Cateterização venosa central guiada por ultrassom --- abordagem ‘‘Syringe-Free’’
Background and objectives
Central venous catheterization of the internal jugular vein is a commonly performed invasive procedure associated with a significant morbidity and even mortality. Ultrasound-guided methods have shown to significantly improve the success of the technique and are recommended by various scientific societies, including the American Society of Anesthesiologists. The aim of this report is to describe an innovative ultrasound-guided central line placement of the internal jugular vein.
Technique
The authors describe an innovative ultrasound-guided central line placement of the internal jugular vein based on an oblique approach – the “Syringe-Free” approach. This technique allows immediate progression of the guide wire in the venous lumen, while maintaining a real-time continuous ultrasound image.
Conclusions
The described method adds to the traditional oblique technique the possibility of achieving a continuous real-time ultrasound-guided venipuncture and a guide wire insertion that does not need removing the probe from the puncture field, while having a single operator performing the whole procedure
Impact of splenic artery ligation after major hepatectomy on liver function, regeneration and viability
It was reported that prevention of acute portal overpressure in small-for-size livers by inflow modulation results in a better postoperative outcome. The aim is to investigate the impact of portal blood flow reduction by splenic artery ligation after major hepatectomy in a murine model. Forty-eight rats were subjected to an 85% hepatectomy or 85% hepatectomy and splenic artery ligation. Both groups were evaluated at 24, 48, 72 and 120 post-operative hours: liver function, regeneration and viability. All methods and experiments were carried out in accordance with Coimbra University guidelines. Splenic artery ligation produces viability increase after 24 h, induces a relative decrease in oxidative stress during the first 48 hours, allows antioxidant capacity increment after 24 h, which is reflected in a decrease of half-time normalized liver curve at 48 h and at 72 h and in an increase of mitotic index between 48 h and 72 h. Splenic artery ligation combined with 85% hepatectomy in a murine model, allows portal inflow modulation, promoting an increase in hepatocellular viability and regeneration, without impairing the function, probably by inducing a less marked elevation of oxidative stress at first 48 hours
Sequential Morphological Changes in the CNV Net after Intravitreal Anti-VEGF Evaluated with OCT Angiography
PURPOSE:
To assess and describe sequential morphological changes in the choroidal neovascularization (CNV) net using optical coherence tomography angiography (OCTA) in patients undergoing treatment with intravitreal antivascular endothelial growth factor (VEGF).
METHODS:
Prospective cohort study. OCTA was performed sequentially: before (t0), 1 h (t1), 1 week (t2) and 1 month after the injection (t3), using Avanti RTVue XR equipped with the AngioVue® software (Optovue, Calif., USA). All images were classified by two independent graders.
RESULTS:
Ten eyes of 10 patients, with a mean age of 72.4 ± 10.5 years, were included. CNV morphology was described as tree-like in 5 eyes, glomerular in 1 and fragmented in 4. A fibrovascular capsule surrounding the CNV net was found in 4 eyes and a feeder trunk was noticed in 6. No changes were observed at t1. Loss of peripheral capillaries, vessel fragmentation and decreased vessel density were evident in 8 eyes at t2. The CNV capillary density and the peripheral anastomosis increased in all of these at t3. Two eyes remained unchanged through the whole length of follow-up.
CONCLUSIONS:
Significant changes in the CNV net can be observable in OCTA at least 1 week after intravitreal anti-VEGF. The safety of frequent examinations may provide a method of gauging treatment effects

Fracture characterization of human cortical bone under mode II loading using the end-notched flexure test
Fracture characterization of human cortical bone under mode II loading was analyzed using a miniaturized version of the end-notched flexure test. A data reduction scheme based on crack equivalent concept was employed to overcome uncertainties on crack length monitoring during the test. The crack tip shear displacement was experimentally measured using digital image correlation technique to determine the cohesive law that mimics bone fracture behavior under mode II loading. The developed procedure was validated by finite element analysis using cohesive zone modeling considering a trapezoidal with bilinear softening relationship. Experimental load-displacement curves, resistance curves and crack tip shear displacement versus applied displacement were used to validate the numerical procedure. The excellent agreement observed between the numerical and experimental results reveals the appropriateness of the proposed test and procedure to characterize human cortical bone fracture under mode II loading. The proposed methodology can be viewed as a novel valuable tool to be used in parametric and methodical clinical studies regarding features (e.g., age, diseases, drugs) influencing bone shear fracture under mode II loading
Aortic Valve Disease and Vascular Mechanics: Two-Dimensional Speckle Tracking Echocardiographic Analysis
PURPOSE:
Degenerative aortic valve disease (AVD) is a complex disorder that goes beyond valve itself, also undermining aortic wall. We aimed to assess the ascending aortic mechanics with two-dimensional speckle tracking echocardiography (2DSTE) in patients with aortic regurgitation (AR) and hypothesized a relationship with degree of AR. Aortic mechanics were then compared with those of similarly studied healthy controls and patients with aortic stenosis (AS); finally, we aimed to assess the prognostic significance of vascular mechanics in AVD.
METHODS:
Overall, 73 patients with moderate-to-severe AR and 22 healthy subjects were enrolled, alongside a previously examined cohort (N = 45) with moderate-to-severe AS. Global circumferential ascending aortic strain (CAAS) and strain rate (CAASR) served as indices of aortic deformation; corrected CAAS was calculated as CAAS/pulse pressure (PP). Median clinical follow-up was 438 days.
RESULTS:
In patients with severe (vs. moderate) AR, CAASR (1.53 ± 0.29/sec vs. 1.90 ± 0.62/sec, P 0.88/sec was significantly higher (log rank, 97.4% vs. 73.0%; P = 0.03).
CONCLUSIONS:
Quantitative measures of aortic mechanics were lower for AS patients, suggesting a more significant derangement of aortic elastic properties. In the context of AVD, vascular mechanics assessment proved useful in gauging clinical prognosis.info:eu-repo/semantics/publishedVersio
Síndroma compartimental aguda ou das locas: Conceitos essenciais
A síndroma compartimental aguda define-se como sendo a falência da circulação intersticial por conflito de espaço. Representa, habitualmente, uma severa complicação das lesões traumáticas, podendo envolver potencialmente qualquer compartimento do organismo. Mais frequente em fraturas de alta energia cinética e em lesões por esmagamento dos membros, é uma verdadeira emergência ortopédica, por forma a conseguir-se a redução da pressão intracompartimental e restabelecer a perfusão dos tecidos em sofrimento.
Assim, a fasciotomia cirúrgica descompressiva, em tempo oportuno, pode evitar a isquémia irreversível dos tecidos mioneurais contidos nos diferentes compartimentos osteomiofasciais, isto é, as retrações fibrosas musculares, a necrose muscular, as lesões nervosas permanentes, a perda permanente da função e, como última consequência, a amputação dos membros.
A definição da melhor altura para a fasciotomia nem sempre é uma tarefa fácil, envolvendo critérios clínicos e, também, a determinação do valor da pressão intracompartimental, mormente nos doentes em estado de inconsciência.
Nos casos em que houver dúvidas e nos doentes em risco de desenvolver esta complicação, é defensável efetuar uma fasciotomia precoce, mesmo sabendo que pode ser desnecessária, uma vez que as complicações relacionadas com as fasciotomias tardias são devastadoras. Com efeito, o risco de infeção e de septicémia são elevados, a taxa de amputações é considerável e, por vezes, perde-se a vida. As sequelas da síndrome compartimental aguda são uma das causas frequentes de litigância contra os cirurgiões ortopedistas
