    The Transbronchial Drainage of a Lung Abscess Using Endobronchial Ultrasonography with a Modified Guide Sheath

    Lung abscess is usually treated with long-term antibiotic therapy. Due to the lack of a safe and easy drainage technique, drainage is only applied in refractory cases. We herein describe three cases in which drainage was successfully performed by endobronchial ultrasonography using a modified guide sheath. This procedure may have advantages in the detection of causative pathogens and early infection source control, and may therefore lead to the appropriate selection of antibiotics and reduce the duration of antibiotic therapy

    Identification of Specific miRNAs in Neutrophils of Type 2 Diabetic Mice: Overexpression of miRNA-129-2-3p Accelerates Diabetic Wound Healing

    Neutrophils are involved in the first stage of acute inflammation. After injury, they are mobilized and recruited to the injured tissue. In diabetes, wound healing is delayed and aberrant, leading to excessive recruitment and retention of neutrophils that fail to promote angiogenesis and prolong inflammation. However, the exact pathological mechanisms of diabetic-derived neutrophils in chronic inflammation remain unclear. Here, miRNA profiling of neutrophils from bone marrow in type 2 diabetic mice was performed using a microarray. miRNAs regulate the posttranscriptional expression of target mRNAs and are important in countering inflammation-related diseases. Our study revealed that miRNAs exhibit differential expression in diabetic-derived neutrophils compared with non–diabetic-derived neutrophils, especially miR-129 family members. miR-129-2-3p directly regulated the translation of Casp6 and Ccr2, which are involved in inflammatory responses and apoptosis.Furthermore, miR-129-2-3p overexpression at the wound site of type 2 diabetic mice accelerated wound healing. These results suggest possible involvement of miR-129-2-3p in diabetic-derived neutrophil dysfunction and that retention kinetics of neutrophils and chronic inflammation may be initiated through miR-129-2-3p–regulated genes. This study characterizes changes in global miRNA expression in diabetic-derived neutrophils and systematically identifies critical target genes involved in certain biological processes related to the pathology of diabetic wound healing

    Background: Bentall procedure is standard procedure for aortic root aneurysm or dissection in patients with Marfan syndrome. The purpose of this study was to evaluate whether valve-sparing root replacement (VSR) can be an alternative to Bentall procedure in patients with Marfan syndrome. Methods: Thirteen patients with Marfan syndrome underwent aortic root replacement between 1999 and 2017. Bentall procedure with a mechanical valve was performed in 9 patients and VSR was performed in 4. The mean durations of follow-up were 103.1 ± 61.6 months in Bentall and 35.0 ± 26.4 months in VSR. Results: There was no in-hospital death in both procedures. Reoperation for bleeding was performed in one patient after Bentall procedure. One patient died because of unknown cause in the Bentall group. No death occurred in the VSR group during a follow-up period. There was no difference in mortality between two groups. In the Bentall group, subarachnoid hemorrhage was developed in one patient and cerebral hemorrhage in one, while there was no hemorrhage or thromboembolism in the VSR group. Echocardiography showed improvement of aortic valve regurgitation grade (pre-operation 2.9 ± 1.4 vs postoperation 0.5 ± 0.4, P = 0.03) in all patients of the VSR group, and there was no reoperation for aortic valve after VSR. Conclusion: The significant inferiority of both early and late result in VSR could not be observed.     Development on Seismic Retrofitting Method for Steel Building by Knee Braces Jointed by Vise with High Hardness Screw Bolt

    The authors have developed a new method of seismic retrofit for steel structures by adding a knee brace which is jointed to the existing structures by vices made of high hardness metal screw bolts. The strength and behavior of the connection by the vice had been already studied in our previous paper. The features of the connection by the vice are easy set up and providing the strength as much as by the normal bolts joint. An experimental study has been conducted for the specimens of steel frames retrofitted by the knee braces. The knee brace was jointed to the specimen by the vices. Two types of failure modes were investigated. The former was slipping behavior of the connection and the later was bucking at the knee braces. The strengths of the specimens were estimated by the simple calculations for the two types of failure modes

