publisher〈Abstract〉Patients with the disease entity termed acute renal failure with severe loin pain and patchy renal ischemia after anaerobic exercise (ALPE) manifests non-oliguric renal failure, usually accompanied by few urinary abnormalities such as hematuria and proteinuria. This condition, diagnosed by the demonstration of hypouricemia in addition to these features, usually responds well to conservative treatment. However, differentiation from other renal disorders occasionally proves difficult, which can lead to unnecessary steroid administration and renal biopsy. In the patient reported here, acute glomerulonephritis (AGN) was initially suspected based on his course and urinalysis findings, although typical AGN symptoms such as oliguria, edema, and hypertension were absent. After the detection of hypouricemia, renal hypouricemia was added to the clinical picture. URAT1 gene mutation (W258X, homozygote) was demonstrated by gene analysis. The patient's mother was heterozygous for mutation at the same site

publisher[概要] 十九世紀後半から二十世紀初頭にかけて続々と発見されたミクロ粒子は, 量子論の誕生にとって極めて重要な契機であった. 量子は, 黒体放射現象の中に懐胎され, ミクロの現象へと適応されることで量子論として理論化されることになるからである. 本論は, まず, 陰極線の研究から説き起こし, それが種々の放射線を発見することへと繋がったことを概観する. そして, こうした研究が「崩壊する原子」という概念を出現させるに至り, 原子概念をも変革していった過程を示す. すなわちこれは, 現代的な原子概念の誕生の歴史でもある. つまり, 広く述べれば, 本論は, 現代物理科学が抱く物質観とそれに基づく世界観の原点の探求と考察を提示するものである. [Abstract] Microscopic particles were discovered in the period from the ending times of the 19th century to the beginning times of the 20th century are the very important moments for the birth of the quantum theory. The notion of quantum was born in black-body radiation and applied to the microscopic phenomena. Since, the notion of the quantum had been polished historically during the process of the applications to microscopic particles phenomena. We will consider the research process of the cathode rays first. This lead to the discoveries of the various radio rays. Furthermore, these researches lead to the revolutionary notion of the self-splitting atom. This is the history of the birth of the notion of the modern atom. This article shows this thought and the historical research process of the origin of the modern physics material view and the basis for the worldpicture

〈Case Reports〉Catheter Ablation of Dual-Loop Macro-Reentrant Tachycardia under Electroanatomical Mapping Guidance Using Multiple-Electrode Sampling Catheter in Enlarged Right Atrium after Repair of Tetralogy of Fallot : A Case Report
publisher〈Abstract〉We describe a 47-year-old woman who underwent tetralogy of Fallot repair at the age of 6 years and radiofrequency catheter ablation of macro-reentrant right atrial tachycardia (rt-AT) 41 years after surgery. A three-dimensional electroanatomical mapping system helped to determine the macroreentrant circuit. Although the right atrium was enlarged by the burden on the right side of the heart, multiple samplings on contact bipolar electrograms generated using a multipolar catheter enabled the production of high-density activation and voltage maps (429 points) within 30 minutes. The activation map revealed a dualloop reentrant circuit surrounding the superior and inferior vena cava, and clarified a critical isthmus located in the low-voltage area on the postero-lateral side of the right atrium. The rt-AT was terminated by ablation of the isthmus without damaging the nearby right phrenic nerve
