Acute kidney injury and peritoneal dialysis in extremely low birth weight newborns
BACKGROUND: In 12.5-56% of extremely low birth weight (ELBW) infants treated in newborn units, acute kidney injury (AKI) develops. Some of these infants may need renal replacement therapy for several reasons including hyperkalemia, hypovolemia and resistant acidosis. METHODS: All ELBW infants who were followed in our hospital between January 2015 and December 2017 and who lived longer than 48 hours were assessed. Patients were followed for AKI and peritoneal dialysis (PD). RESULTS: AKI developed in 25 of 201 ELBW infants. PD was administered to nine patients. PD was initiated at a median of 11 days (2-22 days) for all patients due to hyperkalemia which did not respond to medical treatment. Three of the nine infants who received PD died while dialysis was ongoing. The remaining six patients completed PD successfully. In these patients, the serum potassium value returned to normal in three days, and dialysis was continued for a median of 93 hours (40-172 hours). Dialysis leakage occurred in two patients, and hyperglycemia developed in two patients. On average, diuresis started at the 25(th) hour (8-40(th) hour). CONCLUSIONS: In the renal failure treatment of ELBW infants, PD is the only option which can be used for many units. It was found that in ELBW infants, who had wider peritoneal surface when compared to their body weight, biochemical values recovered rapidly with PD, and diuresis started a short while later in most patients

On Some Topological and Geometric Properties of Some q-Cesaro Sequence Spaces
Mathematical concepts are aesthetic tools that are useful to create methods or solutions to real-world problems in theory and practice, and that sometimes contain symmetrical and asymmetrical structures due to the nature of the problems. In this study, we investigate whether the sequence spaces X-q(p), 0= p<8, and X8, which are constructed by q-Cesaro matrix, satisfy some of the further properties described with respect to the bounded linear operators on them. More specifically, we answer to the question: Which of these spaces have the Approximation, Dunford-Pettis, Radon-Riesz and Hahn-Banach extension properties?. Furthermore, we try to investigate some geometric properties such as rotundity and smootness of these spaces
Estimation of service length with the machine learning algorithms and neural networks for patients who receiving home health care
The main purpose of the study is to develop an estimation model using machine learning algorithms and to ensure the effective and efficient implementation of home health care service planning in hospitals with these algorithms. The necessary approvals for the study were obtained. The data set was created by obtaining patient data (except for data such as Turkish Republic identification number) from 14 hospitals providing Home Health Care Services in the city of Diyarbakir. The data set was subjected to necessary pre-processing and descriptive statistics were applied. For the estimation model, Decision Tree, Random Forest and Multi-layer Perceptron Neural Network algorithms were used. It was found that the number of days of home health care service, which the patients received, varied depending on their age and gender. It was observed that the patients were generally in the disease groups that required Physiotherapy and Rehabilitation treatments. It was determined that the length of service for patients can be predicted with a high reliability rate (Multi-Layer Model Acc: 90.4%, Decision Tree Model Acc: 86.4%, Random Forest Model Acc: 88.5%) using machine learning algorithms. In the light of the findings and data patterns obtained in the study, it is thought that effective and efficient planning will be made in terms of health management. In addition, it is believed that estimating the average length of service for patients will contribute to strategic planning of human resources for health, and to reducing medical consumables, drugs and hospital expenses
Physical and chemical characterization of femur during and after body development period in male and female rats
The aim of this study was to determine the physical and chemical characteristics of the bone structures during four developmental periods. Forty Wistar Albino rats (20 male and 20 female) were divided into four groups including prepubertal period (group I), period between adolescence and adulthood (group II) and later (young adult period as group III and old adult period as group IV). The bones were analysed by morphometric measurements, XRF (X-ray fluorescence) analysis for mineral levels and BET analysis (Brunaurer-Emmett-Teller) for surface area and porosity. In morphometric measurements, the GL (greatest length) and the GLC (greatest length from caput femoris) values increased gradually from the first to the fourth group, and these values were higher in the males than the females. Phosphorus and calcium values were higher in the males in adult groups (third and fourth) compared to that for the females; however, they were higher in the females in groups up to adulthood (first and second). While the Ca/P ratio reached the highest value in the second group in the male, it decreased gradually afterwards. In females, the rate, which was close to each other in the first three groups, increased in the fourth group. Surface area size in the female and the male rats was the highest in the second group. In conclusion, changes in the rat bone structure during the development and adulthood periods of the body were revealed, and it was determined that the gender factor was effective in these changes.Tuerkiye Bilimsel ve Teknolojik Arastirma KurumuTuerkiye Bilimsel ve Teknolojik Arastirma Kurum
Las fases iniciales de la psicosis. ¿Se han olvidado de Klaus Conrad?
