    Field evaluation of an automated mosquito surveillance system which classifies Aedes and Culex mosquitoes by genus and sex

    Background: Mosquito-borne diseases are a major concern for public and veterinary health authorities, highlighting the importance of effective vector surveillance and control programs. Traditional surveillance methods are labor-intensive and do not provide high temporal resolution, which may hinder a full assessment of the risk of mosquito-borne pathogen transmission. Emerging technologies for automated remote mosquito monitoring have the potential to address these limitations; however, few studies have tested the performance of such systems in the field. Methods: In the present work, an optical sensor coupled to the entrance of a standard mosquito suction trap was used to record 14,067 mosquito flights of Aedes and Culex genera at four temperature regimes in the laboratory, and the resulting dataset was used to train a machine learning (ML) model. The trap, sensor, and ML model, which form the core of an automated mosquito surveillance system, were tested in the field for two classification purposes: to discriminate Aedes and Culex mosquitoes from other insects that enter the trap and to classify the target mosquitoes by genus and sex. The field performance of the system was assessed using balanced accuracy and regression metrics by comparing the classifications made by the system with those made by the manual inspection of the trap. Results: The field system discriminated the target mosquitoes (Aedes and Culex genera) with a balanced accuracy of 95.5% and classified the genus and sex of those mosquitoes with a balanced accuracy of 88.8%. An analysis of the daily and seasonal temporal dynamics of Aedes and Culex mosquito populations was also performed using the time-stamped classifications from the system. Conclusions: This study reports results for automated mosquito genus and sex classification using an optical sensor coupled to a mosquito trap in the field with highly balanced accuracy. The compatibility of the sensor with commercial mosquito traps enables the sensor to be integrated into conventional mosquito surveillance methods to provide accurate automatic monitoring with high temporal resolution of Aedes and Culex mosquitoes, two of the most concerning genera in terms of arbovirus transmission.This research was supported by the project VECTRACK.     Detection of pathogenic microorganisms in ticks collected from wild boars (Sus scrofa) in a recreational park in Portugal

    Correspondence[Extract] The Tapada Nacional de Mafra is a Portuguese hunting ground located in the Lisbon district, regularly visited by the public for tourism, environmental education and hunting activities. Its forest consists of an enormous plant diversity with almost 100 plants identified, and with the maritime pine (Pinus pinaster), the stone pine (Pinus pinea), and the cork oak (Quercus suber) being especially abundant (Norte et al. 2012). Furthermore, mammal species such as the wild boar (Sus scrofa, are frequently sighted in close proximity to human trails. These animals are important reproduction hosts for ticks and may play a role in the transmission of tick-borne pathogens. Ticks are vectors of several infectious agents, and consequently, their parasitic relationship with humans can cause various diseases of public health importance. The identification of those pathogens in circulation in different geographic areas contributes to a better understanding of the epidemiology of tick-related diseases, such as Lyme borreliosis, relapsing fever (RF), rickettsioses and tularemia.     Early COVID‐19 XBB.1.5 Vaccine Effectiveness Against Hospitalisation Among Adults Targeted for Vaccination, VEBIS Hospital Network, Europe, October 2023–January 2024

    We conducted a multicentre test-negative case–control study covering the period from October 2023 to January 2024 among adult patients aged ≥18 years hospitalised with severe acute respiratory infection in Europe. We provide early estimates of the effectiveness of the newly adapted XBB.1.5 COVID-19 vaccines against PCR-confirmed SARS-CoV-2 hospitalisation. Vaccine effectiveness was 49% overall, ranging between 69% at 14–29days and 40% at 60–105days post vaccination.     Relatório anual do Programa Nacional de Vigilância da Gripe e de Outros Vírus Respiratórios (PNVG), que descreve a caraterização clínica e laboratorial da atividade gripal na época 2023/2024. O PNVG assegura a vigilância epidemiológica da gripe em Portugal, integrando as componentes de vigilância clínica e laboratorial. A componente clínica possibilita descrever a intensidade e evolução da epidemia no tempo. A componente virológica tem por base o diagnóstico laboratorial do vírus da gripe, SARS-CoV-2 e vírus sincicial respiratório (RSV) o que permite detetar e caraterizar os vírus respiratórios em circulação em cada inverno.     Mortalidade e anos de vida perdidos por COVID-19 em Portugal dois anos de pandemia

