    Current concepts in the assessment and management of multiligament injuries of the knee

    Multiligament knee injuries (MLKIs), though rare, pose significant challenges to the patient and surgeon. They often occur in the setting of high-velocity trauma and are frequently associated with concomitant intra- and extra-articular injuries, the most immediately devastating of which is vascular compromise. A detailed evaluation is required when acute or chronic MLKIs are suspected, and stress radiography, MRI and angiography are valuable adjuncts to a thorough clinical examination. Surgical treatment is widely regarded as superior to non-surgical management and has been demonstrated to improve functional outcome scores, return to work, and return to sport rates, though the incidence of post-traumatic osteoarthritis remains high in affected knees. However, acceptable results have been obtained with conservative management in populations where surgical intervention is not feasible. Early arthroscopic single-stage reconstruction is currently the mainstay of treatment for these injuries, but some recent comparative studies have found no significant differences in outcomes. Recent trends in the literature on MLKIs seem to favour early surgery over delayed surgery, though both methods have distinct advantages and disadvantages. Due to the heterogeneity of the injury and the diversity of patient factors, treatment needs to be individualised, and a single best approach with regards to the timing of surgery, repair versus reconstruction, surgical technique and surgical principles cannot be advocated. There is much controversy in the literature surrounding these topics. Early post-operative rehabilitation remains one of the most important positive prognostic factors in surgical management and requires a dedicated team-based approach. Though outcomes of MLKIs are generally favourable, complications are abundant and precautionary measures should be implemented where possible. Low resource settings are faced with unique challenges, necessitating adaptability and pragmatism in tailoring a management strategy capable of achieving comparable outcomes

    Joint preservation surgery in grade 2 and 3 giant cell tumors of bone around the knee

    Objective: To evaluate the clinical and functional outcomes of joint preservation surgery in high-grade giant cell tumors (GCT) around the knee joint. Methods: A retrospective review of 25 patients of high-grade GCT (Campanacci grade 2 and 3) involving proximal tibia or distal femur managed by extended curettage, bone grafting, and stabilization with knee spanning external fixator between 2016 and 2018 was done. The radiographic outcomes, functional outcomes (Musculoskeletal Tumor Society [MSTS] score for lower limb), and complications including donor site morbidity were evaluated. Results: The mean age of the patient population was 24.04 years with an average follow-up period of 30.24 months. Fourteen patients had involvement of distal femur, and 11 involved proximal tibia. There were 16 cases of grade 2 lesions and 9 cases of grade 3 lesions. Twenty-four out of the 25 patients had radiological consolidation of graft, while one patient had graft subsidence. Twenty-two out of 25 patients had full extension and knee flexion more than 100 degrees. The mean MSTS score was 25.2. Three patients had an MSTS score under 20. All three patients had an extension lag with a restricted range of motion. Conclusion: Joint preservation surgery, when done in line with the basic principles of tumor surgery, gives good radiographic and functional outcomes even in grade 2 and 3 giant cell tumors of bone around the knee and should be attempted before replacement surgeries

    Open approaches for cruciate ligament reconstruction in knee dislocations: A technical note and case series

    Introduction: Arthroscopic surgery is the gold standard for cruciate ligament reconstruction in multi-ligament knee injuries. However, hospitals in limited-resource settings often lack arthroscopic-trained surgeons or equipment. Open approaches for treating knee dislocations can overcome many of these limitations. Methodology: This study aims to describe techniques for open approaches in a supine patient to address the cruciate ligaments in multi-ligament knee injuries and to review associated complications and clinical outcomes in a retrospective case series. Results: Ten patients with multi-ligament knee injuries who had undergone open cruciate ligament reconstruction between July 2016 and November 2018 were retrospectively identified. Open approaches were performed owing to the extravasation of arthroscopy fluid into the posterior compartment (3) or a large traumatic arthrotomy (7). Complications and patient-reported outcomes were analysed. Eight of the 10 patients were followed up at 10 months postoperatively (range, 5–23 months). None had iatrogenic neurovascular damage. Median outcomes scores were: visual analogue scale, 45 (range, 0–100); Knee Injury and Osteoarthritis Outcome Score-Physical Function Short Form, 81.4 (range, 75–100); Lysholm, 85 (range, 67–92). Discussion: Open approaches were safe and useful in treating cruciate ligaments and should be considered in arthroscopy fluid extraversion and large traumatic arthrotomies

    Autograft diameter in ACL reconstruction: size does matter

    Background: Anterior cruciate ligament injuries are commonly seen in orthopedic surgery practice. Although anterior cruciate ligament reconstruction (ACLR) has come a long way, the causes of failure have yet to be fully understood. Objective: The aim of this study was to investigate whether or not the intraoperative 4-strand hamstring autograft diameter does in fact influence the failure rates of ACLR. Methods: Retrospective intraoperative data were collected from ACLR patients from the only tertiary center available in Kuwait. Patients who underwent ACLR from 2012 to 2018 for isolated ACL injuries were included in this study, allowing for a 24 month follow-up period The cohorts were categorized into 3 groups: patients with graft size≤8mm, 2, patients with graft sizes≥8mm with 4-strands and patients with graft sizes≥8mm with 4-strands or more. ANOVA analysis was applied to address group differences between mean graft size and strand numbers and subsequently the failure rates for each group. In addition, the Mann–Whitney U test was used to investigate the relationship between revision and initial ACL graft size. Results: Out of the 711 out of 782 patients were included in this study. Only 42.6% of the patients did not need more than 4-strands to achieve an 8mm sized autograft. The patients who had autografts≤8mm in this study accounted for 17.1% of the population. About 7.2% of these patients required revision surgery. Patients with a 4-strand autograft size that was less than 8mm were 7.2 times more at risk for ACLR failure (RR=7.2, 95% CI: 6.02; 8.35, p=0.007).     A long-term cross-sectional study with modified forgotten joint score to assess the perception of artificial joint after total knee arthroplasty

