Survival results according to Oncotype Dx recurrence score in patients with hormone receptor positive HER-2 negative early-stage breast cancer: first multicenter Oncotype Dx recurrence score survival data of Turkey
Background: The Oncotype Dx recurrence score (ODx-RS) guides the adjuvant chemotherapy decision-making process for patients with early-stage hormone receptor-positive, HER-2 receptor-negative breast cancer. This study aimed to evaluate survival and its correlation with ODx-RS in pT1-2, N0-N1mic patients treated with adjuvant therapy based on tumor board decisions. Patients and methods: Estrogen-positive HER-2 negative early-stage breast cancer patients (pT1-2 N0, N1mic) with known ODx-RS, operated on between 2010 and 2014, were included in this study. The primary aim was to evaluate 5-year disease-free survival (DFS) rates according to ODX-RS. Results: A total of 203 eligible patients were included in the study, with a median age of 48 (range 26-75) and median follow-up of 84 (range 23-138) months. ROC curve analysis for all patients revealed a recurrence cut-off age of 45 years, prompting evaluation by grouping patients as 45 years. No significant difference in five-year DFS rates was observed between the endocrine-only (ET) and chemo-endocrine (CE) groups. However, among the ET group, DFS was higher in patients over 45 years compared to those aged = 18, DFS was significantly higher in the former group within the ET group. However, such differences were not seen in the CE group. In the ET group, an ODx-RS >= 18 and menopausal status were identified as independent factors affecting survival, with only an ODx-RS >= 18 impacting DFS in patients aged = 18 improves DFS
The relationship between psychological flexibility, self-compassion, and posttraumatic growth in cancer patients in the COVID-19 pandemic
PurposeThe COVID-19 pandemic may have an important long-term emotional impact on patients with cancer diagnosis, as they are in the high-risk group. We aimed to evaluate the relationship between self-compassion, psychological flexibility, and posttraumatic growth, and to examine whether psychological flexibility may serve as a mediator between self-compassion and posttraumatic growth.MethodsTwo hundred fifty-three patients with cancer were included in the study. Sociodemographic and Clinical Features Data Form, Self-Compassion Scale (SCS), Freiburg Mindfulness Inventory (FMI), Acceptance and Action Questionnaire-II (AAQ-II), Cognitive Fusion Questionnaire (CFQ), and Posttraumatic Growth Inventory (PTGI) were applied to all patients.ResultsThe multivariate analysis with independent variable SCS, FMI, AAQ-II, and CFQ scores explains 49% of the variance in PTGI (F(4,248) = 60,585, p < 0.001). SC and FMI scores were found to have a positive and AAQ-II and CFQ scores a negative predictive effect on PTGI scores. The partial mediational effect of psychological flexibility on the relationship between self-compassion and posttraumatic growth was found to be statistically significant.ConclusionIn traumatic life events such as pandemics, the importance of self-compassion for posttraumatic growth and the mediator role of psychological flexibility in this relationship should be considered in order to manage the treatment process in cancer patients. These patients are more affected by the pandemic due to the nature of their malignancy and the strict protective measures they must follow as members of a high-risk group. The significance of therapies focused on psychological flexibility should be emphasized in comprehensive biopsychosocial approaches for the management of cancer patients
A Computational Content Analysis on Representation in Mental Health News Media with Global Perspectives
