Late onset neonatal sepsis in very low birth weight premature infants in the neonatal high care unit, Pelonomi Hospital, Bloemfontein: a cohort study
Background: Late onset neonatal sepsis (LOS) is a common problem in very low birth
weight (VLBW) infants and is associated with increased mortality, morbidity and cost of care.
Several known risk factors are reported in the literature, which may be maternal, host or
healthcare related. Hygiene practices, overcrowding, staffing and healthcare interventions all
play a role in the risk of LOS. Pelonomi Neonatal High Care is a 32-bed regional neonatal
unit. The study period was from September 2015 to March 2016.
Methods: This was a prospective cohort study.
Study Population: All VLBW infants with birth weight between 1000g and 1499g excluding
those referred to tertiary centers and those born with major congenital abnormalities were
enrolled and followed up to 28 days of life.
Results: 117 infants were included. 43,6% of infants had at least one episode of LOS
(incidence 435 per 1000 births per annum). There was a higher incidence of gram-negative
(31%) and fungal infections (17%) than reported elsewhere. The use of invasive ventilation,
nasal CPAP, nasal prong oxygen and surfactant were significantly associated with LOS as was
increased duration of umbilical venous catheterisation. Longer periods to the initiation of
feeding and the use of cefotaxime increased the risk of LOS. Breastfeeding appears to confer
protection against LOS. Exposure to human immunonvirus and high maternal viral load may
be a risk factor for LOS. LOS was present in 40% of deaths beyond 72hours of life and 10% of
infants with LOS died. LOS was associated with increased morbidity in the form of increased
length of stay, longer duration to full enteral feeding, longer duration of parenteral nutrition
and increased necrotising enterocolitis. Only 50% of the infants received antenatal steroid
therapy and 96.5% of infants experienced some degree of hypothermia. The unit was
persistently overcrowded and key consumables for hand hygiene were frequently unavailable.
Due to the persistence of overcrowding and the short duration of the study period a
statistical relationship with LOS could not be proven.
Conclusion: LOS was significantly more frequent in this study population than reported in
the literature with serious consequences for patients. Many identified risk factors are
associated with routine practices in the care of VLBW infants in this unit. The unit was
persistently overcrowded and there were several barriers to appropriate hand hygiene.

Post-industrial urban quarries as places of recreation and the new wilderness – a South African perspective
The objective of this article is threefold: To re-evaluate the concept of wilderness as a place to be experienced by human beings as part of their outdoor recreational needs; to investigate if reclaimed post-industrial urban quarries can fulfil these needs, and to investigate the legislative frameworks in South Africa within which such reclamation and re-use can be undertaken. The objective is not to present detailed case studies of post-industrial urban quarries that have been rehabilitated or redeveloped for a different land use, but rather to demonstrate the potential of such quarries. The majority of South Africa’s population is urbanised and has hardly any prospect, due to various constraints, of ever visiting natural or even ‘man-made wilderness’ areas for recreational purposes. The currently held concept of wilderness is critically evaluated and the need to change our perception of wilderness is discussed. The biophysical and socio-economic nature, as well as the legal framework, within which the redevelopment potential of our ubiquitous post-industrial urban quarries must be viewed, are examined to determine whether they can be reclaimed as outdoor recreation places and a new ‘urban wilderness’. It is shown, through a literature review and examining five case studies, that reclaimed quarries can satisfy urban dwellers’ innate need for outdoor recreational spaces and natural or wilderness areas, albeit man-made and despite the procedural challenges posed by the requirements of the South African regulatory urban spatial planning frameworks and Acts.Publisher's versio

