Left ventricular mechanical dispersion by tissue Doppler imaging: a novel approach for identifying high-risk individuals with long QT syndrome.
AIMS: The aim of this study was to investigate whether prolonged and dispersed myocardial contraction duration assessed by tissue Doppler imaging (TDI) may serve as risk markers for cardiac events (documented arrhythmia, syncope, and cardiac arrest) in patients with long QT syndrome (LQTS). METHODS AND RESULTS: Seventy-three patients with genetically confirmed LQTS (nine double- and 33 single-mutation carriers with previous cardiac events and 31 single-mutation carriers without events) were studied. Myocardial contraction duration was prolonged in each group of LQTS patients compared with 20 healthy controls (P < 0.001). Contraction duration was longer in single-mutation carriers with previous cardiac events compared with those without (0.46 +/- 0.06 vs. 0.40 +/- 0.06 s, P = 0.001). Prolonged contraction duration could better identify cardiac events compared with corrected QT (QTc) interval in single-mutation carriers [area under curve by receiver-operating characteristic analysis 0.77 [95% confidence interval (95% CI) 0.65-0.89] vs. 0.66 (95% CI 0.52-0.79)]. Dispersion of contraction was more pronounced in single-mutation carriers with cardiac events compared with those without (0.048 +/- 0.018 vs. 0.031 +/- 0.019 s, P = 0.001). CONCLUSION: Dispersion of myocardial contraction assessed by TDI was increased in LQTS patients. Prolonged contraction duration was superior to QTc for risk assessment. These new methods can easily be implemented in clinical routine and may improve clinical management of LQTS patients
Helseeffekt av influensavaksine til eldre og kronisk syke
BAKGRUNN: På oppdrag fra allmennpraktiker Ottar Grimstad ble vi bedt om å finne dokumentasjonsgrunnlaget for effekten av influensavaksinering av eldre og personer med kronisk sykdom. METODE: Vi søkte etter systematiske oversikter som hadde vurdert effekten av influensavaksine til eldre eller personer med kronisk sykdom. Vi oppdaterte de systematiske oversiktene vi inkluderte, og søkte etter randomiserte kontrollerte forsøk om effekten av influensavaksine til eldre og personer med kronisk sykdom. Vi inkluderte endepunktene totaldødelighet, influensaforårsaket komplikasjon (lungebetennelse), sykehusinnleggelser, influensa (laboratoriebekreftet), influensalignende sykdom og uønskede\ud
hendelser. Vi inkluderte fem systematiske oversikter som vurderte effekten av influensavaksine til eldre, til personer med kronisk obstruktiv lungesykdom, astma, bronkiektasier og cystisk fibrose. Vi inkluderte én randomisert kontrollert studie og 12 observasjonsstudier i oppdatering av oversikten om effekt av influensavaksine til eldre. Vi inkluderte i tillegg fire randomiserte kontrollerte studier hvor effekten ble målt hos pasienter med levercirrhose, multippel sklerose, hiv-infeksjon, og hjerte- og karsykdommer. RESULTAT: Dokumentasjonsgrunnlaget for effekten av influensavaksine til eldre inkluderer over 2,5 millioner mennesker fra seks randomiserte kontrollerte studier og 60 observasjonsstudier. Resultatene viste at influensavaksine muligens reduserer risiko for influensa og influensalignende sykdom hos eldre som bor utenfor institusjon. Influensavaksine påvirker muligens ikke totaldødelighet og lungebetennelse til eldre personer som bor utenfor institusjon. Influensavaksine reduserer muligens risiko for influensalignende sykdom hos eldre som bor i institusjon, personer med kronisk hjerte- og karsykdom og personer med hiv-infeksjon. Dokumentasjonsgrunnlaget er gjennomgående av lav og svært lav kvalitet, og resultatene er dermed beheftet med stor usikkerhet. This review presents the available documentation of the health effects of influenza vaccination to the elderly and people suffering from chronic illness.\ud

