Views of Practitioners and Researchers on the Use of Virtual Reality in Treatments for Substance Use Disorders
From Frontiers via Jisc Publications RouterHistory: received 2020-09-15, collection 2021, accepted 2021-04-27, epub 2021-05-21Publication status: PublishedVirtual Reality Therapy (VRT) has been shown to be effective in treating anxiety disorders and phobias, but has not yet been widely tested for Substance Use Disorders (SUDs) and it is not known whether health care practitioners working with SUDs would use VRT if it were available. We report the results of an interview study exploring practitioners’ and researchers’ views on the utility of VRT for SUD treatment. Practitioners and researchers with at least two years’ experience delivering or researching and designing SUD treatments were recruited (n = 14). Interviews were thematically analyzed, resulting in themes relating to the safety and realism of VRT, and the opportunity for the additional insight it could offer to during SUD treatment. Participants were positive about employing VRT as an additional treatment for SUD. VRT was thought suitable for treating adults and people with mental health issues or trauma, provided that risks were appropriately managed. Subsequent relapse, trauma and over-confidence in the success of treatment were identified as risks. The opportunity VRT offered to include other actors in therapy (via avatar use), and observe reactions, were benefits that could not currently be achieved with other forms of therapy. Overall, VRT was thought to offer the potential for safe, realistic, personalized and insightful exposure to diverse triggering scenarios, and to be acceptable for integration into a wide range of SUD treatments

Finding my voice: A qualitative exploration into the perceived impact of person-centred counsellor training upon counsellors who were adopted as a baby
This small‐scale qualitative study explored how qualified person‐centred counsellors who were adopted as a baby perceived the impact of their person‐centred counselling training. The study focused on the adoptees’ experiences of adoption and how these influenced their experience of person‐centred counselling training. Data were analysed using interpretative phenomenological analysis to gain insight into how the participants made sense of their lived experience. The findings supported the difficulties associated with adoption, which not only are present in existing literature and research but also placed an emphasis on the particular vulnerabilities associated with being adopted as a baby. The findings further highlighted the positive impact of person‐centred counselling training on the participants’ personal development, which included the following: increased self‐awareness, self‐acceptance, identity development and ‘having a voice’. The findings confer implications for clinical practice in understanding the experience of adoptees who were adopted as a baby and for trainers in the planning and provision of person‐centred training. The research also identifies the healing aspects of person‐centred counselling training, which facilitated the participants’ positive self‐development. In addition, unique opportunities for counsellors who were also adopted as a baby are suggested and the need for the Adoption Support Fund to be extended to allow an adoptee of any age to access therapeutic support is also identified. The links made between adoption and person‐centred training are an original area of research and are worthy of further exploration
Protectoral Rule in the North Western Association: the role and consequence of military and civil governance in the north west of England 1655 to 1657
This thesis is a study of the role and consequence of military and civil governance in the Protectoral government’s North Western Association. It seeks to understand how the creation of the association contributed to the security and maintenance of unopposed Protectoral rule. It examines the impact on traditional structures of local government and communities within the association. Ultimately, it shows that uninterrupted control over the association’s regions contributed to the continued stability of Cromwell’s Protectorate.
The first chapter examines North Western society’s religious and political allegiances in the aftermath of the civil wars and finds that, while the parish continued to play a prominent role within the community, some political adversaries of the same ascribed social status within county society continued to maintain pre-war social relationships. Chapter two assesses the role and impact of state imposed martial governance within the association and finds that central government’s policy of promoting godly reform to counter irreligion reaffirmed measures previously pursued by godly officials and magistrates. The third chapter examines the backgrounds and careers of the association’s two major-generals, Charles Worsley and Tobias Bridge, and finds that, before his death, Worsley was the driving force behind the instigation of measures to deal with anti-government activities and godly reformation. Finding that the association’s three county militias were wholly remodelled in 1655, chapter four assesses their reorganisation and role, along with that of the regime appointed commissioners for securing the peace of the Commonwealth, as well as the work of the magistracy. Chapter five considers the efficacy of raising revenues through sequestration and finds that more than sufficient funds were raised by way of the levied decimation tax to maintain the association’s three new troops of horse militia. The sixth chapter examines the parliamentary election campaign of 1656 and considers its relevance to the Northern anti-government rising staged by Sir George Booth in August 1659. It finds that many of the same protagonists at the centre of the election campaign of August 1656 were also at the heart of the events of Booth’s rising.
The thesis concludes that the imposition of military governance ensured that stable unopposed Protectoral rule was maintained throughout the life of the North Western Association and that Tobias Bridge’s oversight of the association lasted well into 1658
Keto-on-the-Clock: A Survey of Dietetic Care Contact Time Taken to Provide Ketogenic Diets for Drug-Resistant Epilepsy in the UK
Medical ketogenic diets (KDs) are effective yet resource-intensive treatment options for drug-resistant epilepsy (DRE). We investigated dietetic care contact time, as no recent data exist. An online survey was circulated to ketogenic dietitians in the UK and Ireland. Data were collected considering feeding route, KD variant and type of ketogenic enteral feed (KEF), and the estimated number of hours spent on patient-related activities during the patient journey. Fifteen dietitians representing nine KD centres responded. Of 335 patients, 267 (80%) were 18 years old or under. Dietitians spent a median of 162 h (IQR 54) of care contact time per patient of which a median of 48% (IQR 6) was direct contact. Most time was required for the classical KD taken orally (median 193 h; IQR 213) as a combined tube and oral intake (median 211 h; IQR 172) or a blended food KEF (median 189 h; IQR 148). Care contact time per month was higher for all KDs during the three-month initial trial compared to the two-year follow-up stage. Patients and caregivers with characteristics such as learning or language difficulties were identified as taking longer. Twelve out of fifteen (80%) respondents managed patients following the KD for more than two years, requiring an estimated median contact care time of 2 h (IQR 2) per patient per month. Ten out of fifteen (67%) reported insufficient official hours for dietetic activities. Our small survey gives insight into estimated dietetic care contact time, with potential application for KD provision and service deliver
Galerkin finite element approximation of a stochastic semilinear fractional subdiffusion with fractionally integrated additive noise

