Unexpected Color Appearance Of Pleural Fluid: A Bilothorax Case

    No full text
Bilothorax, or cholethorax, is a rare cause of exudative pleural effusion characterized by the presence of bile in the pleural space. Recognizing this condition is essential to prevent severe complications such as empyema and acute respiratory distress syndrome. This report presents a 70-year-old male patient who developed right-sided bilothorax following multiple biliary tract interventions. The patient presented with symptoms of jaundice, fever, and abdominal pain. Clinical and radiological evaluations revealed right-sided pleural effusion, and thoracentesis yielded dark yellow-green fluid. The diagnosis of bilothorax was confirmed by a pleural fluid/serum total bilirubin ratio >1.0. Early intervention was performed using a cystofix to drain the pleural fluid. However, despite all interventions, the sepsis condition could not be controlled, and the patient unfortunately passed away. Bilothorax is generally associated with hepatobiliary procedures and is mostly observed on the right side due to anatomical proximity. Diagnosis requires a high index of suspicion, especially in patients with relevant clinical histories and characteristic pleural fluid appearance. Rapid thoracentesis and pleural fluid analysis are crucial for diagnosis. Treatment typically involves pleural drainage and early administration of broad-spectrum antibiotics. In conclusion, bilothorax is a life-threatening condition requiring urgent diagnosis and intervention. This case highlights the importance of recognizing this rare condition and the necessity for early aggressive management in patients with a history of hepatobiliary procedures. © 2025, Pamukkale University. The Mediating Effect of Trust in Meal-Sharing Platforms on Individuals’ Intention To Experience Local Meals

    No full text
Thanks to meal-sharing apps, tourists can experience meals in the homes of local people in the cities they visit. In addition, it is also seen that they participate in various cooking workshops in the homes of local people or visit local markets with them and buy food and beverages. Therefore, meal sharing is also an important gastronomy experience. The aim is to examine the mediating effect of trust in meal-sharing applications on individuals’ intention to experience local meals within the scope of the theory of planned behavior by focusing on the local meal experience offered in sharing economic practices. Questionnaires were collected from 397 Turkish participants. It was found that the participants mostly used sharing economy applications for accommodation (Airbnb) and transportation (Uber), but some participants used meal-sharing applications, albeit on different platforms. The study found that attitudes toward digital meal-sharing platforms and subjective norms are the strongest factors in the intention to taste local meals. It is concluded that the trust factor has a partial mediating role in the relationship between intention, attitude, subjective norm, and perceived behavioral control. © 2025 Taylor ; Francis Group, LLC

Current-Mirror Based Filters With Reduced Circuit Complexity

    No full text
In this Letter, we propose a novel current-mirror-based lossless integration stage. The proposed stage is derived from the lossy integration counterpart by simply incorporating an additional DC current source. The unique offered contribution is the reduction of the circuit complexity in the resulting current-mode systems, where the introduced integrator is utilized. This is verified through the performed comparison with the corresponding stage, which is already introduced in the literature. A multiple-output biquad filter is designed using the proposed lossless integrator, and its performance is evaluated through the utilization of the Cadence IC design suite.Hellenic Foundation for Research and Innovation (HFRI) [19286]The research work was supported by the Hellenic Foundation for Research and Innovation (HFRI) under the 5th Call for HFRI PhD Fellowships (Fellowship Number: 19286). What Factors Drive Cross-Country Economic Freedom Convergence?

