    Czynniki wpływające na przeżycie biorców po transplantacji serca: 5-letnie badanie retrospekcyjne

    Introduction: Cardiac transplantation remains the standard procedure for patients with end-stage heart failure. Assessing the outcomes of heart transplantation, including mortality and its predictors, is highly important for transplant centers. The aim of this study is to explore the possible influence of the quality of life, depressive symptoms, optimism, self-efficacy, and medical data on survival among heart transplant recipients over a five-year observational period. Material and methods: 131 heart transplant recipients were included in this study. Psychological functioning and quality of life were assessed using self-reported measures (Beck Depression Inventory, Life Orientation Test, General Self Efficacy Scale, and World Health Organization Quality of Life). Clinical data were obtained from an electronic health record. Results: During a median follow-up of five years, 24% of patients died. The occurrence of vascular graft disease increased the risk of death twofold in the population studied (HR = 2.17 [1.066, 14.446], p = 0.032). In the patients diagnosed with heart failure on the NYHA scale, the risk of death for patients with a NYHA rating of II–IV was twice as high (HR = 2.18 [1.043, 4.578], p = 0.037) compared with a NYHA rating of I. Conclusions: Neither the quality of life nor psychological factors were associated with five-year survival. Only coronary artery vasculopathy and having a higher NYHA scale rating following heart transplantation were independently predictive of five-year mortality among heart transplant recipients.Wprowadzenie: Przeszczepienie serca pozostaje standardem postępowania w leczeniu schyłkowej niewydolności serca. Ocena wyników przeszczepu serca, w tym śmiertelności i jej czynników prognostycznych, jest bardzo ważna dla ośrodków transplantacyjnych. Celem pracy jest zbadanie możliwego wpływu jakości życia, objawów depresyjnych, optymizmu, poczucia własnej skuteczności oraz danych medycznych na przeżycie wśród biorców serca w 5-letnim okresie obserwacji. Materiał i metody: Badaniem objęto 131 biorców przeszczepu serca. Funkcjonowanie psychologiczne i jakość życia oceniano za pomocą samoopisowych skal (Inwentarz Depresji Becka, Test Orientacji Życiowej, Skala Uogólnionej Własnej Skuteczności oraz Jakość Życia Światowej Organizacji Zdrowia). Dane kliniczne uzyskano z elektronicznego rekordu pacjenta. Wyniki: W okresie obserwacji o medianie 5 lat zmarło 24% pacjentów. Wystąpienie choroby naczyniowej graftu dwukrotnie zwiększało ryzyko zgonu w badanej populacji (HR = 2,17 [1,066, 14,446], p = 0,032). U pacjentów z rozpoznaną niewydolnością serca w skali NYHA II–IV ryzyko zgonu pacjentów było dwukrotnie wyższe (HR = 2,18 [1,043, 4,578], p = 0,037) w porównaniu z oceną NYHA I. Wnioski: Ani jakość życia, ani czynniki psychologiczne nie były związane z 5-letnim przeżyciem. Jedynie waskulopatia tętnic wieńcowych i wyższa skala NYHA po przeszczepieniu serca były niezależnymi predyktorami 5-letniej śmiertelności wśród biorców serca

    Differing concepts of total defence in small states: comparing the cases of New Zealand and Poland

    This article proposes that a small state’s approach to total defence will be strong influenced by the nature of its strategic environment. It compares the defence approaches of Poland and New Zealand to identify whether the different contexts of their strategic environments necessitate divergent strategies for defending their state. The theory of population ecology of organisations will be used to frame the different options available to small states in their strategic environments and applied to the cases in order to explain their different approaches to total defence

