    The Multi-Country Multi-City Collaborative Research Network: An international research consortium investigating environment, climate, and health

    The MCC Collaborative Research Network- Collaborators: Instituto Nacional de Saúde Dr. Ricardo Jorge, Portugal (Susana das Neves Pereira da Silva, Joana Madureira).Short CommunicationsResearch on the health risks of environmental factors and climate change requires epidemiological evidence on associated health risks at a global scale. Multi-center studies offer an excellent framework for this purpose, but they present various methodological and logistical problems. This contribution illustrates the experience of the Multi-Country Multi-City Collaborative Research Network, an international collaboration working on a global research program on the associations between environmental stressors, climate, and health in a multi-center setting. The article illustrates the collaborative scheme based on mutual contribution and data and method sharing, describes the collection of a huge multi-location database, summarizes published research findings and future plans, and discusses advantages and limitations. The Multi-Country Multi-City represents an example of a collaborative research framework that has greatly contributed to advance knowledge on the health impacts of climate change and other environmental factors and can be replicated to address other research questions across various research fields.What this study adds: This is the first contribution that presents the Multi-Country Multi-City (MCC) Collaborative Research Network, detailing the origin and purpose of the collaboration and the research work undertaken so far. In addition to information on the data collection and the multi-location database gathered over the years, this contribution illustrates the study protocol and the mode of collaboration adopted by the MCC Network, based on a flexible framework that promotes data sharing and collective participation. The article also offers an overview of the publications by MCC on different research topics, as well as the methodological developments supporting these works. Finally, this article kickstarts an article collection, with a series of epidemiological works to be published in Environmental Epidemiology, featuring the latest contributions from the MCC Network on topical environmental research issues

    Consumers' perceptions and consumption behaviour concerning red fruits

    Consumers' concern for disease prevention makes them aware of the importance to follow a healthy diet. Red fruits are known for their potential health benefits, namely due to their richness in antioxidants, vitamins and minerals. To assess consumers’ perception of red fruits purchase, conservation, preparation and consumption habits and their potential health benefits. A questionnaire was applied and distributed through e-mails and social media. From January to February of 2023, 507 individuals answered this survey. It focused on individuals’ perception of red fruits consumption and purchase habits; reasons, frequency and season of consumption; preparation and conservation procedures and potential health benefits. Among all participants, 62.7% were aged between 18-29 years, 75.3% were females and 60.4% were from Lisbon. The majority had higher education (79.6%) and a considerable number had health qualifications (36.1%). 387 participants claimed to frequently consume red fruits. Those who do not consume essentially declared high price as the main reason (60.0%). Most people purchase red fruits at super and hypermarkets (88.4%), eat them mostly fresh in spring and summer, and keep them in the fridge until consumption. Strawberry was the most frequently consumed and cranberry was the least one. Vitamins (92.1%) and anthocyanins (44.4%) were the most common substances related to red berries. 49.1% of the participants considered red fruits to be more beneficial than others, associating them with antioxidant effects (87.8%), general health and well-being (69.6%), and prevention of diseases (71.8%) like cardiovascular diseases (73.9%) and cancer (57.4%). Our findings suggest that most of this population consumes red fruits. It was verified a reasonable knowledge of their health benefits; however, several did not recognize their advantages, even having higher education.     Biofilm Formation by ST17 and ST19 Strains of Streptococcus agalactiae

    Bacterial biofilms are an important virulence factor with a vital role in evasion from the host immune system, colonization and infection. The aim of the present study was to evaluate in vitro the effects of three environmental factors (H+ , glucose and human plasma) in biofilm formation, by carrier and invasive S. agalactiae strains of ST17 and ST19 sequence types, including DNase producers and nonproducers. Bacteria ability to assemble biofilms was classified based on crystal violet assay. Biofilm formation was also monitored by scanning electron microscopy. Depending on the growth medium used, each bacterial isolate could fit in different biofilm production categories. Our data showed that optimal conditions for S. agalactiae biofilm assembly were reached after 48 h incubation at pH 7.6 in the presence of glucose and inactivated human plasma. In the presence of inactivated human plasma, the biofilm biomass of ST19 strains experienced a higher increase than ST17 strains. The composition of the extracellular polymeric matrix of the three strongest biofilm producers (all from ST17) was accessed by enzymatic digestion of mature biofilms and proteins were shown to be the predominant component. The detailed identification of the extracellular protein components should contribute to the development of new therapeutic strategies to fight S. agalactiae infections.N/

