    To Compare Blood Glucose Estimation Using the GOD-PAP Technique with the Hexokinase Method

    Glucose is the major source of energy for all the processes working within the human body and hence it's the major carbohydrate required. During starvation or fasting, its concentration in blood is maintained by glycogenolysis and gluconeogenesis otherwise, a routine diet is sufficient for this task. Routine methods like glucose-oxidase coupled dehydrogenase are most commonly used on semiautomatic instruments with mono reagent. Hexokinase is the definite method to measure glucose concentration during different fed and fasting states but it requires an automated instrument and specific reagent which comparatively affects its cost

    A Rare Case of Plasma Cell Myeloma, Myelodysplastic Neoplasm with Low Blast and SF3B1 Mutation and Dyserythropoiesis with Ring Sideroblasts

    Myelodysplastic syndromes (MDS) are heterogeneous hematological neoplasms which lead to dysplasia, cytopenia and hematopoiesis. They have a risk of transforming into Acute Myeloid Leukemia (AML) in some cases. Genetic markers that are determined by cytogenetics, SNP-A karyotyping and molecular mutations have evolved to define conditions of pathogenesis and risk evaluation for the development of AML. Among the concerned are the mutations of the spliceosome (SF3B1, SRSF2, ZRSR2 and U2AF1) epigenetic mutations (DNMT3A, EZH2) and also mutations in transcription factors (RUNX1) and tyrosine kinases (N-RAS, K-RAS). Here, we report a case of an 80-year-old female patient presented for the evaluation of myeloma. A morphology report shows changes that suggest multiple myeloma. Lab data shows anemia, elevated creatinine and calcium; CBC revealed mild neutrophilic leukocytosis and microcytic anemia. Bone marrow demonstrated approximately 50-60% involvement by a plasma cell infiltrate; there was mild dysgranulopoiesis and dyserythropoiesis which are suggestive of plasma cell myeloma. Frequent mutations in MDS and overlap syndromes were discovered due to advances in genomic studies. The differential diagnosis of MDS is challenging because they overlap the disorders sharing features of other illnesses. It is expected that more research on MDS and overlapping disorders will highlight the roles of mutations as “therapeutic targets” and “prognostic indicators”

    Many Faces of Dengue Cholecystitis: A Pictorial Review

    ABSTRACT This article aims to review cholecystitis patterns in known dengue patients as well as the variation in wall thickening involving the gall bladder, which will ultimately aid in imaging-based diagnosis. Awareness of atypical dengue manifestations aids in the early detection of dengue and the prevention of sequelae. The data acquisition of this retrospective study is performed from January 2022 to June 2023 in the Department of Radiology in Shifa International Hospital. Patients with positive dengue serology were identified retrospectively through the RIS database in years, 2022 and 2023. Clinicolab departments identified these patients with Dengue Hemorrhagic Fever, and their ultrasound scans were done and analyzed using several devices, including the Xario 100 and 200. The literature study was also conducted to discover various gall bladder wall thickening patterns associated with this condition. Different gall bladder wall thickening patterns including uniform, striated, asymmetric, tram track, and honeycombing were included. DHF has a wide variety of sonological manifestations. Recognizing diverse gall bladder wall thickening patterns on sonological imaging is crucial for a correct diagnosis and aids in therapy planning. Conservative treatment with continuous observation is advised to avoid morbidity and mortality in dengue fever patients. The crux is that patterns of gall bladder wall thickening may be used to forecast the prognosis of the disease and, when paired with the kind of pattern in serial ultrasonography, can be used to identify the severity of the condition

    Iatrogenic AV Fistula of Breast Following Core Biopsy

    A young lady attended a one-stop breast clinic for assessment of a left breast lump on the background of cosmetic implants. A triple assessment of the lump found a benign cyst. At the time of the biopsy, she had excessive bleeding and developed a hematoma. She was represented subsequently after hearing an unusual noise from her left breast. A palpable pulsatile mass was found on examination. Imaging the new lesion was diagnostic of an arteriovenous fistula (AVF). On review of the literature, six case reports of iatrogenic AVF in the breast were found; however, this case is unique as no other cases occurred in patients with breast implants. The AVF was treated with a surgical tie-off procedure whilst her breast implants remained in situ. She remains well and has been discharged from Breast Surgery care. AVF remains a rare and therefore often unexpected complication of biopsy. It is important to be aware of this, especially when a biopsy is performed on the lateral side of the breast and the patient develops a hematoma

    Cholangiocarcinoma Complicated by Cholangitic Abcsess: A Diagnostic Challenge

    Cholangiocarcinoma (CCA) is the second most frequent type of liver cancer, after hepatocellular carcinoma. The presence of luminal cholangiocarcinoma with cholangitic abscesses complicates diagnosis and management further. Cholangiocarcinoma is one of the commonest liver tumours, and its complications do occur early in the disease course as it blocks the bile ducts. Cholangitic abscess is not one of the common complications, and patients present late because distinguishing it from other disorders is quite difficult. Here, we discuss the case that encountered our hospital setting, discussing clinical signs and symptoms of cholangiocarcinoma with cholangitic abscess, emphasizing the importance of imaging findings from Computed Tomography (CT) and ERCP (endoscopic retrograde cholangiopancreatography) for accurate diagnosis. As CT is readily available, it is the preferred modality because it can assess the original tumor as well as its complications, just like a cholangitic abscess, as in this case