Health dialogue elements present during health communication between patients with diabetes and nurses in a Northern Cape local municipality
Patients and nurses in health facilities interact daily with the purpose of improving
health outcomes. However, it is not known to what extent the elements of health
dialogue are incorporated into each individual patient and nurse interaction. A
concept analysis of “health dialogue” laid the foundation to identify the presence of
the antecedent and empirical referent health dialogue elements during health
communication between patients with diabetes and nurses in the Northern Cape.
The objective of the study was to assess, through observation, the presence of the
health dialogue elements, namely, antecedents, which are a positive attitude,
sensitivity and respect, and training; and empirical referents, which comprise shared
responsibility and decision-making, a mutually beneficial health plan and contextsensitive
communication strategies.
A quantitative, non-experimental, descriptive, cross-sectional research design was
used. Data were collected using an observational checklist aided by a guideline at
public and private health facilities (n=16) in the Sol Plaatje local municipality located
within the Frances Baard District. Patients with diabetes (N=88) and nurses (N=22)
were observed while they interacted during one-on-one consultations (n=88). Each
nurse interacted with more than one patient. Audio-recordings of interactions were
used to verify and support observations.
Data analysis was carried out using descriptive statistics, namely, frequencies and
percentages for categorical data, medians and percentiles for continuous data,
calculated per group, and comparing nurse and patient responses. McNemar’s test or
Bhapkar’s test was applied to compare the statistical difference in responses between
the nurses and the patients.
Inconsistencies (48.8%) in the presentation of the health dialogue antecedent elements
and sub-elements of positive attitude and sensitivity and respect during nurse-patient
interactions, were noted. The antecedent element findings for training noted nurse
training in diabetes (19.3%) and in communication skills (30.6%), whilst patient training
and information about diabetes (48.7%) and communication skills (3.4%) were found.
In accordance with the concept analysis of health dialogue, the inconsistencies noted
in the presentation of antecedent health dialogue elements imply that it was not likely
that the empirical referents could be realised. However, both nurses and patients stated
that the empirical referent elements of shared responsibility/decision-making, a health
care plan of mutual benefit and the application of context-sensitive communication
strategies to convey health messages were experienced during the study.
These findings indicate that both nurses and patients are unable to incorporate the
antecedent health dialogue elements during interaction with one another. In addition,
nurses and patients appear to be ill-prepared to include participatory health
communication and health dialogue in interaction.
Capacity building of both nurses and patients is recommended to enhance participatory

Genetic diversity in fragmented southern African giraffe populations
The giraffe (Giraffa camelopardalis) is distributed throughout sub-Sahara in savannah habitat. It is currently listed as Vulnerable on the IUCN Red Data List, as their numbers are declining. Little is known about the genetic characteristics of giraffe in South Africa. This molecular analysis of the introduced giraffe populations in the Free State Province thus provides new insights into the species’ population genetics across the Province. The specific aims of this study were to quantify the levels of genetic diversity within individual giraffe populations; and to determine the genetic structure of Giraffa camelopardalis in the Free State Province. For this purpose, a total of 129 faecal samples were taken from 20 populations within the Free State Province, and one population from the Northern Cape Province; and with reference sequences from all currently recognized sub-species taken from GenBank. Genetic diversity and genetic differentiation was quantified using sequence data from the Cyt b and D-loop mtDNA regions. Two haplotypes were identified for the Cyt b gene region, with 10 haplotypes identified for the D-loop region. Nucleotide diversity ranged from 0 to 0.132%. The results obtained indicated low levels of genetic diversity within isolated populations; however, there was more diversity present in the larger populations in comparison to the smaller populations, and even higher levels within pooled populations that can potentially be managed as a metapopulation. Various approaches to reconstruct relationships among populations, including Maximum Likelihood, a Bayesian approach and haplotype networks, showed very similar results. The results portrayed northern and southern groups when all samples and reference material were included, with individuals from the current study clustering with the southern clade. Population pairwise FST values and other measures of differentiation confirmed the strength. The extralimital giraffe population in Central South Arica was thus found to consist of more than one subspecies, with G. c. angolensis (or possibly G. c. giraffa x G. c. angolensis hybrids) surprisingly detected in a number of populations. Several recommendations were formulated in terms of the future management and conservation of giraffe in Nature Reserves and private game farms in the Free State Province. The most practical approach for dealing with inbreeding would evidently be to exchange individuals between populations, but this should be supplemented by measures such as the implementation of a database for the Province and monitoring. A metapopulation approach to conserving genetic diversity is strongly recommended, since giraffe frequently occur in low numbers and this situation is unlikely to change. To enhance future studies, sequences of nuclear genes, as well as microsatellite markers should be added to supplement the current mtDNA-based data. Improved geographic coverage within South Africa, and specifically including naturally-occurring populations, would also be beneficial