    Introdução: O impacto da pandemia COVID-19 na população portuguesa não é ainda conhecido na sua totalidade. A caracterização dos anos de vida perdidos prematuramente atribuídos à COVID-19 pode fornecer informação relevante para o desenvolvimento e implementação de estratégias de prevenção e controlo de doenças infeciosas no futuro. Objetivos: Calcular os anos de vida perdidos (YLL) atribuídos às mortes por COVID-19, em Portugal, entre março de 2020 e março de 2022. Métodos: Estudo observacional e transversal. Os dados sobre a população residente média e a esperança de vida à nascença por grupo etário e sexo foram obtidos a partir do Instituto Nacional de Estatística (INE) e do Estudo Global de Carga de Doença 2019, respetivamente. Os dados sobre as mortes por COVID-19 foram extraídos dos relatórios da Direção-Geral da Saúde (DGS). Os YLL foram calculados como o número de mortes por COVID-19 multiplicado pela esperança de vida padrão na idade da morte. YLL foram calculados globalmente, por sexo e por faixa etária (YLL/100 000 habitantes e ajustado por idade). Resultados: Entre 2020 e 2022, foram notificados 3 413 013 casos de COVID-19 em Portugal, dos quais 21 342 (0,6%) morreram devido à doença. Globalmente, os YLL estimados para a COVID-19 em Portugal foram 309 383,8, sendo mais elevados para os homens que para as mulheres na maioria dos grupos etários, exceto nos grupos etários 10-19 anos (76,5 vs 153,1) e > 80 anos (58710,9 vs 68491,4). Os YLL ajustados por idade também mostraram um aumento constante à medida que progredimos na faixa etária [de 3,3 (0-9 anos) para 885,5 (>80 anos)], em que a faixa etária onde se observa um aumento mais acentuado foi a faixa etária de 50-59 anos. Conclusões: COVID-19 teve um impacto nas taxas de mortalidade em Portugal, sendo esse impacto maior na população mais idosa, sobretudo nas pessoas com mais de 70 anos. Os homens apresentaram YLL mais elevados do que as mulheres, sendo que esta diferença aumentou substancialmente à medida que a idade progride.     Tendência epidemiológica de casos de brucelose em Portugal entre 2002 e 2022

    Introdução: Apesar do sucesso das estratégias de prevenção implementadas no controle da brucelose, Portugal permanece um dos países com maior taxa de notificação por 100 000 habitantes da União Europeia. A monitorização da doença é assim um instrumento essencial para a implementação precoce de medidas de prevenção, de forma a limitar a ocorrência de surtos na população. Objetivo: Analisar a tendência temporal dos casos notificados de brucelose em Portugal, entre 2002 e 2022. Material e Métodos Estudo observacional retrospetivo baseado nos casos notificados de brucelose em Portugal (2002-2022). Os dados foram recolhidos a partir da DGS, ECDC e INE. Estatística descritiva e análise da tendência temporal foram realizadas em termos globais, por sexo e faixa etária. Resultados: Entre 2002 e 2022, foram notificados 1435 casos de brucelose, a maioria do sexo masculino (60.1%), com idade compreendidas entre 25 e 64 anos (68.8%). A variação percentual média anual (AAPC) da incidência apresentou uma tendência decrescente estatisticamente significativa (-11.1%, 2002-2022). Analisando a tendência da variação percentual anual (APC) observou-se três períodos distintos: entre 2002-2008 houve uma tendência decrescente estatisticamente significativa (-15.3%), entre 2008-2011 houve uma tendência crescente não significativa (+2.4%), e entre 2011-2022 houve uma tendência decrescente estatisticamente significativa (-14.5%). Apesar da incidência da doença ser maior nos homens do que nas mulheres, na maioria dos anos, a incidência nos homens e mulheres apresentou uma tendência decrescente estatisticamente significativa (AAPC=-11.3% e -10.5%, respetivamente) muito próxima. Foi igualmente observada uma tendência decrescente de incidência estatisticamente significativa em todas as faixas etárias analisadas. Conclusão: Nos últimos vinte anos, a incidência de brucelose diminuiu significativamente, refletindo os avanços significativos no controlo e prevenção da doença. Vários fatores poderão contribuir para esta diminuição: a melhoria do sistema de notificação, mudanças socioeconómicas, educação/consciencialização e os programas de erradicação da brucelose animal. A tendência observada confirma o sucesso das intervenções adotadas, no entanto, também confirma que a brucelose não está erradicada em Portugal.     EVITA - Epidemiologia e Vigilância dos Traumatismos e Acidentes: relatório 2022

