    The Application of Lee Silverman Voice Treatment (LSVT) – BIG with a Patient Diagnosed with Parkinson’s Disease: A Case Study and Literature Review

    Background: Parkinson\u27s disease is the most prevalent progressive neurological disease with motor deficits. The hallmark symptoms include bradykinesia, tremor, rigidity, and postural instability. The primary focus of physical therapy is to address these symptoms. The purpose of this case study is to present the use of the Lee Silverman Voice Treatment (LSVT)-BIG as a Parkinson\u27s disorder intervention, along with a critical review of the literature. Case Description: The patient is a 71-year-old man diagnosed with Parkinson’s disease during the summer of 2019. Before any noticeable symptoms, the patient reported being active and independent without any self-reported limitations. The patient became progressively limited with overall mobility over a period of a year. The patient\u27s primary complaints included poor standing and walking balance with a history of falls as well as difficulty performing activities of daily living. Intervention: The patient participated in LSVT-BIG that included daily exercises, patient-identified functional activities, and gait training. The program was four weeks in duration and consisted of four consecutive one-hour sessions each week. LSVT-BIG is amplitude-based training with the cue for big as the sole focus. Outcome Measures: 10-Meter Walk Test, the Timed-Up and Go Test (with and without a cognitive task), the Functional Gait Assessment, and the Five Times Sit to Stand. Discussion: The patient made improvements in all outcome measures, demonstrating the potential benefits gained following the completion of LSVT-BIG. This case study provides additional research to the limited collection of evidence that currently exists for the support of LSVT-BIG. However, more high-quality randomized trials are needed to further assess the true efficacy of LSVT-BIG

    Modification to Treatment Approach in an Active Individual After Partial Relief from Initial Physical Therapy for Hamstring Injury: A Case Report

    Background: Hamstring injuries are common among traditional and recreational athletes. Diagnosis and treatment of some of the newly described injuries remain difficult and lack supporting evidence. In addition, complete treatment of other components that may contribute to the injury is vastly lacking in research. The purpose of this case report is to highlight the challenges associated with diagnosing and comprehensively rehabilitating hamstring injuries. Case Description: A 72-year-old active female was referred to physical therapy for chronic proximal hamstring pain. This pain had progressed to limiting her ability to turn over in bed and participate in her exercise class. Intervention: Initial examination led to treatment via instrument assisted soft tissue mobilization and eccentric exercise training. After initial benefits, the patient’s improvements plateaued and eventually her pain increased. The treatment plan was modified to address adductor magnus involvement and hamstrings via dry needling and continuation of eccentric exercises. Outcome Measures: The patient’s pain initially decreased to 4/10 with bed mobility by the 4th visit with the initial treatment approach. However, the patient’s improvement then plateaued for 3 weeks. By the 8th visit, the patient’s pain had increased again. Over the course of treatment, the patient’s Lower Extremity Functional Scale score improved from 66% to 89%; thus the treatment modification was put into place. The patient was able to fully return to her exercise class activities and perform rolling over in bed without discomfort. Discussion: This case suggests approaches to differentiate proximal hamstring tendinopathy from other hamstring injury in patients with complaints of hamstring and posterior thigh pain. This case also presents potential treatment approaches for and the regional interdependence of hamstring pain and surrounding musculature

    Use of Functional Electrical Stimulation for Functional Mobility of a Pediatric Patient with Cerebral Palsy: A Case Report

    Background: Children with cerebral palsy (CP) have varying impairments of motor control and muscle tone, impacting functional mobility. One physical therapy intervention for children with limited functional mobility is utilizing functional electrical stimulation (FES) as an intervention to facilitate movement. FES cycling and FES assisted tasks are becoming more readily studied and utilized as an intervention for this population of patients. To date, there are few studies that evaluate the use of FES interventions combined with land based interventions in children with spastic quadriplegic CP. Purpose: Discuss the use of FES cycling and aided functional activities as an intervention for a pediatric patients with spastic quadriplegic CP. Intervention: During a four-week timeframe, a five-year-old boy with spastic quadriplegic CP, participated in physical therapy sessions one time per week utilizing FES cycling and FES assisted functional activities and one time per week utilizing adaptive cycling and non-FES assisted activities. The RT300 FES cycling machine was utilized, as well as the portable SAGE controller component of the RT300 for functional tasks of supine bridging, sit to stand, and static standing. Outcome Measures: Data points from the RT300 were utilized to track objective changes between each session of FES cycling. Qualitative data comparing the level of physical assistance required with each functional activity was utilized to track changes between FES and non-FES sessions each week. Discussion: FES cycling and FES assisted activities may provide feasible and well-tolerated physical activity interventions for children with spastic quadriplegic CP. A multi-modal intervention approach with FES and non-FES activities may provide variety and opportunities for motor learning in the pediatric setting

A large focus in my design was to be able to bring attention to the actor’s face since his expressions informed a lot of the comedy. To do this I created a bright yellow vest paired with a white shirt, and pink bowtie.
