Relationship between lower urinary tract symptoms and frailty
Get PDFObjective:To elucidate the relationship between frailty and lower urinary tractsymptoms (LUTS).
Methods:We longitudinally evaluated the temporal changes and the relationships between frailty and LUTS in 247 community-dwelling adults (45 years or older) at baseline and at a 5-year follow-up. We used the Fried phenotype (phenotype-based frailty), 5-item modified frailty index (5i-mFI; comorbidity-based frailty), and frailty discriminant score (comprehensive frailty assessment) to evaluate frailty. LUTS were evaluated using the international prostate symptom score (IPSS) and overactive bladder symptom score (OABSS).
Results: We analyzed 247 participants with a median age of 60 years. The median IPSS and OABSS were significantly increased over the 5 years. The proportion of frail individuals did not increase significantly over the 5 years. Of the three frailty assessment tools, the 5i-mFI score significantly increased between 2014 and 2019. Multiple linear regression analyses showed that the 5i-mFI score was significantly associated with the severity of LUTS in 2014 to 5i mFI in 2019 but not with 5i-mFI in 2014 to the severity of LUTS in 2019.
Conclusion: The effect of LUTS on frailty might be greater than the effect of frailty on LUTS. Further large-scale studies are needed to elucidate the relationship between LUTS and frailty.弘前大学博士（医学
Predictive symptoms associated with the development of radiographic knee osteoarthritis: a 10-year longitudinal study from the Iwaki cohort
Get PDFPurpose: This 10-year longitudinal cohort study aimed to investigate the annual rate of development of radiographic knee osteoarthritis (drKOA) and its predictive symptoms in the general Japanese population.
Methods: A total of 263 volunteers (154 women) from the Iwaki Health Promotion Project were followed up over 10 years. Standing anteroposterior knee radiographs were obtained and the Kellgren–Lawrence grade was evaluated. drKOA was defined as a change from grade 0 or 1 to 2 over 10 years. Knee symptoms were evaluated using the Knee injury and Osteoarthritis Outcome Score at baseline. Logistic regression analyses were performed to identify symptoms related to drKOA, and a predictive formula was created based on the associated symptoms.
Results: The overall incidence of drKOA was 52.9% (annual rate, 4.34%). Regression analysis revealed that predictive symptoms for drKOA were stiffness [p ¼ 0.033; odds ratio (OR) ¼ 1.89], frequent pain (p ¼ 0.024; OR ¼ 1.38), difficulty in ascending stairs, jumping, twisting, and kneeling (p ¼ 0.009–0.046; OR ¼ 1.71–2.78), and poor knee-related quality of life (p < 0.001–0.005; OR ¼ 1.53–2.18). Furthermore, the prediction formula created using these predictive symptoms demonstrated an area under the curve of 0.663, a cutoff value of 97 out of 100 points, an OR of 2.99, p < 0.001, sensitivity of 0.676, and specificity of 0.589.
Conclusions: The annual incidence rate of drKOA was 4.34% in the general Japanese population. Impact of the irrigation flow technology on contact force-guided pulmonary vein isolation: a prospective, randomized study
Get PDFBackground: Safety and efficacy of the irrigation flow technology on contact force （CF）-guided circumferential pulmonary vein isolation （CPVI） remain unknown. We investigated the impact of the irrigation flow by comparing the application time to achieve unexcitability by pacing from the ablation catheter tip.
Methods: A 56-hole porous tip （PT） irrigation system （ThermoCool SmartTouch Surround Flow; STSF） were compared with a conventional 6-hole catheter tip （ThermoCool SmartTouch; ST）. Forty-two patients with atrial fibrillation（ mean age 61±10 years） undergoing CPVI were randomly assigned to either STSF（ PT group; n=21） or ST （non-PT group; n=21） catheter ablation. Radiofrequency （RF） application at 30W was performed while pacing at 6 sites along the left CPVI line before the CPVI. The CF was adjusted to maintain 10 g. After the study, PVI was performed with the conventional method.
Results: The PT group had a shorter median RF application time from superior to anterior sites than the non-PT group （10 seconds; interquartile range （IQR）, 7–17, versus 29 seconds; IQR, 13–51; P<0.01）. There was no difference in RF application time from inferior to posterior sites. Except for one patient in the PT group who had esophageal erosion, CPVI was performed without complications.
