    Case Report of Treatment Resistant Pyoderma Gangrenosum

    This is a case of a 38-year-old married female diagnosed with Pyoderma Gangrenosum (PG) for 4 years. This patient was very anxious and frustrated. The patient was using all the standard treatments for PG, oral steroids 40mg/day, colchicine 2mg/day, topically tacrolimus 0.1%, topically metronidazole, oral 3rd generation of cephalosporin antibiotics 1gm/day, and oral dapsone 100mg/day but lesion was getting worse. The patient family and her husband were very supportive and all the negative options of physical and mental abuse, and not taking proper medicine have been completely ruled out by taking a detailed history from the patient alone. The patient is willing to get the cure and take and apply for all the medicines properly. All the other diseases have been ruled out that can be associated with PG such as inflammatory bowel diseases, trauma, vasculitis, arthritis, etc. The patient has been to many physicians but has not been able to get better and the lesion is getting worse day by day and because of this patient started the symptoms of depression and start losing weight. PG is a very worrisome disease and usually, PG cure rate is unpredictable. Patients having PG have emotional problems too because of lesions. The patient usually isolates herself from society and lost all her confidence

    A Patient of Fish Odour Syndrome with recurrent Tinea Corporis

    This was a case of the 43-year-old male complaining of severe well-demarcated irregular-shaped, centrally non-cleared, extensive, non-scaly plaques with raised borders. The patient was having severe itching all over the body. The interesting fact about this case is that this patient's sweat has had the smell of rotten fish for the last 5 years. This patient is a known case of Fish odour syndrome diagnosed by the presence of trimethylaminuria in his urine 5 years back. The patient informed us that when he didn't have the bad odour in his sweat, he did not develop the above-mentioned lesions of tinea corporis. The patient was very frustrated and socially isolated because of severe itching and the bad odour in his sweat. The role of trimethylamine (TMA) causes bad odour in the sweat, mouth and urine. We don't encounter cases like this very often and we are reporting this case because after a detailed inquiry about the patient's family history the same condition persists with 4 other patient's family members but they did not report themselves as they are mostly less educated, live in a rural area and they don't want to visit any doctor. We want to increase the knowledge of the medical fraternity to deal with these cases in a better way and maybe we are opening a new horizon for the researchers

    The Pivotal Role of Antihypertensives in the Preoperative Management of Pheochromocytoma

    Pheochromocytomas are rare neuroendocrine tumors with the potential to undergo malignant changes. These tumors are associated with hemodynamic instabilities and may even lead to cardiomyopathy/heart failure due to catecholamine surge. Preoperative intervention with antihypertensives can curb the morbidity and mortality related to the tumor. Adrenergic receptor blockade is an effective preoperative modality. Phenoxybenzamine, along with metyrosine, should be considered a beneficial synergistic combination for a complication-free surgical intervention of the tumor. Standard preoperative therapeutic strategies and guideline protocols, such as the Roizen’s criteria, need to be established for a better overall outcome

    Primary Diffuse Large B-Cell Lymphoma of Breast: A Rare Form of Gestational Breast Cancer

    Primary diffuse large B cell lymphoma of the breast is an uncommon form of gestational breast cancer and its presentation during pregnancy is challenging. In our case report, a patient presented with a right breast lump during the first trimester of her pregnancy. Biopsy was taken from right breast mass, which was diffuse large b cell lymphoma (DLBCL) double expressive type. She underwent termination of pregnancy and received 6 cycles of multiagent intense chemotherapy along with central nervous system prophylaxis followed by consolidation involved site radiotherapy (ISRT) to breast and axilla after achieving a complete response. Studies suggest no improvement in the outcome of gestational breast cancer with the termination of pregnancy. However, the presentation of primary DLBCL during pregnancy is extremely rare and the decision regarding pregnancy termination is weighted upon a case by case basis. The paucity of data makes our case report a valuable source for the management of DLBCL breast in pregnancy

    Acceptability of COVID-19 Vaccines among Patients on Maintenance Hemodialysis: An Assessment from a Healthcare Center in Karachi

    Dialysis patients are among the high-risk group to contract COVID-19 necessitating vaccination to decrease mortality and morbidity. We aimed to evaluate the COVID-19 vaccination status of dialysis patients in our institute (NIKUD). Regular hemodialysis patients (n=38) from Sept’21 to Jan’22 were included after taking written consent. The majority (78%) of patients were fully vaccinated and none was partially vaccinated. Sinopharm (36.6%) was the most frequent vaccine received, followed by Moderna (26%). Among vaccinated patients' families, the complete vaccination rate was (68%) for the COVID-19 vaccine as compared (0%) for the complete vaccination of family members in the unvaccinated group. Non-belief in the vaccine (75%) along with no known case of COVID-19 disease to self or family (100%) were the most common possible reasons for not getting vaccinated. Perceived risk of COVID-19 to themselves as non-dangerous was a concerning belief in the majority (44%) (17) dialysis patients. Approximately 25% of patients did not receive any COVID-19 vaccine including inadequate vaccination among their families. Evidence-Based counseling and education are advised to alleviate any concerns regarding COVID-19 vaccines
