Evaluation of OnabotulinumtoxinA Treatment in Patients with Concomitant Chronic Migraine and Temporomandibular Disorders (vol 55, pg 330, 2018)

Anthropometric differences in the newborns with brachial plexus palsy, clavicle fracture in pregnancies without risk factor
Amaç: Omuz distosisi açısından düşük risk taşıyan olgularda brakiyal pleksus felci, klavikula kırığı ve omuz/humerus kırığı komplikasyonları nı yaşayan yenidoğanların antropometrik verilerinden yararlanarak antenatal değerlendirmeye yönelik öngörü oluşturmak. Yöntem: Distosik doğuma neden olabilecek fetal makrozomi, maternal diyabet, maternal obezite ve gebelikte aşırı kilo alımı, omuz distosisi öyküsü, doğum indüksiyonu, epidural anestezi gibi risk faktörleri dışlandıktan sonra hastane veri tabanından retrospektif olarak çıkarılan doğum komplikasyonu olguları, aynı obstetrik ve demografik özellikleri taşıyan, ancak doğum travması yaşamayan, aynı tartı ve cinsiyette yenidoğan verileri ile birebir eşleştirildi. Bulgular: 185 yenidoğan komplikasyonu gözlendi; bunların 149 tanesi klavikula kırığı, 8 tanesi omuz ve humerus kırığı, 28 tanesi de brakiyal pleksus felci olarak sınıflandırıldı. Doğum komplikasyonu yaşanan olgularda doğumun 2. evresinin anlamlı şekilde daha uzun olduğu görüldü (p=0.01; 22.41±6.98 dakikaya karşılık 24.23±6.43 dakika). Omuz distosisi, komplikasyonların yaşandığı çalışma grubunda daha sık gözlendi (p=0.0001; sıklığı %32.97’ye karşılık %2.7). Antropometrik ölçümler açısından göğüs çevresi / baş çevresi oranları nın ROC çalışması anlamlı saptandı, oranın 0.97’den büyük olması durumunda (AUC=0.903; sensitivite %77.84, spesifisite %89.73, PPD %88.3, NPD %80.2, LR 7.58) omuz distosisi ve komplikasyonları öngörebileceği görüldü. Komplikasyonların 4000 gramın üstündeki olgularda anlamlı olarak artmaya başladığı saptandı (p=0.029). Sonuç: Doğum kanalı içinde omuzlar ve uzantıları travmaya açıktı r. İleri sürdüğümüz hipotezde neonatal antropometrik veriler komplikasyonlar ile anlamlı şekilde korele bulunmuştur. Bir öngörüde bulunmak için, antenatal dönem ve özellikle de eylem sırasında fetal baş ölçümlerinin yanı sıra ayrıca bisakromiyal çap ve göğüs çevresi ölçümleri yardımcı olabilir.Objective: To make predictions for antenatal evaluation by using anthropometric data of the newborns which undergo the complications of brachial plexus palsy, clavicle fracture and shoulder/humerus fracture in cases with low risk in terms of shoulder dystocia. Methods: After the risk factors that may cause deliveries with dystocia such as fetal macrosomia, maternal diabetes, maternal obesity and excessive weight gain during pregnancy, history of shoulder dystocia, labor induction and epidural anesthesia were ruled out, birth complication cases found in the hospital database retrospectively were matched one by one with data of the newborns which had same obstetric and demographic characteristics, same weights and genders but did not undergo labor trauma. Results: A total of 185 newborn complications were observed, and of these complications, 149 were classified as clavicle fracture, 8 as shoulder and humerus fracture, and 28 as brachial plexus palsy. It was seen that the 2nd stage of labor was significantly longer in the cases which underwent birth complication (p=0.01; 22.41±6.98 minutes vs. 24.23±6.43 minutes). Shoulder dystocia was more frequent in the study group which had complications (p=0.0001; 32.97% vs. 2.7%). In terms of anthropometric measurements, the ROC analysis of thorax circumference / head circumference ratio was significant, and it was seen that shoulder dystocia and the complications could be predicted in case that the ratio is higher than 0.97 (AUC=0.903; sensitivity 77.84%, specificity 89.73%, PPV 88.3%, NPV 80.2%, LR 7.58). It was found that the rate of complications increased significantly in cases which were above 4000 g (p=0.029). Conclusion: Shoulders and its extensions are vulnerable to trauma within birth canal. In our hypothesis, neonatal anthropometric data were significantly correlated with the complications. In order to make a prediction, bisacromial diameter and thorax circumference measurements as well as fetal head measurements may be helpful during antenatal period and labor in particular
Effects of trimetazidine on nerve regeneration in a rat sciatic nerve injury model

