    Quantum-Interference and Spin-Selectivity in Diboracene-Based Nanojunctions

    New designs for molecular switches are essential for driving the development of nanoelectronics. The effect of quantum interference in cross-conjugated structures such as anthracene have already been thoroughly explored. A similar structural motif can be found in diboracenes, whose unsubstituted variants could only be synthesized recently. In this contribution, we explore the potential of these structures to produce similar quantum interference effects varying with substitution and how these could be used to produce a spin-filtering device. We investigate the eletronic properties of diboracenes through density functional theory and the global and local transport properties at the quasi-stationary limit using nonequilibrium Green’s function methods. Our findings show how the current may be tuned through the substituent and that larger unsubstituted diboracenes favor a triplet ground state with a destructive quantum interference effect for one of the spin channels

    Fasten, Rheumatoide Arthritis und das Darmmikrobiom

    Fasting may beneficially impact many chronic diseases like rheumatoid arthritis (RA). However, past studies date back several decades before the advent of modern systemic treatments. Moreover, the gut microbiome is becoming focus of RA research as a possible key player in its pathogenesis and treatment. By now, effects of fasting on the gut microbiota in patients with RA remain unexplored. This thesis aims to evaluate RA-specific complaints and disease activity in patients with RA after prolonged fasting and its effects on the gut microbiota. Two explorative clinical studies were conducted to address the research questions. In the randomized controlled trial NutriFast 53 patients with RA were either assigned to a 7-day fast followed by an 11-week plant-based diet (PBD) with time restricted eating (TRE) or a 12-week standard anti-inflammatory diet according to the German Nutrition Society (DGE). Primary endpoint was the difference in the Health Assessment Questionnaire Disability Index (HAQ-DI) at 12 weeks compared to baseline. Of 53 participants, 50 completed the study and were analyzed per protocol. The primary endpoint did not differ statistically significant in both groups. Yet, HAQ-DI improved in the fasting group by day 7 and remained stable until week 12 (Δ-0.29, p=0.001), while the DGE group only improved by weeks 6 and 12 (Δ-0.23, p=0.032). Disease activity (DAS28) improved in both groups by week 12 (Δ-0.97, p<0.001 and Δ-1.14, p<0.001; respectively). Both groups displayed a similar dietary behavior and macronutrient intake in a post-hoc analysis of 41 participants. Post-randomization cluster analysis showed no clear dietary group distinction after three months, possibly explaining the similar disease activity outcomes. The exploratory open-label intervention study BioFast included 20 patients with RA and 10 patients with metabolic syndrome as non-inflammatory controls. Participants underwent bowel cleansing and prolonged fasting for 7-10 days (≤250 kcal/day). Disease activity, measured by DAS28 and SDAI, decreased significantly: 19 out of 20 RA patients improved in disease activity (median ΔDAS28 = -1.23; ΔSDAI = -43%) or achieved remission (DAS28 < 2.6/n=6; SDAI < 3.3/n=3) post-fasting. 16S, 18S and ITS gut microbiota sequencing revealed a decline in potentially arthritogenic bacteria and a shift towards species with diverse metabolic functions. Eukaryotic, particulary fungal presence increased in RA patients, suggesting a potential pathogenic role. This thesis supports dietary interventions as cost-effective, complementary elements in RA treatment and highlights the gut microbiota's link to RA-specific inflammation. Specific nutritional interventions could complement targeted therapies in the future. Larger, confirmatory studies are needed to validate these findings and establish clear dietary recommendations for patients with RA.Fasten kann sich positiv auf chronische Erkrankungen wie rheumatoide Arthritis (RA) auswirken. Erste Studien wurden jedoch vor der Einführung moderner Systemtherapien durchgeführt. Zudem rückt das Darmmikrobiom als Schlüsselfigur bei