The Effects of Attachment Styles and Psychosociodemographic Variables on Alexithymia Levels in Adults
Amaç: Bu araştırmada; erişkin bireylerde bağlanma biçimleri (kaygılı ve kaçınmacı bağlanma boyutları) ile psikososyodemografik değişkenlerin aleksitimi düzeyi üzerindeki etkileri araştırılmıştır. Gereç ve Yöntemler: Araştırmaya, Türkiye’nin çeşitli bölgelerinde yaşayan ve çalışmaya gönüllü olarak katılan 192 (%69,6)’si kadın, 84 (%30,4)’ü erkek toplam 276 birey katılmıştır. Araştırmada, psikososyodemogra- fik özelliklerin belirlenebilmesi için araştırmacı tarafından oluşturulan, “Demografik Bilgi Formu”, aleksitimi düzeylerini ölçmek amacıyla “Toronto Aleksitimi Ölçeği (TAÖ-20)”, bağlanma biçimlerini belirle- mek amacıyla da “Yakın İlişkilerde Yaşantılar Envanteri II (YİYE-II)” kullanılmıştır. Bulgular: Erişkin bireylerde kaygılı ve kaçınmacı bağ- lanma biçiminin aleksitimi toplam puanını ve duygularını tanıma zor- luğu alt boyutunu yordadığı görülmektedir (p<0,01). Bununla birlikte kaçınmacı bağlanma biçiminin, duygularını ifade etme zorluğu alt bo- yutunu yordadığı da gözlenmiştir (p<0,01). Öte yandan aleksitimi top- lam puanı ile dışa-vuruk düşünce puanının cinsiyetler açısından arasında orta düzeyde, düşük güçte anlamlı bir ilişki bulunmaktadır (p<0,01). Analiz sonuçları, TAÖ-T, TAÖ-DTZ, TAÖ-DİEZ ve TAÖ- DVD puanlarının, bireylerin eğitim seviyesine göre farklılaştığını gös- termiştir. Son olarak TAÖ-DVD haricinde TAÖ-T, TAÖ-DTZ ve TAÖ-DİEZ puanlarının, bireylerin romantik ilişki durumlarına göre farklılaştığını göstermiştir (p<,05). Sonuç: Erişkin bireylerde, kaygılı ve kaçınmacı bağlanma biçimi olanlar aleksitimik özellikler göster- mektedir. Kaçınmacı bağlanma biçimi olanlarda, duygularını ifade etme zorluğu saptanmıştır. Aleksitiminin dışa-vuruk düşünce özelliği erkek- lerde kadınlardan fazla bulunmuştur. Eğitim seviyesi arttıkça, aleksiti- mik özellikler taşıma olasılığı azalmaktadır. Romantik ilişkisi olmayan bireylerde, aleksitimik özellikler daha fazla görülmektedir.Objective: In this research, the attachment styles (anx- ious and avoidance attachment styles) and psychosociodemographic variables on alexithymia levels in adults have been researched. Ma- terial and Method: 276 adults, residing in various regions of Turkey, have volunteered for this research, of whom 192 (69.4%) are female and 84 (30.4%) are male. To determine the psychosociodemographic traits a “Demographic Survey” created by the researcher, to deter- mine the alexithymia levels “Toronto Alexithymia Scale (TAS-20)” and to determine the attachment styles “Experiences in Close Rela- tionships-Revised (ECR-R)” have been employed. Results: Anxious and avoidance attachment styles have regressed the total alexithymia score and difficulty in identifying feelings subscale (p<0.01). Fur- thermore it has been observed that avoding attachment style has re- gressed into the difficulty in describing feelings subscale (p<0.01). On the other hand, there was a moderate, low-power significant re- lationship between the total score of alexithymia and the self-effi- cacy score in terms of gender (p<0.01). The results of the analysis showed that TAS-T, TAS-DTZ, TAS-DIEZ and TAS-DVD scores differ according to the level of education of the individuals. showed differentiation (p<.05). Conculusion: In adult individuals, anxious and avoidant attachment styles show alexithymic characteristics. Dif- ficulty in expressing their emotions was found in those with avoidant attachment style. Alexithymia was found to be more manifest in males than females. As the level of education increases, the likeli- hood of carrying Alexithymic properties decreases. Alexithymic fea- tures are more common in individuals who do not have romantic relationships
COGNITIVE STATUS ASSOCIATED WITH VASCULAR RISK FACTORS
Giriş: Vasküler risk faktörlerinin kognitif bozukluğun değiştirilebilir risk faktörlerinden olduğu öne sürülmektedir. Çalışmamızda, kardiyovasküler hastalık ve demans öyküsü olmayan, 60 yaş ve üzerindeki bireylerde on yıllık koroner kalp hastalığı ve inme risk skorları ile kognitif durum arasındaki ilişki araştırıldı. Gereç ve Yöntem:İnme Merkezi “İnme ve Demans Primer Koruma Polikliniği” ne ardışık olarak başvurmuş olan bireylerin verileri retrospektif olarak incelendi. On yıllık koroner kalp hastalığı ve inme risklerini değerlendirmek amacıyla sırasıyla Framingham Risk Skoru (FRS) ve ModifiyeFramingham İnme Risk Profili (FSRP) kullanıldı. Kognitif fonksiyonlar Montreal Bilişsel Değerlendirme