    The effects of structure, process and outcome incentives on primary care referrals to a national prevention programme

We examine the impact of different incentive types on the quantity and effectiveness of referrals made by general practices to a new national prevention programme in England. We measured effectiveness by the number of referrals resulting in programme attendance. We surveyed local commissioners about their use of financial incentives and linked this information to numbers of programme referrals and attendances from 5170 general practices between April 2016 and March 2018. We used multivariate probit regressions to identify commissioner characteristics associated with the use of different incentive types and negative binomial regressions to estimate their effect on practice rates of referral and attendance. Financial incentives were offered by commissioners in the majority of areas (89%), with 38% using structure incentives, 69% using process incentives and 22% using outcome incentives. Compared to practices without financial incentives, neither structure nor process incentives were associated with statistically significant increases in referrals or attendances, but outcome incentives were associated with 84% more referrals and 93% more attendances. Outcome incentives were the only form of pay‐for‐performance to stimulate more participation in this national disease prevention programme

    Detours and Dislocations - Liverpool /Isle of Man / Vancouver: In the Footsteps of Malcolm Lowry

    This exhibition is part of an ongoing body of work which references Malcolm Lowry's life and writing as a point of departure for Quayle's encounters with places, landscape and dwelling spaces which were significant for the life and writing of Malcolm Lowry, who was born in New Brighton in 1909. Lowry's formative experience and life-long interest in the sea was framed by his love hate relationship with Liverpool his adopted city. Lowry visited the Isle of Man twice as a child with his family and upon moving to Vancouver in 1941 was befriended by a retired Manx boat builder named Jimmy Craige. Lowry lived in a 'squatters shack' on the shoreline of the Burrard Inlet near Dollarton, North Vancouver. Quayle's artworks connect the topography and histories, which background Lowry's writing in relation to contemporary narratives based on the artist's response to the same or other locations encountered as part of a process predicated on travel and journeys. The indigenous, cultural and environmental context is manifest in Quayle's visit to Camp Cloud, a road-side encampment-based protest (opposing the trans mountain pipeline) on Burnaby Mountain, at the entrance of the Shellburn Oil refinery, which sat opposite Lowry's shack.An exhibition of artworks by Cian Quayle at the Williamson Art Gallery and Museum, July 7 - August 26, 2018. The installation included a neon artwork/text, a 1/50 scale model, photographs, a lightbox mounted transparency, single channel video, loaned artworks (Chris John Symes and George Cuitt), hand-made photographs from glass plate negatives and an automated 35 mm carousel, slide-projection. The research triangulates, Wirral born author of Under the Volcano (1947), Malcolm Lowry's relationship with Liverpool, the Isle of Man and Vancouver as the basis of a psychogeographic encounter with places and sites of habitation, which held significance for Lowry's life and writing. The exhibition formed part of IB 18 (Independents Biennial) and was exhibited in conjunction with 'Tom Wood: Cammell Laird Shipyard 1993 - 1996'

    Purification of Propylene and Ethylene by a Robust Metal–Organic Framework Mediated by Host–Guest Interactions

Here, we report the excellent separation of equimolar mixtures of propylene/propane and ethylene/ethane, and of a 1/100 mixture of acetylene/ethylene by a highly robust microporous material, MFM‐520, under dynamic conditions. In situ synchrotron single crystal X‐ray diffraction, inelastic neutron scattering and analysis of adsorption thermodynamic parameters reveal that a series of synergistic host–guest interactions involving hydrogen bonding and π⋅⋅⋅π stacking interactions underpin the cooperative binding of alkenes within the pore. Notably, the optimal pore geometry of the material enables selective accommodation of acetylene. The practical potential of this porous material has been demonstrated by fabricating mixed‐matrix membranes comprising MFM‐520, Matrimid and PIM‐1, and these exhibit not only a high permeability for propylene (≈1984 Barrer), but also a separation factor of 7.8 for an equimolar mixture of propylene/propane at 298 K