der Pathogenese und Therapie von RA in den Fokus der Forschung. Ziel dieser Arbeit ist es, die RA-spezifischen Beschwerden und die Krankheitsaktivität bei Patient:innen mit RA nach Fasten und dessen Auswirkungen auf die Darmmikrobiota zu untersuchen. Zur Beantwortung der Forschungsfragen wurden zwei explorative klinische Studien durchgeführt. In der randomisierten kontrollierten Studie NutriFast wurden 53 Patient:innen mit RA entweder 7-tägigem Fasten, gefolgt von 11-wöchiger pflanzenbasierter Diät (PBD) und Intervallfasten (TRE) oder einer 12-wöchigen antiinflammatorischen Diät gemäß der Deutschen Gesellschaft für Ernährung (DGE) zugewiesen. Primärer Endpunkt war der Gruppenunterschied im Health Assessment Questionnaire Disability Index (HAQ-DI) nach 12 Wochen versus Baseline. Von 53 Teilnehmer:innen schlossen 50 die Studie ab und wurden per Protokoll ausgewertet. Der primäre Endpunkt war statistisch nicht signifikant. Allerdings besserte sich der HAQ-DI in der Fastengruppe zu Tag 7 und blieb bis Woche 12 stabil (Δ-0,29, p=0,001), während sich die DGE-Gruppe erst zu Woche 6 und 12 besserte (Δ-0,23, p=0,032). Die Krankheitsaktivität sank in beiden Gruppen zu Woche 12 (DAS28 Δ-0,97, p<0,001 bzw. Δ-1,14, p<0,001). In einer Post-hoc-Analyse zeigten beide Gruppen ähnliches Ernährungsverhalten und Makronährstoffaufnahme. Eine Clusteranalyse konnte nach drei Monaten jedoch keinen klaren Unterschied zwischen den Studiengruppen im Ernährungsmuster aufzeigen, was möglicherweise den ähnlichen Verlauf der Krankheitsaktivität erklärt. An der explorativen open-label Studie BioFast nahmen 20 Patient:innen mit RA und 10 Patient:innen mit metabolischem Syndrom als nicht-entzündliche Kontrollgruppe teil. Die Teilnehmer:innen unterzogen sich einer Darmreinigung und Fasten über 7-10 Tage (≤250 kcal/Tag). Die Krankheitsaktivität (DAS28, SDAI) ging zurück: Bei 19 von 20 Patient:innen mit RA verbesserte sich die Krankheitsaktivität (medianer ΔDAS28 = -1,23) oder sie erreichten eine Remission (DAS28 <2,6/n=6; SDAI <3,3/n=3). 16S-, 18S- und ITS-Sequenzierung der Darmmikrobiota ergab einen Rückgang potenziell arthritogener Bakterien und eine Verschiebung zu Arten mit diversen Stoffwechselfunktionen. Eukaryonten, insbesondere Pilze, waren bei RA vermehrt, was auf eine pathogene Rolle hindeutet. Diese Arbeit unterstützt Ernährungsmaßnahmen als kosteneffiziente, ergänzende Elemente der integrativen RA-Therapie und unterstreicht den Zusammenhang zwischen der Darmmikrobiota und RA-spezifischen Entzündungen. Spezifische Ernährungsinterventionen könnten künftig gezielte Therapien ergänzen. Größere, konfirmatorische Studien sind erforderlich, um diese Ergebnisse zu validieren und klare Ernährungsempfehlungen für Patient:innen mit RA aufzustellen

    Impact of SARS-CoV-2 (COVID-19) pandemic on surgical site infection in patients with chronic inflammatory bowel disease

    Hintergrund: Postoperative Wundinfektionen sind mit ca. 25% eine häufige Komplikation nach kolorektalen Eingriffen und gehen unter anderem mit einer erhöhten Morbidität und Mortalität, längeren Krankenhausaufenthalten sowie verstärkten postoperativen Schmerzen und erhöhten Kosten einher. Zudem haben Patient:innen mit einer chronisch entzündlichen Darmerkrankung (CED) ein erhöhtes Risiko für einen postoperativen Wundinfekt. Als Anfang 2020 die Covid-19 Pandemie ausbrach, wurden eine Reihe von Hygiene-maßnahmen an der Charité – Universitätsmedizin Berlin eingeführt, um die Ausbreitung der Infektion zu reduzieren. Diese Arbeit sollte den Effekt der Hygienemaßnahmen auf die Wundinfektionsrate bei Patient:innen mit einer CED untersuchen, sowie Einflussfaktoren auf einen Wundinfekt darlegen. Methoden: Es handelte sich um eine monozentrische, retrospektive Datenanalyse. Elektive Operationen bei CED-Patient:innen vor der Covid-19 Pandemie (02.03.2015 – 15.12.2018; n = 107) wurden verglichen mit denen während der Pandemie (01.03.2020 – 15.12.2021; n = 97). Anhand der Kriterien der Centers for Disease Con-trol and Prevention (CDC) wurden die postoperativen Wundinfektionen über 30 Tage nachverfolgt. Dazu erhielten die Patient:innen per Post einen Fragebogen. Der Zeit-raum während der Pandemie wurde anhand der Strenge der Hygienemaßnahmen in drei