    Relatório elaborado em setembro de 2023.Relatório EVITA – Epidemiologia e Vigilância dos Traumatismos e Acidentes relativo ao ano de 2022. O sistema EVITA, é um sistema de recolha e análise de dados sobre os acidentes domésticos e de lazer que implicaram recurso às urgências das unidades do Serviço Nacional de Saúde. Em Portugal, a notificação dos acidentes domésticos e de lazer (ADL) realiza-se através do sistema de vigilância EVITA, criado em 2000 na continuidade do sistema ADELIA – Acidentes Domésticos e de Lazer - Informação Adequada, sob a coordenação do Departamento de Epidemiologia do Instituto Nacional de Saúde Doutor Ricardo Jorge. Este sistema conta com a colaboração da entidade Serviços Partilhados do Ministério da Saúde. Os objetivos do sistema EVITA são: a) fornecer informação essencial sobre a epidemiologia dos ADL em Portugal; b) manter um sistema de vigilância que permita a caracterização dos ADL, a identificação das situações de risco, os agentes envolvidos, bem como os produtos perigosos que propiciem a ocorrência de ADL; c) manter uma base de dados disponível para a comunidade cientifica, a partir da qual seja possível realizar estudos epidemiológicos na área dos ADL, e avaliar políticas de prevenção baseadas na evidência. O presente relatório apresenta a análise descritiva dos dados recolhidos pelo sistema EVITA durante o ano de 2022. Dele se destacam os seguintes resultados: - Três picos de frequência diária de ocorrência de ADL: às 10h, às 15h e às 16h. - Uma frequência de ADL superior no sexo masculino nos grupos etários dos 0 aos 54 anos. - Uma frequência de ADL superior no sexo feminino nos grupos etários acima dos 55 anos. - Uma frequência mais elevada de ADL ocorridos na habitação, com destaque para o sexo masculino dos 0-44 anos e para o sexo feminino a partir dos 75 anos. - A frequência de ADL ocorridos em “Escola, área institucional e recintos públicos” foi mais elevada no sexo masculino entre os 0 e os 44 anos. Por outro lado, nos indivíduos com 45 ou mais anos observou-se uma maior frequência de ADL no sexo feminino. - O “Mecanismo de lesão” mais frequente foi a “Queda”. - Cerca de 56% de todos os ADL envolveram uma “Contusão, hematoma”. - A parte do corpo lesada referida com maior frequência foram os “Membros”.info:eu-repo/semantics/publishedVersio

    Nonsense-mediated mRNA decay in genetic diseases and cancer: key players, mechanisms, and a novel approach for suppression therapy