METHOD: We performed end-to-end primary repair of sciatic nerves in rats and showed TMZ's regenerative effect. For this objective 30 male Sprague Dawley albino rats were used. Surgery+water group, rats were assigned to a placebo group and were given water by oral gavage. Surgery+TMZ group, rats were given trimetazidine by oral gavage. All medications were given for 12 weeks. Motor function test was performed. Afterwards, electromyography (EMG) recording was done. Finally, blood samples were taken, the animals were euthanized andsciatic nerve was removed. RESULTS: the amplitudes of compound muscle action potential (CMAP) increased significantly in the Surgery+TMZ group when compared with the group that have been given Surgery+Water. Nerve growth factor (NGF) immunoexpression in the Schwann cell was significantly increased in the Surgery+TMZ group compared with the Surgery+Water group. Moreover, fibrosis score was reduced in the Surgery+TMZ group compared to the Surgery+Water group. CONCLUSIONS: in conclusion, we demonstrated the superiority of TMZ on nerve healing in our experimental study which was evaluated with comparative groups (Tab. 3. Fig. 2, Ref. 31)

TRAUMATIC KNEE INJURY PATTERNS IN ANATOLIAN FOLK DANCERS: A CASE SERIES AND LITERATURE REVIEW
WOS: 000497315600001PubMed: 31543518Lower extremities, especially the knee region, are susceptible to traumatic injuries because of long-lasting hard landings and impacts. Most of the injuries described in the literature are associated with ballet. in this review study, the authors tried to present the traumatic knee injury patterns of the Anatolian folk dance. the Fire of Anatolia dance group consists of 82 dancers (37 males [45.1%] and 45 females [54.9%]) with the mean age of 27.96 (SD = 5.05) years (range: 18-38 years). the major folk dances of the region are Zeybek, Halay, Horon, Teke, Roman, Karsilama, Bar and Lezginka ("the Caucasian"). the dancers suffered from 9 orthopedic injuries requiring surgical treatment (3 meniscus tears, 4 anterior cruciate ligament tears, 1 posterior cruciate ligament tear, 1 patellar dislocation) during a 10-year period. the authors investigated solely the traumatic injuries of these folk dance styles and aimed at revealing the traumatic knee injury patterns in this case series and literature review. on the one hand, the Anatolian folk dancers experienced meniscus tears following frequent squats and twists on single leg stances, typical of Horon and Zeybek. on the other hand, anterior cruciate tears happened after jumps and landings in the Caucasian (Lezginka jump) dance. A posterior cruciate ligament tear was also seen after the Caucasian dance landing. the split figure in the Karsilama dance ended up with patellar dislocation. Certain dance figures seem to be related to specific types of injuries

Postoperative Myocardial Injury Does Not Predict Early and 1-Year Mortality After Living Donor Liver Transplantation

