The history, present situation, and future directions of neoadjuvant chemotherapy for HER2-negative breast cancer
Get PDFChemotherapy has played a significant role in breast cancer therapy and dramatically improved the outcome of breast cancer patients. Neoadjuvant chemotherapy (NAC) is defined as chemotherapy conducted before surgery. The rationale for NAC in operable breast cancer is that the benefit of systemic chemotherapy on the long-term prognosis does not change, regardless of whether chemotherapy is conducted before or after surgery. NAC is now widely used for early and advanced breast cancer patients since it has two significant advantages over conventional adjuvant chemotherapy after surgery. One is that the rate of the breast-conserving surgery increase resulted from tumor shrinkage during NAC, which can contribute to a minimal invasion from surgical therapy and good quality of life following therapy. Another is that a response to chemotherapy can be observed in in-vivo setting. Patients who achieve the pathological complete response (pCR) show a favorable prognosis. Thus, the response to NAC can be used as an indicator for personalized medicine. To date, novel agents for NAC have been explored in randomized trials, setting the pCR rate as an endpoint. Also, response- and residual disease-guided therapy have been ready for prime time in daily practice. Cigarette smoke induces endoplasmic reticulum stress and suppresses efferocytosis through the activation of RhoA
Get PDFImpaired efferocytosis is a key mechanism of inflammatory lung diseases, including chronic obstructive pulmonary disease and cystic fibrosis.Cigarette smoking activates RhoA and impairs efferocytosis in alveolar macrophages, but the mechanism has not been fully elucidated. We investigated the role of endoplasmic reticulum (ER) stress induced by cigarette smoking in the disruption of efferocytosis.Both tunicamycin (10 μg/ml) and thapsigargin (0.1 and 1 μM), which are ER stress inducers, suppressed efferocytosis in J774 cells,and a Rho-associated coiled-coil-forming kinase (ROCK) inhibitor (Y27632) reversed this effect.We validated the effect of tunicamycin on efferocytosis in experiments using RAW264.7 cells.
Then, we investigated the role of the unfolded protein response (UPR) in efferocytosis impaired by ER stress.
A PERK inhibitor (GSK2606414) restored the efferocytosis that had been impaired by TM,
and an eIF2α dephosphorylation inhibitor (salubrinal) suppressed efferocytosis. Cigarette smoke extract (CSE) induced ER stress in J774 macrophages and RhoA activation in J774 cells, and the CSE-induced ROCK activity was successfully reversed by GSK2606414 and tauroursodeoxycholic acid. Finally, we confirmed that ER stress suppresses efferocytosis in murine alveolar macrophages
and that GSK2606414 could rescue this process. These data suggest that cigarette smoke-induced ER stress and the UPR play crucial roles in RhoA activation and suppression of efferocytosis in the lung
Factors that affect empowerment in mothers of children with special needs receiving homecare
No full text目的：障害をもつ子どもを養育する家族には，子どもと家族が置かれた状況に気づき，問題を自覚し，生
活の調整と改善を図る力，家族エンパワメントがより必要とされる．本研究の目的は障害をもつ子どもの
母親のエンパワメントに影響を及ぼす要因を明らかにすることである．
方法：Ａ障害児施設に外来通院中で，在宅療養中の障害をもつ10歳以下の子どもの母親で，自記式質問票
に有効回答のあった117人を解析対象とした．「家族エンパワメントスケール（FES）」得点を従属変数とし，
関連要因についてMann-WhitneyのＵ検定，重回帰分析を行い検討した．
結果：対象者の平均年齢は39.3±5.3歳，家族形態は核家族92人（78.6％）が最も多かった．子どもの平均年
齢は5.6±2.8歳で第 1 子が多く，診断からの期間の平均は4.2±3.0年であった．重回帰分析の結果，「診断か
らの期間が長い」（p=0.002）「子どもの性別が男児」（p=0.016）「医療職への相談あり」（p=0.029）の順にエンパワメントの高さに有意に関連していた．
結論：母親のエンパワメントを高めるための支援では，より早い専門家からの支援と子どもの性別を考慮
する必要性が示唆された