Ölçeği (MoCA) ile değerlendirilmişti. Bu testin Türkiye standardizasyon çalışmasına göre MoCA? 21 olanlar kognisyonu bozuk olarak kabul edildi. Kognitif durum ile vasküler risk arasındaki ilişki multivariat lojistik regresyon analizi ile araştırıldı. Yaş, cinsiyet, eğitim düzeyi, diğer kognisyonla ilişkili olabilecek etkenler, antihiperlipidemik, antidiyabetik ve antihipertansif tedavi kullanımı analize alındı. Bulgular: Örneklem 167 bireyden (40 erkek ve 127 kadın) oluşmaktaydı. Ortalama yaş 68 (SD: 6 Aralık: 28) idi. Ortalama FRS ve FSRP sırasıyla 8(20-3) ve 7(11-4) idi. Elli beş kişinin (%33) kognitif durumu bozuktu. Framingham Risk Skorundaki her %10’ luk artış kognisyonun bozuk olmasıyla ilişkiliydi (OR:1,669, 95%CI 1,038-2,682). İleri yaş, düşük eğitim düzeyi, alkol ve antihiperlipimik tedavi kullanmıyor olmak da ilişkili diğer bağımsız faktörlerdi. Ancak FSRP ile kognitif durum arasında ilişki saptanmadı. Sonuç: Kardiyovasküler hastalık öyküsü olmayan yaşlı bireylerde, FRS ile ölçülen global vasküler risk kognitif bozuklukla ilişkiliyken FSRP ile benzer ilişki saptanmamıştır. Bu ilişki, rutin klinik değerlendirmede kullanılan ve hafif kognitif bozukluğa duyarlı nöropsikolojik bir test olan MoCA ile gösterilmiştir.Introduction:Vascular risk factors have been showed to contribute to cognitive impairment. This study examined the relationship between 10-year risk for coronary heart disease (CHD) or stroke and cognitive function in older people, free of cardiovascular disease. Material and Method: Participants were consecutive attenders of a “stroke and dementia primary prevention clinic”. The Framingham Risk Score (FRS) and Modified Framingham Stroke Risk Profile (FSRP) were used to assess 10-year risk of CHD and stroke, respectively. Cognitive function was measured with Montreal Cognitive Assessment Scale (MoCA). Cognitive status (CS) was classified as impaired (MoCA?21) vs. normal, which had been previously validated in theTurkish population. Correlations between cognitive status and global vascular risk were analyzed with ultivariate logistic regression analyses. Age, gender, education level, other potential correlates of cognitiveability, treatment for hyperlipidemia, hypertension and diabetes were included in the analyses. Results:The sample consisted of 167 individuals (40 men and 127 women). Mean age was 68 (SD: 6 Range: 28). Mean FRS and FSRP were 8(20-3) and 7(11-4), respectively. Fiftyfive individuals (%33) had impaired CS. Individuals with higher FRS (increments by 10% in FRS) had more impaired CS (adjusted OR:1, 669, 95%CI 1,038 to 2,682). No association was shown between FSRP and CS. Higher age, lower education level, absence of alcohol consumption and absence of treatment for hyperlipidemia were the other independent predictors of impaired CS. Conclusion: Our findings indicated that in older individuals free of cardiovasculardisease, global vascular risk is associated with impaired cognitive function which was accounted forby FRS rather than FSRP. This association was demonstrated with the use of a neuropsychological test which is sensitive for mild cognitive impaiment in routine clinical setting

Serum chymase levels in obese individuals: the relationship with inflammation and hypertension
WOS: 000582566800010Background: Inflammation related hypertension is reported in obesity due to synthesis of angiotensinII (Ang-II) and proinflammatory compounds in obese adipose tissue. Mast cell chymase (MC) also stimulate Ang-II synthesis, and activate transforming growth factor beta-1 (TGF-beta 1) and matrix metalloproteinase-9 (MMP-9). the aim of our study is to evaluate the relation of serum chymase levels, a serine protease enzyme secreted from mast cells, in obese patients with hypertension and cytokines that lead to cell damage. Materials and methods: Three study groups are composed of individuals aged between 19 and 63 with following characteristics; (1) control (n=30): healthy subjects with body mass index (BMI) 30; (3) obese+ HT (n=20): patients BMI >30 and hypertension. Serum Ang-II, MC, TGF-beta 1 and MMP-9 are determined by commercial ELISA. Angiotensin converting enzyme (ACE) activity is determined with enzymatic colorimetric assay. Results: Serum chymase levels did not vary between groups. Chymase levels showed significant negative correlation with ACE activity (r = -0.278, p= 0.013) and positive correlation with Ang-II levels (r=0.251, p=0.024). No correlation was evident between chymase levels and hsCRP, TGF-beta 1 and MMP-9. Conclusion: Serum chymase, Ang-II, TGF-beta 1 and MMP-9 levels did not change in obese and hypertensive-obese patients despite evident hyperinsulinemia, increased insulin resistance and elevated hsCRP levels.Research Fund of Istanbul UniversityIstanbul University [21867]This work was supported by the Research Fund of Istanbul University; Project Number: 21867. Comparison of the Effects of Extracorporeal Irradiation and Liquid Nitrogen on Nerve Recovery in a Rat Model