We searched for systematic reviews of the effects of influenza vaccine to the elderly and chronic ill. We updated the systematic reviews that we included, and we searched for randomized controlled trials (RCTs) of the effect of influenza vaccine to people with chronic illness. We included the following outcomes; all cause mortality, complications due to influenza (pneumonia), hospital admissions, influenza (laboratory confirmed), influenza like illness and adverse events.\ud

We included five systematic reviews that assessed the effect of influenza vaccination to the elderly, to people with chronic obstructive lung disease, asthma, bronchiectasis, and cystic fibrosis. We included one RCT and 12 observational studies for update of the effect of influenza vaccination on elderly. We additionally included four RCTs where the effect had been measured in people with cirrhosis of the liver, multiple sclerosis, HIV- infection, and cardio vascular disease.\ud

The evidence for influenza vaccine to elderly included more than 2,5 million people who had been included in six RCTs and 60 observational studies. Results indicate that influenza vaccine possibly reduce the risk of influenza and influenza like illness for community dwelling elderly. Influenza vaccine does possibly not influence the risk of all cause mortality or pneumonia for community dwelling elderly. Influenza vaccine possibly reduce the risk of influenza like illness for elderly living in institution, people with chronic hearth disease and people with HIV-infection. The quality of the evidence is low and very low, and results are uncertain. For the other outcomes and the other chronic diseases, evidence were either lacking or of very low quality.\ud

We performed a systematic search for systematic reviews in international research databases. We selected studies that met our inclusion criteria, scored their quality and summarized the results.\ud

We have only identified systematic reviews within rehabilitation and elderly care and have not found reviews in this context assessing mental health or drug addictions. Most included reviews have focused on measures taken in care, and not on the qualifications of the personnel. Only two of the reviews specifically referred to personnel. We identified studies on two occupations with specialized expertise in functional training and activation: occupational therapists and functional therapists. We therefore have little knowledge about other professions. In addition there are personal assistants - that have no formal competence in training function and activation. Several of the reviews included interdisciplinary measures and group therapies. We have limited information about what personnel were used in these studies. \ud

These reviews can only partially answer our question. There is little available research on the effects of using personnel with special expertise in municipal home care. We have not found systematic reviews that evaluated these measures for the areas of mental health or alcohol and drug addictions. Concerning the elderly, group activities seem to reduce isolation and loneliness. For stroke patients, occupational therapy probably has a small positive effect on social participation and activity, but present research does not address the applicability of these results to other patients

Nasjonalt kunnskapssenter for helsetjenesten fikk i april 2009 i oppdrag fra Arbeiderpartiets helse– og omsorgsfraksjon på Stortinget å oppsummere tilgjengelig forskning om tvangsbehandling av opioidavhengige. Denne kunnskapsoppsummeringen er tenkt som et dokumentasjonsgrunnlag for hva som er kunnskapsstatus med hensyn til tvangsbehandling av opioidavhengige.\ud

Arbeidet ble utført som en kunnskapsoppsummering med søk etter forskning av høy kvalitet. Vi har gjort systematiske søk for å identifisere relevant vitenskapelig litteratur på den forhåndsdefinerte problemstillingen. To prosjektmedarbeidere leste alle unike titler identifisert i litteratursøket, uavhengig av hverandre. Vi søkte også etter litteratur for å identifisere etiske problemstillinger knyttet til behandlingen av pasienter med opioidavhengighet.\ud

In April 2009, the Labour Party commissioned the Norwegian Knowledge Centre for the Health Services (NOKC) to complete a review about the effects of coercive treatment of persons dependent on opioids. The review would help answer the questions of whether and how adults with opioid dependence should be treated with coercive interventions. We addressed one main question: What are the effects of coercive treatment of persons dependent on opioids?\ud

