    Reverse mortgages: Exploring the impact of risk factors by source

    In recent years, reverse mortgages have gained attention as suitable financial instruments for individuals of retirement age, partic- ularly those classified as “house-rich but cash-poor.” Despite growing interest in countries such as the US, the UK, and Australia, the Italian market remains underdeveloped in this area. From a lender’s perspective, these contracts are perceived as complex, mainly due to the management of various risk components: longevity risk, financial risk, and house price risk. In this study, we model house price risk using a Geometric Brown- ian Motion, financial risk with a CIR model, and longevity risk with the Lee-Carter model, using Italian data. Our goal is to provide lenders with insights into the impact of each risk factor through a time-dependent profit/loss function. This approach aids in identifying the most critical sources of risk, enabling lenders to take appropriate measures to mitigate potential financial threats. We conduct Monte Carlo simulations of the chosen stochastic risk models and apply a single-component VaR assessment procedure. Our findings indicate that house price risk is the most significant risk factor of a Re- verse Mortgage portfolio. Moreover, the lender can control the financial risk through the analysis of the risk premium values. The results also highlight that, beyond a certain time horizon, the lender’s exposure sta- bilizes, with profitability emerging in the long run

    Il territorio contemporaneo affronta una sfida decisiva: innovare l’urbanistica e il progetto della città attraverso i principi dell’economia circolare e con un approccio ecologico alla rigenerazione urbana. Questo volume raccoglie gli esiti della ricerca “EcoRegen. Economie Circolari e Rigenerazione dei Territori”, condotta da un gruppo multidisciplinare del Dipartimento di Architettura dell’Università degli Studi di Napoli Federico II, impegnato nella sperimentazione di strategie circolari per la trasformazione urbana e il progetto dello spazio pubblico. Il libro adotta una prospettiva metabolica per rigenerare la città come organismo complesso attraversato da flussi di risorse, energia e scarti, e come ecosistema sociale capace di contrastare le diseguaglianze e definire nuovi standard di benessere collettivo. Le aree urbane concentrano oltre il 70% del consumo globale di risorse e generano più della metà dei rifiuti mondiali: da questa consapevolezza, la ricerca esplora la trasformazione di aree di scarto, spazi dismessi, quartieri marginali e territori in abbandono, in opportunità concrete per una rigenerazione sostenibile. La costa orientale di Napoli – tra i territori a maggiore densità abitativa d’Italia, segnata da dismissione industriale e rischio vulcanico – costituisce il laboratorio sperimentale di un modello trasferibile di piano e di progetto. Analizzando i sette comuni costieri tra Napoli e Castellammare di Stabia, la ricerca sviluppa un metodo operativo articolato in vision strategiche, schemi direttori e un abaco di soluzioni circolari, che individua come prioritari i possibili spazi di welfare nei quartieri di edilizia residenziale pubblica. L’approccio multiscalare proposto – dal territorio al dettaglio urbano – dimostra come rigenerare in chiave circolare: dalla riqualificazione della linea di costa al recupero delle aree agricole periurbane, fino alla creazione di spazi operativi per il riciclo, concepiti come attrezzature pubbliche multifunzionali.     Comparison between cold incremental and stretch forming of flax fiber-reinforced polypropylene composites

    Incremental sheet forming, born as a versatile and cost-effective alternative to traditional procedures to deform incrementally and without dedicated dies flat metal sheets into complex three-dimensional shapes, has recently been gaining attention as a forming technique for polymers and composites. Among these, natural fiber-reinforced thermoplastics have been climbing in popularity because natural fibers are widely available, act as effective polymer reinforcement, and make the composite semi-biodegradable. Through compression molding and using natural woven fabrics, it is also possible to achieve good fiber/matrix coupling without compromising the environmental benefits with chemical coupling agents and/or treatments. This experimental study compares cold incremental and stretch forming processes of flax woven fabric-reinforced polypropylene composites for the manufacture of spherical caps. By considering formability, geometrical accuracy, forming forces, power, and energy consumption, the study highlights the effectiveness of incremental forming applied to these biobased composites, without resorting to full counter dies and heating stages, to manufacture components with increased mechanical performance

