Hospital Frailty Risk Score Predicts Outcomes in Chronic Obstructive Pulmonary Disease Exacerbations
Get PDF三重大学博士(医学)application/pdfIntroduction: Patients with chronic obstructive pulmonary disease (COPD) are at high risk for frailty and prone to complications after admission for an acute exacerbation. We aim to investigate the association between frailty risk and functional outcomes in patients with acute exacerbations of COPD, using a nationwide database.
Methods: This retrospective cohort study included patients with acute exacerbations of COPD who were admitted by ambulance. We assessed frailty using the Hospital Frailty Risk Score (HFRS) and compared the outcomes between low frailty risk (HFRS < 5) and frailty at risk (HFRS ≥ 5) groups. The primary outcome was prolonged hospitalization (≥30 days). The secondary outcomes were in-hospital mortality, readmission (≤90 days), poor activities of daily living (ADL) at discharge, and difficulty in returning home.
Results: There were 3,396 eligible patients (mean age, 75.9 ± 11.2 years; 20.4% female). The rate of frailty at risk patients was 14.0%. Frailty at risk patients were significantly higher rates of prolonged hospitalization (32.9% vs. 17.5%), more in-hospital mortality (16.4% vs. 12.5%), more difficulty in returning home (34.6% vs. 22.9%), and poorer ADL at discharge (8.7% vs. 12.4%) than those of low frailty risk. Multivariate analysis with adjusted covariates showed that HFRS was independently associated with prolonged hospitalization (odds ratio, 2.0; 95% confidence interval, 1.4–2.9).
Conclusions: HFRS can be used to predict the outcome of patients with acute exacerbations of COPD. Get PDFUnityは開発ゲームエンジンと呼ばれ、VR(VirtualReality)アプリの開発によく使われているツールである。VRメガネで動いているゲームアプリを開発している技術者は使い方に慣れているだろうが、教育用教材の開発をゲームアプリの開発のように行えないかと思う開発初心者には、使いこなすまで大変であると思われるツールである。本論文では、開発の仕方を示すことで、教育用教材を開発しようとする人への一助になれば幸いである。用いるVRメガネは、Meta Quest2という比較的安価な製品であり、ゲームアプリが多数出ているため、多くの方はゲーム用に使っているゴーグル型のVRメガネである。本論文で示す内容は、開発の最初の段階の、WindowsパソコンとMeta Quest2を接続させる場面の操作も含め、VRメガネの中でコントローラーを使って視点移動したり、VR内に表示されている物を移動させたりするプログラム等を示す。開発中にWindowsパソコンとMeta Quest2間の接続が切れたり、ビルドエラーが出て苦労する点も含めて記述している。最後の場面で作られるVRアプリは、教育用教材には程遠いが、開発が今後も続く初期段階のものである。プログラムの大変さは理解していただけると思う。departmental bulletin pape
Development of a One-Tap Teaching Material Application that Increases Interest in the Shapes of Everyday Objects and Leads to Shape Concept Formation
Get PDF本研究の目的は、知的障害がある肢体不自由児を対象に、iPadを用いて、わずかな手の動きでも操作を可能にし、対象児が図形を答えて○×で判別でき、さらに対象児の身の回りのものを提示して、ワンタップで答えて図形の概念形成（形を捉える力）を育成できるワンタップ教材アプリ「どーれかな？」（図形探し版）を開発することである。本アプリは、はじめに基本図形について学習し、次の段階として身の回りのものの形を捉える学習を行うことができる。アプリを開発後、授業実践を行ってその有用性を検討した。成果は、以下の通りである。①対象児の習得状況に応じて教材の内容をカスタマイズできるため、対象児の図形の概念形成を育むことができた。基本図形の問題だけでなく、身の回りのものの問題に対しても、対象児の馴染みのあるものが表示されるため、iPad の画面をよく見て取り組むことができた。②①の成果の背景には、アプリによる賞賛機能と教師による言語的賞賛が挙げられる。両方の賞賛により、対象児は達成感を得ることができ、更なる課題解決への意欲につながった。departmental bulletin pape