Bereiche (mild, mittel, streng) unterteilt. Ergebnisse: Die Wundinfektionsrate lag während der Covid-19 Pandemie bei 25,8% im Vergleich zu 31,8% davor (OR 0,94; 95% KI 0,40 – 2,20; p = 0,881). Die Strenge der Hygienemaßnahmen hatte keinen Einfluss auf die Wundinfektionsrate (p = 0,553). Während der Pandemie hatten die Patient:innen einen schlechteren ASA-Score (p = 0,021) und einen höheren Kontaminationsgrad der Wunde (p = 0,047). Die OP-Zeiten waren länger während Covid-19 (p = 0,004), es gab mehr postoperative intra-abdominelle Abszesse (p = 0,021) und der Krankenhausaufenthalt war kürzer (p < 0,001). Einflussfaktoren, die die Wundinfektionsrate erhöhten, waren in der multivariaten Analyse der offene Operationszugang (OR 2,02; 95% KI 1,31 – 3,11; p = 0,001), intraoperative Serosadefekte des Darmes (OR 60,91; 95% KI 3,92 – 946,66; p = 0,003) und transmurale Darmläsionen (OR 22,00; 95%; KI 1,73 – 280,74; p = 0,017) sowie Re-Operationen (OR 21,50; 95% KI 1,99 – 232,00; p = 0,011). Schlussfolgerung: Die Verschärfung der Hygienemaßnahmen im Krankenhaus während der Covid-19 Pandemie hatte keinen signifikanten Einfluss auf die Ausbildung einer postoperativen Wundinfektion bei elektiven, darmresezierenden Operationen bei Patient:innen mit CED. Die möglicherweise verzögerte Durchführung der Darmresektion aufgrund des zeitweisen Verbotes von elektiven Operationen führte wahrscheinlich zu einer Verschlechterung der Gesundheitssituation, wodurch sich die Wundinfektionszahlen erhöhten und vermehrt tiefere Wundinfekte auftraten. Diese Veränderung konnte durch die Hygienemaßnahmen nicht ausgeglichen werden.Background: Surgical site infection (SSI) is a frequent complication after colorectal surgery and is associated, among other things, with increased morbidity and mortality, longer hospital stay as well as increased postoperative pain and overall costs. In colo-rectal surgery, SSIs occurs in approximately 25% of patients. Those with inflammatory bowel disease (IBD) are at increased risk. When the Covid-19 pandemic started in early 2020, several hygienic measures were implemented at Charité – Universitätsmedizin Berlin to reduce the spread of the disease. The purpose of this trial was to investigate the effect of hygienic measures on the rate of SSI in patients with IBD and to identify risk factors for SSI. Methods: This was a monocentric, retrospective data analysis. Elective surgery with bowel resection in IBD patients before Covid-19 pandemic (03/02/2015 - 12/15/2018; n = 107) was compared to surgery during the pandemic (03/01/2020 - 12/15/2021; n = 97). SSIs were followed up for 30 days using criteria according to Centers for Disease Control and Prevention (CDC). For this purpose, the patients received a questionnaire by mail. The period during the pandemic was divided into three areas (mild, medium, strict) based on the strictness of the hygiene measures. Results: The rate of SSI was 25.8% during the Covid-19 pandemic compared to 31.8% before (OR 0.94; 95% CI 0.40 – 2.20; p = 0.881). The strictness of the hygienic measures had no influence on the rate of SSI (p = 0.553). During the pandemic, patients had worse ASA scores (p = 0.021) and higher grade of wound classification (p = 0.047). Operation time was longer (p = 0.004), there were more postoperative intraabdominal abscesses (p = 0.021), and the hospital stay was shorter (p < 0.001). In multivariate analysis, risk factors for SSI were open surgical access (OR 2.02; 95% CI 1.31 – 3.11; p = 0.001), intraoperative serosal tear of bowel (OR 60.91; 95% CI 3.92 – 946.66; p = 0.003), intraoperative transmural bowel lesion (OR 22.00; 95%; CI 1.73 – 280.74; p = 0.017) and reoperations (OR 21.50; 95% CI 1.99 – 232.00; p = 0.011). Conclusion: The increase of hygiene measures in hospital during the Covid-19 pandemic had no significant impact on the development of SSIs during elective bowel resection surgery in patients with IBD. The possible delayed performance of bowel resections due to the temporary ban on elective surgeries probably led to a deterioration of their health situation, resulting in increased rates of SSIs and led to an increase in deeper wound infections. This change could not be compensated by the hygiene measures

