Survey of genomic features putatively contributing for antimicrobial resistance in Neisseria gonorrhoeae

    No full text
Globally, the impact of gonorrhea and antimicrobial resistance (AMR) in Neisseria gonorrhoeae are major public health concerns. This master's dissertation aimed to analyze the genetic diversity and AMR markers of N. gonorrhoeae strains circulating in Portugal between 1983 and 2022. First, AMR trends were evaluated from two collections of N. gonorrhoeae strains from a national network of laboratories, known as the "Legacy Collection" (n= 714) and PTGONONET (n= 5339). Within this dataset, it was observed that 46% of the strains showed resistance to ciprofloxacin, and from 2020 onwards, there was a significant increase in resistance to azithromycin until 2022. An analysis of trends in the circulation of genogroups over time was conducted, investigating the relative frequency of the ten most prevalent genogroups in Portugal from 1983 to 2022. The results revealed fluctuations in the genogroups over this period. The integration of the N. gonorrhoeae genomes from Portugal into a public European dataset allowed a detailed analysis of genetic diversity and genogroup circulation, revealing that the genogroups G002.001 and G001.002 not only predominate in Portugal but also in other parts of Europe. The second achieved objective was the development of an efficient technique to capture AMR markers directly from clinical samples through the application of an NGS amplicon-based scheme. This methodology was then applied to a set of 367 clinical samples and proved efficient in amplifying the target genes. Despite the reduced amplification rates observed in the folP and 23S rRNA genes, the overall amplification rate of the genes was high, around 84%. An in silico analysis was performed on clinical samples from different anatomical sites to identify antimicrobial resistance profiles. It was observed that more than 50% of the samples from all anatomical sites showed resistance or intermediate susceptibility to tetracycline, azithromycin, and ciprofloxacin. About cephalosporins, molecular data indicated a potentially reduced susceptibility. This study provided a detailed analysis of the evolution of antimicrobial resistance in N. gonorrhoeae in Portugal over nearly four decades. This historical analysis is essential for understanding the development of antibiotic resistance. One of the most innovative aspects of this dissertation is the implementation and validation of an NGS amplicon-based methodology.

In 2018, the National Prison Monitoring Bank - BNMP 2.0 was instituted and regulated, for the registration of arrest warrants and other documents. This research aims to propose the use of existing information in the National Bank for Prison Monitoring (BNMP) in Brazil by the Portuguese security forces and services, through a cooperation agreement between the two countries. The study is therefore related to the Public Security System (SSP) in Brazil and the Internal Security System (SSI) in Portugal. It is intended to demonstrate the importance and advantages of using BNMP information in Portugal to prevent citizens from Brazil who have a valid arrest warrant and not served in that country from being able to enter the national territory or, if it has already occurred, from being detained. As it is a database available on the world wide web on open data, the information can be consulted freely. However, in order to make access and consultation official, a cooperation agreement between the authorities of the two nations would be important, which could also include the training of Portuguese security forces and services. It must be considered that the Portuguese Republic would be the biggest beneficiary of a cooperation agreement for the use of information because it would have no cost to access and research the system, since the BNMP has been implemented since 2011 in Brazil

Exploring the link between iron availability and fluconazole activity against Candida glabrata

    No full text
"Invasive fungal infections (IFIs) represent a significant health concern worldwide, as recently acknowledged by the World Health Organization with the release of the first priority list of fungal pathogens. Fluconazole remains the most frequently prescribed antifungal for treating IFIs due to its affordability, oral bioavailability, broad spectrum of activity, and safety profile. This triazole drug targets lanosterol 14α-demethylase, Erg11, an enzyme that catalyzes the conversion of lanosterol to ergosterol, whose inhibition arrests cell growth by causing the accumulation of toxic sterols and depletion of ergosterol, which together compromise membrane integrity. Iron overload has been shown to promote fungal growth and is considered a significant risk factor for IFIs. Preliminary results from our laboratory suggest that iron-loading conditions reduce the antifungal activity of fluconazole against Candida glabrata, the second most prevalent cause of these infections.(...)"N/

ABSTRACT - Background: It is estimated that 47% of the global population cannot afford a healthy diet. The number of individuals facing food insecurity and hunger is rising, leading to an economic and food crisis. Food assistance programmes (FAPs) play a vital role in providing a safety net for individuals and families, significantly impacting their well-being. Therefore, the study aims to understand the overall impact of FAPs on secondary economic indicators. Methods: A systematic review was conducted to identify relevant literature. PubMed, Web of Science, Scopus, Cochrane, and JSTOR databases were used. The search scope encompassed peer-reviewed journal articles published in English and Portuguese between 2013 and March 2023. Results: The search yielded 4,189 articles, of which 13 met the inclusion criteria and covered different forms of FAPs across the USA, UK, Rwanda, and East Africa. Most studies employed quasi-experimental designs, while two employed cluster randomized trials and three employed quantitative methodologies. Eleven studies were assessed to be of moderate quality. The various forms of FAPs included in the review were found to have complex effects on different outcomes, although FAPs was found to have a positive effect on increasing employment and total income for households. Conclusion: When effectively designed and implemented, FAPs have the potential to break the cycle of poverty. They address immediate food insecurity and promote economic stability for families.

Understanding how Streptococcus pneumoniae adapts to stresses imposed by innate immunity

    No full text
"Streptococcus pneumoniae is a commensal microorganism in the human nasopharynx that can become pathogenic when invading sterile body regions, leading to difficult-to-treat infections due to strain diversity and antibiotic resistance. The innate immune response to S. pneumoniae is primarily driven by phagocytes like macrophages, which generate antimicrobial reactive oxygen species (ROS) and reactive nitrogen species (RNS). To survive, S. pneumoniae has evolved mechanisms to resist these stresses and relies heavily on glucose metabolism for survival at infection sites. While phosphotransferase systems (PTS) are linked to stress resistance in other Gram-positive bacteria, their role in the pneumococcus remains poorly understood.(...)
