How are the Results of Allogeneic Stem Cell Transplantation in Elderly Patients? A Single-center Experience
INTRODUCTION: In this study, we aimed to present allogeneic hematopoietic stem cell transplantation (allo-HCT) experience in elderly patients with hematological malignancy. METHODS: Thirty-five patients aged 60 years and older who underwent allo-HSCT between 2017 and 2021 were retrospectively analyzed. Patient's demographic/clinical features, and the outcomes of transplantation were reviewed. RESULTS: The median age was 63 (range, 60-74) years and 25 (77,1%) were male. Twenty-seven (60%) were diagnosed with AML, followed by MDS (n: 7, 20%). Twenty-three (65,8%) patients had intermediate, and 6 (17,1%) patients had a high hematopoietic cell transplantation-specific comorbidity index score. Karnofsky performance status of ? 90% was detected in 15 (42,9%) patients. Busulfan plus fludarabine plus anti-thymocyte globulin was used mainly as a reduced-intensity conditioning regimen, which was used in 18 (51,4%) patients. The median duration of neutrophil and platelet engraftments were 18 (range, 11-27) and 18 (range, 11-33) days, respectively. The median follow-up time was 4 months (range, 0-51), with the OS rate %14,2. The transplant-related mortality rate within the first 30 days after allo-HSCT was detected in 10 patients (28,6%) due to infection and/or GvHD. Response assessment could be performed in 25 (71,4%) patients after transplantation. The duration of PFS was 6 (range, 1-51) months in patients with response evaluation. The rate of PFS was 72% in 1 years and 5 (14,2%) patients were still alive with complete response at the last visit. DISCUSSION AND CONCLUSION: Reduced-intensity conditioning regimen has provided the important advantage in allo-HSCT, for elderly patients with hematological malignancies such as AML and MDS
Improved performance of the NaFePO4/Hardcarbon sodium-ion full cell
Chronoamperometry and galvanostatic procedures were applied in the LiFePO4-based ion exchange approach for the production of NaFePO4 cathodes. The materials obtained by various processes were employed as the cathode electrode in Na-ion half cells, and their structural and electrochemical properties were investigated. According to XRD patterns for both processes, it is seen that the Na-ions were successfully replaced with Li-ions. In addition of this, XPS and EDX-dot mapping analyses were supports the ion exchange procedure. The electrochemical properties of the cathode were investigated using CR2032 cells, and the diffusion coefficient and diffusion activation energy were calculated using the CV graphs at various scan rates and environmental temperatures. It was found that the Na-diffusion rate in the cell is faster at higher temperatures than at lower temperatures. The galvanostatic cycling tests were performed for C/10 and 1C rates at room temperature, 10 degrees C, and 50 degrees C and determined that the first capacities for C/10 and 1C at room temperature were 140mAh/g and 60mAh/g, respectively, with good stability. The full cells of NaFePO4/hard carbon were fabricated by pre-sodiation tech-nique and the battery performance tests revealed that the cells have a 74 mAh/g for C/10-rate with a voltage window of 2-4 V.University Research Council [FBA -2021-2417]Prof. Dr. Serdar ALTIN also would like to thank the I ? no ? n? Effects of combined aerobic exercise and diet on cardiometabolic health in patients with obesity and type 2 diabetes: a systematic review and meta-analysis