After discussing the existence, uniqueness and regularity results, we approximate the noise with the piecewise constant function in time in order to obtain a regularized stochastic fractional subdiffusion problem. The regularized problem is then approximated by using the finite element method in spatial direction. The mean squared errors are proved based on the sharp estimates of the various Mittag-Leffler functions involved in the integrals. Numerical experiments are conducted to show that the numerical results are consistent with the theoretical findings
The Long-Term Effects of Developmental Hypoxia on Cardiac Mitochondrial Function in Snapping Turtles

However, many vertebrates also encounter oxygen deprivation during embryonic development. The effects of developmental hypoxia on mitochondrial function are likely to be more profound, because environmental stress during early life can permanently alter cellular physiology and morphology. To this end, we investigated the long-term effects of developmental hypoxia on mitochondrial function in a species that regularly encounters hypoxia during development—the common snapping turtle (Chelydra serpentina). Turtle eggs were incubated in 21% or 10% oxygen from 20% of embryonic development until hatching, and both cohorts were subsequently reared in 21% oxygen for 8 months. Ventricular mitochondria were isolated, and mitochondrial respiration and reactive oxygen species (ROS) production were measured with a microrespirometer. Compared to normoxic controls, juvenile turtles from hypoxic incubations had lower Leak respiration, higher P:O ratios, and reduced rates of ROS production. Interestingly, these same attributes occur in adult vertebrates that acclimatize to hypoxia. We speculate that these adjustments might improve mitochondrial hypoxia tolerance, which would be beneficial for turtles during breath-hold diving and overwintering in anoxic environments
A Systematic Review Exploring the Reflective Accounts of Applied Sport Psychology Practitioners

The aim of this review was to synthesize the reflective accounts of applied sport psychology practitioners and highlight common themes that provide focus to their reflective practice. The insight into current progress on reflective content in applied sport psychology provides a foundation to build on as we continue to understand this topic. Following a systematic search of the literature, a total of 73 studies were included within the review, which were analyzed using thematic content analysis. Analysis of the reflective accounts resulted in the creation of nine higher-order themes: Process and Purpose of Reflective Practice; Ethical Practice; Supporting Person and Performer; Practitioner Individuation; Relationships with Clients; Cultural Awareness; Competence-Related Angst; Support of Practitioner Development; and Evaluating Practitioner Effectiveness. The review includes recommendations for future research, such as the use of narrative analysis to provide further insight into applied practitioners’ experiences. We also provide practical implications, which are tailored to match the specific demands of practitioners at different stages of development and include increased engagement in critical reflection for trainee practitioners and engaging with ‘critical friends’ to facilitate the process of meta-reflection for newly qualified practitioners
Simulations of Neutrino and Gamma-Ray Production from Relativistic Black-Hole Microquasar Jets

In this work, we concentrate on the investigation of electromagnetic radiation and particle emissions from the jets of stellar black hole binary systems characterized by the hadronic content in their jets. Such emissions are reliably described within the context of relativistic magneto-hydrodynamics. Our model calculations are based on the Fermi acceleration mechanism through which the primary particles (mainly protons and electrons) of the jet are accelerated. As a result, a small portion of thermal protons of the jet acquire relativistic energies, through shock-waves generated into the jet plasma. From the inelastic collisions of fast (non-thermal) protons with the thermal (cold) ones, secondary charged and neutral particles (pions, kaons, muons, η-particles, etc.) are created, as well as electromagnetic radiation from the radio wavelength band to X-rays and even very high energy gamma-rays. One of our main goals is, through the appropriate solution of the transport equation and taking into account the various mechanisms that cause energy losses to the particles, to study the secondary particle concentrations within hadronic astrophysical jets. After assessing the suitability and sensitivity of the derived (for this purpose) algorithms on the Galactic MQs SS 433 and Cyg X-1, as a concrete extragalactic binary system, we examine the LMC X-1 located in the Large Magellanic Cloud, a satellite galaxy of our Milky Way Galaxy. It is worth mentioning that, for the companion O star (and its extended nebula structure) of the LMC X-1 system, new observations using spectroscopic data from VLT/UVES have been published a few years ago
Visual impairment and medication safety: a protocol for a scoping review

People can be visually impaired from birth, childhood, early adulthood or later in life. Those with visual impairment are subject to health inequities and increased risk for patient safety incidents in comparison to the general population. They are also known to be at an increased risk of experiencing medication errors compared to those without visual impairment. In view of this, this review aims to understand the issues of medication safety for VI people. Methods/design: Four electronic bibliographic databases will be searched: MEDLINE, Embase, PsycInfo and CINAHL. Our search strategy will include search combinations of two key blocks of terms. Studies will not be excluded based on design. Included studies will be empirical studies. They will include studies that relate to both medication safety and visual impairment. Two reviewers (SG and LR) will screen all the titles and abstracts. SG, LR, RM, SCS and PL will perform study selection and data extraction using standard forms. Disagreements will be resolved through discussion or third party adjudication. Data to be collected will include study characteristics (year, objective, research method, setting, country), participant characteristics (number, age, gender, diagnoses), medication safety incident type and characteristics. Discussion: The review will summarise the literature relating to medication safety and visual impairment
