Actuación enfermera ante la estenosis aórtica sintomática leve congénita detectada durante el embarazo: caso clínico
Este trabajo expone el caso de Elena, una mujer embarazada de 28 semanas que
acude al servicio de urgencias presentando síntomas de disnea y agitación. Tras las
pruebas diagnósticas, se descubre una estenosis aórtica congénita, una condición
cardíaca que había pasado desapercibida hasta ese momento. Esta patología
puede complicar el embarazo debido al aumento de la demanda metabólica y
energética.
El plan de cuidados de enfermería se estructura en torno a la evaluación de las 14
necesidades básicas de Virginia Henderson, identificando cuatro áreas prioritarias
afectadas: respiración, hábito tabáquico, manejo de la ansiedad y educación para la
salud. Se diseñan intervenciones personalizadas para abordar estas necesidades,
centrándose en la reducción de riesgos para la madre y el bebé, el apoyo emocional
y la educación sobre la enfermedad y el embarazo. Se emplea la taxonomía NNN
consult para establecer objetivos específicos y actividades de enfermería adaptadas
a las necesidades individuales de Elena. Se planifica una evaluación continua de
los resultados obtenidos para ajustar las intervenciones según sea necesario.
En la discusión pondremos de manifiesto la falta de investigación de la estenosis
aórtica en el embarazo, para determinar los resultados de las intervenciones
propuestas. También consideraremos los obstáculos a la hora de ofrecer cuidado
continuo, especialmente en el contexto de saturación actual de nuestro sistema
sanitario
El Patrimonio Jacobeo y su gestión desde las Humanidades Digitales: presente y futuro del Camino de Santiago en Castilla y León
Esta tesis doctoral centra su estudio en los Bienes de Interés Cultural (BIC) localizados en el Camino de Santiago Francés a su paso por la comunidad autónoma de Castilla y León, desde la perspectiva de las Humanidades Digitales. El objetivo es analizar estos bienes culturales sistemática, detallada, cuantitativa y cualitativamente para profundizar en su gestión y valorización, contribuyendo así al campo de la Gestión del Patrimonio Cultural. Los objetivos específicos abarcan la síntesis de los elementos clave tanto del Patrimonio Cultural y su gestión como del Camino de Santiago, el análisis bibliométrico de estudios sobre el Camino de Santiago, la elaboración de una base de datos detallada de dichos Bienes de Interés Cultural y el estudio de la gestión, protección y enfoque turístico de estos. Además, se evalúa también el papel de herramientas digitales en la investigación de la huella digital de los Bienes de Interés Cultural, facilitando datos cruciales para la toma de decisiones en su gestión. La tesis se enmarca en una metodología interdisciplinar que combina Humanidades Digitales, Ciencia Social Computacional y análisis de Sistemas Complejos, ofreciendo un panorama integrado, combinando diversas perspectivas que hasta ahora habían sido tratadas de manera aislada. Al integrar aspectos históricos, artísticos, turísticos y digitales, el trabajo proporciona una perspectiva holística de cómo estos diferentes elementos interactúan y contribuyen a la gestión y valorización del patrimonio. Este enfoque multidimensional permite a los gestores del Patrimonio Cultural, académicos y responsables políticos obtener una comprensión más completa y matizada, facilitando la toma de decisiones informadas y la implementación de estrategias más efectivas. La investigación aborda específicamente el impacto digital en la gestión del patrimonio, utilizando herramientas como Twitter, Google News y Google Trends para analizar la huella digital del Camino de Santiago. Esta exploración pionera proporciona insights valiosos sobre cómo las tecnologías digitales están remodelando la percepción y la interacción con el Patrimonio Cultural, ofreciendo nuevas oportunidades para su promoción y preservación en la era digital.This doctoral thesis focuses its study on the Assets of Cultural Interest (BIC) located on the French Way of Saint James as it passes through the autonomous community of Castile and Leon, from the perspective of Digital Humanities. The objective is to analyze these cultural assets systematically, in detail, quantitatively and qualitatively in order to deepen their management and valorization, thus contributing to the field of Cultural Heritage Management. The specific objectives include the synthesis of the key elements of Cultural Heritage and its management as well as the Way of Saint James, the bibliometric analysis of studies on the Way of Saint James, the development of a detailed database of the Assets of Cultural Interest and the study of the management, protection and tourism approach of these. In addition, the role of digital tools in the investigation of the digital footprint of the Assets of Cultural Interest is also evaluated, providing crucial data for decisión-making in their management. The thesis is framed in an interdisciplinary methodology that combines Digital Humanities, Computational Social Science and Complex Systems analysis, offering an integrated view, combining different perspectives that until now had been treated in isolation. By integrating historical, artistic, tourism and digital aspects, the work provides a holistic perspective of how these different elements interact and contribute to heritage management and enhancement. This multidimensional approach enables cultural heritage managers, academics and policy makers to gain a more complete and nuanced understanding, facilitating informed decision-making and the implementation of more effective strategies. The research specifically addresses the digital impact on heritage management, using tools such as Twitter, Google News and Google Trends to analyze the digital footprint of the Way of Saint James. This pioneering exploration provides valuable insights into how digital technologies are reshaping the perception of and interaction with cultural heritage, offering new opportunities for its promotion and preservation in the digital age

Interacción, discurso y tecnología en la enseñanza del español
Actas del XXXIII Congreso Internacional de la Asociación para la Enseñanza del Español como Lengua Extranjera, realizado del 29 de agosto al 2 de septiembre de 2023, organizado por la Universidad de BurgosEl volumen temático "Interacción, discurso y tecnología en la enseñanza del español" es una compilación multidisciplinaria que aborda los desafíos y avances en la enseñanza del español como lengua extranjera, segunda lengua y lengua de herencia. Se organiza en tres ejes principales: la interacción en el aprendizaje, el discurso en la enseñanza y la integración de la tecnología.