Background Lifestyle modifications involving diet and exercise are recommended for patients diagnosed with obesity and type 2 diabetes mellitus (T2DM). The purpose of this review was to systematically evaluate the effects of combined aerobic exercise and diet (AEDT) on various cardiometabolic health-related indicators among individuals with obesity and T2DM.Methodology A comprehensive search of the PubMed/Medline, Web of Science, Scopus, Science Direct, Cochrane, and Google Scholar databases was conducted for this meta-analysis. The Cochrane risk of bias tool was used to evaluate eligible studies, and the GRADE tool was used to rate the certainty of evidence. A random-effects model for continuous variables was used, and the results were presented as mean differences or standardised mean differences with 95% confidence intervals.Results A total of 16,129 studies were retrieved; 20 studies were included, and data were extracted from 1,192 participants. The findings revealed significant improvements in body mass index, body weight, waist circumference, systolic blood pressure, diastolic blood pressure, total cholesterol, triglycerides, fasting blood glucose, fasting plasma insulin, glycated hemoglobin, leptin, interleukin-6, C-reactive protein, and adiponectin (p < 0.05) compared to the standard treatment (ST) group. No significant differences were observed between the AEDT and ST groups in fat mass, hip circumference, waist-to-hip ratio, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, and tumor necrosis factor-alpha. The present findings are based on low- to moderate-quality evidence.Conclusions AEDT may be a critical behavior for holistic cardiometabolic health-related benefits as a contemporary anti-obesity medication due to its significant positive impact on patients with obesity and T2DM. Oxidative stress and antioxidants in health and disease
The increase in the formation of reactive oxygen and reactive nitrogen species of endogenous or exogenous origin causes oxidative stress due to pro-oxidant and antioxidant imbalance that causes cellular damage in metabolism. This can increase inflammation of cells, apoptosis and necrosis, damage to DNA base damage, DNA and protein cross-links, lipid membrane peroxidation, and mitochondrial dysfunction. Antioxidants can be described as a system that protects biomolecules and the organism against the harmful effects of free radicals, reduces or repairs the damage done by reactive oxygen species (ROS) to the target molecule, and this is called antioxidant defense. It is known that the mechanisms caused by the increase in ROS resulting from oxidative stress are positively related to the pathology of many diseases such as cancer, metabolic syndrome, atherosclerosis, malaria, Alzheimer's disease, rheumatoid arthritis, neurodegenerative diseases and preeclampsia
Investigation of the dimensional change of magnetite (Fe3O4) magnetic nanomaterials depending on working conditions
Manyetik nanomalzemelerin (MNP) biyomedikal uygulamaları 30 yıldan fazla bir süredir araştırılmaktadır. MNP'ler tıpta; ilaç hedefleme sistemleri, hipertermi, biyosensör, manyetik hücre ayırma ve manyetik rezonans görüntülemede (MRG) kontrast ajanları gibi birçok alanda kullanılmaktadır. Özellikle vücut içi MNP kullanımında manyetik malzemelerin küçük boyutlu olması istenmektedir. Bu malzemelere kaplama yapıldığında ya da ilaç bağlandığında boyutu artmaktadır. MNP ne kadar küçük boyutta üretilirse kaplama sonrası vücut içi kullanım olasılığı o kadar artmaktadır. Bu çalışma MNP'lerin boyut değişiminin çalışma koşullarına bağlı değişimini incelemek amacıyla yapılmıştır. Birlikte çökelme yöntemi ile elde edilecek manyetik nanomalzemelerin pH, sıcaklık, karıştırma hızı ve kullanılan bazın türü gibi koşullar değiştirilerek boyut değişimi izlenmiştir. Ayrıca bu boyut değişimleri manyetik nanomalzemelerin termal özelliklerini, mekanik özelliklerini, optik özelliklerini elektrik özelliklerini ve manyetik özelliklerini değiştirecektir. Yeni özellikler yeni uygulama alanları doğuracaktır. Bu değişimler sayesinde hangi koşullarda en iyi özelliklere sahip olan ve en küçük boyutta manyetik nanomalzemenin oluştuğu tespit edilmiştir. Farklı çalışma koşulları kullanılarak elde edilen örneklerin karakterizasyon testlerinde; XRD ile sentezlenen MNP'lerin manyetit (Fe3O4) olup olmadığı kontrol edilmiştir. FESEM, demir oksit çekirdeğinin morfolojisini gözlemlemeye ve gerçek çapını tahmin etmeye izin verir. SEM analizinde MNP' lerin küresel yapıda olup olmadıkları ve MNP çekirdeklerin çapları dolayısıyla boyutları ölçülmüştür. MNP' lerin ön boyut incelenmesi dispersiyon durumundaki