Preoperative cardiac troponin-I (cTnI) elevation has been shown to be a predictor of mortality after liver transplantation. Myocardial injury after non-cardiac surgery (MINS) has been defined as elevation of serum cardiac troponin levels in the perioperative period that does not fulfill the criteria for myocardial infarction. MINS has been shown to be a prognostic factor for in-hospital and long-term mortality, but there is limited data in patients undergoing living-donor liver transplantation (LDLT). in this study, we aimed to evaluate the relationship between MINS and postoperative mortality. Material and methods. Patients who had undergone adult LDLT at Florence Nightingale Hospital Liver Transplantation Unit between December 2012 and December 2015 were retrospectively analyzed for 30-day in-hospital and 1-year mortality. Myocardial injury was defined as cTnI level above 0.04 ng/mL. Patients (N = 214) were divided into 2 groups according to postoperative cTnI levels. the following were the exclusion criteria: 1. patients under 18 years old, 2. patients undergoing deceased-donor liver transplantation or dual liver-kidney transplantation, 3. cTnI elevation due to other causes (sepsis, renal failure, pulmonary embolism, myocardial infarction), and 4. patients without postoperative troponin levels. Results. MINS occurred in 123 (57.4%) patients after LDLT. There was no difference between the groups according to age, sex, creatinine levels, presence of ischemic heart disease, hypertension, diabetes mellitus, and tobacco use. the presence of MINS did not predict 30-day and 1-year mortality in the study population. Conclusion. Myocardial injury detected by serum cTnI elevation was frequent after LDLT; however, it was not associated with 30-day in-hospital and 1-year mortality
Türkiye’de akciğer kanserinde genetik mutasyonların bölgesel dağılımı (REDIGMA)
Introduction: The results of standard chemotherapy in lung cancer are not very satisfactory, so it is important to identify genetic mutations that provide targeted therapies. Recent reports have suggested influences of racial difference on the frequency of mutation in lung cancer. We aimed to determine the frequency and regional distribution of genetic mutations of non-small cell lung cancer (NSCLC) in Turkey. Materials and Methods: Regional distribution of genetic mutations in lung cancer in Turkey (REDIGMA) study was carried out as a prospective, cross-sectional, observational study in a large number of centers in which lung cancer patients were followed and could perform genetic mutation analysis on patients’ biopsy materials. Results: The 703 patients (77.7% male, mean age 63.3 ± 12.5 years) who were diagnosed as NSCLC from 25 different centers were included in the study. Tumor samples from patients were reported as 87.1% adenocarcinoma, 6.4% squamous cell carcinoma and 6.5% other. Mutation tests were found to be positive in 18.9% of these patients. The mutations were 69.9% EGFR, 26.3% ALK, 1.6% ROS and 2.2% PDL. Mutations were higher in women and non-smokers (p< 0.000, p< 0.001). Again, the frequency of mutations in adenocarcinoma was higher in metastatic disease. There was no difference between the patient’s age, area of residence, comorbidity and clinical stage and mutation frequency. Conclusion: Our study revealed that the EGFR mutation rate in Turkey with NSCLC was similar to East European, African–American and Caucasian patients, and was lower than in East Asia.Giriş: Akciğer kanserinde standart kemoterapinin sonuçları çok tatmin edici değildir, bu nedenle hedefe yönelik tedavileri sağlayan genetik mutasyonları belirlemek önemlidir. Son raporlar, ırksal farklılığın ve bölgesel değişikliğin akciğer kanserinde mutasyon sıklığı üzerindeki etkilerini göstermiştir. Çalışmamızda küçük hücreli dışı akciğer kanseri (KHDAK)'nde genetik mutasyonların Türkiye'deki sıklığını ve bölgesel dağılımını belirlemeyi amaçladık. Materyal ve Metod: Türkiye'de akciğer kanserinde genetik mutasyonların bölgesel dağılımı (REDIGMA) çalışması, akciğer kanseri hastalarının takip edildiği ve hastaların biyopsisinde genetik mutasyon analizi yapılabilecek çok sayıda merkezde prospektif, kesitsel ve gözlemsel bir çalışma olarak gerçekleştirildi. Bulgular: Çalışmaya 25 farklı merkezden KHDAK tanısı konan 703 hasta (%77.7 erkek, ortalama yaş 63.3 ± 12.5 yıl) alındı. Hastalardan alınan tümör örnekleri %87.1 adenokarsinom, %6.4 skuamöz hücreli karsinom ve %6.5 diğer olarak bildirildi. Mutasyon testleri bu hastaların %18.9'unda pozitif bulundu. Mutasyonlar %69.9 EGFR, %26.3 ALK, %1.6 ROS ve %2.2 PDL idi. Mutasyonlar kadınlarda ve sigara içmeyenlerde istatistiksel olarak daha yüksek tespit edildi (p< 0.000, p< 0.001). Yine, adenokarsinomdaki mutasyonların sıklığı metastatik hastalıkta daha yüksekti. Hastanın yaşı, ikamet alanı, komorbiditesi, klinik evresi ve mutasyon sıklığı arasında farklılık saptanmamıştır. Sonuç: Çalışmamızın sonucunda Türkiye geneli mutasyon pozitifliği literatür ile karşılaştırıldığında Türkiye’de KHDAK'lı hastalarda EGFR mutasyon oranının Doğu Avrupa, Afrikalı-Amerikalı ve Kafkasyalı hastalara benzer ve Doğu Asya'dan daha düşük olduğu ortaya koyulmuştu
The protective effects of Delta(9)-tetrahydrocannabinol against inflammation and oxidative stress in rat liver with fructose-induced hyperinsulinemia

Delta(9)-tetrahydrocannabinol (THC) is known as a therapeutic agent for clinical and experimental applications. The study aims to investigate the effects of THC treatment on inflammation, lipid profiles and oxidative stress in rat liver with hyperinsulinemia. Methods Sprague-Dawley rats were divided into groups: control, fructose (10% fructose in drinking water for 12 weeks), THC (1.5 mg/kg/day for the last 4 weeks, intraperitoneally) and fructose+THC groups. Biochemical parameters were measured spectrophotometrically. ELISA method was used for insulin measurement. Apoptosis and inflammation markers were detected by the streptavidin-biotin peroxidase method. Key findings The consumptions of food and fluid are inversely proportional to fructose and non-fructose groups. Insulin levels were the highest in fructose group. The reduced glutathione-S-transferase level significantly increased in fructose + THC group compared with fructose group. Total cholesterol level in the fructose + THC group was higher than the fructose group. Caspase-3 and NF-kappa beta immunopositive cell numbers increased in fructose + THC rats compared with fructose group. The number of IL-6 immunopositive cell decreased in fructose + THC group compared with fructose group.
