Acute Kidney injury
This chapter helps the reader to understand the different types and causes of acute kidney injury (AKI) and helps and supports the patient and family during an episode of AKI. It describes the signs and symptoms of AKI, analyses the management of those with AKI, and evaluates the various treatment options for AKI. The chapter emphasises the vital role the nurse plays in the prevention of progression to AKI, progression of AKI through the stages, and of the delivery of care for the patient in AKI. AKI may be divided into three major categories: pre-renal, renal, and post-renal. Testing home dream precognition and exploring links to psychological factors
Precognition refers to the idea that an individual may be able to obtain information about a future event via a non-usual route, prior to the event occurring. Over time various paradigms have been developed to elicit and test for such effects. Here, we focused on dream precognition as dreaming represents a naturally occurring altered state of consciousness that may facilitate such anomalous experiences. Using a home-dream paradigm we examined whether participants would be able to dream about and rate a future target image as more similar to their dreams compared to decoy images. We also examined the potential relationship between dream precognition and sensory processing sensitivity, transliminality, boundary thinness and anomalous experiences and belief. One hundred and one participants completed an initial practice trial, to familiarise themselves with the procedure, followed by the main trial. In each trial participants were required to dream of a future target image that they would later be shown. During each trial, after they had completed their dreaming, participants were randomly presented with a set of four images and required to rate them in terms of how much similarity there was between each image and their dream on a scale from 1 to 100. Results of the main trial showed that ratings for the target image were significantly higher than the decoy images. However, no clear relationships were found between precognitive target ratings and sensory processing sensitivity, transliminality, boundary thinness or anomalous experiences and belief. We conclude that the result is not due to any methodological artifacts and as such represents suggestive evidence for dream precognition. Given the logistical simplicity of the home-dream paradigm we would encourage other researchers to replicate our findings and explore what may be a fruitful avenue of research
Tele-controlled upper arm exoskeleton for post-stroke recovery
Clinical studies have shown that post-stroke patients can greatly benefit from exoskeleton-assisted rehabilitation exercises, providing a compelling alternative to traditional physiotherapy. Most existing exoskeletons are locally controlled, prompting the development of a tele-controlled spring-driven upper arm exoskeleton. This paper describes the design of an exoskeleton featuring arm support for enhanced grip strength using a single motor for elbow and finger mobility, along with string compliance, offering a remotely controllable and monitorable system in a home setting, particularly beneficial for stroke survivors and individuals lacking technical expertise. Details of the 3D design of the exoskeleton have been reported, with an emphasis on the incorporation of user anthropometric data and biomechanical considerations. The paper further discusses the initial findings of tele-control using an electronic control circuit based on an Arduino IoT module connected to a cloud-based interface accessible through Android smartphones
The potential impact of universal screening for vasa previa in the prevention of stillbirths
Objectives
To estimate the number of pregnancies complicated by vasa previa annually in nine developed countries, and the potential preventable stillbirths associated with undiagnosed cases. We also assessed the potential impact of universal screening for vasa previa on reducing stillbirth rates.
Methods
We utilized nationally-reported birth and stillbirth data from public databases in the United States, United Kingdom, Canada, Germany, Ireland, Greece, Sweden, Portugal, and Australia. Using the annual number of births and the number and rate of stillbirths in each country, and the published incidence of vasa previa and stillbirth rates associated with the condition, we estimated the expected annual number of cases of vasa previa, those that would result in a livebirth, and the potential preventable stillbirths with and without prenatal diagnosis.
Results
There were 6,099,118 total annual births with 32,550 stillbirths, corresponding to a summary stillbirth rate of 5.34 per 1,000 pregnancies. The total expected vasa previa cases was estimated to be 5,007 (95 % CI: 3,208–7,201). The estimated number of livebirths would be 4,937 (95 % CI: 3,163–7,100) and 3,610 (95 % CI: 2,313–5,192) in pregnancies with and without a prenatal diagnosis of VP. This implies that prenatal diagnosis would potentially prevent 1,327 (95 % CI: 850–1,908) stillbirths in these countries, corresponding to a potential reduction in stillbirth rate by 4.72 % (95 % CI: 3.80–5.74) if routine screening for vasa previa was performed.