    Roman Dmowski et son camp, dont Erazm Piltz, ont utilisé l’évolution de la politique étrangère française et présence du Comité national polonais à Paris pour faire pression sur le gouvernement français afin qu’il inclue l’indépendance de la Pologne dans les objectifs de guerre des Alliés. L’intérêt accru de la France pour les affaires polonaises à la fin de 1917 résulte d’une part de la perte de son allié russe et d’autre part d’un fort désir d’affaiblir l’Allemagne autant que possible. La France s’est alors mise à modifier sa politique dans l’Est européen prévoyant dans ses plans futurs la possibilité de compter les nouveaux États d’Europe centrale dans son système d’alliance. Finalement, en 1921, la France et la Pologne conclurent une alliance en deux volets : un accord politique et une convention militaire secrète. Pour la France, la Pologne n’est dans les domaines politique et militaire qu’un «allié de revers», un allié contre l’Allemagne, mais aussi un allié qui soulève de nombreuses objections au Quai d’Orsay. Elle joue également un rôle très important pour les investisseurs français. Pour la Pologne, en revanche, l’alliance avec la France en tant que puissance victorieuse était facteur essentiel dans sa politique étrangère, le pays ayant absolument besoin d’un partenaire parmi les puissances européennes.Roman Dmowski and his camp, including Erazm Piltz, used the evolution of French foreign policy and the existence of the Polish National Committee in Paris to pressure the French government to include Polish independence in the Allies’ war aims. France’s increased interest in Polish affairs in late 1917 resulted partly from the loss of its Russian ally and partly from a strong desire to weaken Germany as much as possible. France then began to modify its policy in Eastern Europe and included in its future plans the possibility of including the new Central European states in its alliance system. Finally, in 1921, France and Poland concluded a two-part alliance: a political agreement and a secret military convention. For France, in the political and military spheres, Poland was nothing more than a “rear ally”, an ally against Germany, as well as an ally that raised many objections in the Quai d’Orsay. It also played a very important role for French investors. For Poland, on the other hand, the alliance with France as a victorious power was a very important factor in its foreign policy, because Poland absolutely needed a partner among the European powers.Roman Dmowski i jego obóz, w tym Erazm Piltz, wykorzystywali ewolucję francuskiej polityki zagranicznej i istnienie w Paryżu Komitetu Narodowego Polskiego w celu wywierania presji na rząd francuski, aby włączył on sprawę niepodległości Polski do celów wojennych aliantów. Wzrost zainteresowania Francji sprawami polskimi pod koniec 1917 roku po części wynikał z utraty rosyjskiego sojusznika, a po części z dążenia do maksymalnego osłabienia Niemiec. Francja zaczęła wówczas modyfikować swoją politykę w Europie Wschodniej i w swoich planach na przyszłość uwzględniała możliwość włączenia nowych państw Europy Środkowej do swojego systemu sojuszniczego. Ostatecznie w 1921 roku Francja i Polska zawarły dwuczęściowy sojusz: umowę polityczną i tajną konwencję wojskową. Dla Francji, w sferze politycznej i militarnej, Polska była jedynie allié de revers, sojusznikiem przeciwko Niemcom, przy tym sojusznikiem budzącym wiele zastrzeżeń w Quai d’Orsay. Sojusz odegrał także bardzo ważną rolę z perspektywy francuskich inwestorów. Dla Polski natomiast sojusz z Francją jako mocarstwem zwycięskim był kluczowym czynnikiem w jej polityce zagranicznej, ponieważ Polska bezwzględnie potrzebowała partnera wśród mocarstw europejskich

Już dzisiaj dostrzega się, że jest i będzie wyznacznikiem głębokich zmian w polityce europejskiej. 24 lutego tego roku siły zbrojne Federacji Rosyjskiej napadły na Ukrainę, dokonując aktu niczym nieusprawiedliwionej agresji na suwerenne państwo. Zgromadzenie Ogólne ONZ uznało tę agresję za pogwałcenie art. 2 ust. 4 Karty Narodów Zjednoczonych jako normy bezwzględnie wiążącej (ius cogens). Co więcej, Federacja Rosyjska była jednym z trzech gwarantów bezpieczeństwa Ukrainy, związana podpisanym 5 grudnia 1994 roku memorandum budapeszteńskim, którego pierwszy paragraf brzmi: „Federacja Rosyjska, Zjednoczone Królestwo Wielkiej Brytanii i Irlandii Północnej oraz Stany Zjednoczone Ameryki potwierdzają swoje zobowiązanie wobec Ukrainy, zgodnie z zasadami Aktu Końcowego KBWE, do poszanowania niepodległości, suwerenności i istniejących granic Ukrainy”"(...