    Effectiveness of tezepelumab in severe asthma: A multicenter real-world study

    Severe asthma is a complex, heterogeneous disease that remains a major therapeutic challenge. Despite several biologics targeting type 2 (T2) inflammation, some patients remain uncontrolled, highlighting the need for upstream interventions. Tezepelumab, a monoclonal antibody against thymic stromal lymphopoietin (TSLP), has shown broad efficacy in randomized trials regardless of eosinophilic status or biomarker levels. We conducted a prospective, multicenter, observational study to assess real-world effectiveness and safety of tezepelumab in severe asthma. Thirty patients were enrolled at two tertiary centers in Italy between September 2023 and December 2024. Inclusion criteria were a severe asthma diagnosis per ERS and GINA 2024 guidelines and inadequate control despite maximal inhaled therapy. Tezepelumab was given at 210 mg every 4 weeks. Clinical, functional, and biomarker data were collected at baseline, 1 month, and 6 months. After six months, patients showed significant improvement in Asthma Control Test (ACT) score, with marked reductions in oral glucocorticoid use and exacerbation rate. Sinonasal symptoms improved over time. Blood eosinophils and FeNO decreased significantly, while total IgE remained unchanged. Lung function improved in both FEV1/FVC ratio and FEF25–75, suggesting benefit on airflow limitation and small airway function. No serious adverse events occurred. Improvements were consistent in both T2-high and T2-low subgroups. This real-world study confirms tezepelumab’s clinical effectiveness and safety in severe asthma. Benefits across inflammatory phenotypes support TSLP blockade as a broad-spectrum therapeutic approach. Larger, longer-term studies are warranted to confirm results and identify response predictors

    Peptide receptor radionuclide therapy (PRRT) in high grade neuroendocrine neoplasms: a systematic review and meta-analysis

    Purpose: We performed a systematic review and meta-analysis to investigate the efficacy of peptide receptor radionuclide therapy (PRRT) by using radiolabeled somatostatin analogues in high grade gastro-entero-pancreatic neoplasms (GEP-NEN). Methods: All clinical studies published up to March 2025, including patients with grade 3 (G3) GEP-NEN treated with [177Lu] Lu-DOTA-TATE and/or [90Y] Y-SSA, were identified based on systematic searches in the PubMed and Embase databases. Eligible studies had to report at least one of the following outcomes: 1) response to therapy evaluated according to RECIST (version 1.1 as complete response, partial response, stable disease, progressive disease; 2) median progression free survival (PFS) with 95% confidence intervals (CI); 3) median overall survival (OS) with 95% CI. Objective response rate (ORR) and disease control rate (DCR) were considered as primary outcomes. Results: The final analysis included 7 studies accounting for a total of 317 patients with G3 disease (ranging from 18 to 112). Six studies evaluated response to therapy according to RECIST in 288 patients, and in 4 of them patients were also stratified according to ki67 values. The pooled ORR and DCR were 34% (95% CI: 22-46) and 64% (95% CI: 52-76), respectively. The funnel plot indicated no publication bias among these studies. Five studies reported in 239 patients median PFS with 95% CI, with a pooled estimate of 13.88 (95% CI: 10.33-18.64) months. Three studies fully reported in 174 patients median OS with 95% CI, with a pooled estimate of 29.95 (95% CI: 19.80-45.30) months. Conclusions: In patients with G3 GEP-NEN, PRRT may provide substantial disease control and response rates, and it seems able to delay the progression of the disease

    SHORT-COURSE RADIOTHERAPY FOR OLDER PATIENTS WITH LOCALLY ADVANCED RECTAL CANCER AND UNFIT FOR CHEMOTHERAPY: THE SOFT STUDY

    Background: The treatment of locally advanced rectal cancer (LARC) in older patients is not sufficiently standardized in international guidelines. Short-course radiotherapy (SCRT) followed by delayed surgery has emerged as a promising therapeutic option for these patients, offering potential benefits in terms of reduced treatment time and toxicity, along with improved convenience. Materials and Methods: From February 2019 to April 2024, a total of 141 older patients aged ≥ 70 years [median age = 79 (range, 70–91)], unfit for chemotherapy and with LARC (stage II-III) adenocarcinoma, underwent SCRT (5 daily fractions of 5 Gy each for a total dose of 25 Gy) followed by delayed surgery (no earlier than 6 weeks after the end of treatment). The study was conducted at 11 centers in Italy. Down-staging rates, relapse-free survival (RFS), overall survival (OS), cancer-specific survival (CSS), safety, and mortality were analyzed. Results: Down-staging occurred in 87 cases (61.7 %). Complete radiological responses after SCRT and before surgery were reported in 10 patients (7.1 %), while partial responses were reported in 77 (54.6 %) cases. All patients underwent delayed surgery. The R0 resection rate was 93.6 %. Eight patients (5.7 %) had a pathological complete response (pCR). At a follow-up of 70.5 months, the median RFS, OS and CSS were 31.5, 40.5 and 41.5 months, respectively. Post-operative severe morbidity was 23.4 %, and mortality was 2.8 %. Conclusions: Overall, SCRT offers a viable alternative to conventional long-term radiotherapy plus chemotherapy in older patients with LARC, with favorable results in terms of efficacy, limited toxicity and low mortality