    A glimpse into darkness: Diversity of culturable cyanobacteria, green algae and fungi from subaerial cave biofilms

    Caves and hypogean environments provide stable microclimates characterized by uniform temperatures, constant humidity, and low light levels. In such sites, extremely low light irradiance can support the growth of subaerial biofilms (SABs) dominated by unique photosynthetic communities of cyanobacteria accompanied by chlorophytes, heterotrophic bacteria, and fungi. This study aimed to apply a culture‐dependent approach to uncover the diversity of cyanobacteria, green algae, and fungi from SABs of a cave in Northern Spain. We isolated a total of 58 cyanobacteria grouped into 21 genera based on their 16S rRNA gene sequences; 24 green algae grouped into 10 genera based on their SSU rRNA gene sequences, and 41 fungi fell into 13 genera according to phylogenies based on the ITS rRNA region between the 16S and 23S rRNA genes (ITS rRNA region). The SABs were dominated by cyanobacteria, which also reflected by high diversity, including calcium‐carbonate sheath‐producing species such as Geitleria calcarea and Scytonema julianum. Typical cave‐inhabiting species such as Chalicogloea cavernicola , Timaviella karstica or Oculatella subterranea were also isolated, alongside potentially new genera and species. Associated green algae were predominantly located closer to the cave entrance and included various lineages of the genera Jenufa and Chromochloris . The fungal community directly derived from the SABs was dominated by decomposers, saprophytes, and phytoparasitic representatives such as Sporobolomyces , Stereum , and Phlebia , with a corresponding set of enzymes that were evaluated for all fungal isolates. The results strongly support the hypothesis that specialized cyanobacterial communities are often located in caves as a result of niche specialization

    Effiziente Erzeugung molekularer Boltzmann-Verteilungen mit Methoden des maschinellen Lernens

    Generating equilibrium samples from the Boltzmann distribution of biomolecular systems, such as proteins, remains a fundamental challenge in computational biochemistry and statistical physics. Conventional approaches, including molecular dynamics and Markov chain Monte Carlo, rely on iterative sample generation and can become prohibitively expensive for large systems or long timescales, limiting their utility in applications like drug discovery. This dissertation investigates how machine learning models, particularly normalizing flows, can accelerate this sampling. Normalizing flows are invertible deep‐generative models that produce novel configurations while providing exact likelihoods. We introduce several normalizing flow architectures that embed the intrinsic rotational, translational, and permutation symmetries of molecular systems into both their structure and training objectives. We prove that these equivariant flows generate samples whose distributions inherit the same symmetries as the target Boltzmann distribution. Moreover, we introduce equivariant optimal transport flow matching, a novel training objective for equivariant continuous normalizing flows that leverages the symmetry of the target energy for simulation‐free training and yields near-optimal transport paths at inference. We propose two complementary sampling strategies for the different models: independent sample generation with so called Boltzmann Generators, enabling importance reweighting against known energy functions for unbiased observable estimation, and long‐range, proposal‐based moves within Markov chain Monte Carlo methods, achieving time jumps up to six orders of magnitude over standard molecular dynamics. Additionally, and somewhat orthogonally, we also leverage our models to generate force estimates for training machine learned coarse‐grained force fields. Our methods are validated on both many‐particle systems (e.g., Lennard‐Jones clusters) and small peptides (alanine dipeptide and di‐ and tetrapeptides), showing accurate reproduction of free‐energy landscapes, high effective sample sizes, and transferability across temperatures and small peptide sequences. These advances suggest a promising path toward scalable, machine learning driven