk factor for chronic kidney disease (CKD). Previous studies argued that leptin levels increase significantly with the progression of CKD. But the association between leptin and CKD has not been investigated in patients with NAFLD. Therefore, we conducted this study to establish whether increased leptin level is associated with CKD in NAFLD patients. Patients and methods: In our prospective study with a follow up period of six months thirty-five teetotaller biopsy-proven NAFLD patients were divided as groups with mild, versus advanced, fibrosis. Liver fibrosis was also assessed with Fibroscan. Serum leptin levels were measured by radioimmunoassay. For insulin resistance we used the homeostasis model assessment method (HOMA-IR). For the kidney function, we used the abbreviated formula Modification of Diet in Renal Disease (MDRD) formula, which estimates GFR. For statistical analysis, Student\"s-t test, Mann-Whitney test, linear regression-binary logistic regression analyses and the ROC curve analysis were used. Results: Advanced fibrosis and increased HOMA-IR were risk factors for decreased eGFR. Leptin correlated inversely with advanced fibrosis (p: 0.03) and low leptin was a risk factor for CKD (p: 0.02). In ROC curve analysis, advanced fibrosis and low leptin were risk factors for decreased eGFR (p: 0.007 and 0.004, respectively). Low leptin level was dependently associated with decreased eGFR. Conclusion: Advanced fibrosis in NAFLD patients is a risk factor for CKD. Leptin correlated inversely with advanced fibrosis. Unlike the previous studies, which were not performed in NAFLD patients, we found decreased leptin in NAFLD patients with decreased eGFR. Low leptin level was found to be a dependent predictor for differentiating NAFLD patients with high risk for CKD

    A Catalyst for Social Economy: Society 5.0

    As our planet moves towards the middle of the 21st century, it faces a dizzying digital transformation process. This transformation brings along with it unique and innovative solutions to numerous social, organizational, and managerial issues. The most tangible of these solutions so far is the vision of Society 5.0 announced by the Japanese government in 2016. Within this scope, the purpose of this chapter is to develop conceptual insight into the reasons for the emergence, components, and goals of Society 5.0. Up-to-date research and current reports on the topic have been examined in this context, and a conceptual framework is formed. The findings show that many digital tools of Society 5.0 are already used in economies, but policymakers discuss these tools’ human-centered reuse. As a result, a positive futuristic perspective has been drawn that humanity will continue its life on a more habitable planet for decades to come and unite digital and physical space.     Acute interhemispheric subdural hematoma with neuroimagings of two trauma cases in the emergency department

    Backround: Subdural hematomas are clinically important in that they occur commonly in elderly trauma patients and cause a high mortality. Since interhemispheric subdural hematoma is rare among cases of subdural hematoma and is a diagnosis that can be missed in the emergency department, it is aimed to discuss this condition with two case reports and the relevant neuroimages which were overlooked initially. Two case reports: The first report of neuroimages of two geriatric patients with head trauma was reported as normal. The images were re-evaluated due to persistent headache during the emergency observation. An overlooked interhemispheric subdural hematoma was diagnosed. Why should an emergency physician be aware of this?: We aimed to contribute to the literature with these two cases, because an acute interhemispheric subdural hematoma may be overlooked on CT images, because it is in proximity to the falx and is usually a small lesion

    Cannabidiol as a potential novel treatment for endometriosis by its anti-inflammatory, antioxidative and antiangiogenic effects in an experimental rat model

    Research question: Can cannabidiol (CBD) be used in the treatment of endometriosis for its anti-inflammatory, antioxidative and antiangiogenic effects? Design: Endometrial implants were surgically induced in 36 female Wistar albino rats. After confirmation of endometriotic foci, the rats were randomized into four groups. In the leuprolide acetate group, rats were given a single 1 mg/kg s.c. leuprolide acetate injection. The other groups were 5 mg/kg CBD (CBD5), saline solution and 20 mg/kg CBD (CBD20); daily i.p. injections were administered for 7 days. After 21 days, the rats were euthanised, and total antioxidant status (TAS), total oxidant status (TOS), oxidative stress index (OSI), interleukin-6 (IL-6) and tumour necrosis factor-alpha (TNF-α) measurements in blood and peritoneal fluid samples, and immunohistochemical staining for TNF-α, IL-6 and vascular endothelial growth factor (VEGF) of endometriotic tissues were evaluated. Results: Significant reductions in the endometriotic implant surface area (P = 0.0213), serum TOS (P = 0.0491), OSI (P = 0.0056), IL-6 (P = 0.0236), TNF-α (P = 0.0083) and peritoneal fluid OSI (P = 0.0401), IL-6 (P = 0.0205) and TNF-α (P = 0.0045) concentrations were observed in the CBD5 group when compared with the saline solution group. Compared with the saline solution group, increased TAS concentrations in serum (P = 0.0012) and peritoneal fluid (P = 0.0145) were found in the CBD5 group. The CBD5 and leuprolide acetate groups were similar regarding inflammatory and oxidative stress parameters of serum and peritoneal fluid samples. The CBD5 group showed significantly lower mean intensity in both surface epithelium and stromal cells for VEGF (both P = 0.002) and only in surface epithelium cells for IL-6 (P = 0.0108), when compared with the leuprolide acetate group. Conclusion: Due to its anti-inflammatory, antioxidative and antiangiogenic effects, CBD might be a therapeutic agent candidate for endometriosis

    Spontaneous Multifocal Intracranial Hemorrhage In A Young Patient

    Introduction: A stroke is an acute neurologic condition resulting from a disruption in cerebral perfusion, either due to ischemia (ischemic strokes) or hemorrhage (hemorrhagic strokes). Hemorrhagic strokes are further classified as intracerebral or subarachnoid. Clinically, strokes are characterized by the acute onset of focal neurologic deficits, including hemiparesis, paresthesias, and hemianopsia. Systemic hypertension and other cardiovascular diseases are common risk factors for both ischemic and hemorrhagic strokes. For both ischemic and hemorrhagic strokes, age is the most important nonmodifiable risk factor and arterial hypertension is the most important modifiable risk factor. Case: A 25-year-old man presented to our emergency department with a headache, Broca aphasia, and difficulty in vision that started two days ago. The patient stated to have blurry eyes in the morning for a month. However, aphasia had just started before he came to the Emergency room. The patient has had no known past medical history and no medication use. The general situation was bad. Glasgow was 10. He was conscious but was disoriented and non- cooperated, with no verbal response. Blood pressure was 149/82, Pulse was 113, spO2 was 95%, and fever was 36 °C. Ophthalmologic examination showed that visual acuity of the right eye was 2 MPS, 0.1 MPS of the left eye. Intraocular pressures were normal bilaterally. DIIR +/+, no RAPD. Movements of globes were normal and without pain. Bilateral corneas were lucent, and the anterior chambers were quiet. Bilateral hemorrhagic areas were seen in dilated fundus examination