WOS: 000512563000001PubMed: 32013622Aim of the study: Biologic reconstruction using tumor-bearing bone autografts devitalized by liquid nitrogen or extracorporeal irradiation (oncological sterilization) is a safe and effective method in musculoskeletal surgery. the purpose of this study was to examine the effects of these two oncological sterilization methods on nerve recovery. Methods: A total of 48 rats were randomly divided into 3 groups as autograft, irradiation and liquid nitrogen groups. A nerve defect created in the right sciatic nerve was reconstructed with an autograft obtained from the nerve itself. Group I underwent reconstruction with standard nerve autograft. Group II and Group III underwent reconstruction with devitalized nerve autograft treated through extracorporeal irradiation and liquid nitrogen, respectively. the left sciatic nerves of the rats served as control. Electromyography, motor function test and histomorphological analysis were performed to assess the nerve recovery on the 3rd (early stage) and 4th months (late stage). Results: Electrophysiological assessment revealed better results in irradiation group compared with liquid nitrogen group in terms of myelinization and axonal regeneration. Motor performance of the autograft group was slightly better than the other groups. Histologically, autograft group demonstrated better results compared with other groups. Late-stage assessments revealed high rates of myelinization in the graft segment in liquid nitrogen group and in the segment distal to the graft in irradiation group. Conclusions: This study has demonstrated that nerve autografts treated by oncological sterilization methods may be used for nerve reconstruction in limb salvage surgery. The effectiveness of basic life support training on nursing students’ knowledge and basic life support practices: A non-randomized quasi-experimental design
Background: Provision of up-to-date information and skills training related to basic life support practices is very important for nursing students’ professional development and practitioner and education related roles. Objective: the purpose of the study was to examine the effectiveness of basic life support training on knowledge and practices among nursing students. Methods: A non-randomized quasi-experimental design (one group pre-test-post-test) was used in this study. the study was conducted in the laboratory of an undergraduate nursing school. the sample consisted of a convenience sample of 1st-year students enrolled in the undergraduate nursing class. the study sample consisted of 65 nursing students. Basic life support training included both theoretical and practical components. the students’ knowledge and practices were assessed before basic life support training. Data were collected using the knowledge assessment questionnaire. the practical skills for basic life support were observed and assessed using a checklist. the pre-and post-assessment practice scores were compared. Results: After basic life support training, level of knowledge and practical skill scores were higher compared to pre-training scores (t=-12.442, p=0.000; t=-22.899, p=0.000). There was a significant and moderate association between the adult basic life support knowledge form scores and the adult basic life support practice assessment form scores obtained after the training (r = 0.39, p<0.01). Conclusion: the study showed that basic life support training improved knowledge and skills related to basic life support practices in nursing students. Periodic basic life support training is very important for competency in this area among nursing students. © 2020 Kose S et al
The Ameliorative Effects of Ascorbic Acid on Critical Illness Polyneuropathy in Rodent Sepsis Model
