    The SWITCH algorithm: An expert consensus on treat‐to‐target criteria for chronic prurigo

    International audienceAbstract Background To date, the management of chronic prurigo (CPG; prurigo nodularis) relies on expert consensus and guidelines without providing detailed recommendations on long‐term treatment management. Objectives This study was initiated by an expert committee to define, validate and reach consensus on treat‐to‐target (T2T) criteria and a decision‐supporting treatment algorithm for CPG. Methods A prospective, single‐centre, non‐interventional study was conducted in adult CPG patients at a moderate‐to‐severe disease stage, experiencing intense itch (NRS ≥7; Group A), and in patients who considered themselves successfully treated (Group B). The patients answered questions about their most severe symptoms and essential therapy goals. A committee of 14 field experts evaluated the study results, identified outcome tools from clinical trials and defined and weighted T2T criteria. Based on this, a treatment algorithm was developed through an iterative consensus‐building method and anonymous voting. Results 171 patients (Group A, n = 96; Group B, n = 75) were interviewed. Itch (99.4%), the need to scratch (48%), and pruriginous lesions (45.6%) were the most frequently reported severe symptoms. Pruritus relief (over complete pruritus resolution) and healing of lesions were the key therapeutic goals. In clinical trials, the most frequently used instruments were assessments of worst/peak itch intensity, disease severity via IGA, DLQI and sleep quality. According to the results, the experts defined initial itch control as NRS ≤3, lesion healing with IGA‐S 0/1, and burden, along with their respective clinically measurable parameters, as T2T criteria. These criteria, with predefined cut‐off values, serve as the decision‐tree keys that constitute the treatment algorithm presented here, facilitating a decision to maintain or switch the treatment. Conclusions The T2T criteria and the SWITCH treatment algorithm offer a valid and systematic method for evaluating and refining CPG treatment based on patient‐reported and expert‐validated data. This will help to improve the treatment of patients with CPG

    Décrypter les changements de communautés biologiques le long du littoral breton au cours de l'Anthropocène par paléogénomique

    International audienceL’Anthropocène, définie comme la période au cours de laquelle l’impact des activités humaines est devenu significatif à l’échelle planétaire, se caractérise par de profondes altérations de la biodiversité. Ces perturbations ont pu être documentées grâce aux avancées majeures de la génomique environnementale. En particulier, l’étude de l’ADN ancien extrait des sédiments (sedaDNA) permet une évaluation fine et exhaustive de ces dynamiques, à différentes échelles temporelles (i.e. allant jusqu’à plusieurs millénaires) et géographiques (e.g. océan profond, pergélisol, fjords et lacs).Dans ce contexte, et afin d’évaluer l’impact des pressions anthropiques mais également climatiques sur la biodiversité au cours des 100 dernières années en région Bretagne, nous avons initié une étude de paléogénomique. L’aquaculture, les pratiques agricoles intensives, ainsi que l’urbanisation et l’industrialisation accélérées après la Seconde Guerre mondiale, propre à cette région, ont sans aucun doute profondément influencé la biodiversité locale. Dans le cadre des projets Pacte, PaleoBreizh et de l’expédition européenne TREC, plusieurs carottes de sédiments d’environ 1 mètre de long ont été prélevées en Rade de Brest, dans la Baie de Lorient et à Roscoff. La situation géographique de ces sites, chacun associé à un bassin versant distinct, permet de considérer ces archives sédimentaires comme de précieux témoins de l’impact des activités humaines. Par ailleurs, les datations radio isotopiques ont confirmé que l’ensemble des carottes couvre des périodes associées à des événements de pollution et/ou à des changements écosystémiques. L’étude de la diversité microbienne a alors été initiée par séquençage d’amplicon (ADNr 18S V4, V9 et 16S V4-V5) en sélectionnant des échantillons correspondant à des périodes marquées par les activités humaines, telles que corroborées par les profils de contaminants.Des comparaisons inter-sites permettront de tester plusieurs hypothèses concernant (i) l’érosion de la biodiversité, (ii) les trajectoires convergentes des communautés face à des pressions anthropiques spécifiques, et (iii) la dynamique temporelle régionale des espèces non indigènes. Les résultats préliminaires issus de ce projet seront présentés et discutés dans le contexte plus large des changements globaux liés à l’anthropisation des milieux

