PROGNOSIS OF GERIATRIC COVID-19 PATIENTS ADMITTED TO INTENSIVE CARE UNIT ACCORDING TO VACCINATION STATUS
Introduction: As of June 27, 2022, the COVID-19 pandemic has caused over 540 million infections and 6.3 million deaths. We aimed to investigate the effect of the vaccine on the clinical course of elderly patients hospitalized in the intensive care unit and to determine the prognosis of the patients according to their vaccination status. Materials and Methods: The study included 157 patients over the age of 65. Patients were divided into two groups. The first group consisted of patients who were vaccinated with two doses of CoronaVac, and the second group consisted of patients who were not vaccinated. Demographic data of the patients, prehospital clinical frailty scales, Charlson Comorbidity Indexes, APACHE II scores, laboratory values, and patient prognoses were recorded. Results: Of the 157 patients, 93 (59.2%) were female, and the median age was 76 years (65-99). 96 (61.1%) patients were vaccinated and 61 (38.9%) patients were unvaccinated. Patients were grouped as survivors (n=26) and deceased. We found that APACHE II, prehospital clinical frailty scales, and Charlson Comorbidity Indexes scores were higher in patients who died. There was a significant difference between blood lymphocyte and ferritin levels and survival. The 28-day survival rate was higher and intensive care unit overall survival time were longer in the vaccinated group. Conclusions: We observed that the vaccinated patients had higher survival times and lower mortality rates than those who were not vaccinated. We think that it is important to vaccinate elderly patients and that additional doses may be needed
Temperament Characteristics of Preschoolers and Maternal Acceptance and Rejection Statuses of Their Mothers
The study was conducted to examine the relationship between maternal acceptance-rejection and children's temperament characteristics and whether children's temperament characteristics predicted maternal acceptance-rejection. The data of the study, which employed a predictive correlational design, were obtained from 349 children in the 4-6 age group attending preschool education and their mothers. The data were collected using the Parental Acceptance-Rejection Questionnaire and the Child Behavior Checklist and analyzed using Pearson's Correlation Analysis and Multiple Linear Regression Analysis. As a result of the study, a significant relationship was found between maternal acceptance-rejection and all temperament traits except discomfort, fear, shyness, impulsivity, and approach/positive participation. It was also found that maternal coldness and lack of affection were predicted by perceptual sensitivity, low-intensity pleasure, and smiling and laughter, maternal hostility and aggression were predicted by falling reactivity and soothability, maternal indifference and neglect were predicted by sadness, and maternal undifferentiated rejection was predicted by the temperament characteristics of attentional focusing and anger/frustration
Investigating the correlation of health literacy with eating behavior and health perception in adult individuals
Limited levels of health literacy can have a negative impact on individuals, society and the health care system. This study was conducted to determine the relationship of health literacy among adults to eating behavior and health perception. The population of the study was composed of adults in the age group of 30-60 years in the service regions of 10 Family Health Centers affiliated with the city center of Malatya located in eastern Turkey. Descriptive Information Form, the Adult Health Literacy Scale, the Dutch Eating Behavior Questionnaire, and the Perception of Health Scale were used to collect the data. The data were collected with face-toface interview technique and analyzed by using number, percentage, and linear regression analysis. It was determined that there was a statistically positive linear effect between health literacy and eating behavior (p < 0.05) and between health literacy and health perception (p < 0.001). It was determined that health literacy, health perception, and its subscales accounted for 1.3% (R-2 = .013) of the change in the eating behavior, the dependent variable. It was determined that the eating behavior subscales and health literacy accounted for almost 19% (R-2 = .188) of the change in the health perception, the dependent variable. According to the results of the study, eating behaviors and health perceptions can be improved by increasing the health literacy of the individuals. It is recommended for nurses to provide individuals with trainings increasing their knowledge levels about health information, health literacy and eating behavior and consulting them
Counterfeit Probiotic Drugs
Aim: Counterfeit drugs are a threat to human health worldwide. It can be seen that insufficient research has been conducted on the subject of counterfeit drugs related to potentially addictive drugs seized by the authorities. The aim of this study was to investigate whether or not there are drugs for sale in the Turkish market, which are counterfeit drugs under the heading of probiotics which are defined as micro-organisms with a positive effect on health when taken in certain quantities. Material and Method: A total of 17 different probiotic products in capsule, drops or sachet form, which were permitted for sale, were obtained from randomly selected chemist’s shops in the province of Malatya. Results: There was nothing missing or counterfeit on the packaging of the 17 products examined. When the contents were investigated, no deficiencies or excess micro-organism production was determined in the first 12 products. In product #13, there was no production of one of the mentioned micro-organisms. In products # 14,15, 16, and 17, more than one micro-organism stated in the prospectus could not be obtained. Conclusion: As the checking of products sold in chemist’s but controlled by the Ministry of Food, Agriculture and Livestock is not easy, there is a need for tighter supervision. This can be provided by sending the samples taken during audits to the correct laboratories, making detailed examinations, and evaluation the amount of active substance