    Ochrona lokatorów została zinstytucjonalizowana w polskim systemie prawa w okresie międzywojennym. Analiza celów i kształtu regulacji wprowadzonych w ustawach z 1920 i 1924 r. prowadzi do wniosku, że na przestrzeni lat regulacje kształtujące ochronę lokatorów uległy modyfikacji nie tylko w warstwie merytorycznej, ale i aksjologicznej. Regulacje wprowadzone w okresie międzywojennym miały równoważyć prawa najemców i wynajmujących. Wprowadzone instytucje znajdowały zastosowanie nie tylko do najemców lokali mieszkalnych, ale też użytkowych. Odpowiedni kształt regulacji i zakres ich zastosowania przyczynić się miały zarówno do ochrony mieszkalnictwa, jak i rozwoju gospodarki, chroniąc inwestorów na rynku budownictwa oraz konsumentów. Istotne jest także, że wprowadzone regulacje z założenia miały mieć charakter przejściowy i obowiązywać jedynie na czas sytuacji kryzysowej spowodowanej zniszczeniami I wojny światowej.Protection of tenants was institutionalized in the Polish legal system in the interwar period. The analysis of the objectives and shape of the regulations introduced in the Act of 1920 and then of 1924 leads to the conclusion that over the years the regulations shaping the protection of tenants have been modified not only in terms of their content but also in the axiological aspect. The regulations introduced in the interwar period were to balance the rights of tenants and landlords. The introduced institutions were applied not only to tenants of residential but also commercial premises. The appropriate shape of the regulations and the scope of their application were to contribute to the protection of housing, but also to the development of the economy, protecting investors in the construction market and consumers. It is also important that the introduced regulations were supposed to be of a temporary nature, and they were to be in force only for the duration of a crisis situation caused by the devastation of World War I

