Comparison of wooden temporary housing with prefabricated temporary housing in the Kumamoto earthquake
The purpose of this study is to evaluate the performance between wooden temporary housing and raise future issues. This survey is useful method to understand the transition of wooden temporary housing. In addition, we compare the performance of wooden temporary houses with prefabricated temporary houses
in the Kumamoto earthquake. As a result, it was found that the evaluation of wooden housing is better than that of prefabricated temporary houses in terms of the living performance
Dialysis as a new pre-treatment technique for online bacterial counting
Real-time bacteriological counting technology is capable of providing an online profile of bacterial removal during the wastewater treatment process, and can enhance the safety of recycled water for potable water reuse. However, autofluorescence emanating from dissolved organic compounds present in treated wastewater interferes with the analysis. In this study, a novel approach is adopted, viz., dialysis treatment for the removal of dissolved interfering substances from treated wastewater, and the efficiency of this treatment protocol is evaluated as a pre-treatment technique for real-time bacteriological counting. Dialysis using membranes having a molecular weight cut-off (MWCO) of 1000 kDa and 6–8 kDa were found to successfully reduce the intensity of autofluorescence emitted from the interfering substances; whereas the courser dialysis membrane having a MWCO of 1000 kDa was found to be more effective in removing the interfering substances. Here we demonstrate for the first time that continuous online dialysis treatment aids in the direct determination of the bacterial counts in ultrafiltration- and membrane bioreactor-treated wastewaters. The results of the study indicate that the dialysis pre-treatment technique is effective for continuously reducing the concentration of interfering substances in treated wastewater, and thus allows for direct online counting of bacteria
Influence of Liver Intoxication by Carbon Tetrachloride or D-Galactosamine on Absorption of Fluorescein Isothiocyanate-Dextran-10 and Other Marker Compounds with Different Molecular Weights from the Rat Liver Surface
We examined the influence of liver disease on the absorption from the liver surface of fluorescein isothiocyanate (FITC)-dextran 10 (FD-10, MW: 11000) and several marker compounds with different molecular weights. The purpose of this study was to determine the feasibility of liver surface application of macromolecular compounds in the disease state. We used male Wistar rats treated with carbon tetrachloride (CCl4) or D-galactosamine (GAL). FD-10 and other marker compounds were applied to the liver surface using a cylindrical diffusion cell in liver-intoxicated rats. The blood, bile, urine, and the remaining solution in the diffusion cell were collected for assay. FD-10 was absorbed by first-order kinetics from the liver surface in the liver-intoxicated rat models. The calculated rate constant ka values in the normal, CCl4 and GAL groups were 0.000965, 0.00125 and 0.00104 min-1, respectively. Increased absorption of FITC-dextrans in the liverintoxicated rats was observed. In both CCl4 and GAL groups, an inverse relationship was observed between the molecular weight and ka from the rat liver surface of the marker compounds. The limits of the molecular weight absorbed from the liver surface were extrapolated to be 71200, 135000, and 105000 in the normal, CCl4, and GAL groups, respectively. In conclusion, increased absorbability from the rat liver surface indicates that liver surface application for liver targeting of macromolecules in the diseased state is indeed feasible. Therefore, our findings can support further research on liver surface application of drugs under liver disease
Maximum and minimum lactate levels within 24 hours after veno-arterial extracorporeal membrane oxygenation induction are risk factors for intensive care unit mortality: a retrospective observational study
Introduction: Lactate level and clearance were hypothesized to be potential prognostic factors for mortality in patients with
refractory cardiogenic shock who underwent veno-arterial (VA) extracorporeal membrane oxygenation (ECMO). This study aimed to determine the prognosis of VA-ECMO patients and whether the lactate level at intensive care unit (ICU) admission(La) and at 24 h after VA-ECMO induction (L24), minimum (L24min) or maximum (L24max) lactate level within 24 h after VAECMO induction, and/or maximum lactate level after ICU admission (Lmax) could predict ICU mortality in VA-ECMO patients.Materials and Methods: This retrospective observational study included consecutive patients who underwent VA-ECMO for severe cardiogenic shock and admitted to the ICU in a hospital from April 2009 to March 2017. Risk factors for ICU mortality
with respect to lactate levels after VA-ECMO induction were determined through multiple logistic regression analysis.Results: VA-ECMO induction was performed in 67 adult patients, of whom 23 (34.3%) survived to ICU discharge. La, L24min,L24max, and Lmax were risk factors for ICU mortality in VA-ECMO patients after adjustment for the Acute Physiology and Chronic Health Evaluation II score and use of continuous renal replacement therapy and refractory ventricular arrhythmia after VA-ECMO induction, which were confounding factors in univariate analysis (La: odds ratio [OR], 1.44; 95% confidence interval[CI], 1.13-2.05; L24min: OR, 1.20; 95% CI, 1.01-2.56; L24max: OR, 1.44; 95% CI, 1.11-2.02; Lmax: OR, 1.52; 95% CI, 1.14-2.21).Conclusion: Lactate levels can be a therapeutic target and indicator of the need for improved patient management after VAECMO induction
Age, Symptomatic Metastatic Disease, and Malignant Pleural Effusion as Predictors of Poor Prognosis in Patients with Differentiated Thyroid Carcinoma Treated with Lenvatinib.