    The Mind’s Presence to Itself: In Search of Non‐intentional Awareness

That is to say, in addition to being aware of whatever objects it is aware of, it is also (co‐presently) aware of itself. This paper explores the proposal that we should think about this kind of experiential‐presence in terms of a form of non‐intentional awareness. Various candidates for the relevant form of awareness, as constituting supposed non‐intentional experiential‐presence, are considered and are shown to encounter significant problems. The fact that a plausible account of the non‐intentional awareness which experience putatively has of itself cannot be framed with reference to such forms of awareness is grounds for scepticism concerning the cogency of non‐intentional experiential presence

    Seismic Design of Offshore Wind Turbines: Good, Bad and Unknowns

Offshore wind farms comprise of both offshore wind turbines (OWTs) and balance of plants (BOP) facilities, such as inter-array and export cables, grid connection etc. An OWT structure can be either grounded systems (rigidly anchored to the seabed) or floating systems (with tension legs or catenary cables). OWTs are dynamically-sensitive structures made of a long slender tower with a top-heavy mass, known as Nacelle, to which a heavy rotating mass (hub and blades) is attached. These structures, apart from the variable environmental wind and wave loads, may also be subjected to earthquake related hazards in seismic zones. The earthquake hazards that can affect offshore wind farm are fault displacement, seismic shaking, subsurface liquefaction, submarine landslides, tsunami effects and a combination thereof. Procedures for seismic designing OWTs are not explicitly mentioned in current codes of practice. The aim of the paper is to discuss the seismic related challenges in the analysis and design of offshore wind farms and wind turbine structures. Different types of grounded and floating systems are considered to evaluate the seismic related effects. However, emphasis is provided on Tension Leg Platform (TLP) type floating wind turbine. Future research needs are also identified

    Speed-Related Energy Flow and Joint Function Change During Human Walking

Joint mechanics of the human body are coordinated with each other to adapt to speed change. The aim of this study is to analyze the functional behaviors of major joints during walking, and how joints and segments alter walking speed during different periods (collision, rebound, preload, and push-off) of stance phase. In this study, gait experiment was performed with three different self-selected speeds. Mechanical works of joints and segments were determined with collected data. Joint function indices were calculated based on net joint work. The results show that the primary functional behaviors of joints would not change with altering walking speed, but the function indices might be changed slightly (e.g., strut functions decrease with increasing walking speed). Waist acts as strut during stance phase and contributes to keep stability during collision when walking faster. Knee of stance leg does not contribute to altering walking speed. Hip and ankle absorb more mechanical energy to buffer the strike during collision with increasing walking speed. What is more, hip and ankle generate more energy during push-off with greater motion to push distal segments forward with increasing walking speed. Ankle also produces more mechanical energy during push-off to compensate the increased heel-strike collision of contralateral leg during faster walking. Thus, human may utilize the cooperation of hip and ankle during collision and push-off to alter walking speed. These findings indicate that speed change in walking leads to fundamental changes to joint mechanics

    The problem this dissertation addresses is at face a simple one: in the very specific case of Hebrews 7.1-3, what interpretive move is the author using to interpret the Old Testament, when he offers a comparison between Jesus and the mysterious figure of Melchizedek? However, the answer quickly becomes complicated due to the inadequacy of our terminology, where “typology” and “allegory” – the most common interpretive moves assigned to Hebrews 7.1-3 in the scholarship – take on medieval or modern meanings rather than definitions available to the ancient authors themselves. This dissertation explores the historical background to figural and non-literal readings of sacred texts, considering in turn Greek, Jewish, and Christian readers. Each group of readers considered provides necessary context for interpretive activity in Hebrews. Greek allegorists provide the idea of a religiously or philosophically encoded text via the Homeric allegorists and their critic Plato. They also provide the actual ancient definition of the term “allegory”, as a rhetorical trope involving extended metaphor and poetic hinting by an author, which might include techniques ranging from metonymy to numerology to concept-for-concept substitution. Jewish allegorists – Aristeas, Aristobulus, and Philo – make the distinctive move to seeing a text as encoded not by a human poet like Homer or Orpheus, but by the great divine Author, God. When turning to Christian allegorists, a natural touchstone is Paul – who uses the term allegory in Galatians – but Jesus himself and (Pseudo)-Barnabas also provide very important context for distinctively Christian allegorical reading, particularly involving the Christological fulfilment of hints laid by God in the sacred history of the Old Testament (that is, “typology”). Trajectories in allegorical exegesis in early Christianity are considered, to examine the latent tendencies within the form. Finally, the definitions and understanding gained are turned to use in analysing exegesis in Hebrews, where 7.1-3 – and several other texts – are read against the background of Hellenistic literary allegory