The Difficult Differential Diagnosis for A Pediatric Patient with Shwachman-Diamond Syndrome; A Case Report and Literature Review
Shwachman-Diamond syndrome (SDS) is a rare autosomal recessive disease characterized by bone marrow dysfunction, exocrine pancreatic insufficiency and skeletal abnormalities. Persistent or intermittent neutropenia caused by bone marrow hypoplasia is the most common hematological abnormality in SDS. It can be difficult to diagnose the disease that usually occurs in early childhood. SDS should be kept in mind in the differential diagnosis of neutropenic patients. If the signs of pancreatic insufficiency are not observed, the diagnosis may be missed. Investigating export performance of international companies with the Hesitant Fuzzy SWARA
Dünyada ticaretin küreselleşmesi, yeni pazarların ortaya çıkmasıyla birlikte ihracatın önemi de artmıştır. Firmaların ihracata yönelmeleri ve oluşan rekabet ortamında var olabilmeleri için dış pazarda önem vermeleri gereken faktörler vardır. Bu çalışmanın amacı ihracatı etkileyen kriterleri belirlemek ve ihracatta ne derecede önemli olduklarını tespit etmektir. Öncelikle konunun önemine değinmek amacıyla ihracatın faydaları araştırılmıştır, ihracat performansını etkileyen kriterler belirlenmiştir. İhracat performansını hesaplamak için 3 ana kriter ve 16 alt kriter belirlenmiştir ve ihracat alanında 5 uzman kişi ile görüşülmüştür. Kriterlerin önem derecelerinin hesaplanması için dilsel ifadeler kullanılmıştır. Uygulamada kullanılan kriterlerin ağırlıklarını hesaplamak için Tereddütlü Bulanık SWARA yöntemi kullanılmıştır ve firmaların ihracat performansları hesaplanmıştır. Sonuç olarak ihracat yapan firmaların ihracat performansını etkileyen tek bir kriter olmadığı yönetici özelliklerinin, sürdürülebilirliğin ve üretilen ürün özelliklerinin ihracat yapabilmede öne çıkan kriterler olduğu belirlenmiştir.With the globalization of trade in the world and the emergence of new markets, the importance of exports has also increased. There are factors that need to be given importance in the foreign market in order for companies to tend to export and exist in the emerging competitive environment. The aim of this study is to determine the criteria affecting exports and to determine how important they are in exports. First of all, in order to emphasize the importance of the subject, the benefits of export have been researched and the criteria affecting export performance have been determined. In order to calculate export performance, 3 main criteria and 16 sub-criteria were determined and 5 experts in the field of export were interviewed. Linguistic expressions were used to calculate the importance of the criteria. Hesitant Fuzzy SWARA method was used to calculate the weights of the criteria used in the application and the export performances of the firms were calculated. As a result, it has been determined that there is not a single criterion affecting the export performance of exporting companies, managerial characteristics, sustainability and product characteristics are the prominent criteria in exporting
Foreign Language Teacher Competences: A Systematic Review of Competency Frameworks
The aim of this study is to determine competences needed in the language teaching profession. For this aim, six frameworks for language teacher competency are examined. Data is analysed by using content analysis method. The required skills and knowledge are categorized under the aspects learning, learner, teaching, assessment and evaluation, linguistic competences, language awareness, professional development, leadership, culture, school and environment, teaching experience and technology. The results revealed that linguistic and language teaching core competences are crucial competences for language teachers. Yet, the frameworks lay diverse focus on knowledge and skills related to these competences. Out of the results, it is evident that a common framework for language teachers is needed, as all frameworks seem to be insufficient to give a full overview regarding the required competences.Bu çalışmanın amacı, yabancı dil öğretmenleri için gerekli olan yeterliklerin belirlenmesidir. Bu amaç kapsamında altı yabancı dil öğretmeni yeterlik çerçevesi içerik analizi yöntemi ile incelenmiştir. Çerçevelerde, yabancı dil öğretmenleri için gerekli olan yeterlikler bilgi ve beceri açısından incelenmiş ve kategorilere ayrılmıştır. Yeterliklerin, öğrenme, öğrenci, öğretme, ölçme ve değerlendirme, dilsel yeterlik, dilsel farkındalık, profesyonel gelişim, liderlik, kültür, okul ve çevre, öğretim deneyimi ve teknoloji başlıkları altında ele alınabildiği görülmektedir. Elde edilen bulgular, çerçevelerde ana öğretim yeterlikleri için gerekli olan dilsel ve dil öğretme konularında bilgi ve becerilere farklı ağırlıklar koyarak tanımlama yaptığını göstermektedir. İnceleme sonucunda, yabancı dil öğretmenleri için gerekli olan yeterlikler konusunda çerçevelerde uzlaşıya varılmış genel bir standardın olmadığı, bu konuda yabancı dil öğretmenleri için temel yeterlik konularını kapsayacak çerçeve çalışmalarına gereksinim duyulduğu anlaşılmaktadır
