    Renal Cysts: An Issue of Concern

    Simple renal cysts are frequent benign conditions that rarely evolve to malignancy. Complications include rupture, hemorrhage, infection, and local compression effects. Benign cysts are most often asymptomatic and incidentally detected by imaging tests, which reveal that these changes can increase in size doubling their volume in a decade. The Bosniak classification is widely utilized as a useful guide for diagnosis and control, and the uncomplicated cysts usually do not need invasive procedures or interventions. An elderly female with asymptomatic simple cysts in the left kidney and antecedent of partial mastectomy and total thyroidectomy due to malignant tumors is here reported.     Effect of Prone Positioning in Patients with Moderate COVID-19 Pneumonia Admitted to Ward at a Tertiary Care Hospital

    COVID -19 pneumonia can progress to severe disease in 5% of patients requiring intensive care management, which can put an excessive burden on health care systems. Prone positioning has been shown to improve oxygenation and decrease lung injury in patients with COVID-19 pneumonia and can be used as an adjunctive treatment to delay intubation. The objective of this study is to assess the effect of prone positioning in patients admitted to the COVID ward, with Moderate COVID-19 Pneumonia. This experimental study was conducted at Liaquat National Hospital. A quasi-experimental study design was applied. Patients with hypoxemia SpO2 < 94% were assisted to prone and semi-prone for up to 2 hours at a time for multiple sessions. Parameters like SaO2, PaO2/FiO2, hours of proning and changes and X-rays were recorded daily and pre and post-intervention values were compared. Paired t-test and Wilcoxon sign test were used to compare continuous parameters. A two-tailed p-value less than 5% of the level of significance was defined as statistically significant. 20 patients fulfilling the inclusion criteria were enrolled. Median hours of prone positioning were 48.5. The median hospital stay was 7.5 days. At baseline, mean PaO2/FiO2 ratio was 342±91.87 and at the time of discharge, it was 412.30±105.97 which is a statistically significant improvement from baseline (p=0.040). 50% of patients showed improvement in X-rays. One patient was intubated and all the patients were discharged. The sample collected in the current showed that prone positioning is a safe and feasible approach to improve oxygenation in moderate-severe COVID-19 pneumonias. However, studies with a larger sample size are recommended to further verify the findings of this study

    Case Report of Herpes Zoster in the 17-year-old Boy

    Herpes zoster has been caused because of the reactivation of latent Varicella-Zoster virus. It is a proportionally uncommon condition in the young population, as compared with adults. After taking consent from the legal guardian of the patient, we report a 17-year-old boy with a history of varicella, who came to us with the complaint of very severe painful vesicular lesions on the right side T1 and T2 dermatomal distribution of the chest, arm, and dorsal side of the trunk. Ultimately, the patient was diagnosed with herpes zoster. We focus to highlight that herpes zoster could occur in the healthy and immunocompetent young population and this may be due to the previous subclinical infection or vaccine strain reactivation

    Merkel cell carcinoma is a rare aggressive non-melanomatous cutaneous malignancy with rising incidence. It has two distinct etiologies Merkel cell polyomavirus associated and non-associated, usually presents as painless, indurated, solitary violaceous dermal nodule on sun-exposed sites, frequently in the head and region followed by extremities. The workup includes dermoscopy, histopathology with immunohistochemical staining and imaging. Treatment modalities for Merkel cell carcinoma are surgery, radiation therapy, chemotherapy etc. however, role of immunotherapy and targeted agents is emerging

    Monkeypox: A Global Challenge

    Monkeypox has emerged as the most significant human pathogen in recent times and is a rapidly growing threat to public health globally. Monkeypox virus is an orthopoxvirus, that belongs to the Poxviridae family. The smallpox virus also belongs to this family. Monkeypox virus, endemic to Central and West Africa can infect various animal species but can also transmit to humans. Monkeypox viruses circulate among wild animals and usually spread to people when they eat or have other close contacts with infected animals. Getting bitten or scratched by the infected animal, direct contact with its bodily fluids, blood, blisters or scabs; indirect contact with lesion material, e.g. contaminated bedding, linens and even eating the undercooked animal infected with monkeypox, could result in transmission. Among humans, Monkeypox is contracted through close physical contact, and contact with contaminated materials. Monkeypox presents with fever, headaches, myalgia, and enlarged lymph nodes. This is followed by a rash that starts from mouth to face and spreads to the trunk and arms. The illness is usually mild and patients fully recover within four weeks. Nucleic acid testing (NAT) is the primary diagnostic tool for detection. Polymerase chain reaction (PCR) is the preferred method for NAT. Treatment is mainly supportive and is directed to alleviate symptoms, prevent long-term sequelae and mitigate disease spread. Patients require isolation and symptomatic care. The smallpox vaccine, antiviral agents and vaccinia immunoglobulin (VIG) have been used in earlier outbreaks but sufficient evidence to recommend their use is still lacking