sampling tools that can significantly reduce the computational burden of molecular simulations, paving the way for faster, more efficient exploration in drug discovery and beyond.Die Erzeugung von Samples aus der Gleichgeweichts Boltzmann-Verteilung biomolekularer Systeme, wie beispielsweise Proteinen, stellt nach wie vor eine grundlegende Herausforderung in der computergestützten Biochemie und statistischen Physik dar. Herkömmliche Ansätze, darunter Molekulardynamik und Markov chain Monte Carlo, basieren auf iterativer Generierung und können für große Systeme oder lange Zeiträume unerschwinglich aufwendig werden, was ihre Nützlichkeit in Anwendungen wie der Arzneimittelentwicklung einschränkt. In dieser Dissertation untersuchen wir, wie Machine Learning Modelle, insbesondere Normalizing Flows, das Boltzmann‐Sampling beschleunigen können. Normalizing Flows sind invertierbare generative Modelle, die neue Konfigurationen erzeugen und gleichzeitig exakte Wahrscheinlichkeiten liefern. Wir stellen mehrere Normalizing Flows Architekturen vor, die die intrinsischen Rotations-, Translations- und Permutationssymmetrien molekularer Systeme sowohl in ihrer Architektur als auch in ihren Trainingszielen verankern. Wir beweisen, dass diese equivarianten Flows Konfigurationen erzeugen, deren Verteilungen dieselben Symmetrien aufweisen wie die Boltzmann-Verteilung. Darüber hinaus führen wir equivariant optimal transport flow matching ein, ein neuartiges Trainingsziel für equivariante Continuous Normalizing Flows, das die Symmetrie der Zielenergie für simulationsfreies Training ausnutzt und nahezu optimale Transportpfade bei der Inferenz liefert. Wir schlagen zwei komplementäre Sampling-Strategien für die verschiedenen Modelle vor: Einerseits die unabhängige Sample-Generierung mit sogenannten Boltzmann Generatoren, die eine Neugewichtung gegen die Energiefunktion ermöglicht und so unverzerrte Schätzungen von Observablen erlaubt. Andererseits langfristige, vorschlagsbasierte Zeitschritte innerhalb von Markov chain Monte Carlo Methoden, die bis zu sechs Größenordnungen größere Zeitschritte gegenüber standard Molekulardynamik erreichen. Zusätzlich nutzen wir unsere Modelle auch, um Kräfte für das Training von coarse-grained Kraftfeldmodellen zu generieren. Unsere Methoden wurden sowohl an Vielteilchensystemen (z. B. Lennard-Jones Klustern) als auch an kleinen Peptiden (Alanin-Dipeptid sowie Di- und Tetrapeptiden) validiert und zeigen eine genaue Reproduktion der freien Energielandschaften, hohe effektive Stichprobengrößen und Übertragbarkeit über Temperaturen und kleine Peptidsequenzen hinweg. Diese Fortschritte deuten auf einen vielversprechenden Weg hin zu skalierbaren Sampling-Tools, die den Rechenaufwand für Molekularsimulationen erheblich reduzieren können und damit den Weg für eine schnellere und effizientere Erforschung in der Arzneimittelentwicklung und darüber hinaus ebnen

    Genomische Insertionen in der Vielfalt menschlicher Antikörper

    Human antibodies variety is crucial for normal immune system performance. Antibodies are synthesized by the B cells and diversified by genomic rearrangement of V(D)J segments, somatic hypermutation and isotype switching. Recently, fragments of LAIR1 and LILRB1 genes were found to be inserted in the immunoglobulin heavy chain locus of B cells extracted from malaria-exposed individuals. These insertions encoded an additional domain binding a broad spectrum of repetitive interspersed family (RIFIN) proteins of malaria-causing Plasmodium falciparum. This doctoral study aims to address the following questions: what is the frequency of such events, how malaria exposure contributes to it, and what can be a mechanism of these phenomena. To profile the insertions incorporated in the antibody genes, a technique called suppression PCR was developed. It predominantly amplifies the products of a certain length, allowing to selectively enrich the insertion-containing immunoglobulin transcripts. Applying this method to 56 donors from Africa and Europe, we detected 1822 chimeric heavy chain immunoglobulin transcripts in 89.3% of the donors. The frequency of the insertions varied between 1.67·10-6 and 1.74·10-4 per B cell and did not differ significantly