    About one third of all genetic diseases and many forms of cancer are caused by nonsense or frameshift mutations that introduce premature translation-termination codons (PTCs) (1,2). Indeed, PTCs contribute significantly to the spectrum of inherited human diseases such as cystic fibrosis, Duchenne muscular dystrophy, beta-thalassemia, and many forms of cancer. Generally, the presence of a PTC results in premature termination of mRNA translation and in rapid degradation of the PTC-containing mRNAs through the mechanism of nonsense-mediated decay (NMD). Eukaryotic mRNA translation initiates with the recruitment of the cap-binding eukaryotic initiation factor 4F (eIF4F), which comprises eIF4E, eIF4A and eIF4G, to the mRNA 5’ end (3). eIF4G has a binding site for eIF4E and the cytoplasmic poly(A)-binding protein 1 (PABPC1), which in turn is bound to the poly(A) tail, resulting in mRNA circularization (4). The unwinding of the 5’UTR by the helicase eIF4A, enables binding of the 40S ribosomal subunit. The association of eIF1, eIF1A and eIF3 to the 40S subunit facilitates binding of the ternary complex eIF2-GTP-Met-tRNAi (3). The resulting 43S preinitiation complex can land next to the cap and scans in a 5’ to 3’ direction until it recognizes an AUG codon in a consensus sequence, base-pairing with Met-tRNAi (3). Then, there is joining of 60S subunit to form an 80S ribosome, and elongation can start and the polypeptide is synthesized (3). The termination event occurs when an elongating ribosome encounters an in-frame stop codon. The eukaryotic release factor 1 (eRF1) recognizes stop codons within the ribosomal A site and triggers the hydrolysis of the ester bond, stimulated by eRF3 (3). Interactions of the eRFs with cellular proteins playing key roles in other gene expression processes may be the means by which termination is adjusted and linked to mRNA translation and NMD. NMD controls the quality of eukaryotic gene expression and also degrades and controls the levels of physiologic mRNAs (1,2,5,6). The NMD pathway is found in all eukaryotes. Several NMD key factors are highly conserved among diverse species, including UPF1, UPF2, and UPF3 (1,2). Translating ribosomes normally displace the UPF2/UPF3 containing exon junction protein complexes (EJCs) from the open reading frame (ORF) during the pioneer round of translation (1,2). However, if an mRNA contains a PTC located more than 50-54 nucleotides upstream the last exon-exon junction, the ribosome will fail to displace distal EJC(s). If a PTC prohibits removal of distal EJCs from an mRNA during the initial round of translation, UPF1 and the SMG1 kinase associate with the eRF1 and eRF3 release factors on the ribosomal termination complex at the PTC. UPF1 then interacts with the UPF2/UPF3 proteins at the downstream EJC complex. This interaction induces UPF1 phosphorylation by SMG1 and marks the mRNA as PTC-containing (2). A complex composed of SMG5, SMG6, SMG7, and the PP2A phosphatase then dephosphorylates UPF1, and the mRNA is subsequently triggered to rapid decay by SMG6 endonucleolytic attack and exonucleolytic degradation from both 5’ and 3’ ends by a not yet completely understood process that recruits decapping and 5’-to-3’ exonuclease activities, as well as deadenylating and 3’-to-5’ exonuclease exosome activities (2,7). It has been shown that the catalytic subunits of the RNA exosome are the RNase II-family exoribonucleases DIS3 and DIS3L1 (8,9). Interestingly, DIS3L1 is mainly cytoplasmic, whereas DIS3 is mainly localized in the nucleoplasm (8,9). More recently, another RNase II homologue (DIS3L2) has been characterized (10), which is active in 3’-5’ cytoplasmic RNA decay, independently of the exosome (11). Despite the fact that DIS3L1 and DIS3L2 localize in the same compartment where NMD occurs, little is known about their role in this process. Nevertheless, it has been shown that mutations in the DIS3 locus are associated with aberrant accumulation of processing intermediates and aberrant forms of some RNAs (8), which evidences its essential role in RNA surveillance processes. In addition, significant findings over the last years have shown that human DIS3 paralogous are involved in growth, mitotic control, and important diseases such as cancer (8-10). For example, DIS3L2 inactivation was associated with mitotic abnormalities and altered expression of mitotic checkpoint proteins (10). Genetic diseases and cancer attributable to PTCs affect millions of patients worldwide. Thus, the high incidence of PTCs suggests that therapeutic strategies aimed at suppressing PTCs to restore deficient protein function – so-called suppression therapies – have the potential to provide a therapeutic benefit for many patients and with a broad range of genetic disorders (12,13). This therapeutic approach uses readthrough drugs, such as aminoglycosides, that induce the translational machinery to recode an in-frame PTC into a sense codon (12,13). Suppression therapy increases the frequency that near-cognate aminoacyl-tRNAs bind at a PTC and subsequently transfer their amino acid to the nascent polypeptide (12). However, there are several obstacles that must be overcome before aminoglycosides can be used long term in the suppression of nonsense mutations. First, the efficiency of suppressing PTCs is greatly influenced by the identity of the stop codon (TAA, TAG or TGA) and the surrounding mRNA sequence. Second, the long-term use of aminoglycosides is limited due to side effects (12,13). New strategies developed to overcome this issue include the discovery of non-aminoglycoside agents such as PTC124 (Ataluren®) (14), among others (12). While the most promising drug, PTC124, was found to be safe and offers a therapeutic benefit to many patients, not all patients respond equally well to its administration. One factor that possibly affects the response to suppression therapy in many patients is the high efficiency of NMD. Based on these data, this project included the following aims: (A) To study the role of DIS3-like proteins in the mRNA decay pathway inherent to NMD; (B) To analyze how DIS3L1 regulates the human transcriptome and how its functional interactions modulate the transcriptional reprogramming of colorectal cancer (CRC) cells; (C) To study the interplay between the mechanisms of PTC definition, mRNA translation, and NMD; (D) To establish an efficient PTC suppression therapy for beta-thalassemia.FCTN/