La interacción en el aprendizaje del español se destaca como fundamental para la adquisición lingüística. Se examinan competencias interaccionales, géneros dialogados y dinámicas de grupo innovadoras que fomentan una interacción efectiva, así como el papel del feedback interactivo y la evaluación en el desarrollo de la autonomía del estudiante.
En cuanto al discurso en la enseñanza, se enfoca en aspectos léxicos, gramaticales y pragmáticos esenciales para el aprendizaje. Se analiza el impacto del discurso del docente en la formación del profesorado y la importancia de estudios críticos del discurso en los materiales didácticos. También se abordan el discurso oral y escrito, la multimodalidad y la diversidad lingüística.
La tecnología y mediación tecnológica se presentan como elementos transformadores en la enseñanza del español. Se exploran plataformas de aprendizaje, entornos híbridos y virtuales, y herramientas lingüísticas digitales, incluyendo la inteligencia artificial para desarrollar destrezas lingüísticas.
Se trata, por tanto, de una obra de referencia indispensable para educadores, investigadores y profesionales en la enseñanza del español en diversos contextos y niveles
Comparative Selective Conversion of Biomass-Derived Mono- and Polysaccharides into Lactic Acid with Lanthanide Lewis Acid Catalysts
Lanthanides were tested (Ce3+, Er3+, and Yb3+) as catalysts to produce lactic acid (LA) from the monosaccharides present in corn stover (glucose, xylose, and arabinose) resulting in ytterbium being the most active. A MW-heated system led to similar LA yield as a conventionally heated pressurized system. The maximum value of LA yield was 40% at 240 °C after 20 min of isothermal treatment regardless the starting monosaccharides, which allowed to propose a similar LA production route based on the products profile determined along time for the three monosaccharides. Temperature and time determined the product profile, observing furfural degradation at severity factors higher than 3.5, while values higher than 5.5 were needed to observe LA degradation. By increasing temperature, catalyst solubility decreased, increasing its presence in the solid residue after treatment. Xylan conversion to LA was similar as for xylose, but lower yield was obtained from microcrystalline cellulose. Corn stover presented more amorphous regions leading to higher hydrolysis yields of its cellulose fraction.Open Access funding provided thanks to the CRUE-CSIC agreement with Springer Nature. Cultural and linguistic adaptation and validation of the Function and Life Impact scales of the HAND-Q instrument for the Portuguese population
Introduction: Clinical conditions affecting the hands are common and can have a significant impact on a person's quality of life. Patient-Reported Outcome Measures (PROMs) are considered the preferred method of assessing the results achieved from the patient's perspective. However, most of them have psychometric limitations. The HAND-Q is a new instrument built using a modern methodology that makes it possible to collect information on hand appearance, health-related quality of life (HRQoL), care experience and treatment from the perspective of patients with any type of congenital or acquired condition affecting the hands.
Aim: The aim of this study was to translate, linguistically and culturally adapt and psychometrically validate the Function and Life Impact scales of the HAND-Q instrument, from the original in English, for the Portuguese population, guaranteeing their validity and reliability so that they can be used in clinical practice.
Methods: The cultural and linguistic adaptation process will be carried out in two phases: Semantic equivalence, consisting of translation and retroversion; Content validation through a cognitive pre-test, using a sample of 5 to 15 patients with clinical hand conditions and clinical review of the quality of the translation by a panel of 5 to 7 experts.
The pilot validation process will be carried out using a convenience sample of 30 participants with clinical conditions of the hand in the Adult Physical Medicine and Rehabilitation services of the ULS São José. Data collection will involve assessing grip strength (Activ5) and pain perception (VAS) and applying a set of questionnaires (Portuguese versions of the MMSE, the Function and Life Impact scales, the DASH and the EQ-5D) and a form on sociodemographic and clinical data.
