Spinal anaesthesia for caesarean section with low dose hyperbaric bupivacaine with or without fentanyl in patients with pregnancy induced hypertension - a comparative study.
Objectives: 1. To compare the effects of low dose intrathecal bupivacaine and combination of bupivacaine and fentanyl in pregnancy induced hypertension patients.2. To compare the haemodynamic changes between the two groups.3. To compare the quality of analgesia between the two groups.4. To compare foetal outcome between two groups.
Study Design & Setting: Prospective Double Blind, Hospital based Study.
Materials & Methods: 50 ASA grade II patients with pregnancy induced hypertension (PIH) were divided into 2 groups as Group I (Bupivacaine) and Group II (Bupivacaine-Fentanyl). Haemodynamic variables like systolic and diastolic blood pressure, heart rate were recorded every 2 minutes upto delivery of baby and then every 5 minutes until end of surgery. Sensory block and motor block alongwith side effects were recorded. Pain was evaluated using visual analogue scale and neonatal outcome assessed using APGAR scoring.
Results: Haemodynamic parameters like heart rate, systolic blood pressure, diastolic blood pressure and mean arterial blood pressure between the study and control group, were comparable and statistically not significant. On comparing the highest sensory level achieved between the two groups, statistically significant difference was observed.. The time taken for sensory regression to T12 level (minutes) between the two groups was statistically significant with a longer duration (209.9 min) in the group II compared to 162.6 min in group I.
Conclusion: The spinal fentanyl- bupivacaine mixture produces a significant prolongation of sensory block, prolongs the duration of analgesia and achieves a higher level of sensory block as compared to bupivacaine alone. This combination does not hasten the onset of sensory or motor block and does not prolong the duration of motor block or produce haemodynamic instability

Effect of integrated nutrient management and planting dates on growth, yield and quality attributes of potato (Solanum tuberosum L.)
The present investigation entitled “Effect of integrated nutrient management and planting dates on growth, yield and quality attributes of potato (Solanum tuberosum L.)” was undertaken to standardise the suitable planting date and appropriate nutrition level for profitable yield of potato crop in Kashmir valley. The experiment was laid during kharif seasons of 2008 and 2009. The treatments comprised of the 3 planting dates (10th March, 25th March and 11th April) assigned to main plots and 6 nutrition levels ( RDF =160:100:100 N:P:K kg /ha, 75% RDF +FYM 20 t/ha, 75% RDF +8 t/ha vermicompost , 75%RDF +Azotobacter and PSB , 75% RDF +FYM 20 t/ha +Azotobacter and PSB and 75% RDF +8 t/ha vermicompost + Azotobacter and PSB) randomized in sub-plots.
All climatological indices, viz., GDD, PTU and HTU favoured planting on 25th March with nutrition level of 75% RDF +8 t/ha vermicompost + Azotobacter and PSB whereas, late planting on 11th April (with 75%RDF + Azotobacter and PSB were found unfavourable for the crop.
Amongst phenological parameters, late planting on 11th April produced tallest plants, maximum shoot number along with fresh and dry weight thereof but mid planting on 25th March proved superior in respect of LAI, number of stolons/plant, length of stolon and fresh and dry weight of tubers. Application of 75% RDF+8 t/ha vermicompost + Azotobecter and PSB proved superior in improving these parameters. Planting of potato crop during 13th standard meteorological week (25th March) was found most suitable in realizing highest yield (357.19 q/ha) of good quality tubers. Among nutrition levels 75% RDF+8 t/ha vermicompost + Azotobacter and PSB was found appropriate in improving yield potential (326.00 q/ha) and quality attributes of potato crop.