WOS: 000583627400006PubMed: 33133742Although the exact pathophysiology of critical illness polyneuropathy (CIP) is still unknown, there are several hypotheses, some of which are increased inflammation and oxidative stress. We used rodent sepsis model in which we induced sepsis through cecal ligation followed by cecal puncture. We then administered ascorbic acid (AA) and evaluated outcomes. the levels of malondialdehyde (MDA), tumor necrosis factor alpha (TNF-alpha), interleukins (IL)-6 in the plasma, and heat shock protein-70 (HSP-70) levels in the sciatic nerve were measured, and also electromyography analyses were performed. While plasma MDA, TNF-alpha, and IL-6 levels were decreased significantly with AA treatment, sciatic nerve levels of HSP-70 were significantly elevated in the AA group. A significant increase in compound muscle action potential (CMAP) amplitude and a significant decrease in CMAP latency were detected in the AA group. We observed healing effects of AA on a rat model of CIP and these effects seem to be related to its anti-inflammatory and antioxidant properties
Behçet ve Multipl Skleroz Hastalarında Huzursuz Bacaklar Sendromu: Sıklık, İlişkili Olduğu Durumlar, Klinik Özellikler
Introduction: To investigate the prevalence and characteristics of Restless Legs Syndrome (RLS) in patients with Behçet’s Disease (BD) and Multiple Sclerosis (MS). Methods: Consecutive patients with BD and MS seen in the outpatient clinic were included in the study. As a control group, volunteer subjects without a known peripheral or central nervous system disorder were included. The BD group was divided into two sub-groups as BD with neurological involvement [Neuro-Behçet’s Disease (NBD)] and BD without any neurological involvement (other BD) for further evaluation. Data on demographic characteristics, medical history and family history were collected, and all patients underwent neurological examination. The patients were evaluated for the presence of diagnostic criteria for RLS. The features and severity of RLS were evaluated in patients with RLS using Restless Legs Syndrome Identification Form, and the International Restless Legs Syndrome Study Group (IRLSSG) Rating Scale. The clinical and radiological findings of patients with BD and MS were retrieved from their medical files. Results: The study included a total of 96 patients with BD (mean age 39.9±11.8; 51 males; 41 patients with NBD) and 97 patients with MS (mean age 34.97±4.1 years; 24 males). There were 100 healthy control subjects (mean age 36.18±11.10 years; 46 males). RLS was more prevalent in MS (22.8%) and NBD (22%) groups compared to the control group (10%; p=0.004 and 0.029, respectively) with a statistically significant difference. The prevalence of RLS was higher in MS patients with less disability. Most severe RLS symptoms were observed in the MS group. The rate of sleep disorders was also higher in MS group. Although stress appeared to be a factor worsening RLS in all groups, its prevalence was higher in the MS group (p=0.011). There was no correlation between the distribution of magnetic resonance imaging lesions and RLS in both MS and NBD groups. Conclusions: It is well established that RLS can accompany disorders involving the peripheral and central nervous systems such as all types of peripheral neuropathy, myelopathy, and Parkinson’s disease. The present study showed that MS and NBD also seem to be a risk factor for RLS, being associated with more severe symptoms.Amaç: Behçet Hastalığı (BH) ve Multipl Skleroz (MS) hastalarında Huzursuz Bacaklar Sendromunun (HBS) prevalans ve özelliklerini araştırmak. Yöntem: Poliklinikten takipli BH ve MS hastaları ardışık olarak çalışmaya alındı. Kontrol grubuna bilinen bir periferik veya merkezi sinir sistemi bozukluğu olmayan gönüllü denekler alındı. BH grubu nörolojik tutulumu olan BH [Nöro-Behçet Hastalığı (NBH)] ve herhangi bir nörolojik tutulumu olmayan BH (diğer BH) olmak üzere iki alt gruba ayrıldı. Demografik bilgiler, öz ve soygeçmiş özellikleri toplandı ve tüm hastaların nörolojik muayeneleri yapıldı. Hastalar HBS tanı kriterlerinin varlığı açısından değerlendirildi. Huzursuz Bacaklar Sendromu Tanımlama Formu ve Uluslararası Huzursuz Bacaklar Sendromu Çalışma Grubu (UHBSÇG) Derecelendirme Ölçeği kullanılarak HBS özellikleri ve şiddeti değerlendirildi. BH ve MS hastalarının klinik ve görüntüleme bulguları tıbbi dosyalarından derlendi. Bulgular: Çalışmaya toplam 96 BH (ortalama yaş 39,9±11,8 yıl; 51 erkek, 41’i NBH) ve 97 MS (ortalama yaş 34,97±4,1 yıl, 24 erkek) hastası alındı. 