Methods\ud
We searched systematically for relevant literature in international scientific databases. We evaluated published systematic reviews, randomized controlled trials (RCTs) and controlled studies for inclusion using a pre-designed inclusion form. We planned to appraise the methodological quality of the included studies with appropriate check lists, and summarize the results in tables and text. We searched for literature concerning ethical issues related to coercive treatment of persons dependent on opioids, and we used 23 sub questions to identify ethical issues.\ud

Results\ud
We found neither systematic reviews nor primary studies that evaluated the effects of coercive treatment of persons dependent on opioids. We excluded systematic reviews and studies which did not report results regarding the effects of coercive treatment of persons dependent on opioids, as well as interventions which did not meet the inclusion criteria of being coercive treatment for opioid dependence. We identified several ethical issues concerned with this treatment, which answered to our 23 sub-questions.\ud

Conclusion\ud
Our concerted efforts at identifying literature about the effects of coercive treatment of persons dependent on opioids showed that there is little available high quality research on the effects of coercive treatment in general, and on the effects of coercive treatment of persons dependent on opioids specifically. Ethical issues should be considered when designing and planning randomized controlled studies and controlled clinical trials for populations with this type of severe dependence

The Norwegian Knowledge Centre for the Health Services was commissioned by the Norwegian Directorate of Health to perform a systematic review of the scientific evidence concerning effects of rehabilitation on social and community participation among patients with reduced functional capacity. All patients were included regardless of their diagnosis. According to a parliamentary bill, rehabilitation is defined as: time-limited, planned processes with clear objectives and means, in which several parties cooperate to provide necessary assistance to the user’s own efforts to achieve the best possible coping and functional ability, independence and social and community participation.\ud

We searched for controlled studies of effect in databases for medical research literature. The criteria for inclusion were:\ud

Population: patients with reduced functional capacity regardless of diagnosis\ud
Intervention: multidisciplinary interventions based on the patient’s own aims and needs, and where the patients actively participate in their own rehabilitation process\ud
Outcome: participation socially and/or in the community\ud

The search identified 4876 references; we included three controlled clinical trials and three controlled before and after studies in this report.\ud
\ud
The included studies differed regarding various aspects. The patients had different diagnoses, the professional groups that participated in the multidisciplinary teams varied, the types of patient’s participation and which social activities or ways of participating in the community that was addressed all varied between the studies. The studies had different study designs, although they all included a control group. According to the quality assessment tool used the studies were all evaluated to have unclear or high risk of bias.\ud

The quality of the documentation for the effect of the intervention is too low for us to draw conclusions on whether rehabilitation contributes to the patients social and community participation. Whether the results can be generalised to other groups or other interventions is unclear. We need larger end better studies before a clearer conclusion can be drawn
Oppsummering av en toårig undersøkelse av selvmordssaker i psykisk helsevern
NORSK SAMMENDRAG: I årene 2005 og 2006 foretok Statens helsetilsyn en systematisk registrering av selvmord som ble begått av pasienter innskrevet i psykisk helsevern og som ble kjent for Helsetilsynet i fylkene. \ud

Vi fant at virksomhetene ikke overholdt sin lovpålagte meldeplikt til tilsynsmyndighetene i nesten en av fem tilfeller, og hendelsene ble i liten grad brukt til kvalitetsforbedrende arbeid. Virksomhetene sviktet i å utarbeide rutiner/prosedyrer for selvmordsforebyggende arbeid, opplæring av sine ansatte, journalføring og i å ivareta etterlatte mv. Vi fant videre store ulikheter i Helsetilsynet i fylkenes håndtering av selvmordssaker.SUMMARY IN ENGLISH: During 2005 and 2006, the Norwegian Board of Health Supervision conducted a systematic registration of suicides committed by patients registered in the mental health care service and that were brought to the notice of the county branches of the Board of Health Supervision. \ud

We found that the health enterprises did not comply with their statutory obligation to report to the supervisory authorities in nearly one of five cases, and that the events were only to a small extent used for quality improvement work. The health enterprises failed in preparing routines/procedures for suicide prevention work, in training their employees, in keeping records and in taking care of the bereaved, etc. We also found great differences in how the county branches of the Board of Health Supervision handled suicide cases
Simulering som opplæringsmetode i spesialistutdanning av leger - evaluering av effekt