23rd International Conference on Computational Science and Its Applications, ICCSA 2023 -- 3 July 2023 through 6 July 2023 -- Athens -- 297179Media representations can provide important symbolic resources in the construction of perception and agendas in diverse domains particularly concerning public health and social awareness. In view of this standpoint, dominant media frames are said to be powerful in defining social problems and shaping public discourses. The majority of the relevant literature demonstrates that representations of people with mental disorder are often negative, individuals being depicted in a passive role. A grammatical analysis of language use in press reveals different perspectives regarding the assignment of a more or less prominent role. The use of active and passive verb voice is one constituent of such analysis. Framing process constitutes another mode of analysis of news media content, which is stated to be dynamic in the concept of communication. Accordingly, this study aims to provide insights into the representation of individuals that experience mental health problems. For this purpose, newspaper stories are analyzed by examining the volume and content of a sample of selected news stories on mental illness in Türkiye for a time period of six years. Six national daily newspapers make up the sample of the study which provides a content analysis on the news media dataset. The analysis results demonstrate statistical findings on the depiction of people with mental disorder. Based on the findings and considering the sensitivity of the issue, it can be noted that more emphasis can be placed on humanizing mental illness by indicating that mental disorder is real, common and possible to be treated. Health Risks Management Program in Schools: An Operational Study in Türkiye
This study was carried out to improve the quality of school health services with the operational epidemiology method. The study aimed to describe the current status of the School Health Protection and Improvement Program (SHPIP), the difficulties experienced during its implementation, to develop evidence-based solution methods, and to test the proposed solutions, in a district with a population of 400,513, 20.4% of which is of school age between the ages of 5–19. The “Health Risk Management Program at Schools”, which consists of the stages of putting the results into practice by sharing the results with the relevant parties, wasdeveloped. In this study, a cross-sectional research design was employed using questionnaire forms, while qualitative data were collected through the use of the phenomenological analysis method, specifically through the conduct of focus group interviews. SHPIP year-end evaluation forms of 191 schools were reviewed retrospectively, questionnaires were applied to 554 school staff and 146 family health center staff between 21 October 2019 and 21 November 2019 using the probabilistic sampling (simple random) method, and semi-structured focus group interviews were conducted with 10 school health study executives. The most common health risks in schools and the risks identified during the execution of school health services were identified. In order to eliminate the lack of in-service training, training modules for “School Health Management Teams” were developed and impact analyses were conducted. After the intervention, a significant difference was found in the compliance of schools with SHPIP, and the application of all components of the school health program increased from 10.0% to 65.6% (p < 0.05). The program has been integrated into the “School Health Protection and Improvement Program” (SHPIP) with the decisions of the District School Health Board and District Hygiene Council. © 2023 by the authors
Intersectionality: its origins, evolution, and reflections in the United Nations
ÖZET: Bu çalışma kesişimsellik kavramına odaklanır. Kavramın tarihini, gelişimini ve günümüzde insan hakları hukuku alanındaki yansımalarını Birleşmiş Milletler üzerinden inceler. Siyah feminizmin ürünü olarak kesişimsellik fikirlerinin kökeni 1800’lerdeki ırkçılık karşıtı ve oy hakkı mücadelelerine kadar uzanır. Bu dönemdeki siyasi çatışmaları ve Siyah kadınların bu çatışmalar içindeki konumlarını anlamak kavramın kökenini açıklamak için gereklidir. Dolayısıyla bu çalışma başlangıç noktasını Birinci Dalga Feminizm içinde anaakım feminizme karşı Siyah kadınların durumuna dayandırır. Bir kavram olarak gelişmeden önce kesişimselliğin hangi terimlere başvurularak ifade edildiğinin izi Siyah kadınların metinlerinde sürülür. Kimberle Crenshaw tarafından kesişimsellik kavramının dolaşıma sokulmasını takiben toplumsal hareketlere ve ayrımcılığa kesişimsel yaklaşımın ne ifade ettiği incelenir. Buradan yola çıkarak kesişimselliğin tahakküm ilişkilerinin incelenmesine ve dönüştürülmesine odaklanan yapısının hukukta nasıl karşılık bulduğuna ve bulacağına