Advance Care Planning Implementation in "Serviced Housing for Older Adults" and Related Factors. - Focusing on the Presence or Absence of Nurses On-site -
Get PDF1050867611172347520application/pdf【要旨】北海道のサービス付き高齢者向け住宅において，アドバンス・ケア・プランニングの実施状況を調査した。136件中41.9％に看護師が配置され、6割以上がアドバンス・ケア・プランニングを行っている，今後行うと回答した。アドバンス・ケア・プランニングを行う上で困難な点は，「開始時期の見極め」が最も多かった。アドバンス・ケア・プランニングの実施は，看取り数と有意に関連していたが，看護師配置に関連はなかった。また，内容の確認は，看護師不在の場合，介護支援専門員が提案した際に行われており，アドバンス・ケア・プランニングを行う際の困難と看護師配置の有無では，「人的な体制が不十分」であることに関連が見られた。サービス付き高齢者向け住宅において，アドバンス・ケア・プランニングを行う意義は大きく，そのためには，看護師の配置や訪問看護師との連携を有機的に行う必要がある。Abstract: This study examined the implementation status of Advance Care Planning (ACP) in 457 "serviced housing for older adults" in Hokkaido. There were 136 responses from care managers in facilities, resulting in a response rate of 29.8%, and 41.9% of the "serviced housing for older adults" facilities had a nurse on staff. More than 60% of the respondents reported that they had either conducted ACP or plan to do so in the future. The most commonly cited difficulty in implementing ACP was determining the appropriate "period to start advance care planning." A statistically significant difference was observed between the "implementation of ACP" and "the number of residents cared for at the end of life;" however, no significant difference was found between the "implementation of ACP" and "having a nurse on staff.” Furthermore, in facilities without an on-site nurse, "intentions toward ACP" were reviewed upon suggestions from the “care manager.” Regarding difficulties, there was a significant correlation between "the presence or absence of nurses on-site" and "insufficient personnel resources." ACP holds great importance in "serviced housing for older adults," and it is essential to systematically arrange nurse placements and coordinate with visiting nurses to support its implementation.departmental bulletin pape