100 sağlıklı kontrol birey (ortalama yaş 36,18±11,10 yıl, 46 erkek) vardı. HBS, MS (%22,8) ve NBH (%22) gruplarında en sıktı ve kontrol grubuyla (%10) kıyaslandığında anlamlı fark bulundu (sırayla p değerleri 0,004 ve 0,029). Özürlülüğü daha az olan MS hastalarında HBS daha sıktı. HBS semptomlarının şiddeti MS grubunda en ağırdı. MS grubunda uyku bozukluğu oranı da yüksekti. Stres, tüm gruplarda HBS’yi kötüleştiren faktör olmakla birlikte MS grubunda en fazlaydı (p=0,011). Hem MS hem de NBH grubunda manyetik rezonans görüntülemedeki lezyonlarının dağılımı ile HBS arasında anlamlı korelasyon yoktu. Sonuç: Huzursuz bacaklar sendromunun, periferik nöropatilerin tüm tipleri, miyelopati ve Parkinson Hastalığı gibi periferik ve merkezi sinir sistemini etkileyen hastalıklara eşlik edebileceği iyi bilinir. Bu çalışma, MS ve NBH’nin de HBS için bir risk faktörü oluşturduğunu ve semptomların daha ağır seyretmesine neden olabileceğini göstermiştir
MR imaging pattern of tibial subchondral bone structure: considerations of meniscal coverage and integrity
WOS: 000543608800001PubMed: 32591855Objectives To compare regional differences in subchondral trabecular structure using high-resolution MRI in meniscus-covered/meniscus-uncovered tibia in cadaveric knees with intact/torn menisci. Materials and methods 3D proton density CUBE MRI of 6 cadaveric knees without significant osteoarthritis (OA) was acquired, 0.25-mm resolution. Menisci were evaluated and classified intact or torn. MR data were transferred to ImageJ program to segment tibial 3D volume of interest (VOI). Data was subdivided into meniscus-covered/meniscus-uncovered regions. Segmented VOI was classified into binary data, trabeculae/bone marrow. the trabecular bone data was used to measure MR biomarkers (apparent subchondral plate-connected bone density (adapted from spine MR), apparent trabecular bone volume fraction, apparent mean trabecular thickness, apparent connectivity density, and structure model index (SMI)). Mean value of parameters was analyzed for the effects of meniscal tear/tibial coverage. Results Nine torn menisci and 3 intact menisci were present. MR measures of bone varied significantly due to meniscal coverage/tear. Subchondral plate-connected bone density under covered meniscus regions increased from 10.9 to 23.5% with meniscal tear. Values increased in uncovered regions, 19.3% (intact) and 32.4% (torn). This reflects higher density when uncovered (p = 0.048) with meniscal tear (p = 0.007). Similar patterns were found for trabecular bone fraction (coveragep < 0.001, tearp = 0.047), trabecular thickness (coveragep = 0.03), connectivity density (coveragep = 0.002), and SMI (coveragep = 0.015). Conclusion Quantitative trabecular bone evaluation emphasizes intrinsic structural differences between meniscus-covered/meniscus-uncovered tibias. EFFECT OF DIETARY CURCUMIN ON OXIDANT-ANTIOXIDANT STATUS IN TESTIS TISSUES OF RATS FED A HIGH-FAT DIET
WOS: 000548277100011Objective: High fat diet (HFD) induced oxidative stress is known to adversely affect testicular functions. Curcumin, which is an active ingredient of turmeric spice, has been shown to reduce lipid peroxidation and stimulate cellular antioxidant enzymes. the aim of this study is to investigate the effect of dietary curcumin on the testicular oxidant-antioxidant status in rats fed on a HFD. Material and Method: Male Sprague-Dawley rats were divided into four groups. Group 1 was fed with a control diet (10% of total calories from fat). Group 2 was fed with a HFD (60% of total calories from fat). Groups 3 and 4 received a HFD and a control diet with curcumin (1g/kg diet; w/w) respectively for 16 weeks. the testis tissue was homogenized and divided into postmitochondrial and mitochondrial fractions and the reactive oxygen species (ROS) levels were measured with the fluorometric method. the malondialdehyde (MDA) and glutathione (GSH) levels and glutathione peroxidase (GPx), superoxide dismutase (SOD) and glutathione transferase (GST) activities were measured with spectrophotometric methods. Results: HFD supplementation increased postmitochondrial and mitochondrial MDA levels and decreased cytosolic GSH levels in testis. MDA levels decreased with curcumin supplementation. Moreover, GSH levels and GST activity increased. However, curcumin supplementation alone did not affect oxidant-antioxidant status. ROS levels, SOD and GPx activities did not change significantly between the groups. Conclusion: Our data has shown that curcumin supplementation with HFD may prevent testicular oxidant damage. However, further studies are needed to investigate the underlying molecular mechanisms