    La France a considéré l’alliance franco-polonaise de 1921 comme un élément d’un système de sécurité collective prévoyant une concertation avec la Tchécoslovaquie en cas d’agression allemande. Sa position a également évolué en raison du manque de coordination entre le Quai d’Orsay et l’État-major, car il est impossible de concilier le réseau d’alliances en Europe centrale avec la préparation d’une guerre strictement défensive. Le système de sécurité collective se dégrada dans les années 1930 et se déforme de plus en plus par la suite. Le droit des peuples à l’autodétermination fut instrumentalisé dans la crise de la minorité polonaise en Tchécoslovaquie qui s’intensifia à partir de 1937. Le potentiel de coopération franco-polonaise s’est considérablement réduit lors de la crise des Sudètes. Bonnet chercha à rendre la partie polonaise responsable afin de disculper la France et de justifier sa propre attitude conciliante à l’égard de Munich. Au Quai d’Orsay, en revanche, des tendances conscientes de la dévastation de la relation franco-polonaise cherchèrent à évaluer ce qu’il est possible d’en sauver.France considered the Franco-Polish alliance of 1921 to be part of a collective security system that included consultation with Czechoslovakia in the event of German aggression. Its position also changed because of the lack of coordination between the Quai d’Orsay and the General Staff, as it was impossible to reconcile the network of alliances in Central Europe with the preparation of a strictly defensive war. The system of collective security deteriorated in the 1930s, and became increasingly distorted thereafter. The right of peoples to self-determination was instrumentalized in the crisis over the Polish minority in Czechoslovakia, which intensified from 1937. The potential for Franco-Polish cooperation was severely undermined by the Sudetenland crisis. Bonnet sought to blame the Polish side in order to exonerate France and justify his own conciliatory stance towards Munich. In the Quai d’Orsay, on the other hand, there were tendencies aware of the devastation of Franco-Polish relations that sought to assess what could be salvaged from them.Francja uważała sojusz polsko-francuski z 1921 r. za ważną część systemu bezpieczeństwa zbiorowego, przewidującego konsultacje z udziałem Czechosłowacji w przypadku agresji ze strony Niemiec. Na zmianę postrzegania sojuszu przez Francję w okresie międzywojennym wpłynął brak koordynacji między Quai d’Orsay a Sztabem Generalnym, gdyż niemożliwe było pogodzenie sieci sojuszy w Europie Środkowej z przygotowaniem do wojny ściśle defensywnej. System bezpieczeństwa zbiorowego uległ w latach 30. degradacji, a następnie stopniowo ulegał wypaczeniu. Prawo narodów do samostanowienia zostało wykorzystane instrumentalnie w czasie kryzysu wokół mniejszości polskiej w Czechosłowacji, który nasilał się od 1937 r. Współpraca polsko-francuska została znacząco naruszona podczas kryzysu sudeckiego. Georges Bonnet za kryzys sudecki starał się obciążyć odpowiedzialnością stronę polską, aby poprawić wizerunek Francji i uzasadnić własne ugodowe stanowisko wobec układu monachijskiego

    Enhancing resilience: the state of play in NATO, European Union, and Poland

    This article presents the current state of play on resilience in NATO, the EU, and Poland. It argues that in the past couple of years, the international security environment has undergone dramatic transitions. On top of existing challenges, new threats (including hybrid) and large-scale crises (i.e., pandemics) are emerging. This complex security situation requires a whole-of-government and whole-of-society approach. Building resilience at state, local, and societal level is key. This notion is well understood both in the EU and NATO. Resilience is rooted in the Alliance’s founding Treaty. Article 3 of the Washington Treaty claims that each Ally must first take care of its individual security. Russia’s annexation of Crimea and, subsequently, the COVID-19 pandemic stressed – both in NATO and the EU – the need to be better prepared and able to respond to complex crisis. In the last couple of years, the EU’s approach to resilience has been more inward-looking. From building resilience by the EU, it has shifted to resilience of the EU. Russia’s invasion of Ukraine puts the West to a test, including a resilience test. Opening their homes to ca. 2 million Ukrainian refugees, the Polish people have passed the test and proved, inter alia, that they are able to deal effectively with uncontrolled movement of people

    PRZEDMOWA: "Polifenole są metabolitami wtórnymi roślin i grzybów. Powszechnie występują w kwiatach, korze, korzeniach, łodygach, liściach i owocach roślin. Ich strukturę chemiczną charakteryzuje obecność dwóch lub więcej grup –OH przyłączonych do pierścieni aromatycznych. Do polifenoli zaliczamy lignany, kurkuminoidy, taniny, stilbenoidy, kwasy fenolowe oraz flawonoidy. Ta ostatnia klasa obejmuje: flawony (np. apigenina), flawanony (np. naringenina), flawonole (np. kwercetyna), flawanole (np. katechiny), izoflawony (np. genisteina), antocyjanidyny (np. malwidyna), chalkony (np. buteina), aurony (np. aureuzydyna) i ksantony (np. α-mangostyna). Do tej pory opisano ponad dziesięć tysięcy związków polifenolowych i wciąż nie poznano ich wszystkich. Właściwości polifenoli to temat intensywnie badany na całym świecie ze względu na możliwe wykorzystanie związków polifenolowych w medycynie, farmakologii, kosmetologii, rolnictwie, dietetyce i przemyśle."(...