Virtual training of manual and practical skills in skills laboratories gives possibilities for training without the involvement of real patients by use of physical models, or different types of simulators. Virtual training can be used for development of component skills or as a full scale simulation to practice an entire procedure in a realistic environment, but without living patients. The National knowledge centre for the health services was given an assignment by the Spesialitetsrådet, The Norwegian Medical Association, to summarise available research on the effect of virtual training of skills by using models or different categories of simulators which are used in specialist training of physicians. The report is based on the results of six systematic reviews that were published within January 2008. All of the reviews deal with training of surgical skills. We have not identified any systematic reviews from other medical fields where virtual training has been used, despite growing number of primary studies. Summed up research shows:\ud

* Virtual training of skills without the use of real patients, but with models and simulators, given in addition to standard training, may improve surgical skills and may increase general competence.\ud
* Surgical skills practiced in skills laboratories or simulator training can probably be transferred to clinical practice, but documentation is still limited and the results should be interpreted with care.\ud
* The research does not give grounds for conclusion about which type of virtual training is most effective or how training should best be organised.\ud
* The quality of available research is too low to decide if virtual training of skills has an impact on patient oriented outcomes such as pain, mortality, morbidity or satisfaction.\ud
* The impact of virtual training of skills with simulators and models is best documented with regard to training of certain technical and manual skills.\ud
* The impact of virtual training is still poorly documented with regards to whether this training actually improves the quality of health services and improves patient security
Læring av feil og klagesaker
NORSK SAMMENDRAG: Boka er en oppfølger av heftet ”Kollegabasert læring av klagesaker” fra 1995, utgitt av Legeforeningen i samarbeid med Statens helsetilsyn. Senere har både direktøren i Statens helsetilsyn og Legeforeningens president i flere sammenhenger påpekt behovet for å lære av feil som begås i helsetjenestene.\ud

Vi har derfor lagt vekt på å utarbeide en eksempelsamling. I denne finnes et bredt spekter av alvorlige og mindre alvorlige hendelser og sykdomsforløp som er hentet fra klager behandlet av tilsynsmyndighet og Norsk pasientskadeerstatning. Sakene skriver seg fra allmennpraksis og legevakt – og er selvsagt anonymisert. \ud

Den teoretiske delen tar sikte på å gi kunnskap om feil i allmennmedisinsk virksomhet, klagebehandling, legers reaksjoner på å begå feil og hvordan klagesituasjonen best kan håndteres. De pedagogiske utfordringene og mulighetene blir beskrevet og drøftet. Det har de senere årene vært satt fokus på feil gjort av leger uten norsk opprinnelse. Denne problemstillingen er omtalt i boka, sammen med en gjennomgang av hvordan allmennlegevirksomhet kan organiseres for å unngå feil. Forfatterne er alle toneangivende innenfor sine fagfelt innen undervisning, forskning og tilsynsvirksomhet. \ud

Vi ønsker at boka skal gi grunnlag for diskusjoner blant medisinske studenter, turnusleger, allmennleger under spesialistutdanning og i smågrupper for allmennleger i etterutdanningen. (“Kollegabasert læring av klagesaker”) from 1995, published by the Norwegian Medical Association in cooperation with the Norwegian Board of Health Supervision. Later, on several occasions, both the Director General of the Norwegian Board of Health Supervision and the President of the Norwegian Medical Association have pointed out the need for learning from errors that are made in the health services. \ud