nanoparçacıkların hidrodinamik çapını ölçen dinamik ışık saçılımı (DLS) ile karakterize edilmiştir. VSM analizinde oda sıcaklığında elde edilen manyetizasyonun, manyetik alanla değişimine bakılarak MNP' lerin manyetik davranışları belirlenmiştir. Tanecikler arasındaki itme ve çekme değerine etkisi olan yüzey yükü zeta (ζ) potansiyel analizi ile karakterize edilmiştir.The biomedical applications of magnetic nanomaterials (MNPs) have been explored for more than 30 years. MNPs have been used in various areas such as in medicine, drug targeting systems, hyperthermia, biosensors, magnetic cell separation and contrast agents in magnetic resonance imaging (MRI). Especially in the use of in-body MNP, magnetic materials are desired to be small in size. These materials increase in size when coating or drug binding. The smaller the MNP is produced, the higher the possibility of in-body use after coating. This study has been carried out to examine the size change of MNPs depending on the working conditions. The size change of the magnetic nanomaterials to be obtained by the co-precipitation method has been monitored by changing the conditions such as pH, temperature, stirring speed and the type of base used. In addition, these dimension changes will change the thermal properties, mechanical properties, optical properties, electrical properties and magnetic properties of magnetic nanomaterials. New features will create new application areas. Thanks to these changes, it has been determined under which conditions the magnetic nanomaterial with the best properties and the smallest size is formed. In the characterization tests of the samples obtained using different working conditions; it has been checked whether the MNPs synthesized by XRD were magnetite (Fe3O4). FESEM allows to observe the morphology of the iron oxide core and to estimate its true diameter. In the SEM analysis, whether the MNPs are spherical or not and the diameters of the MNP nuclei are measured. Pre-size examination of MNPs has been characterized by dynamic light scattering (DLS), which measures the hydrodynamic diameter of dispersed nanoparticles. In the VSM analysis, the magnetic behavior of the MNPs has been determined by looking at the change of the magnetization obtained at room temperature with the magnetic field. Formülsüz Fen Eğitimi: Basit Makineler Örneği
Bu araştırmada, ortaokul 8. sınıf öğrencilerinin doğrudan formül kullanmadan sınıf içerisinde yapılan etkinlikler ile matematiksel ifadelere aşamalı olarak ulaşmalarının sağlanması ve bu aşamaların konunun öğretimindeki etkisinin araştırılması amaçlanmıştır. Araştırmanın çalışma grubunu Adıyaman ili Çelikhan ilçesindeki bir ortaokulda öğrenim gören 15 (10 kız, 5 erkek) 8. sınıf öğrencisi oluşturmaktadır. Deney grubunda 7, kontrol grubunda 8 öğrenci bulunmaktadır. Araştırma ön test son test kontrol gruplu yarı deneysel desene göre tasarlanmıştır. Kontrol grubuna 5E modeli uygulanırken, deney grubunda 5E modelinin keşfetme basamağında Tasarım Temelli Fen Eğitimi (TTFE) kapsamında öğrencilerin tasarlanan araçlardan yola çıkarak ölçümler yapıp veriler elde etmeleri ve matematiksel ifade türetmeleri istenmiştir. Değerlendirme aşamasında “Basit Makineler Başarı Testi” her iki gruba son test ve dört hafta geçtikten sonra kalıcılık testi olarak uygulanmıştır. Verilerin analizinde bağımsız iki grup arasında fark olup olmadığını belirlemek için Mann Whitney U ve Wilcoxon testleri uygulanmış ve p<0.05 düzeyi anlamlı kabul edilmiştir. İstatistiksel analizler sonucunda deney grubuna ait ön-test son-test puan farkının daha fazla olduğu, ancak kontrol ve deney grupları arasında anlamlı bir farklılığın görülmediği belirlenmiştir. Ayrıca sıra ortalamaları ve sıra toplamlarına göre analiz sonuçlarının deney grubu lehine olduğu gözlenmiştir. Kalıcılık testi sonuçlarına göre ise deney grubu ve kontrol grubuna ait son test puan ortalamaları ile kalıcılık testi puan ortalamalarının birbirine yakın olduğu gözlenmiştir. Optimization with artificial intelligence of the machinability of Hardox steel, which is exposed to different processes