bakılır. İnsan hakları alanına hakim olan tek yönlü bakış açıları ayrımcılıkla mücadelede yetersiz kalır. Çünkü bu anlayış insan haklarını kompartımanlara böler. Cinsiyet, ırk, etnik köken, cinsel yönelim gibi ayrımcılık zeminleri birbirlerinden ayrıştırılarak ele alınır. Ancak kesişimsellik ayrımcılık zeminlerini birbirlerini kuran etkileşimli yapılar olarak ele alır. İnsanı oluşturan kimliklerin birbirinden ayrılamaz olduğunu vurgular. Böylelikle ayrımcılıkla mücadele konusunda insan hakları alanında paradigma değişiminin anahtarını sunar. Keza 2000’lerden günümüze BM yapısında kesişimselliğin anaakımlaşması bize bunu göstermektedir.ABSTRACT: This thesis focuses on the concept of intersectionality. It examines the concept's origins, evolution, and today's reflections in the field of human rights through the United Nations. Intersectionality as a product of Black feminism can be traced back to the anti-racist and suffrage struggles of the 1800s. Understanding the political conflicts of this period and the position of Black women in these conflicts is necessary to preserve the origin of the concept. So this thesis relies on the use of Black women against mainstream feminism at the start of First Wave Feminism. Before it developed as a concept, the expression of intersectionality is traced in Black women's texts. Following the addition of the concept of intersectionality by Kimberle Crenshaw, what the intersectional approach to social movements and discrimination means is examined. From this point of view, it is looked at how intersectionality will respond and find a path in law that focuses on examining and transforming the relations of domination. The one-sided perspectives that dominate the field of human rights are insufficient in dealing with the broad. Because this understanding divides people into compartments. It separates the grounds such as gender, race, ethnicity, and sexual orientation. However, it emphasizes that the identities that make up the human being are inseparable. It offers the key to a paradigm shift in the field of human rights. Likewise, the mainstreaming of the intersectionality of the UN structure from the 2000s to the present shows us this
Neoliberal abandon or care society? thoughts on the aftermath of the Kahramanmaraş earthquakes
ÖZET: Bu yazıda 6 Şubat 2023 Kahramanmaraş Depremleri sonrasındaki durum, neoliberal umursamazlık, dayanışma ve bakım toplumu kavramları çerçevesinde irdeleniyor. Yazıda, depremin yol açtığı olağanüstü yıkımın kısmen on yıllardır sürdürülen neoliberal politikalardan kaynaklandığı iddia ediliyor. Kâr ve ranta öncelik veren ve afet risklerini göz ardı eden bu politikalar yazıda neoliberal umursamazlık olarak nitelendiriliyor. Deprem sonrasında ise kentleri doğal afetlere daha dayanıklı hale getirmek için dayanışmayı büyütmek ve bakım odaklı topluluklar oluşturmak mümkün olabilir. Ancak neoliberal umursamazlık, bu ihtimalin önündeki en önemli tehdidi oluşturmaya devam ediyor.ABSTRACT: This essay reflects on the aftermath of the Kahramanmaraş Earthquakes on 6 February 2023 through the concepts of neoliberal abandon, solidarity, and care society. It argues that the extraordinary destruction caused by the earthquake is partly attributable to several decades of neoliberal policies that disregarded disaster risks in favor of policies that prioritized profit and rent-seeking, which it calls neoliberal abandon. In the wake of the earthquake, it may be possible to enlarge the scope of solidarity and build caring communities so as to make cities more capable to withstand natural disasters. However, neoliberal abandon still stands as the biggest threat in front of that possibility
Gendered familialism in a Mediterranean context: women's labor market participation and early childhood education and care in Turkey