between donors of African and European origin. The insertions were incorporated either in between the VDJ segments of the variable domain (VDJ inserts) or in between the variable and constant domains (J-CH1 inserts). We characterized the insertions based on their genomic origin, developmental stage of B cells in which they were detected, GC composition, expression level, proximity to telomeres, sources of DNA damage, and AID or RAG off-targets. These features allowed us to classify the insertion-containing transcripts into four classes: fragments of nuclear genome (nucVDJ) and mitochondrial DNA (mtVDJ) incorporated in the variable domain, nuclear DNA originating from telomere- and R-loop-proximal regions (telJC), and AID-mediated insertions originating from ERFS-proximal genomic regions (nucJC). 13.2% of the insertions did not interfere with normal heavy chain reading frame and a set of the insertions bears somatic hypermutations, pointing at the affinity maturation. A selection of chimeric antibodies were expressed in vitro as grafts on various antibody backbones. While most of the chimeric antibodies lost the original target binding activity, a subset of them acquired an unspecific binding, providing a putative explanation of chimeric antibody presence in human repertoire. Collectively, this dissertation sheds light on a new layer of antibody diversification.Die Vielfalt der menschlichen Antikörper ist entscheidend für die normale Leistung des Immunsystems. Antikörper werden von den B-Zellen synthetisiert und durch genomische Umlagerung von V(D)J-Segmenten, somatische Hypermutation und Isotypwechsel diversifiziert. Kürzlich wurde festgestellt, dass Fragmente der LAIR1- und LILRB1-Gene in den Immunglobulin-Schwerketten-Locus von B-Zellen eingefügt sind, die aus Malaria-exponierten Personen extrahiert wurden. Diese Insertionen kodierten für eine zusätzliche Domäne, die ein breites Spektrum von Proteinen der repetitiven interspersierten Familie (RIFIN) des Malaria-verursachenden Plasmodium falciparum bindet. Diese Doktorarbeit zielt darauf ab, die folgenden Fragen zu beantworten: Wie häufig treten solche Ereignisse auf, wie trägt die Malaria-Exposition dazu bei und was kann ein Mechanismus für diese Phänomene sein? Um die in die Antikörpergene eingebauten Insertionen zu profilieren, wurde eine Technik namens Suppressions-PCR entwickelt. Es verstärkt vor allem die Produkte einer bestimmten Länge und ermöglicht so eine selektive Anreicherung der insertierten Immunglobulin-Transkripte. Bei der Anwendung dieser Methode auf 56 Spender aus Afrika und Europa konnten wir bei 89,3% der Spender 1822 chimäre Immunglobulin-Transkripte der schweren Kette nachweisen. Die Häufigkeit der Insertionen variierte zwischen 1,67·10-6 und 1,74·10-4 pro B-Zelle und unterschied sich nicht signifikant zwischen Spendern afrikanischer und europäischer Herkunft. Diese Merkmale ermöglichten es uns, die insertierungshaltigen Transkripte in vier Klassen zu klassifizieren: Fragmente des Kerngenoms (nucVDJ) und der mitochondrialen DNA (mtVDJ), die in die variable Domäne eingebaut sind, Kern-DNA, die aus Telomer- und R-Loop-proximalen Regionen stammt (telJC) und AID-vermittelte Insertionen, die aus ERFS-proximalen Genomregionen (nucJC) stammen. 13,2% der Insertionen störten den normalen Leserahmen der schweren Kette nicht und ein Satz der Insertionen weist somatische Hypermutationen auf, die auf die Affinitätsreifung hinweisen.     Chronic Tinnitus denotes the longstanding, conscious awareness of a tonal or composite noise for which there is no identifiable external acoustic source. Depending on the psychological makeup of affected individuals, tinnitus can be highly distressing. Current guidelines view the symptom together with its cognitive-emotional processing as a psychosomatic phenomenon – and suggest (1) medico-audiological treatment for underlying medical influences – if known, and (2) psychological treatment for highly distressed individuals. Anchored within a psychosomatic vulnerability-stress-coping framework, the present thesis sets out to examine chronic tinnitus-related distress. It asks two questions: 1. In how far do direct or indirect psychological factors – that may or may not correlate with ‘somatic’ variables – influence individuals’ experience of a tinnitus symptom as distressing? And: 2. In how far are psychosomatic (i.e. psychologically or medico-audiologically anchored) treatment approaches effective in ameliorating tinnitus-related distress? Chapter 1 [Introduction] briefly reviews evidence on vulnerability-stress-coping contributors to chronic tinnitus symptomatology and related distress-reactions. Chapter 2 [Vulnerability] presents the first two studies of the thesis, which examine chronic tinnitus-related distress at the junctions of (1) biological markers of tinnitus symptom vulnerability or perceived stress experiences and (2) psychological vulnerability-stress interactions. Chapter 3 [Stress] presents the third study, which investigates, whether transdiagnostically relevant psychological variables underlie chronic tinnitus- and pain-related distress experiences. Chapter 4 [Coping] examines the efficacy of psychosomatic treatment approaches. The final three studies of this thesis examine (1) whether a transdiagnostic psychological treatment approach ameliorates different functional symptom clusters, (2) whether a ‘somatic’ treatment approach (hearing amplification) bears psychological benefit, and (3) psychological effects on hearing amplification via hearing aid use-time.     Patientinnen und Patienten, die bei koronarer Herzkrankheit (KHK) an Herzinsuffizienz versterben, nehmen eine beachtenswerte Gruppe innerhalb der Todessursachenstatistiken der westlichen Welt ein. Eine der häufigsten invasiven Behandlungsmethoden ist die aortokoronare Bypass-Operation (ACB-OP). Bei Patientinnen und Patienten mit stark fortgeschrittenem Krankheitsbild ist die Datenlage zur optimalen Versorgung jedoch un-eindeutig. Die Kunstherztherapie bietet sich als vielversprechende Alternative für ausge-wählte Patientinnen und Patienten an. Vor diesem Hintergrund stellt sich die Frage, ob diese der ACB-OP bei Patientinnen und Patienten mit KHK und fortgeschrittener Herzinsuffizienz überlegen/gleichwertig ist. Es erfolgte eine retrospektive Datenauswertung von Patientinnen und Patienten, die an KHK und fortgeschrittener Herzinsuffizienz (LVEF < 26 %) litten und entweder ein Kunstherz erhielten oder mittels ACB-OP versorgt wurden. Die Behandlung erfolgte zwischen 2015 und 2018 am Deutschen Herzzentrum Berlin (DHZB). Eingeteilt wurde in permanente Herzunterstützungssystem-(Assist Device)-Kohorte (n = 97) und aortokoronare Bypass-Kohorte (n = 100). Als primärer Endpunkt wurde die Einjahresmortalität bestimmt. Folgend konnten die einflussreichsten Risikofaktoren für Therapieversagen (Versterben und/oder Einsatz von Extracorporeal-Life-Support-Systemen und/oder permanenten Her-zunterstützungssystemen) während des stationären Aufenthalts durch eine Subgrup-penanalyse der aortokoronaren Bypass-Kohorte identifiziert werden. Aus diesen identifi-zierten Risikofaktoren wurde ein Risiko-Score gebildet. Es zeigte sich kein signifikanter Unterschied im Einjahres-Überleben zwischen den Kohorten (p = 0,598; HR(LVAD) = 0,80). Aufgrund der geringen Fallzahlen zu Rehospitalisierung und Lebensqualität erfolgte die Auswertung deskriptiv und vergleichend. In der Bypass-Kohorte kam es zu 33 Rehospitalisierungen (34,55 %). Eine Rehospitalisierung in der Assist Device Gruppe erfolgte 197 Mal (57,29 %). Numerisch fand sich in Bezug auf die Lebensqualität ein Unterschied zwischen den Kohorten. So betrug der Median der visuellen Analogskala-(VAS)-Daten in der Assist-Device-Gruppe 62,5, bei 48 nacher-fassbaren Patientinnen und Patienten. Der Median der VAS-Daten in der Bypass-Gruppe betrug 55, bei 28 nacherfassbaren Patientinnen und Patienten. Die Subgruppenanalyse und Modellselektion für den Risiko-Score der aortokoronaren Bypass-Kohorte identifizierte Diabetes mellitus (OR = 44,66), reduzierte LVEF (OR = 1,11) und Höhe des INTERMACS-Levels (OR = 4,09) als einflussreichste Risikofaktoren. Die Assist Device Therapie stellt einen vielversprechenden therapeutischen Ansatz bei Patientinnen und Patienten mit KHK, fortgeschrittener Herzinsuffizienz und entsprechenden Risikofaktoren für ein Therapieversagen der ACB-OP dar. Weitere prospektive, mul-tizentrische Studien können helfen, eine klare Behandlungsempfehlung