Conclusions
Our study highlights the importance of universal screening for vasa previa and suggests that prenatal diagnosis of prevention could potentially reduce 4–5 % of stillbirth
Supporting young children’s physical development through tailored motor competency interventions within a school setting
Background/Objectives: This study investigated how tailored motor competency (MC) interventions within a bespoke Scheme of Work (SOW) can support young children’s physical development (PD) by enhancing fine motor skills (FMS) and gross motor skills (GMS). The objective was to enable children to meet curriculum developmental physical milestones. Methods: The longitudinal case study design was conducted over 19 weeks across three academic terms and focused on 25 children (12 boys and 13 girls aged between 5 and 6 years old). Interventions within the SOW were evaluated at six points to assess effectiveness and to make adjustments. Data collection included observations on FMS and GMS development. Results: Statistically significant improvements were observed in FMS and GMS after implementing the MC interventions. Notable improvements included hopping, running, climbing, kicking, catching, and using scissors (p < 0.05). Conclusions: The study underscores the importance of holistic approaches to PD, highlighting the need for early intervention and the crucial role of educators. The findings advocate for strategically planned MC interventions and practitioner observations to achieve longitudinal improvements in PD. The study recommends nationwide implementation to enhance PD outcomes, preparing children for lifelong and life-wide MC

Impact of CDIO framework pedagogical approach adoption on the student learning and experience
In 2018, Canterbury Christ Church University (CCCU) welcomed its first cohort of BEng (Hons) Chemical Engineers, who graduated in 2022. Subsequently, the inaugural cohorts of BEng (Hons) Mechanical Engineers, BEng (Hons) Biomedical Engineers, and BEng (Hons) Product Design Engineers completed their programs in 2023. The first cohort of Engineering Manufacturing Degree Apprentices is anticipated to graduate in 2024. The curriculum, adopting the Conceive, Design, Implement, and Operate (CDIO) pedagogical approach formulated by Crawley in 2004 (Crawley et al., 2014), has evolved to emphasize good practice industrial engagement (Fortin et al., 2019). This evolution encompasses curriculum co-design (Sparsø et al., 2011), guest lectures (Törngren et al., 2016), and industry-sourced CDIO capstone projects (Manna et al., 2023).
CCCU's unique curriculum is designed to cultivate 'Industry Ready' engineering graduates through industry-sourced CDIO projects spanning from the foundation year to the final year. This approach facilitates practical application and evaluation of learning, nurturing both technical and employability skills for future employment. This paper presents the initial results of a project evaluating the impact of CCCU's CDIO pedagogical approach on students and employers.
The research methodology, following Cohen et al.'s (2017) quantitative and qualitative active learning approach, employs surveys to gauge student and employer perceptions, comprising Likert scale and qualitative questions. The survey results will guide subsequent qualitative interviews, aiming to comprehend experiences associated with CDIO, and address the following research hypotheses:
1. Has CCCU's CDIO enriched student learning and experience?
2. Has CCCU's CDIO improved graduate prospects?
3. Is CCCU's CDIO approach valued by employers/industry?
The paper will present the initial evaluation results, draw conclusions, and identify any optimizations of the CDIO approach's efficacy in improving learning outcomes and employer relevance
What makes a successful whole-school mental health intervention? A comparison of stakeholders’ perspectives and implications for schools and the future of English education
This small-scale study compares stakeholders’ perspectives regarding what makes a successful whole-school Mental Health Intervention. Four study groups were included from an all-ability coeducational secondary school in south-east England: Students (n = 8), Teachers (n = 5), School-Leaders (n = 3), and YMCA Youth Workers (n = 3). Students took part in a focus group whereas the adult groups participated in semi-structured interviews. Subsequently, all participants ranked potential aspects of what might make a successful whole school Mental Health Intervention. No theme was mentioned consistently across the four groups, suggesting a diversity of perspectives regarding the benefits of Mental Health Interventions. All participants, except teachers, valued student involvement, a feature confirmed by the literature. Teachers occasionally adopted misinformed perspectives – arguably a result of insufficient training. Generally, participants’ job roles informed their responses. However, a loose trend existed whereby School-Leaders’ results were more aligned to an external charity association – the Young Men’s Christian Association (YMCA) – than Teachers’ despite the fact that the Teachers and School Leaders worked in the same context. Although the single context and small sample sizes for this research compromise generalisability, there are clear recommendations to inform future studies and for those who are leading and providing whole-school Mental Health Interventions. This study is set within the political and policy contexts of a competitive, market drive English education system and can comment on the wider system within which this intervention nests