    Monitoring the nutritional composition of drinks (soft drinks and flavored milks) available on the Portuguese market in 2024: the reduction in sugar and the presence in sweeteners?

    O consumo excessivo de açúcar está relacionado com o desenvolvimento de doenças crónicas, nomeadamente a obesidade e a diabetes tipo 2, con forme o evidenciam diversos estudos científicos. Para reduzir a prevalência deste tipo de doenças, o Governo português aprovou a Estratégia Integrada para a Promoção da Alimentação Saudável (EIPAS) que, entre outros objeti vos, pretende uma redução progressiva do consumo diário de açúcar. Na qualidade de parceiros estratégicos na implementação da EIPAS, pro cedeu-se à monitorização do teor de açúcar nas categorias de alimentos refrigerantes e leites aromatizados, disponíveis no mercado português. Os dados foram recolhidos no comércio eletrónico das maiores cadeias de su permercados, durante o ano de 2024. Decorrente dos objetivos de reformu lação e da consequente redução de açúcar, considerou-se relevante avaliar também a presença de edulcorantes (aditivos alimentares utilizados para adoçar os alimentos). As medianas dos teores de açúcar foram de 4,10 g/100 mL para os refrige rantes e 2,4 g/100 mL (excluindo o valor da lactose), para os leites aromati zados. Estes valores foram comparados com os do estudo de monitorização do teor de açúcar nestes alimentos realizado em 2019, e confrontados com os limites da EIPAS de 2,5 g/100 mL, tendo sido verificada uma redução progressiva do teor de açúcar por parte da indústria alimentar. Na categoria refrigerantes, com base na lista de ingredientes apenas 22% não continham edulcorantes. No caso dos leites aromatizados, verificou-se que 73% não continham edulcorantesExcessive sugar consumption is related to the development of chronic diseases, namely obesity and type 2 diabetes, as shown by several scientific studies. To reduce the prevalence of this type of disease, the Portuguese government approved the Integrated Strategy for the Promotion of Healthy Eating (EIPAS), which, among other objectives, aims to progressively reduce daily sugar consumption. As strategic partners in the implementation of EIPAS, we monitored the sugar content in the soft drink and flavored milk food categories available on the Portuguese market. Data was collected in e-commerce from the largest supermarket chains, during the year 2024. Due to the reformulation objectives and the consequent reduction in sugar, it was considered relevant to also evaluate the presence of sweeteners (food additives used to sweeten food). The median sugar levels were 4.10 g/100 mL for soft drinks and 2.4 g/100 mL (excluding the lactose value) for flavored milks. These values were compared with those from the study monitoring the sugar content in these foods carried out in 2019, and compared with the EIPAS limits of 2.5 g/100 mL, with a progressive reduction in sugar content by the industry being verified. In the soft drinks category, based on the list of ingredients, only 22% did not contain sweeteners.