In this study, different process types were processed on Hardox 400 steel. These processes were carried out with five different samples as heat treatment, cold forging, plasma welding, mig-mag welding and commercial sample. The aim here is to determine the changes in properties such as microstructure, microhardness and conductivity that occur in the structure of hardox 400 steel when exposed to different processes. Then, the samples affected by these changes were processed in WEDM with the box-behnken experimental design. Ra, Kerf, MRR and WWR results were analyzed in Minitab 21 program. In the continuation of the study, using these data, a prediction models were created for Ra, Kerf, MRR and WWR with Deep Learning (DL) and Extreme Learning Machine (ELM). Anaconda program Python 3.9 version was used as a program in the optimization study. In addition, a linear regression models are presented to comparison the results. According to the results the lowest Ra values were obtained in heat-treated, cold forged, master sample, plasma welded and mig-mag welded processes, respectively. The best Ra (surface roughness) value of 1.92 mu m was obtained in the heat treated sample and in the experiment with a time off of 250 mu s. Model F value in ANOVA analysis for Ra is 86.04. Model for Ra r2 value was obtained as 0.9534. The lowest kerf values were obtained in heat-treated, cold forged, master sample, plasma welded and mig-mag welded processes, respectively. The best kerf value of 200 mu was obtained in the heat treated sample and in the experiment with a time off of 200 mu s. Model F value in ANOVA analysis for Kerf is 90.21. Model for Kerf r2 value was obtained as 0.9555. Contrary to Ra and Kerf, it is desirable to have high MRR values. On average, the highest MRR values were obtained in mig-mag welded, plasma welded, cold forged, master sample and heat-treated processes, respectively. The best mrr value of 200 gmin- 1 was obtained in the mig-mag welded sample and in the experiment with a time off of 300 mu s. Model for MRR r2 value was obtained as 0.9563. The lowest WWR values were obtained in heat-treated, cold forged, master sample, plasma welded and mig- mag welded processes, respectively. The best wwr value of 0.098 g was obtained in the heat treated sample and in the experiment with a time off of 200 mu s. Model F value in ANOVA analysis for WWR is 92.12. Model for wwr r2 value was obtained as 0.09561. In the analysis made with artificial intelligence systems; The best test MSE value for Ra was obtained as 0.012 in DL and the r squared value 0.9274. The best test MSE value for kerf was obtained as 248.28 in ELM and r squared value 0.8676. The best MSE value for MRR was obtained as 0.000101 in DL and the r squared value 0.9444. The best MSE value for WWR was obtained as 0.000037 in DL and the r squared value 0.9184. As a result, it was concluded that different optimization methods can be applied according to different outputs (Ra, Kerf, MRR, WWR). It also shows that artificial intelligencebased optimization methods give successful estimation results about Ra, Kerf, MRR, WWR values. The effect of spinal anesthesia that is performed in sitting or right lateral position on post-spinal headache and intraocular pressure during elective cesarean section
Background: Although spinal anesthesia can be applied in different patient positions, the most frequently used positions are sitting and lateral positions. It is known that different patient positions during spinal anesthesia have effects on hemodynamic parameters, postdural puncture headache, and intraocular pressure.Aim: The study aimed to determine the effect of spinal anesthesia performed in either sitting or right lateral position on postspinal headache and intraocular pressure during elective cesarean section. Patients and Methods: The study was a randomized controlled study of 104 eligible pregnant women scheduled to undergo elective cesarean section. The women were randomized into two groups. Spinal anesthesia was performed either in the sitting (Group S, n = 53) or the right lateral position (Group L, n = 51). Heart rate and blood pressure were recorded throughout the operation. The participants were informed and monitored for postspinal headaches. Intraocular pressure before and after the operation was measured with Icare PRO. The obtained data were statistically compared between the two groups.Results: There was no difference between the groups in terms of demographic data. Postdural puncture headache was observed in five patients in Group S and one patient in Group L (P =0.04). There was no difference between the groups in terms of intraocular pressure (P >.05). Heart rate was not significantly different between the groups; however, there was a significant difference in average blood pressure in 1, 5, 30, and 40 minutes (P <.05). The number of trials administered to patients for spinal anesthesia was significantly higher in Group L (P =0.01).Conclusion: Spinal anesthesia performed in the sitting position for cesarean section caused a higher postspinal headache than in the right lateral position, but the position did not affect intraocular pressure

Yüce Rabbimiz, insanlara cenneti kazandıracak rehber kitap olarak Kur'ân-ı Kerim'i göndermiştir. Kur'ân-ı Kerim'i insanlara zorluk olarak göndermediğini, aksine sakınanlar için bir öğüt olarak indirdiğini ilahi kelamında bizlere açıklamıştır. Kullar da bu ilahi kelamı anlamak, daha iyi kavramak ve insanlara daha iyi anlatıp hidayeti daha kolay bulmalarını sağlamak amacıyla birçok eserler kaleme almışlardır. Bu eserlerin bazıları Kur'ân-ı Kerim'i daha iyi anlamaya yönelik olan Tefsir eserleri, bazıları Kur'ân-ı Kerim'i daha iyi okuyabilmek için kıraat ve tecvit eserleri gibi Kur'ân-ı Kerim'in ışığında birçok ilim dalı ortaya çıkmıştır. Bu ilim dallarının birbirine etkileri oldukça fazladır. Birbirini tamamlayan ilimlerdir. İlk dönemlerden itibaren ayrı ilim dalları olarak değerlendirilen tefsir ve kıraat ilimleri aslında birbirleriyle iç içedirler. İlk dönem tefsir eserlerine bakıldığında bu açıkça görülebilmektedir. Birbiriyle bu kadar bağlantılı olan bu iki ilim birbirine anlam bakımından yardımcı konumundadır. Kur'ân'ın, çağlar ötesi mesajlarının kullara doğru bir şekilde ulaşması, onun doğru okunması ve anlamının doğru açıklanması yani tefsir edilmesi ile mümkün olmaktadır. Bu mesajların anlaşılıp, açıklanması konusunda kıraat ilminin payı da pek büyüktür. Geçmişten beri insanlar Kur'ân-ı Kerim'i anlamanın yanında onun okunuş ve mûsikî alanına ilgi göstermişlerdir. Bu alan da genel çerçeve de kıraat ilmiyle alakalı olmaktadır. Bu araştırmada Hicrî 7.asrın büyük âlimlerinden olan büyük müfessir Beyzâvî (ö. 685/1286)'nin tefsiri olan Envârü't-Tenzîl ve Esrârü't-Te'vîl'de Bakara ve En'âm Sûreleri özelinde kıraatlerin tefsire etkisi incelenmiştir.Our Almighty Lord has sent the Qur'an as the guide book that will bring people to Paradise. Many branches of science have emerged in the light of the Qur'an, such as Tafsir works, some of which are aimed at better understanding the Qur'an, and some of them, such as recitation and tajvid works in order to be able to read the Qur'an better. out. These branches of science have a lot of influence on each other. These are sciences that complement each other. The sciences of tafsir and recitation, which have been considered as separate branches of science since the early periods, are actually intertwined with each other. This can be clearly seen when looking at the first period exegetical works. These two sciences, which are so related to each other, are in the position of helping each other in terms of meaning. The correct transmission of the messages of the Qur'an beyond the ages to the servants is possible by reading it correctly and explaining its meaning correctly, that is, by interpreting it. The share of the science of recitation in understanding and explaining these messages is also great. Since the past, people have shown interest in its recitation and music, as well as understanding the Qur'an. Both this field and the general framework are related to the science of recitation.