Based on the findings of this study, application of 75% RDF + 8 t/ha vermicompost alongwith Azotobacter and PSB to potato crop planted on 25th March recorded higher average yield (385.30 q/ha) with quality tubers and also provided highest gross (Rs. 1,92,650.00 /ha) and net return (Rs. 1, 29, 050.00 /ha). The practice can be recommended for Kashmir valley after conducting confirmative trials
Studies on Foliar Application of Boron and Calcium on Yield, Quality and Shelf Life of Sweet Cherry cv. Bigarreau Noir Grossa (Misri
The goal of the present investigation was to examine the response of Sweet cherry (Prunus avium L.) to boric acid and calcium chloride foliar fertilization. The experiment was carried out in the private orchard located at Dara, Harwan, Srinagar during 2006-2007. The boric acid sprays were performed (1) at the beginning of flowering and (2) at full bloom, whereas calcium chloride was applied (1) at pit hardening stage of fruit and (2) at 15 days after pit hardening stage. The results showed that lower concentration of boric acid at full bloom stage had increased the fruit yield in terms of fruit weight and no effect on fruit set (%) and yield efficiency was recorded. Moreover, fruit of boric acid sprayed trees had higher content of soluble solids, sugars and anthocyanin compared to control. Fruits treated with calcium chloride @ 1.50g/l at 15 days after pit hardening stage showed higher physico-chemical characteristics viz, fruit firmness, colour, organolepetic rating, less spoilage, less physiological loss in weight, acidity, TSS, sugars and higher calcium content in leaf and fruits. Fruits treated with 1.5g/l calcium chloride at 15 days after pit hardening stage had longer shelf life than fruits treated with calcium chloride @ 0.75g/l. Fruit cracking was significantly reduced by application of calcium chloride @ 1.50g/l at 15 days after pit hardening stage when applied individually and in combination with boric acid at @ 0.5g/l at full bloom stage. Furthermore, fruit quality characteristics like TSS, reducing sugars, non-reducing sugars, total sugars, ascorbic acid, anthocyanin content, juice content and sensory characteristics were significantly increased by the combined effect of boric acid and calcium chloride application. Fruits treated with 1.50g/l calcium alone or in combination with boric acid 0.5g/l increased all the physical and chemical characteristics even after 10 days of storage. It is concluded that foliar application of boric acid and calcium chloride showed a remarkable improvement in most studied characteristics and can be recommended for improving fruit quality, appearance and shelf life in sweet cherry to a greater extent

Intravenous regional anaesthesia: Lidocaine versus combination of lidocaine and dexamethasone.
Objectives: 1) To determine the efficacy of dexamethasone as an adjuvant to lidocaine for intravenous regional anaesthesia (IVRA). 2) To compare the onset and duration of sensory block, motor block and analgesia when lidocaine is used alone and in combination with dexamethasone for intravenous regional anaesthesia.
Study Design & Settings: Prospective, randomized.
Material & Methods: 50 patients of ASA status I & II aged 15 to 65 years undergoing ambulatory hand surgery were randomly divided to receive either 40 ml of 0.5% lidocaine (control group) or 40 ml of 0.5% lidocaine & 8mg dexamethasone for IVRA (study group). Onset, duration & recovery time of motor & sensory block were compared. Pain was assessed intraoperatively & postoperatively using Visual Analog Scale Score (VAS). Time to request for first analgesia & total analgesic consumption in 24 hrs were noted.
Results: There was no statistically significant difference in the onset and duration of sensory and motor block. The recovery time of sensory & motor block was significantly longer in the study group. The patients in the study group had a significantly longer pain free interval. The total analgesic consumption in the first 24 hrs & the number of patients requiring analgesia was significantly lower in the study group.
Conclusions: The addition of 8mg dexamethasone to 0.5% lidocaine for IVRA significantly prolongs the recovery time of sensory & motor block, reduces the analgesic consumption postoperatively for 24 hrs and prolongs the time to request for first analgesic
The optimum intravenous fluid to be used during transurethral resection of prostate (TURP) for the prevention of hyponatremia
: Development of hyponatremia is a known problem with TURP, which can lead to cerebral edema, seizures, coma and death; but there is paucity of literature to know which Intravenous (iv) fluid is optimum to be used during TURP to minimize or prevent this complication.
Objectives: To find out the optimum iv fluid out of Normal Saline (NS), Dextrose Normal Saline (DNS) and Ringer’s Lactate (RL) to be used during TURP to minimize the development of hyponatremia immediately post-operatively and 24 hours post-operatively