Therefore, we have produced a collection of examples. We present a broad range of serious and less serious events and case histories, based on complaints that have been dealt with by the supervision authorities and the Norwegian System of Compensation for Injuries to Patients. The cases are from general practice and the emergency services, and are, of course, made anonymous.\ud
\ud
The theoretical part presents information about the following: errors in general medical practice, dealing with cases of complaint, doctors’ reactions to making errors, and how complaints can best be dealt with. Pedagogical challenges and possibilities are described and discussed. During the last few years, focus has been directed towards errors made by doctors from other countries. This issue is discussed in the book, including a review of how general practice can be organised in order to prevent errors. The authors are all authoritative persons in the fields of teaching, research and supervision of services.\ud

BAKGRUNN: Demens er en samlebetegnelse på en rekke sykdommer som varig nedsetter mental kapasitet, og er sannsynligvis den viktigste årsaken til funksjonshemning hos eldre. For å målrette og tilpasse det kommunale omsorgstjenestetilbudet bedre til personer med demens og sammensatte lidelser, har regjeringen i 2008 iverksatt et utredningsarbeid for å utvikle et sterkere kunnskapsgrunnlag knyttet til konkret utforming av bo- og institusjonstilbud. METODE: Det ble utført systematiske søk i følgende databaser t.o.m. 14.01.2009: Cochrane Database of Systematic Reviews, Cochrane CENTRAL, DARE (Database of Abstracts of Reviews of Effects), HTA, MEDLINE, EMBASE, PsycINFO, SveMed, OT Seeker og SINTEF Byggforsk publikasjoner for å identifisere systematiske oversiktsartikler og primærstudier. I tillegg ble det utført manuelle søk i referanselistene i relevante rapporter og ekskluderte oversiktsartikler. RESULTATET: Søket resulterte i 3388 unike titler. Etter gjennomlesning av tittel og sammendragene ble 161 titler identifisert som mulig relevante og innhentet i fulltekst. Ytterligere 53 artikler ble innhentet etter manuelle søk i referanselister. Kunnskapsoppsummeringen rapporterer resultater fra 8 oversikter som til sammen oppsummerte 96 ulike enkeltstudier, og 11 primærartikler. KONKLUSJONER: Forhold i fysiske omgivelser har vist bedring for personer med demens på fire hovedområder: nivå på dagligfunksjoner, adferdsforstyrrelser, positiv adferd og livskvalitet. Resultatene må tolkes med forsiktighet på grunn av metodiske begrensninger i de foreliggende studiene.1-PAGE KEY MESSAGES:\ud

This review examines effects of the built environment and elements therein on people with dementia.\ud

Dementia diseases lead to degrees of disabilities that progressively involve caretakers up to stages of complete helplessness and is at present the most common reason for transfer of elderly to care programs. 80% of insitutionalized elderly in Norway have cognitive impairments. We have summarized results from 8 systematic reviews and 11 primary studies on the therapeutic design of environments for people with dementia.\ud

The results should be interpreted cautiously because of methodological limitations in the present studies. Adaptations in the physical environment can lead to positive effects on activities of daily life, behaviors, and quality of life. The overall configuration of a building can affect level of orientation amongst people with dementia. Uniqueness in common rooms, short corridors, calm surroundings and few decision points eased orientation. Spatial orientation and accessability was better in small grouped units with associated common places. Neutral design and colours, and low sensory stimulus are associated with fewer catastrophe reactions and less medication. Visual modification and adaptions can lead wanderers to move in safe areas. There is some evidence to support camouflaging exit door or doorknob against elopement. There were not significant effects of preventing falls amongst people with dementia. Signs and colour codes have small effects on people with cognitive impairments, depending on degree of dementia. Increasing environmental lighting (towards daylight levels) reduced behavioural disturbances, supported spatial orientation and had a modest effect on length of sleep.\ud

Small units with a noninstitutional environment gave less behavioural challenges among people with dementia, but specific features are ill-defined. Less combined multiuse spaces, greater variation and less crowding resulted in less cognitive stress, depression and social isolation amongst Alzheimers patients. Single rooms with the opportunity for individual and home-like character, personal furnishing and objects resulted in less psychiatric symptoms. Highly accessible toilets (visible, open-door and close to bed and common rooms) led to increased use among residents.\ud