This article explores the relationship between women's labor market participation and early childhood education and care (ECEC) in Turkey within a broader Mediterranean context. Since the 1990s, there have been significant changes in the familialist models in the Mediterranean region driven by women's increased labor market participation and the expansion of ECEC services. The transformations in the region have unveiled a significant link between the expansion of preschool education and an increase in women's labor market participation. Turkey missed this critical juncture in the 1990s, as indicated by the low employment rates of women and below-the-Organisation for Economic Co-operation and Development (OECD)-average preschool enrolment. Through a comparative perspective that examines the slow progress in both areas in Turkey as well as the gendered feature of its familialist model, the article emphasizes the need for closer analysis of the link between ECEC and the low labor force participation of women. Given that the expansion of ECEC in the 2000s has taken place through market-driven services, the article concludes that the link between ECEC and women's labor market participation exhibits a class dimension. Thereby, women from lower socio-economic groups are increasingly experiencing the impact of the gendered characteristics inherent in the familialist regime in Turkey
Challenging ocularcentric fairness assumptions of the video assistant referee (VAR) system in football
Football's video assistant referee (VAR) system is based on the assumption that reviewing plays on a screen refines precision and accuracy, resulting in fairer calls. The system thus reinforces ocularcentrism, the Western, modernist, rationalist thinking that privileges sight over other senses. Based on Turkey, I investigate the stakes involved in reinforcing and the conceptual opportunities opened in challenging these assumptions. The VAR system's ocularcentrism, as foreshadowed by televisation, creates an ocular mode separate from that of the field by muting it and visually dissecting its plays. This leads to false transparency along with novel relations and socialities between referees, VARs and players. As such, rather than carrying referees to objectively based or clean moments of fairness, the VAR process in fact identifies the complex socialness of fairness itself
Spatial and Temporal Returns to/in Beyoğlu: Bir Üretim Mekanı Olarak: Beyoğlu Düşerse
ÖZET: İstanbul’un kent kültürünü hem şekillendiren hem de yansıtan mekanlardan biri olmayı sürdüren Beyoğlu, Türkiye’deki kültür politikalarını anlamak için kritik önemdedir. Ebru Pelvanoğlu’nun 2022 yılında yayınladığı Bir Üretim Mekanı Olarak: Beyoğlu Düşerse kitabının, kültür politikası külliyatına katkısının ve bu amaçta ilerlerken eksik kaldığı yönlerin ele alındığı bu yazıya, tarihyazımının öznelliği ve nostalji üzerine bir tartışmayla başlanmıştır. Devamında, kitabın sosyopolitik bağlamla kurduğu ilişki gözden geçirilmiş ve “Beyoğlu Kültür Yolu” üzerine yapılan tartışmalarla beraber, Beyoğlu’ndaki kültür ve sanat dönüşümleri zamansal ve mekansal olarak değerlendirilmiştir. Beyoğlu üzerine kurulan nostalji biçimlerinde, farklı geçmiş zaman kesitlerinin türlü biçimlerde hatırlanmasına ve kültürün, farklı toplumsal grupları yüceltmek ya da karalamak amacıyla araçlaştırılmasına sıklıkla rastlanmaktadır. Buna bağlı olarak, nostaljik bakışın Beyoğlu’nda dönüştürmeyi önüne koyduğu meselelerin sosyopolitik bağlamı yeniden vurgulanmış ve “düşünsel nostalji” kurmak mümkün mü sorusu tartışılmıştır.ABSTRACT: Beyoğlu, which continues to be one of the places that both shapes and reflects Istanbul’s urban culture, is of critical importance for understanding cultural policies in Turkey. In this article, I discuss the contribution of Ebru Pelvanoğlu’s book, Bir Üretim Mekanı Olarak: Beyoğlu Düşerse [If Beyoğlu Falls as a Place of Production], published in 2022, to the literature of cultural policies and its deficiencies. The article begins with the discussions on the subjectivity of historiography and on the concept of nostalgia. Then, the book’s relationship with the sociopolitical context is reviewed and, together with the discussions on the “Beyoğlu Culture Road,” the cultural and artistic transformations in Beyoğlu are evaluated temporally and spatially. When yearning for the past of Beyoğlu, it is common to recall different periods from the past positively or negatively and to instrumentalize the culture of Beyoğlu, to exalt or vilify different social groups. In relation with this nostalgic perspective evoked for Beyoğlu, the sociopolitical context of the issues that this nostalgia sets out to transform in Beyoğlu is emphasized once again. Consequently, the question of whether it is possible to construct “ironic nostalgia” is discussed