Do presente relatório, destacam-se os seguintes resultados: - A confirmação bacteriológica da TB é essencial para a monitorização da doença e para o conhecimento do perfil de suscetibilidade aos antimicrobianos, pelo que devem ser sempre solicitadas análises laboratoriais auxiliares ao diagnóstico; - De acordo com os dados atuais do relatório da DGS, em 2021 foram confirmados (casos de TB com cultura positiva para MTC ou exame direto e teste de amplificação de ácidos nucleicos positivos) 862 casos de TB (57% do número total de casos), 366 casos foram classificados como prováveis (caso de TB exame direto positivo ou teste de amplificação de ácidos nucleicos positivo ou exame anatomopatológico compatível) e 285 (18.7%) casos como possíveis (caso de TB apenas com critérios clínicos e/ou imagiológicos e sem resultados laboratoriais).N/

    Health impact of lithium mining: preliminary results of the ILiFOOD project

    A atividade mineira para exploração de lítio está em expansão devido à utilização de lítio em baterias de produtos eletrónicos ou veículos elétricos. Os níveis de lítio nas águas, solos e alimentos em diferentes regiões geográficas são variáveis, especialmente nas zonas próximas de explorações de lítio. O projeto exploratório ILiFOOD tem como objetivo avaliar o impacto da mineração de lítio nas explorações agrícolas/hortas familiares de locais circundantes e na ingestão alimentar das populações. A investigação contribuirá para avaliar o risco de exposição das populações locais ao lítio, e a outros elementos químicos, com base nos seus níveis em vegetais (couves e batatas) e em águas (de rega e de consumo) e solos provenientes de zonas rurais próximas e afastadas de minas ativas com recursos de lítio, localizadas nas regiões de Barroso- -Alvão e Guarda. Para estimar a exposição da população, serão aplica dos inquéritos sociodemográficos e de consumo alimentar aos residentes dos locais em estudo. Trata-se de um estudo inovador em Portugal que irá, pela primeira vez, explorar a relação entre a exposição ali mentar e a atividade mineira da exploração de recursos litiníferos. Os resultados preliminares obtidos na envolvente da mina C57 (Guarda) revelaram teores mais elevados de Li nas couves amostradas nas hortas localizadas até 0,5 km de distância da mina.Lithium mining activity is expanding due to the use of lithium in batteries for electronic products or electric vehicles. Lithium levels in the water, soils, and food vary from region to region, especially in areas within active mining lithium explorations. ILiFOOD aims to evaluate the impact of lithium mining on surrounding local farms and populations' dietary intake. This exploratory project will contribute to enlightening the risk of exposure of local populations to lithium and other chemicals elements, by evaluating its levels in vegetables (cabbages and potatoes) and in water (drinking and irrigation) and soil from rural areas close and far from active lithium mines resour ces, located in Barroso-Alvão and Guarda regions to assess popula tion´s exposure, socio-demographic and food consumption surveys will be applied to inhabitants of local farms. This is an innovative study in Portugal, which will for the first time; assess the relationship between food exposure and mining activity of lithium resources.     Investigating the association between ambient particulate matter (PM10) exposure and blood pressure values: Results from the link between the Portuguese Health Examination Survey and air quality data

    Introdução e objetivo: A pressão arterial elevada (PA) continua a ser um importante fator de risco cardiovascular (CV) modificável. Vários estudos epidemiológicos têm sido realizados para avaliar a associação entre a exposição à poluição do ar e este fator de risco CV, mas os resultados permanecem inconsistentes. Este estudo tem como objetivo estimar o efeito da exposição de curta duração às PM10 (concentração média dos últimos três dias) nos valores da pressão arterial diastólica (PAD) e sistólica (PAS) da população residente em Portugal Continental. Métodos: O nosso estudo baseou-se nos dados disponíveis de PAD e PAS de 2272 participantes do primeiro Inquérito Nacional de Saúde com Exame Físico (INSEF, 2015) que viviam num raio de 30 km de pelo menos uma estação de monitoração da qualidade do ar com medições disponíveis de material particulado com diâmetro aerodinâmico ≤10 m (PM10). Foram utilizados os dados da rede de monitoração da qualidade do ar da Agência Portuguesa do Ambiente para atribuir as exposições individuais de PM10. Os modelos lineares generalizados foram utilizados para estimar o efeito da exposição às PM10 nos valores de PAD e PAS. Resultados: Nenhuma associação estatisticamente significativa foi encontrada entre a exposição a PM10 e os valores de PAD e PAS (0,42% de incremento de PAD por cada aumento de 10 g/m3 de PM10 [IC de 95%: −0,85; 1,70] e de 0,47% de incremento de PAS por cada aumento de 10 g/m3 [IC 95%: −0,86; 1,79]). Os resultados permaneceram inalterados após restringir a análise aos participantes hipertensos ou aos participantes obesos ou após alteração da metodologia para atribuir as exposições individuais de PM10. Conclusões: Tendo em consideração os níveis de PM10 observados em 2015, os nossos resultados sugerem que a exposição às concentrações de PM10 terá um efeito pequeno ou nenhum efeito sobre os valores da pressão arterial.     Viral genetic clustering and transmission dynamics of the 2022 mpox outbreak in Portugal