    No full text
Are the economic freedom levels of all countries converging now that the Cold War is over? If not, are they converging into a subset of economic freedom groups or 'clubs' based upon underlying legal origins and country characteristics? This study investigates these questions using recent methodological developments in panel data convergence analysis. Our tests indicate non-convergence of economic freedom across all countries. However, club convergence tests reveal three distinct convergence clubs. Our subsequent results demonstrate that countries belonging to the higher economic freedom convergence clubs are less likely to have French legal origin and lower reliance on natural resource rents, and more likely to have long tenured and democratic governments, easier exitability, more net migration, faster economic growth, more control of corruption, as well as more elderly and dense populations. The authors appreciate the comments from session participants at the 2023 Southern Economic Association meetings in New Orleans, LA as well as feedback on an earlier draft from Nicolas Cachanosky and Louis Rouanet. We also thank the editors and referees for their comments which strengthened the manuscript. Photonic crystals are periodic optical structures that guide and confine the movement of photons. One of the most effective tools for this guidance and confinement is the use of cavity structures. Photonic cavity structures enable the precise control of many important parameters, such as frequency, light speed, and delays. Moreover, these structures play a critical role in enhancing light-matter interactions and improving the efficiency of photonic devices. Photonic crystal cavity structures form the foundation of numerous photonic devices, including optical sensors, optical logic gates, and both narrow and broad-band filters, in addition to applications like buffering and filtering of light. In this study, simulations were conducted to investigate the use of photonic cavity structures as coupled cavity waveguides. The effects of symmetry reduction on parameters such as frequency, group velocity, and group velocity dispersion (GVD) were analyzed using the plane wave expansion (PWE) and finite-difference time-domain (FDTD) methods. Specifically, we explored how symmetry breaking contributes to the phenomenon of rainbow trapping (RT). Rainbow trapping is a phenomenon where light of specific wavelengths is slowed and trapped at different positions, enabling applications such as optical delay lines and data storage. This study focuses on how the performance of photonic crystal cavity structures can be enhanced by combining low-symmetry effects with RT techniques. As a result of our work, both the improvement of optical parameters through low-symmetry effects and the successful realization of these improvements were achieved RT

Uzun, Suleyman Utku/0000-0002-8876-2848; Mut, Hakan/0000-0003-0589-6072Objective: This study aimed to assess the opinions, attitudes, and behaviors of family physicians in T ; uuml;rkiye regarding mandatory communicable disease notification. Design: Cross-sectional study. Setting: A national online survey in T ; uuml;rkiye. Participants: This study was conducted among 397 family physicians from different geographical regions in T ; uuml;rkiye. Main Outcome Measure(s): Family physicians' opinions, attitudes, and behaviors regarding infectious disease notification. Results: The majority of family physicians (69.3%) felt inadequate in terms of knowledge about the notification system and 70.3% desired post-graduation training on communicable disease reporting. About one-third of the participants were dissatisfied with the current notification system. Reasons for under-reporting included lack of time, lack of feedback, and uncertainty about post-notification procedures. Factors significantly associated with higher infectious disease notification attitude scores (IDNAS) were receiving training on communicable disease reporting, feeling competent about reporting, and being satisfied with the reporting system (P .001). However, physicians who admitted choosing a different ICD code to avoid filling out an infectious disease notification form had lower IDNAS scores (P .001). Conclusion: This study highlights knowledge gaps and negative attitudes among family physicians in T ; uuml;rkiye regarding infectious disease reporting. Addressing these challenges requires continuous education and training programs. Feedback mechanisms should be improved to enhance physicians' engagement in the notification process. Reducing the workload of family physicians and simplifying the reporting process may also contribute to higher compliance with mandatory communicable disease reporting

This study focuses on the use of negation, absence, and question constructions in the Turkish divans of Ali ġîr Nevâyî: Garâ’ibü’s-Sıgar, Nevâdirü’Ģ-ġebâb, Bedâyi’ül-Vasat and Fevâidü’l-Kiber. The starting point of this analysis is based on published translations of Ali ġîr Nevâyî’s Turkish divans, which include significant amounts of material. This material includes words, phrases, and sentences where negation, absence, or question markers are employed. The study examines structures that contain these explicit markers, as well as those that express negation without any formal markers. Additionally, the study explores the place of absence and question forms within the broader concept of negation. It is also necessary to address and evaluate question structures alongside negation and absence forms. In fact, question forms are not analyzed independently in this study, but are considered within the concept of negation. On the other hand, the study progresses contextually, and information about the origin of the common suffix in the selected examples is provided in the title. As a result, the focus is placed on how these structures are articulated and the effect of their expression and meaning in relation to different participants. The standard markers of negation, such as the -mA suffix, and the words yok and irmes, are analyzed in terms of how they reflect negation. The study also examines how negation is conveyed when no formal marker is present, the ways in which negation is perceived by the reader, and the categories of absence and question, all within the context of the selected divans. In the history of Turkish linguistics, there are theoretical studies on negation, absence, and question forms. In this context, it is anticipated that the present study will contribute to Nevâyî research by providing a foundational framework for understanding how negation, absence, and question categories are addressed in his works
