OSCA has been established to help pre-registrant students overcome practice learning challenges, and particularly to ease the achievement of a range of more challenging proficiencies and skills within the Nursing and Midwifery Council’s (2018) Future Nurse: Standards of Proficiency for Registered Nurses. Using simulation as a learning and assessment strategy, OSCA offers learners an opportunity to experience and practice the realities of professional practice in a controlled and accessible environment

    Cremation in the Early Middle Ages: Death, fire and identity in North-West Europe

    Introducing this first-ever edited collection drawing together the latest research on the cremation of the dead in early medieval North-West Europe, the chapter identifies, rationalises and contextualises the book’s aims: to rethink the burning of the dead in the context of other dimensions to mortuary practices in the early medieval period and showcasing the significant theoretical, methodological and investigative advances of recent decades. The scope and rationale of the book are outlined, focusing on providing a rich and accessible resource for students and scholars of archaeology and cognate fields. The chapter then explains the distinctive process of the book’s creation: largely by transcribing and editing structured interviews. The nineteen constituent chapters of the book are then reviewed, and key themes and future directions are identified for the investigation of death and fire in the Early Middle Ages

    Identification and support of autistic individuals within the UK Criminal Justice System: a practical approach based upon professional consensus with input from lived experience

    Background: Autism spectrum disorder (hereafter referred to as autism) is characterised by difficulties with (i) social communication, social interaction, and (ii) restricted and repetitive interests and behaviours. Estimates of autism prevalence within the criminal justice system (CJS) vary considerably, but there is evidence to suggest that the condition can be missed or misidentified within this population. Autism has implications for an individual’s journey through the CJS, from police questioning and engagement in court proceedings through to risk assessment, formulation, therapeutic approaches, engagement with support services, and long-term social and legal outcomes. Methods: This consensus based on professional opinion with input from lived experience aims to provide general principles for consideration by United Kingdom (UK) CJS personnel when working with autistic individuals, focusing on autistic offenders and those suspected of offences. Principles may be transferable to countries beyond the UK. Multidisciplinary professionals and two service users were approached for their input to address the effective identification and support strategies for autistic individuals within the CJS. Results: The authors provide a consensus statement including recommendations on the general principles of effective identification, and support strategies for autistic individuals across different levels of the CJS.     Disease Burden, Clinical Outcomes, and Quality of Life in People with Hemophilia A without Inhibitors in Europe: Analyses from CHESS II/CHESS PAEDs

    Introduction: Limited data relating to treatment burden, quality of life, and mental health burden of hemophilia A (HA) are currently available. Aim: To provide a comprehensive overview of unmet needs in people with HA (PwHA) using data generated from the Cost of Haemophilia in Europe: a Socioeconomic Survey-II (CHESS II) and CHESS in the pediatric population (CHESS PAEDs) studies. Methods: CHESS II and CHESS PAEDs are cross-sectional surveys of European males with HA or hemophilia B (HB) aged ≥18 and ≤17 years, respectively. Participants with FVIII inhibitors, mild HA, or HB were excluded from this analysis, plus those aged 18 to 19 years. Annualized bleeding rates (ABRs), target joints, and other patient-reported outcomes were evaluated. Results: Overall, 468 and 691 PwHA with available data for the outcomes of interest were stratified by hemophilia severity and treatment regimen in CHESS II and CHESS PAEDs, respectively. In these studies, 173 (37.0%) and 468 (67.7%) participants received FVIII prophylaxis, respectively; no participants received the FVIII mimetic emicizumab or gene therapy. ABRs of 2.38 to 4.88 were reported across disease severity and treatment subgroups in both studies. Target joints were present in 35.7 and 16.6% of participants in CHESS II and CHESS PAEDS; 43.8 and 23.0% had problem joints. Chronic pain was reported by a large proportion of PwHA (73.9% in CHESS II; 58.8% in CHESS PAEDs). Participants also reported low EQ-5D scores (compared with people without HA), anxiety, depression, and negative impacts on their lifestyles due to HA. Conclusions: These analyses suggest significant physical, social, and mental burdens of HA, irrespective of disease severity.     FireNet-Micro: Compact Fire Detection Model with High Recall

    Fire occurrences and threats in everyday life incur substantial costs on ecological, economic, and even social levels. It is crucial to equip establishments with fire prevention systems due to the notable increase in fire incidents. Numerous studies have been conducted to develop efficient and optimal fire detection models in order to prevent such mishaps. Initially, thermal/chemical methods were used, but later, image processing techniques were also employed to identify fire occurrences. Recent approaches have capitalized on the advancements in deep learning models for computer vision. However, most deep learning models face a trade-off between detection speed and performance (accuracy/recall/precision) to maintain a reasonable inference time (for real-time applications) and parameter count. In this paper, we present a bespoke and highly lightweight convolutional neural network specifically designed for fire detection. This model can be integrated into real-time fire monitoring equipment and potentially applied in future methods suhc as CCTV surveillance cameras, traffic lights, and unmanned aerial vehicles (drones) for fire monitoring in futuristic smart city scenarios. Despite having significantly fewer trainable parameters, our customized model, FireNet-Micro, outperforms existing low-parameter-count models in fire detection. When evaluated on the FireNet dataset, FireNet-Micro, with only 171,234 parameters, achieved an impressive overall accuracy of 96.78%. In comparison, FireNet-v2 attained 94.95% accuracy with 318,460 parameters (which is almost double the parameter count of the proposed FireNet-Micro)