Background: Lenvatinib is one of the few therapeutic options available for radioiodine-refractory thyroid cancer. However, the factors that determine the therapeutic outcomes remain unknown.
Methods: Patients with thyroid carcinoma treated with lenvatinib who had been dead or who had survived for longer than a halfyear
were retrospectively compared. We evaluated the clinical parameters when lenvatinib was started, and also studied the tumor volume reduction ratio, the duration until re-growth of the largest metastatic lesion, the thyroglobulin (Tg) reduction rate,and the duration until re-elevation of Tg after lenvatinib between survivors and dead patients.Results: We identified 16 patients, with an average age of 73.1±7.6 yrs and a male-to-female ratio of 5 to 11, who had advanced differentiated thyroid cancer that was treated with lenvatinib. Nine patients had died after 8.9±6.1 months, whereas 7 survived for 13.0±2.0 months after starting lenvatinib. The patients who died were older than the survivors (76.7±6.5 vs. 68.6±6.6 yrs, p=0.03).Malignant pleural effusion (p=0.017) and symptomatic metastatic disease (SMD) (p=0.039) were associated with death in a Kaplan-Meier survival analysis. Age (p=0.012, HR 1.150, CI 1.030-1.320) and SMD (p=0.014, HR 8.069, CI 1.503-61.34) were
associated with poor outcome in a multivariate Cox proportional hazard model. The duration until the re-elevation of Tg was
longer in survivors than in patients who died (6.43±4.55 vs. 2.17±1.39 months, p=0.025).Conclusions: We identified multiple factors, including SMD, that were related to poor outcomes after lenvatinib treatment. This study suggests that lenvatinib might be started before patients develop SMD
A case of surgical resection for well-differentiated squamous cell carcinoma arising in a ciliated hepatic foregut cyst
Ciliated hepatic foregut cysts (CHFC) are extremely rare, and most are benign cysts of the liver arising from remnants of the embryonic foregut. CHFC is usually found incidentally and as mostly asymptomatic cysts. We report squamous cell carcinoma (SCC) arising in a CHFC in a 50-year-old Japanese woman. She consulted our hospital for upper abdominal pain.A computed tomography and an ultrasound showed a cystic region including calcification and a solid mass in segment 4 of the liver. Left hepatectomy, B6 bile duct resection, and biliary-jejunal anastomosis were performed. Microscopic examination revealed that part of the cyst was lined by a characteristic ciliated pseudostratified columnar epithelium surrounding a connective tissue, a slightly thick fibrotic smooth muscle stromal layer, and an outer fibrous capsule. The cyst wall contained a low-papillary mural nodule showing atypical squamous hyperplasia with high-grade dysplasia. Stromal invasion was identified at the base of the nodule, leading to the diagnosis of well-differentiated SCC arising from a CHFC. We recommend careful clinical follow-up for patients with relatively large CHFCs as potentially malignant lesions and excision if they show any clinical manifestation

Objective: Accumulating evidence suggests an association between periodontitis and several systemic diseases, such as atherosclerosis. In the lesions of these diseases, nucleotide-binding domain leucine-rich repeat-containing protein 3 (NLRP3), apoptosis-associated speck-like protein containing a caspase activation and recruitment domain (ASC) and caspase-1 form inflammasome complex, which leads to the functional maturation of interleukin (IL)-1β via cleavage of caspase-1 in macrophages. IL-1β plays a critical role in the etiology of these diseases; however, inflammasome priming—specifically, IL-1β and NLRP3 upregulation—is necessary for effective IL-1β production. We investigated the effect of initial periodontal treatment on the inflammasome priming of peripheral blood mononuclear cells (PBMCs). Methods: Twenty-two patients with chronic periodontitis were enrolled in this study and given initial periodontal treatment. Peripheral blood samples were collected at baseline and re-evaluation (41.1 ± 29.1 d after the treatment), and the relative expression of IL-1β, and three inflammasome components, ASC, NLRP3 and Caspase-1, mRNA was determined using quantitative reverse transcription PCR. PBMCs were stimulated with silica crystals, and the IL-1β secretion was measured via enzyme-linked immunosorbent assay. Results:
Probing pocket depth and bleeding on probing (BOP) were significantly improved after the treatment. Expression of IL-1β and ASC in the PBMCs decreased after the treatment. PBMCs stimulated with silica crystals secreted IL-1β. The treatment attenuated IL-1β secretion by PBMCs in low BOP percentages group whereas IL-1β secretion was increased in high BOP percentages group. Conclusion:

The Effects of a Year-Long English for Academic Purposes Program: The SCAS Program in 2018-19
The purpose of this report is to disseminate results of the Special
Course in Academic Skills (SCAS) Program, an optional English for
Academic Purposes preparation course for Nagasaki University
undergraduate students. In the 2018-19 academic year, TOEFL ITP
tests were administered prior to, during, and after completion of the
SCAS Program to measure receptive language abilities (i.e., reading,
listening, structure and written expression of English). Additionally,
students completed pre and posttests to measure productive language
abilities (i.e., writing, speaking) prior to instruction and following
completion of the program. Results from these assessments were
examined using paired sample t-tests, which revealed statistically
significant improvements in student abilities following completion of
the SCAS Program. Effect sizes observed through calculation of
Cohen’s d displayed a range of results from small to large, indicating
that while SCAS Program instruction was responsible for change in
students’ language skills, that change was not seen to be consistent
throughout all language skill areas