This study aims to identify people’s symptoms through technology. However, machine learning (ML) can classify Cognitive Normal (CN) and Mild Cognitive Impairment (MCI) and Early Mild Cognitive Impairment (EMCI) using standard assessments from the Alzheimer’s Disease Neuroimaging Initiative (ADNI); Montreal Cognitive (MoCA), Mini-Mental State Examination (MMSE), Functional Activities Questionnaire (FAQ). Consequently, a Multilayer Perceptron (MLP) model was assembled into tables; MCI vs CN, MCI vs EMCI, and CN vs MCI. Additionally, an MLP model was developed for CN vs MCI vs EMCI. As a result, of advanced model performance, a cascade 3-path categorisation approach was created. Similarly, the exploitation of meta-analysis indicated a combination of MLP models (MCI vs CN, MCI vs EMCI, and CN vs MCI) with an overall accuracy within an acceptable limit. In addition, better results were found when assessments were combined rather than individually. Furthermore, applying class weights and probability thresholds could improve the MLP framework by performance achieving a balanced specificity and sensitivity ratio. Altering class weights and probability thresholds when training the MLP neuro network model, the sensitivity and Accuracy could be progressed further. In conclusion, ML, VR and electrodermal activity are constrained. Introducing the possibility of activity-based applications to enhance innovative solutions for cognitive impairment diagnosis and treatment

    Invisible but Exposed: The Lived Experience of Disabled Female Academics in Neoliberal Universities

    Neoliberal academia represents a challenging and competitive environment which privileges long working hours and performativity. Though previous research has addressed the experiences of female academics, there has been relatively little consideration of disabled faculty, who are often marginalised and poorly accommodated. In the present study, I focus on the lived experience of academics with long-term conditions that limit energy levels and / or impact on cognitive function. These conditions may be particularly inconsistent with the neoliberal academic culture and are not easily addressed by institutional accommodations. I interviewed ten female academics; all academics reported a condition that impacted on their energy levels and / or cognitive function (e.g., arthritis, depression). Academics were employed at British Higher Education Institutions, though institution type, role, and subject discipline varied. Due to COVID-19 restrictions, interviews were conducted online via the Zoom platform. Average length of interview was 55 minutes, ranging from 45 minutes to 69 minutes. I employed Interpretive Phenomenological Analysis (Smith, 1996) to analyse the interviews and six themes were identified. Identity and the Concept of Disability indicated that academics were often reluctant to describe themselves as disabled, with important consequences for the reporting and disclosure process. Dependence and Vulnerability highlighted a lack of institutional support and frequent dependence on individual Managers for access to appropriate accommodations. Legitimacy, Convention, and Conformity described the ‘hierarchy’ of health conditions that influences the acceptability of specific health issues and the privileging of specific forms of academic practice that may disadvantage those with disabilities. Workload, Intensification, and Marketisation focused on excessive academic workloads, the intensification of academic work, and the impact of this on faculty health and wellbeing. Insecurity, Competition, and Comparison highlighted the precarious and competitive nature of academia and the impact of this (e.g., on wellbeing, willingness to disclose), particularly when disabled academics are compared to those without disabilities. Perception, Othering, and Isolation described a lack of understanding of energy limiting conditions and / or those that impact on cognitive function and the extent to which the actions of individual colleagues were exacerbated by ableist policies and practice. I discuss each theme in relation to existing education and disability-oriented literature and make specific recommendations for education practice and policy