für diese Patienten- und Patientinnengruppe bereitzustellen.Patients suffering from heart failure and coronary artery disease represent a remarkable group in the Western world’s cause mortality statistics. Coronary artery bypass grafting is the mainstay of surgical treatment. However, there’s a lack of eminent data on optimal treatment for patients with the severest forms of heart failure. Assist-Device-Implantation may be a promising alternative for selected patients. The question is whether it’s superior or equal to coronary artery bypass graft surgery in this population. A retrospective data analysis was performed on patients suffering from coronary artery disease and severe heart failure (left ventricular ejection fraction < 26 %), treated with either coronary artery bypass graft surgery or with assist device implantation. Treatment was performed at the German Heart Center Berlin (DHZB) between 2015 and 2018. Patients were divided in an artificial heart cohort (n = 97) and a coronary artery bypass graft surgery cohort (n = 100). One year mortality was defined as the primary endpoint. In addition, a subgroup analysis identified major risk factors for treatment failure (death and/or need for extracorporeal life support and/or assist device implantation) in the coronary artery bypass surgery cohort. These were used to create a risk score. There was no significant difference in one-year survival between the cohorts. Due to the small number of cases of rehospitalization and quality of life cases, the analysis was descriptive and comparative. In the bypass cohort, patients had to be rehospitalized 33 times. This represented 34,55 % of the patients who could be followed up. In comparison, rehospitalization occurred 197 times in the assist-device-group. This represented 57,29 % of the patients who could be followed up. Numerically, there was a difference in quality of life between the cohorts. The median VAS data in the assist device group was 62,5, with 48 patients available for follow up. The median VAS data in the bypass group was 55, with 28 patients available for follow up. Subgroup analysis and model selection for the risk score of the aortocoronary bypass cohort identified diabetes mellitus (OR = 44,66), reduced left ventricular function (OR = 1,11) and INTERMACS level (OR = 4,09) as the most influential risk factors. Assist-Device-Treatment may be an important therapeutic approach in patients with coronary artery disease, severe heart failure, and major risk factors for coronary artery bypass graft surgery treatment failure. Further prospective, multicenter studies may help to establish a clear treatment recommendation for this patient population

    Understanding change from the patient perspective in a transdiagnostic Internet-based intervention for emotional disorders: a qualitative content analysis

    Qualitative investigations that openly explore changes and facilitators of changes from the patient’s perspective might offer valuable insights on impacts of therapy and helpful and hindering aspects. Our aim for this study was to explore the perspective of patients on a transdiagnostic Internet-based intervention to understand (1) which changes (positive as well as negative effects) responders and non-responders experienced, and (2) which aspects of the intervention they found helpful or hindering in facilitating those changes. We interviewed 21 patients that showed response or non-response to treatment using the Change Interview Schedule following a 10-week Internet-based intervention based on the Unified Protocol. Interviews were analyzed using qualitative content analysis. Both responders and non-responders reported positive changes, with few negative changes mentioned. Across both groups, increased positive affect was reported most frequently (81%). Both groups reported helpful factors, with guidance mentioned most frequently across both responders and non-responders (85.7%). Mainly, aspects of the specific framework were perceived as hindering (e.g. lack of personalization) (66.7%). Overall, patients reported mostly positive impacts, even if they did not respond to treatment. Results highlighted that what patients find helpful or hindering is individual