    Analysis of humpback whale song units from the study of their vocal generator

    Bioacoustique et acoustique environnementale; GABE - Acoustique du Bâtiment et de l'Environnement: GBIO - BioacoustiqueNational audienceHumpback whales are known for their longer migratory routes and for the songs that males emit mainly during the breeding seasons. Over 60 years, researchers described the acoustic characteristics of these vocalizations emitted in a precise order making several repeated phrases. Our previous study published in 2016 showed that some vocalizations exhibited acoustic nonlinearities. The frequency jump is abrupt change in the fundamental frequency during the temporal duration of the vocalization. There may be multiple jumps within the same vocalization. The chaotic mode is the presence of noise during the vocalization, as if the cetacean suddenly lost the vibrational control of its vocal generator, then very quickly adjusted it to return to the original fundamental frequency. In this present study, we confirmed the presence of these 2 nonlinearities in the songs of humpback whales, and we found 2 new ones. Some vocalizations exhibit two frequencies that evolve differently over time, representing the first description of biphony in humpback whales. Finally, we also detected subharmonics. We propose to explain these 4 acoustic non-linearities by the mechanical description of their vocal generator, in particular by the vibrations of the membranes covering the arytenoid cartilages which hold the laryngeal sac at the level of the trachea. The individual can adjust the spacing of these cartilages as well as the intensity and direction of the air flow to select a part of these membranes. Because these non- linearities were found in 3 different songs from 3 different oceans at 3 different years, our assumption is that these acoustic features are finally non rare, and does not depend on a specific anatomical abnormality of the vocal generator of a particular singer. This study can be seen as a contribution to the identification of an individual acoustic signature, which has never been shown in mysticetes

    Rational design of mesoporous NiWO4 / Co3O4/ g-C3N4 based heterostructure for high performance asymmetric supercapacitors

    International audienceHerein we have designed a nickel-based tungsten oxide as a cathode material for hybrid supercapacitor owing to its better theoretical capacitance values. However, the material lacks good conducting behaviour and rate capability which affect its extensive utilisation as electrode material. In order to overcome the defects heterostructure composite was designed to enhance its electrochemical behaviour. The prepared materials were characterised with several physiochemical and electrochemical techniques. The ternary nanocomposite has portrayed cube like morphology with mesoporous nature. In specific, the ternary composite in three electrode cell have delivered a high specific capacitance of 819 F/g at 1 A/g when compared to other materials and retained a initial capacitance of 91 % after 5000 charge discharge cycles. Then an asymmetric hybrid supercapacitor with NiWO4/Co3O4/g-C3N4 as positive electrode and rGO as negative electrode in 3 M PVA-KOH electrolyte using Swagelok cell was assembled. The fabricated device exhibited a specific capacitance of about 113 F/g at 1 A/g in the potential of 1.4V with a specific energy of 35 Wh/kg at an specific power of 1500 W/kg. Further the device exhibited a better cycle life of 93 % even after 10000 cycles. Thus, the transition metal tungstate based heterostructure could be employed as a potential electrode material for efficient supercapacitors

    Video rate widefield sensitive polarized light microscopy by spectrally structured illumination

    International audienceIn this Letter, we propose a setup to realize quantitative and sensitive widefield microscopy of optical retardation and azimuth of a birefringent specimen at video rate. The originality of the technique lies in the spectral structuration of the microscope illumination using spectral coding of polarization. The optical retardation and azimuth map of the specimen are obtained from a combination of three intensity images captured after successive illumination of the microscope by three phase-shifted spectrally structured spectra from light-emitting diodes (LEDs).     In Situ Characterization of Lithium-Ion Battery Dynamic Impedance Spectrum for Real-World Driving Applications

    International audienceDynamic electrochemical impedance spectroscopy (DEIS) provides critical insights into the kinetic pathways of Lithium-ion batteries under dynamic operating conditions, establishing its significance in advanced onboard diagnostics. However, accurate DEIS implementation faces challenges due to voltage drifts arising from load fluctuations, state of charge (SOC) variations, and temperature changes. To address these issues, this work proposes a novel differencing framework that systematically suppresses drifting components while preserving the integrity of perturbation and response signals, enabling precise in situ frequency response analysis. Analytical criteria are established to select differencing lags and orders, effectively mitigating disturbances without compromising frequency-domain responses under dynamic driving conditions. Comprehensive experimental investigations across diverse SOC levels, driving profiles, thermal environments, and battery aging states validate the method, achieving consistently accurate and stable DEIS measurements with maximum absolute residuals below 0.6% under Kramers-Kronig validatio