The first phases of psychosis have been the object of psychopathological attention, with special emphasis from the contributions of Jaspers and his school on. It is traditionally known that the evaluation of these phases requires an attentive and careful clinical examination. However, at the beginning of this century a current of biomedical enthusiasm led to the conviction that it was possible to identify people at risk of psychosis by means of biomarkers. In this way, preventive measures could be applied in order to avoid progression. Early-stage or first-episode programs spread widely. The promoters of these programs made a clean slate of previous psychopathological knowledge and searched in a natural space in which they found nothing. The keys to understanding psychosis are in the experience, not in biomarkers. Klaus Conrad made essential contributions to identify and describe the experiential phenomena that appear in the early stages of psychosis. Despite the time that has passed and neuroscientific contributions, they remain valid and up-to-date.Las primeras fases de la psicosis han sido objeto de atención psicopatológica, con especial empuje a partir de las aportaciones de Jaspers y su escuela. Tradicionalmente, se sabe que la evaluación de estas fases requiere de una exploración clínica atenta y cuidadosa. Sin embargo, a principios de este siglo una corriente de entusiasmo biomédico llevó al convencimiento de que era posible identificar a las personas en riesgo de psicosis mediante biomarcadores. De esta forma se podrían aplicar medidas preventivas para evitar su progresión. Los programas de fases iniciales o de primeros episodios se extendieron profusamente. Los promotores de estos programas hicieron tabla rasa del saber psicopatológico previo y buscaron en un espacio natural donde no encontraron nada. Las claves para entender la psicosis están en la vivencia, no en los biomarcadores. Klaus Conrad hizo aportaciones indispensables para identificar y describir los fenómenos vivenciales que aparecen en las primeras etapas de la psicosis. A pesar del tiempo transcurrido desde entonces y de los hallazgos neurocientíficos, conservan vigencia y actualidad
Efectos de la neurorrehabilitación sobre el trastorno del espectro alcohólico fetal: un estudio de caso único
The present investigation consisted of a single case study of a 10-year-old child from international adoption with symptoms of Fetal Alcohol Spectrum Disorder (FASD). Due to the symptomatological complexity of the disorder and its diagnosis, currently there are no effective treatments. The study started with a neurocognitive evaluation where alterations in attention, memory and executive functions were observed, which manifested themselves in great impulsivity, as well as in difficulties in self-regulation. A 96-session neurorehabilitation program was designed, focusing on a multidisciplinary approach to the altered cognitive, behavioral and functional aspects. The application of the rehabilitation program produced a general improvement in the patient's functional capacity, specially in attention processes and the capacity for self-regulation. The results obtained highlight the value of the application of multidisciplinary neurorehabilitation programs as an effective tool in the treatment of the clinical symptoms of FASD and the improvement in the adaptation of patients to the demands of their daily lives.La presente investigación consistió en un estudio de caso único de un niño de 10 años, procedente de adopción internacional, con síntomas compatibles con trastorno del espectro alcohólico fetal (TEAF). Debido a la complejidad sintomatológica del trastorno y de su diagnóstico, en la actualidad no existen tratamientos contrastados. El estudio comenzó con una evaluación neurocognitiva donde se observaron alteraciones en atención, memoria y funciones ejecutivas, que se manifestaron en una gran impulsividad, así como en dificultades para la autorregulación. Se diseñó un programa de neurorrehabilitación, de 96 sesiones, centrado en el abordaje multidisciplinar de los aspectos cognitivos, conductuales y funcionales alterados. La aplicación del programa de rehabilitación produjo mejora general en la capacidad funcional del paciente, destacando los procesos de atención y en la capacidad de autorregulación. Los resultados obtenidos ponen en valor la aplicación de programas de neurorrehabilitación multidisciplinares al revelarse como una herramienta eficaz en el tratamiento de la sintomatología clínica del TEAF y la mejora en la adaptación de los pacientes a las exigencias de su vida diaria
THE TURKISH VERSION OF BODY UNDERSTANDING MEASURE FOR PREGNANCY SCALE: VALIDITY AND RELIABILITY STUDY
The objective of this study was to adapt the Body Understanding Measure for Pregnancy Scale into the Turkish language and to examine the psychometric properties in pregnancy. The methodological study was conducted with 508 healthy pregnant women. Language validation, content validation, and pilot scheme are the three phases of cultural adaptation of the scale. The construct validity of the scale was analyzed using both Exploratory and Confirmatory Factor Analyses. The reliability of the scale was tested using parallel-form reliability, test-retest reliability, Cronbach's ? coefficient of internal consistency, and item-total correlation. The Confirmatory Factor Analysis conducted supports the three-factor structure of the scale that is constructed by Exploratory Factor Analysis. We determined Cronbach’s ? coefficient of internal consistency to vary between 0.78 and 0.88 for the scale total and the subscales. Besides, there was a high correlation between the parallel-form reliability, test-retest reliability, and item-total correlation of the scale. The Turkish version of Body Understanding Measure for Pregnancy Scale is a validated and reliable tool designed to evaluate the body image concerns of Turkish pregnant women