Here, we combine genomic and epidemiological data of the 2022 mpox outbreak to better understand early viral spread, diversification and transmission dynamics. By sequencing 52% of the confirmed cases in Portugal, we identified the mpox virus sublineages with the highest impact on case numbers and fitted them into a global context, finding evidence that several international sublineages probably emerged or spread early in Portugal. We estimated a 62% infection reporting rate and that 1.3% of the population of men who have sex with men in Portugal were infected. We infer the critical role played by sexual networks and superspreader gatherings, such as sauna attendance, in the dissemination of mpox virus. Overall, our findings highlight genomic epidemiology as a tool for the real-time monitoring and control of mpox epidemics, and can guide future vaccine policy in a highly susceptible population.info:eu-repo/semantics/publishedVersio

    Programa Nacional de Vigilância da Gripe: relatório da época 2022/2023

    Relatório anual do Programa Nacional de Vigilância da Gripe (PNVG). A presente publicação descreve a caraterização clínica e laboratorial da atividade gripal na época 2022/2023. O PNVG assegura a vigilância epidemiológica da gripe em Portugal, integrando as componentes de vigilância clínica e laboratorial. A componente clínica possibilita o cálculo de taxas de incidência permitindo descrever a intensidade e evolução da epidemia no tempo. A componente virológica tem por base o diagnóstico laboratorial do vírus da gripe, SARS-CoV-2 e vírus sincicial respiratório (RSV) o que permite detetar e caraterizar os vírus respiratórios em circulação em cada inverno. As atividades do PNVG são desenvolvidas pelo Laboratório Nacional de Referência para o Vírus da Gripe e Outros Vírus Respiratórios do Departamento de Doenças Infeciosas e pelo Departamento de Epidemiologia do Instituto Nacional de Saúde Doutor Ricardo Jorge, em colaboração com a Direção-Geral da Saúde. Da descrição da atividade gripal no inverno de 2022/2023 agora apresentada, destacam-se os seguintes resultados: - Na época 2022/2023 a atividade gripal foi de baixa intensidade, tendo sido verificada a epidemia de gripe mais precoce ocorrida nas últimas épocas; - O vírus da gripe A(H3) foi o predominante durante o período epidémico, tendo sido detetados em cocirculação os vírus da gripe A(H1)pdm09 e do tipo B; - A análise virológica demonstrou que os vírus da gripe circulantes eram na sua maioria semelhantes aos vírus que integraram a vacina de 2022/2023. A análise do genoma viral mostrou alguma diversidade genética nos vírus em circulação; - Os vírus SARS-CoV-2 pertenciam na sua maioria à linhagem BA.5 e XBB; - Foi nas crianças entre os 5 e os 14 anos que se detetou a maior percentagem de casos de gripe (33%), seguidas dos jovens e adultos entre os 15 e os 29 anos (26%); - Observou-se um excesso de mortalidade de cerca de 1959 óbitos, no período coincidente com a epidemia de gripe e de frio extremo; - A taxa de admissão de casos confirmados de gripe em UCI registou o valor máximo nas semanas 47/2022 e 10/2023. O vírus influenza tipo A foi identificado na quase totalidade dos casos. Os resultados obtidos no relatório constituem informação útil para a orientação e planeamento de medidas de prevenção e controlo da gripe, SARS-CoV-2, RSV e de outros vírus respiratórios de forma precisa.info:eu-repo/semantics/publishedVersio
