No full text110009428335application/pdf本稿の目的は児童養護施設におけるソーシャルワーク実践事例を用いて,ソーシャルワークの機能を明確にしていくことである。その意義は実践事例の分析と検討からソーシャルワーク実践理論の研究的積み上げに多少なりとも貢献できるからである。
その結果,ソーシャルワーク機能は子どもの生活援助・支援においては随所に活用されていた。直接的援助機能やケースマネージャー機能,保護機能などが実践では多く活用され,子どもの成長と発達によってソーシャルワーク機能は広範囲にわたり活用されることなどが明らかになった。This paper reviews an example of social work practices at a foster home to clarify the social work functions at such homes. The objective is that an analytical review of social work practices such as the one described here can more or less contribute to the accumulation of the data for the social work practice theory. The review of the example revealed that social work functions were employed in every aspect to support children's lives. In particular, functions such as residential care, case management, and protection were employed frequently, demonstrating that social work functions are employed more widely as children become older.departmental bulletin pape
Relationship among conditions for self-control and guardian care of bronchial asthmatic children in Hokkaido
No full text110009428353application/pdf気管支喘息と診断され通院する北海道在住の小児と保護者を対象に,喘息コントロール状態をJapanesePediatric Asthma Control Program:JPAC で評価し自己管理との関連を分析し,看護支援の課題検討を目的に自作の自記式質問紙により2010 年2 〜 4 月に調査を実施した。有効回答403 部を分析対象に評価すると,喘息コントロール状態は「完全コントロール群」24.8%,「コントロール良好群」38.5%,「コントロール不良群」36.7%であった。家庭での喘息管理状況は内服薬飲み忘れ5 割,保護者喫煙5 〜 6 割,ペット飼育2 割,喘息日記記載2 割,ピークフローメーター(PEF)測定1 〜 3 割であった。喘息コントロール状態と家庭での管理状況との間には喘息コントロール状態が良好な程,救急外来受診率は低く,PEF 測定率は高いという有意な関連性が示された。患者教育においては客観的指標を日常生活に取り入れ,患児・家族がセルフ・コントロールを実感できるように動機づけ,セルフ・モニタリングを推進し,主体的な健康管理に取り組むための基盤づくりが課題と思われる。We surveyed children in Hokkaido with a diagnosis of bronchial asthma, visiting the hospital regularly as anoutpatient, and their parents between February and April 2010 to evaluate asthma control in children using the JapanesePediatric Asthma Control Program (JPAC). We analyzed the relationship between asthma control and self-managementto identify nursing support issues using our unique self-completion questionnaire. We assessed 403 valid questionnaireforms. The results revealed that asthma control was "completely controlled" in 24.8%, "well controlled" in 38.5%, and"poorly controlled" in 36.7%. In terms of asthma control in the home, 50% forgot to take drugs, 50 to 60% had parentswho smoked, 20% had pets, 20% kept an asthma diary, and 10 to 30% measured peak expiratory flow (PEF). Asignificant relationship between asthma control and management in the home was shown such that when asthma controlwas better, the rate of emergency room visits was lower while the rate of PEF measurement was higher. Topicsidentified for patient training were: to use objective parameters in daily living, to motivate patients and their families toachieve self-control, to promote self-monitoring and build a basis for actively working on health management.departmental bulletin pape