    Background and purpose: The ultimate goal for an arthroplasty surgeon is to provide the patient a joint that feels more like a natural joint. The Modified Forgotten Joint Score (MFJS) is a newly introduced functional scoring system that has a superior ability to assess this property among arthroplasty patients. The objective of this study is to evaluate the long-term temporal association of the MFJS and total knee arthroplasty (TKA). Methods: We assessed 360 patients post TKA with MFJS questionnaire. The patient groups were distributed at follow-up intervals of 3 weeks (n = 55), 6 months (n = 45), 1 year (n = 57), 2 years (n = 40), 3 years (n = 49), 5 years (n = 49), 7 years (n = 39), and 10 years (n = 26). Higher score suggests a forgotten artificial joint. Results: Post-operative mean MFJS scores were 64.4 ± 7.6 at 3 weeks, 87.7 ± 5.6 at 6 months, 89.2 ± 3.1 at 1 year, 89.9 ± 2.6 at 2 years, 89.4 ± 3.2 at 3 years, 89.1 ± 4 at 5 years, 84.5 ± 8.8 at 7 years, and 82.7 ± 11.9 at 10 years. The score at 3 weeks was significantly lesser than the average scores at other follow-up intervals. The score at 6 months was significantly higher compared to the score at 10 years. The average score at 1 year, 2 years, 3 years, and 5 years were significantly higher compared to the average score at 7 years and 10 years. Conclusion: The trend of the MFJS score was found to drastically improve from 3 weeks to 6 months and peak in 2 years after which the score tends to attain a plateau up to 5 years following which there is a decline in the score at 7- and 10-years post-surgery. Age did not have an influence on the variation in functional score in any of the follow-up groups. MFJS has a strong positive correlation with the well-recognised KOOS scoring system.     Platelet-rich plasma as an effective biological therapy in early-stage knee osteoarthritis: One year follow up

The principal objective of our study was to evaluate the effectiveness and safety of standardized intra-articular injection of autologous PRP in early osteoarthritis knee.     Misdiagnosed cartilaginous PCL avulsion in young children

    Posterior Cruciate Ligaments injuries are rare in children and usually due to bony avulsion fractures or midsubstance tears. This study focused on cartilaginous avulsions initially misdiagnosed despite of MRI assessment. Two 6-year-old boys had cartilaginous avulsion fracture injury at the femoral attachment of the PCL. One had associated medial meniscal lesion and was reinserted. The other conducted to non-union. MRI second lecture reveals an original description with nail-biting sign on cartilage surface of anterior notch, and a close PCL angle without anterior tibial translation. No bone bruise was associated. Similarly, to ACL cartilaginous tibial avulsions, PCL cartilaginous femoral avulsions are underdiagnosed. When knee hemarthrosis occurs under the age of nine, clinician and radiologist should be aware that cartilaginous avulsion of ACL and PCL also could be the main pattern of lesion

    Posterior shoulder dislocation (PSD) with a reverse Hill-Sachs lesion is a rare injury with challenging management. This article is a technical note, describing the combination of both, modified McLaughlin procedure with posterior Bankart repair, for the surgical treatment of traumatic PSD associated with a substantial reverse Hill-Sachs lesion. Two patients with mid-term follow-up are presented. Approaching and repairing both sides of the joint, balance and congruency are restored, the humeral head is centralized in the glenoid and the patient starts early mobilization and rehabilitation safely

    Latissimus dorsi tendon transfers: a historical journey

    Several latissimus dorsi tendon transfer techniques for shoulder problems have been previously described and developed. These techniques involve the transfer in obstetric palsy, transfer in irreparable posterosuperior rotator cuff tears and subscapularis tears, and transfer in reverse shoulder arthroplasty. We detail the differences in planning and surgery and the need for different approaches. This historical and technical description provided in this study will benefit surgeons wishing to use the procedure

    Subacromial impingement by a lipoma arborescens

    Subacromial impingement syndrome (SIS) is the leading cause of shoulder pain. A systemic approach for abnormal causes of SIS is recommended to avoid misdiagnosing rare or sinister pathologies. To our knowledge, only nine cases of subacromial lipoma arborescens associated with impingement syndrome have been reported in the literature. In this report, we briefly discuss histopathologic and radiological signs of an unusual case of impingement syndrome caused by subacromial “lipoma arborescens” and describe arthroscopic synovectomy after the failure of conservative management. The patient remains symptom-free five years after surgery