    An Ultra-Energy-Efficient Reversible Quantum-Dot Cellular Automata 8:1 Multiplexer Circuit

    Energy efficiency considerations in terms of reduced power dissipation are a significant issue in the design of digital circuits for very large-scale integration (VLSI) systems. Quantum-dot cellular automata (QCA) is an emerging ultralow power dissipation approach, distinct from traditional, complementary metal-oxide semiconductor (CMOS) technology, for building digital computing circuits. Developing fully reversible QCA circuits has the potential to significantly reduce energy dissipation. Multiplexers are fundamental elements in the construction of useful digital circuits. In this paper, a novel, multilayer, fully reversible QCA 8:1 multiplexer circuit with ultralow energy dissipation is introduced. The power dissipation of the proposed multiplexer is simulated using the QCADesigner-E version 2.2 tool, describing the microscopic physical mechanisms underlying the QCA operation. The results show that the proposed reversible QCA 8:1 multiplexer consumes 89% less energy than the most energy-efficient 8:1 multiplexer circuit previously presented in the literature

    Economic and Humanistic Burden of Moderate and Severe Hemophilia A and B in Spain: Real-World Evidence Insights from the CHESS II Study

    Background: Hemophilia is a congenital disorder characterized by deficiency or absence of clotting factor VIII in hemophilia A (HA) or clotting factor IX in hemophilia B (HB), resulting in frequent, repeated, and prolonged spontaneous or traumatic bleeding into joints or soft tissue. Severity is classified by the patient’s baseline level of clotting factor activity as mild (>5%-40%), moderate (1%-5%), or severe (<1%). In Spain, there is limited information on the societal economic burden of disease. Objective: To estimate the economic and humanistic burden of disease in adult patients with non-inhibitor moderate and severe HA and HB in Spain. Methods: Spanish data from the CHESS II study (2018-2020) on patients’ clinical characteristics, health-related quality of life (HRQoL) and hemophilia-related healthcare resource utilization were analyzed. Economic burden was determined by estimating condition-related annual per-patient direct (medical and nonmedical) and indirect costs, stratified according to hemophilia type and severity and presented as 2022 Euros. HRQoL was assessed via the EQ-5D-5L. Results: Of 341 patients in the Spanish CHESS II cohort, 288 patients met the inclusion criteria: 181 had HA (37% [n = 66] moderate and 63% [n=115] severe) and 107 had HB (26% [n = 28] moderate and 74% [n = 79] severe). Mean annual direct cost was higher in HB than in HA, and higher in severe than in moderate patients, resulting in an annual cost/patient of €17 251 (moderate HA), €17 796 (moderate HB), €116 767 (severe HA) and €206 996 (severe HB). The main direct cost component in all groups except moderate HA was factor replacement therapy. Mean per-patient indirect cost was €4089 (moderate HA), €797 (moderate HB), €8633 (severe HA) and €8049 (severe HB). Finally, the mean total cost (direct and indirect) for moderate and severe patients were €91 017 (HA) and €163 924 (HB). EQ-5D-5L [SD] scores were lower in patients with severe HA (0.77 [0.18]) and severe HB (0.70 [0.22]) compared with patients with moderate HA (0.81 [0.15]) and moderate HB (0.86 [0.17]). Conclusions: Independently of the type of hemophilia, greater condition severity was associated with increased costs and a decrease in HRQoL

    @ 2024 The British Journal of Community JusticeThe aim of this paper is to explore the development of peer mentorship within Youth Justice, including the value and utilisation of lived experience. Children and young people who have acquired specific experience of system contact can accrue experiential knowledge and become ‘experts by experience’. These children and young people are potentially capable of providing unique insights, which include sharing knowledge and experiences of navigating welfare and justice services. This research paper provides in-depth insight from an ongoing study about the experiences of those involved in delivering a peer mentoring scheme within a youth justice context. Data from semi-structured interviews with lived experienced peer mentors and practitioners were analysed using thematic analysis to explore participants’ opinions, attitudes and beliefs regarding the design and development of a peer mentoring scheme. The Version of Record is available online at: http://dx.doi.org/10.1007/s10336-024-02249-xThe Cape Vulture (Gyps coprotheres) has the smallest range of any vulture species in Africa, Europe or Asia and is substantially impacted by anthropogenic factors because of their low productivity and long maturation times. Almost year-round presence at breeding colonies makes understanding Cape Vulture breeding behaviour essential for their conservation. Camera traps, a first for this species, were used to investigate the effect of time of day and temperature on the presence and behaviours performed at nest sites. The number of vultures at a nest site was likely to be higher during the early morning and late afternoon when temperatures were lower, with significantly fewer individuals present at higher temperatures in the days before and after laying. Attendance of at least one adult at the nest was recorded for 86.9% and 99.8% of time points in the days before and after laying, respectively. Almost constant attendance during incubation may also be necessitated by predation pressure, with this study providing observation of possible predation pressure by White-necked Ravens (Corvus albicollis) at the colony. Here, we demonstrate that camera trapping is an effective method of studying Cape Vulture breeding behaviour, which improves understanding and allows more informed conservation measures to be implemented. Cape Vulture nest attendance is affected by temperature, so this species may be vulnerable to climatic changes and subsequent changes in predation pressure.Open access funding provided by University of KwaZulu-Natal.