    It is very important to understand the long-term exposed concrete durability in actual environment to conduct appropriate maintenance and long-term performance predict of concrete structure. In this study, the experiment is conducted about compressive strength, carbonation, and chloride ion penetration to research Gunkanjima shore core after long-term service. From the experiment result, it is conformed that shore core in Gunkanjima is general healthy while supersonic waves spread speed and compressive strength of shore core in the surface is slightly small. Moreover it is found that large quality deference exists in respect of carbonation and total chloride ion concentration distribution of core in both of sea side and island inside

    Fluctuation of Hepatic Focal Nodular Hyperplasia Size with Oral Contraceptives Use

    BACKGROUND Focal nodular hyperplasia (FNH) of the liver is a rare benign nodular lesion that arises in women of reproductive age. Although a role of female hormones has been suggested, their influence on the course of FNH has remained controversial. CASE REPORT A 44-year-old woman with a 12-year history of oral contraceptive use was referred to our hospital for examination of an asymptomatic liver mass (3 cm in diameter) identified by computed tomography. We diagnosed FNH using imaging methods and fine-needle biopsy. Oral contraceptives were discontinued because the mass increased over a period of 21 months. Four months later, the mass had decreased in size, indicating that FNH can spontaneously regress when oral contraceptives are discontinued. CONCLUSIONS Discontinuation of oral contraceptives use can reduce the size of FNH, as in this case

    Hydrazide Derivatives of Luminol for Chemiluminescence-Labelling of Macromolecules

    A series of chemiluminescent compounds containing a hydrazide group as a nucleophilic functional group has been synthesized. The syntheses were started from chemiluminescent luminol and isoluminol. The linker moiety was easily introduced onto non-nucleophilic exocyclic amino groups of luminol and isoluminol by gentle heating with cyclic acid anhydrides such as glutaric anhydride. The resulting carboxy group was converted to hydrazide by a simple condensation reaction using carbodiimide. Although majority of the synthesized compounds did not emit strong light, a sufficient chemiluminescence intensity was obtained from luminolamido- C2-hydrazide (L2H) comprising of luminol scaffold with a dimethylene linker. The ability of L2H to form a covalent bond with a macromolecule was further investigated by incubation with oxidized horseradish peroxidase. The analysis on matrix assisted laser desorption/ionization-time of flight (MALDI-TOF) MS revealed that the coupling efficiency of L2H was similar to that of commercially available labelling reagent having a hydrazide group. These results suggested that L2H, the luminol hydrazide containing a dimethylene linker, could be useful for the labelling of macromolecules in the sensitive bioassay such as chemiluminescence immunoassay

    Epidemiology and Pathogenesis of Providencia alcalifaciens Infections

    Providencia alcalifaciens is a member of the family Enterobacteriaceae that has been commonly implicated as a causative agent of diarrheal infection in humans and animals. Recent outbreaks of P. alcalifaciens in both developing and developed countries have raised public health concerns. Several studies have suggested that P. alcalifaciens can cause diarrhea by invading the intestinal mucosa, although its pathogenicity has not been well established. Often routine laboratory investigations that seek etiological agents of diarrhea do not actively pursue P. alcalifaciens detection. Therefore, routine laboratory diagnosis should be given more attention for better understanding the epidemiology and pathogenicity of P. alcalifaciens

    Oral surgery in patients with antiphospholipid syndrome

    Antiphospholipid syndrome (APS) is a systemic autoimmune disorder characterized by recurrent thrombosis, other associated autoimmune disease, and/or obstetrical morbidity along with persistent production of antiphospholipid antibodies. Because of the nature of this systemic disease, most patients are medicated with antithrombotic agents and abundant glucocorticoids. This study reports a cohort of 16 patients with APS, who underwent oral surgery between 2010 and 2017 at the Nagasaki University Hospital. Because oral antithrombotic therapy was continued in the perioperative period, all wounds were strictly closed by suturing to prevent postoperative bleeding. Perioperative laboratory dates and medications were assessed. All patients achieved local postoperative hemostasis and did not exhibit systemic complications. Moreover, there were no postoperative systemic and/or local infections. Oral surgeons should suture oral wounds and ensure local hemostasis to prevent postoperative bleeding. Because patients with APS are likely to develop thrombosis despite continued administration of antithrombotic medications, strict perioperative examination of blood coagulation is needed. Furthermore, it is important to consider the damage and stress caused due to oral surgery. Moreover, when necessary, glucocorticoid therapy should be carefully administered, in accordance with the degree of invasion and judgment of the attending physician