Results: We hope to obtain a consensus translation, adapted to the Portuguese population, which meets the expectations of professionals in the field and is clear to patients. It is also hoped that the translated scales will preliminarily have the validity and reliability to make them viable for use in clinical practice
Cascade approach to valorize soybean by-product by using subcritical water. First step: bioactive compounds
Trabajo presentado en: EIFS2024, 3er Encuentro Ibérico de Fluidos Supercríticos = Third Iberian Meeting on Supercritical Fluids = 3º Encontro Ibérico de Fluidos Supercríticos, 22-24 de julio, Ourense, Spain.Okara is the soybean by-product generated in the production of soymilk, tofu, and soy protein isolate, at the filtration stage. It’s quite expensive to dry it (necessary for its conservation), so it is generally discarded; however, its nutritional value makes it very interesting. It presents fiber, protein, good amino acid profile, healthy lipids and great isoflavone content; being these last compounds particularly noteworthy.[1, 2] Subcritical water (subW) was the green technology selected as the main hydrolytic agent in this study, focusing on the isoflavone fraction. OKW extracts were subjected to independent subW extractions in six assays from 110 °C to 180 °C, using N2 to pressurize up to 5 MPa, for 5 h,in a batch reactor of 0.5 L capacity. It was analyzed total isoflavone content at different treatment times for each selected temperature. 8 isoflavones (daidzein, genistein, glycitein and its derivates) and the difference in extraction of its malonyl forms, β-glucosides and aglycones, were analyzed. Different assays were carried out on OKW (oven-dried washed okara) and best conditions were applied posteriorly to OKA (okara as received without pretreatment). Total isoflavone extraction (Fig. 1) and antioxidant capacity were analyzed in all of these kinetics. At low temperatures (lower than 140 °C), there was observed a maximum in the extraction of isoflavones, being the highest at 120 ° C, after 30 min of treatment (27.8 μg/mL; 585.6 μg/g OKW). On the contrary, at the highest temperatures essayed, 160 and 180 °C, there was observed a continuous decrease due toits degradation at these temperatures. Opposite kinetics were found for antioxidant capacity, where the highest temperatures (mainly 160 and 180 °C) report highest results, although lower isoflavone content was determined. This could be attributed to Maillard reaction between other compounds release at those temperatures (such as reducing sugars and amino groups). Focusing on isoflavone extraction, 120 °C was selected to treat OKA (as received, with a humidity content of 82.5 %). At 35 min, it was reported 1229.2 μg isoflavones /g OKA, being 210 % higher than using OKW. The study of the composition of the isoflavones chemical forms was also carried out (Fig. 2), showing how malonylglucosides decrease losing their malonyl group and forming βglucosides, that increase even faster, due to the high hydrolizing power of subW; at the same time, aglycones increase slowly without showing high content in any moment. Other novel extraction technologies were applied to OKA, Microwave (895.2 μg /g OKA) and ultrasound extraction (927.5 μg /g OKA), reporting high values of total isoflavone extraction, but lower results (72.8 % and 75.4 %, respectively) than subW (Fig. 3). Besides, subW results present higher ratio βglucoside/malonylglucoside, being the first more noteworthy. As a way to reach an integral valorisation of this by-product, additional subW hydrolysis can be carried out on the solid residue after subW at 120 °C, with the isoflavones from OKA already extracted. Exploring the bioactive potential of algae residue extract via subcritical water extraction: Insights into chemical composition and biological activity
Gelidium sesquipedale is valued in the Spanish agar industry, but its production generates substantial waste, often discarded despite its nutritional and bioactive content. Subcritical water extraction (SWE) at 175 °C and 50 bar for 130 min was performed on this waste after agar extraction, comparing it to conventional ethanol extraction. The SWE extract exhibited superior nutritional profile, including proteins (170.6 ± 1.0 mg/gfreeze-dried-extract), essential amino acids (18.1%), carbohydrates (148.1 ± 0.3 mg/gfreeze-dried-extract), total phenolic content (57 ± 7 mg-EqGA/gfreeze-dried-extract), and also containing Maillard reaction compounds, such as 5-hydroxymethylfurfural, furfural, 2-furanmethanol, 1-(2-furanyl)-ethanone, and 5-methyl-2-furfural, influencing color, aroma and flavor. This extract showed better antioxidant and anti-inflammatory properties than the conventional extract, and higher xanthine oxidase, tyrosinase, and acetylcholinesterase inhibition activities. Toxicological assessment on human cells indicated the safety of the SWE extract. Therefore, SWE technology offers a promising method to valorize G. sesquipedale residue, yielding a bioactive and nutrient-rich extract suitable for food and nutraceutical applications.This work was supported by PT national funds (FCT/MCTES, Fundação para a Ciência e Tecnologia and Ministério da Ciência, Tecnologia e Ensino Superior) through the project UIDP/50006/2020.