THE RELATIONSHIP BETWEEN GLYCAEMIC INDEX, DAILY ENERGY INTAKE, METABOLIC PARAMETERS, AND BODY COMPOSITION IN THE ELDERLY: A CROSS-SECTIONAL STUDY
Introduction: This study aimed to evaluate the relationship between dietary glycaemic index, daily energy intake, metabolic parameters and body composition in elderly individuals.Materials and Methods: This is a cross-sectional study that included 198 elderly individuals who applied to the endocrinology outpatient clinic. Participants were asked to record the foods they consumed on three-day diet forms, which would be collected the following week at the time when also the metabolic parameters would be measured and body composition analyses performed at the polyclinic. Nutrient content was recorded with the BeBIS software. The body composition was analysed by using the bioelectrical impedance method.Results: The mean age of 198 people included in the study was 67.78 +/- 2.76 (65-78). HbA1c, insulin and HOMA-IR, visceral adiposity rates, body fat mass and metabolic age were found to be higher in participants with a dietary glycaemic index >= 70, and the effect of the glycaemic index on these parameters was statistically small. The daily energy intake showed a positive correlation with fasting blood glucose, triglyceride and triglyceride-glucose (TyG) index. Both the daily energy intake and glycaemic index showed a weakly positive correlation with visceral adiposity, metabolic age, body fat mass and body mass index. A dietary glycaemic index >= 70 was observed to increase the risk of obesity OR=3.7 times (95% CI=1.72 - 7.94), and the risk of HbA1C higher than 8 to increase OR=3.13 times (95% CI=1.0 - 9.74).Conclusion: An increase in the dietary glycaemic index and the daily energy intake in the elderly results in poor glycaemic control
Effects of early physical therapy on motor development in children with Down syndrome
OBJECTIVE: The objective of the study was to compare the motor development of children with Down syndrome (DS) who received physical therapy (PT) and did not receive PT, and to show the effect of PT programs started before the age of one on movement development. METHODS: The study included aged between 6 and 42 months, 58 children with DS. Children with DS were divided into two groups as receiving PT and non-receiving PT. Children with DS who received PT were further divided into two groups according to the age of starting PT as before and after 1 year of age. Gross motor and fine motor development of the cases were evaluated with Bayley Scales of Infant and Toddler Development III. RESULTS: Gross motor scaled scores (GM-SS: 3.88±3.46–1.67±1.23), fine motor scaled scores (FM-SS: 4.29±3.24–1.79±0.93), and composite scores (64.4±19.5–50.38±5.38) of PT group were statistically higher than the non-PT group (p<0.05). In addition, GM-SS (5.22±4.23–2.38±1.20), FM-SS; (5.61±3.85–2.81±1.37), and composite scores (72.33±23.85–55.56±5.7) of the cases who started PT before the age of one were statistically higher than those who started after the age of one (p<0.05). CONCLUSION: Our results revealed that PT especially when started early childhood under had a positive effect on the development of gross and fine motor in children with DS and provided a scientific basis for referring children with DS to PT programs before the age of one. Clinicians should recommend PT for children with DS in the early period
Copeptin: A potential marker for the prediction of poor ovarian reserve in the infertile women
Objective: There is actually no evidence regarding the physiological effects of copeptin in infertile women with different ovarian reserve types. This study aimed to investigate the relationship of serum copeptin level with poor ovarian reserve (POR) and to reveal the predictive value of copeptin for POR development in the infertile women. Materials and Methods: All participant women were classified as the control group (n=77) included the women with diagnosis of unexplained infertility and the POR group (n=61) was composed of the women who met the European Society of Human Reproduction and Embryology consensus on POR [serum anti-Mullerian hormone (AMH) concentrations below 1.1 ng/mL]. The biochemical tests, including estradiol (E2), follicle stimulating hormone (FSH), luteinizing hormone, AMH and copeptin were analysed. The analyses of serum copeptin concentrations were measured by the means of competitive enzyme immunoassay. Results: A significant increase in the serum copeptin level existed only in the POR group. There was a significant positive correlation between serum copeptin with E2 and FSH levels in the POR group. Significant negative correlations between copeptin and AMH concentrations (r=-0.310, p=0.015) and between copeptin concentration and antral follicle counts (r=-0.284, p=0.027) were detected only in the POR group. The estimated areas under receiver operating characteristic curves for serum concentration were found to be statistically significant with a cut-off value of 3.52 (95% confidence interval 0.519-0.709), sensitivity 0.90 and specificity 0.72. Conclusion: This study confirmed that there was an elevated serum copeptin concentration in the infertile women with POR and that serum copeptin concentration may have a predictive value for POR diagnosis
